Absence of cytotoxic antibody to human immunodeficiency virus-infected cells in humans and its induction in animals after infection or immunization with purified envelope glycoprotein gp120.

The presence of antibody-dependent complement-mediated cytotoxicity (ACC) was assessed in humans and chimpanzees, which are capable of infection with human immunodeficiency virus isolate HTLV-IIIb, and examined in the goat after immunization with the major viral glycoprotein (gp120) of HTLV-IIIb. In infected humans no antibody mediating ACC was observed regardless of the status of disease. Even healthy individuals with high-titer, broadly reactive, neutralizing antibodies had no ACC. In contrast, chimpanzees infected with HTLV-IIIb, from whom virus could be isolated, not only had neutralizing antibody but also antibodies broadly reactive in ACC, even against distantly related human immunodeficiency virus isolates, as well as against their own reisolated virus. In the goat, the gp120 of HTLV-IIIb induced a highly type-specific response as measured by both ACC and flow cytofluorometry of live infected H9 cells. Normal human cells were not subject to ACC by animal anti-HTLV-III gp120-specific sera. Induction of ACC and neutralizing antibody were closely correlated in the animal experimental models but not in humans. The presence of ACC in gp120-inoculated goats and HTLV-III-infected chimpanzees represents a qualitative difference that may be important in the quest for the elicitation of a protective immunity in humans.

Chemoradiotherapy as initial management in patients with squamous cell carcinoma of the head and neck.

Thirty-nine previously untreated or minimally treated patients with squamous cell carcinoma of the head and neck were entered onto a chemoradiotherapy protocol employing multiple courses of simultaneous radiation therapy, cisplatin, and a 4-day 5-FU infusion. Thirty-eight patients were evaluable for response and toxicity. Twenty-three patients underwent surgical resection midway through this therapy and 11 (48%) were pathologically free of disease. Thirty-five of the 38 patients (92%) were ultimately rendered disease-free by this combined modality protocol. Thirty-one patients remain disease-free, with a projected 2-year disease-free survival of 74%. Although the treatment-associated mucositis and myelo-suppression were significant, this chemoradiotherapeutic approach is promising and merits further study.

Combination chemotherapy prior to definitive local therapy in squamous cell carcinoma of the head and neck.

30 patients with locally advanced squamous cell carcinoma of the head and neck were given chemotherapy with methotrexate, bleomycin and cis-Platinum prior to planned definitive surgery and/or radiation therapy. Chemotherapy was well tolerated and 90% of patients responded. Of 14 initially unresectable patients, 6 became operable after chemotherapy. After definitive therapy 70% of patients were considered complete responders, and 43% of the entire cohort remain disease-free. All partial responders have died. Patients receiving chemotherapy, surgery and radiation had a significantly greater likelihood of achieving a complete response than those receiving chemotherapy and radiation alone. Although this initial chemotherapy is highly active in this disease its contribution to ultimate survival is unclear. Overall treatment success is determined by the results of definitive therapy.

Purification and characterization of human lymphoid poly(adenosine diphosphate ribose) polymerase.

Poly(ADP-ribose) polymerase has been purified 12 000-fold from human tonsils with an 83% recovery of enzymatic activity relative to that of the initial homogenate. The specific activity of the purified enzyme is 862 units/mg of protein. The isolated protein has a molecular weight of approximately 116 000 as determined by sodium dodecyl sulfate-polyacrylamide gel electrophoresis. The apparent Km for NAD+ is estimated to be 185 microM at pH 8.0 and 37 degrees C. The purified enzyme has an absolute requirement for exogenous DNA for catalytic activity, and the reaction is enhanced by the addition of purified histone H1. The enzyme does not require magnesium or other divalent cations for activity. Enzyme activity is inhibited by p-(hydroxymercuri)benzoate and N-ethylmaleimide. Thymidine, theophylline, nicotinamide, and 5-methylnicotinamide markedly inhibit enzyme activity whereas ADP-ribose, 3',5'-cAMP, and sodium fluoride have a minimal effect on enzyme activity. Autoradiograms of labeled products of the reaction catalyzed by the purified enzyme at different concentrations of NAD+ and at different incubation times show that at low concentrations of NAD+ and after short incubations, poly(ADP-ribosyl)ation of the enzyme occurs preferentially over that of histone H1; at higher concentrations of NAD+ or after longer incubations, poly(ADP-ribosyl)ation of histone H1 is increased.

Inorganic phosphate assay with malachite green: an improvement and evaluation.

An improvement over existing procedures for the determination of nanomole quantities of inorganic phosphate (Pi) is described. The protocol is simplified, and the effective concentration range of Pi in which the assay may be used is increased to 60 nmol/ml. Many of the substances commonly used in association with Pi assays (i.e. phosphohydrolase studies) are shown not to interfere with the measurement of Pi by this method. The effects of detergents and protein on the assay also were investigated, and methods for avoiding interferences by them are described.

Characterization and molecular cloning of a bovine lentivirus related to human immunodeficiency virus.

An infectious virus which causes persistent lymphocytosis, lymphadenopathy, lesions in the central nervous system (CNS), progressive weakness and emaciation was previously isolated from the leukocytes of cattle. Our present studies show that this virus encodes a reverse transcriptase (RT) with Mg2+ cation preference, replicates and induces syncytia in a variety of embryonic bovine tissues in vitro, and has a morphology most similar to the human immunodeficiency virus (HIV). Moreover, serologic analyses have demonstrated a conservation of epitopes between the major core protein of this bovine retrovirus and HIV. Shared antigenic determinants were also observed with other pathogenic retroviruses of the lentivirus subfamily. To resolve the phylogenetic relationship of this virus, proviral molecular clones were derived and used to determine the nucleotide sequence of the highly conserved RT domain. The sequence data and serologic analyses together show that this bovine retrovirus is a novel lentivirus related to HIV and other lentiviruses. We propose that this virus be tentatively named bovine immunodeficiency-like virus (BIV) to reflect its genetic relationship and biological similarity to HIV.

Syndrome of inappropriate secretion of antidiuretic hormone after infusional vincristine.

A 77-year-old woman with refractory multiple myeloma was treated with a 4-day continuous intravenous infusion of vincristine and doxorubicin and 4 days of oral dexamethasone. Nine days after her second cycle she presented with lethargy and weakness associated with hyponatremia. Evaluation revealed the syndrome of inappropriate secretion of antidiuretic hormone, which was attributed to the vincristine infusion. After normal serum sodium levels returned, further doxorubicin and dexamethasone chemotherapy without vincristine did not produce this complication.
