Chronic infection with cutaneous herpes simplex in a patient with systemic lupus erythematosus.

A 68-year-old black woman with systemic lupus erythematosus being managed by systemic corticosteroids developed large ulcerated lesions in a dermatomal distribution on the flank and abdomen. Subsequently, large ulcers developed progressively in the perianal region, the buttocks, the perivaginal region, and the thighs. The clinical diagnosis was vasculitis of systemic lupus erythematosus. Biopsy from the margin of an ulcer showed changes in the epidermis that are specific for infections by herpesvirus. Electron microscopy revealed viral particles of herpesvirus. Cultures from the perianal lesions grew Herpesvirus hominis. This is the first report to our knowledge of "chronic cutaneous herpes infection" in a patient with systemic lupus erythematosus.

Desmoplastic malignant melanoma: a study by conventional and electron microscopy.

A 61-year-old white man presented himself with a mass that had recurred on the chin. Conventional microscopy of sections from this mass showed atypical spindle cells surrounded by abundant collagen and resembled fibrosarcoma. Stains for melanin revealed pigment in the cytoplasms of the atypical spindle cells. Study of sections by electron microscopy demonstrated round, oval, and spindle-shaped cells having desmosomes and containing abundant melanosomes in varying stages of maturation. On the basis of both conventional microscopy and electron microscopy, the diagnosis was desmoplastic malignant melanoma. An unusual finding by electron microscopy was the presence of tubuloreticular intracytoplasmic inclusions in the neoplastic melanocytes.

Angiosarcoma arising in a plexiform neurofibroma: a case report.

A left cervical mass developed in a 14-year-old white boy with stigmata of von Recklinghausen's disease. Excision biopsy examination showed a plexiform neurofibroma with angiosarcoma. Ultrastructural studies confirmed the endothelial nature of the malignant cells.

Benign lipoblastoma: report of a case.

Light and electron microscopic studies of a benign, well circumscribed lipoblastoma in a 10-year-old boy were performed. Ultrastructural examination demonstrated lipocytes, lipoblasts, undifferentiated mesenchymal cells, and occasional hibernoma cell types. Myxoid liposarcoma as a differential diagnosis was considered. However, an absence of mitosis and the hyperchromatic and bizarre nuclei of the lipoblasts ruled out the possiblity of malignancy.

Multiple periorbital fibrous histiocytomas: a light and electron microscopic study.

Recurrent, bilateral, multiple, periorbital fibrous histiocytomas occurred in a 62-year-old woman. There were no associated serum lipid abnormalities. Histologically, the tumor was composed of an admixture of lipid-laden histiocytes and fibroblasts with a variable amount of collagen production. In many areas the fibroblasts were arranged in a storiform pattern. No cytologic evidence of malignancy was observed. Ultrastructural studies of the tumor showed histiocytic cells with a variable amount of lipid in the cytoplasm and fibroblasts with mild to moderate collagen production. Differential diagnosis and histogenesis are discussed.

Thyroglossal duct carcinomas: light and electron microscopic studies.

Two cases of papillary carcinoma arising in thyroglossal duct cysts are reported. Electron microscopic studies were done on both cases and similar ultrastructural features were noted. When compared with a case of papillary adenocarcinoma arising in the thyroid gland proper, no difference was seen both at the light microscopic and ultrastructural levels.

Beta 1 integrin expression: a marker of lymphatic metastases in cutaneous malignant melanoma.

BACKGROUND: Experimental evidence suggests that integrins are key regulators of the development of melanoma metastases, influencing both the likelihood and site of metastases. Whereas effective treatment of cutaneous melanoma remains surgical, elective lymph node dissection (ELND) is controversial. The present study was designed to investigate the relationship between integrin expression by a given primary melanoma and occult regional lymph node metastases. MATERIALS AND METHODS: We studied beta 1 integrin expression, by quantitative immunohistochemistry using an image analyzer, in the primary melanomas of 90 ELND patients. RESULTS: beta 1 integrin was expressed in > or = 10% of the primary tumor in 92% of cases eith lymph node involvement versus 9% of node negative cases (p < 0.001). CONCLUSIONS: Our data demonstrate that quantitative immunohistochemistry for beta 1 integrin expression in primary melanomas can identify patients likely to have occult lymph node metastases. This suggests that beta 1 integrins play a role in the lymphatic dissemination of cutaneous melanoma.
