Machinery-related occupational fatalities in the United States, 1980 to 1989.

The National Traumatic Occupational Fatalities surveillance system identified machinery-related incidents as the second leading cause of traumatic occupational fatalities in the United States between 1980 and 1989. These incidents resulted in 8,505 civilian worker deaths and an average annual fatality rate of .80 per 100,000 workers. Workers aged 65 years and older had 5.8 times the fatality rate of workers aged 16 to 64 years (4.06 vs. 70). The highest industry-specific rate was noted in agriculture, forestry, and fishing (7.47). Tractors and other agricultural machinery were associated with nearly 9 of every 10 fatal machinery-related incidents involving workers aged 65 or older. Although numerous studies of agricultural machinery-related fatalities are found in the literature, detailed analyses of machinery-related fatalities in the construction industry as well as analyses of work situations and risk factors associated with fatal injuries are needed.

Mohs micrographic excision of sebaceous carcinoma of the eyelids.

BACKGROUND: Sebaceous carcinoma of the eyelid is a difficult tumor to manage because of diagnostic difficulties and pagetoid-type spread in the conjunctivae. OBJECTIVE: We report our experience with eight patients using the Mohs technique of excision with reconstruction. METHODS: Six cases were managed with paraffin-embedded hematoxylin and eosin sections. One case was managed by frozen histologic sections and one was managed with frozen section processing followed by paraffin-embedded section review. RESULTS: The correct pathologic diagnosis was made initially in only 50% of the cases. Eighty-eight percent of the cases revealed intraepithelial neoplasm. There has been one recurrence with metastatic disease. The average follow-up period has been 56.8 months. CONCLUSIONS: We advocate early recognition and excision with the Mohs technique with paraffin-embedded sections followed by reconstruction. The use of paraffin-embedded histologic sections helps with interpretation of intraepithelial and pagetoid tumor spread.

Evisceration techniques.

Evisceration is the removal of the intraocular contents of the globe. I favor evisceration with retention of the cornea when at all possible. Surgical techniques for evisceration with and without keratectomy are discussed. A promising new implant material, hydroxyapatite, is described. The outstanding advantage of this material is its transformation into "living tissue." Although extremely rare, evisceration can be associated with postoperative sympathetic ophthalmia in the remaining eye.

Anti-Bacterial Agents

The Marcus Gunn phenomenon. A review of 71 cases.

We have reviewed the clinical findings, natural course, pathologic observations, and management of 71 patients with the Marcus Gunn phenomenon. We found individuals with this syndrome to have a significant incidence of amblyopia (59%), double elevator palsy (25%), anisometropia (25%), and superior rectus muscle palsy (23%). Long-term follow-up did not reveal a case that improved with age. In most patients requiring surgery, we recommend a unilateral levator excision on the affected side plus a bilateral frontalis suspension.
