[ASD plaque formation of the mitral valve in secundum atrial septal defect and the timing for operation].

Non-rheumatic mitral regurgitation (MR) was found in 33 patients (10.2%) in a total of 324 patients with secundum atrial septal defect (ASD) who were operated on during 13 years period at the Mitsui Memorial Hospital. This combination was more frequent in women (27 patients: 13.5%) than in men (10 patients: 6.5%), and the incidence tended to increase with age. Inspection at operation revealed a marked fibrous thickening (ASD plaque) of the anterior and posterior mitral valve leaflets with a localized bulging into the left atrium at the posteromedial commissural area. Similar but less prominent change was found in nearly all patients above 20 years of age in the absence of mitral regurgitation. A friction between the anterior and posterior mitral leaflets caused by the abnormal left ventricular motion secondary to the right ventricular volume overload might be responsible to the genesis of this lesion. I proposed a new entity, "ASD-MR complex", for this combined lesion, as it demonstrates a specific pathophysiology and necessitates a different procedure for surgical correction from that of a secundum atrial septal defect without mitral or tricuspid involvements. As ASD plaque begins to appear in the second decade, surgical closure of the defect should be indicated before ten years of age. The incidence of severe MR or tricuspid regurgitation (TR) was significantly higher in patients older than 40 years of age. In cases with ASD-MR complex with or without TR, mitral or tricuspid annuloplasty with direct closure of the defect should be recommended.

Exocrine pancreatic cancer with humoral hypercalcemia.

Humoral hypercalcemia associated with malignancy has rarely been reported in exocrine pancreatic cancer. We report a patient with cancer of the exocrine pancreas who presented with hypercalcemia which did not respond to indomethacin. Her serum levels of parathyroid hormone and vitamin D derivatives were low. Technetium diphosphonate bone scan revealed no evidence of bone metastasis, a finding which was confirmed at autopsy. On light microscopy, histological classification of the tumor was moderately differentiated tubular adenocarcinoma. The electron microscopic study, however, revealed a few zymogen-like granules containing cancer cells lying between ductal-type cancer cells. A review of humoral hypercalcemia in cancer of the exocrine pancreas is presented. A humoral factor(s) other than parathyroid hormone, prostaglandin E, and vitamin D derivatives is considered responsible for hypercalcemia in this patient.

Low molecular weight (7s) immunoglobulin M antibody against hepatitis B core antigen in the serum for differentiating acute from persistent hepatitis B virus infection.

Sera from persons infected with hepatitis B virus were fractionated into 19s and 7s moieties by high performance liquid chromatography and then tested for the antibody to hepatitis B core antigen (anti-HBc) of immunoglobulin M (IgM) class by radioimmunoassay. Sera from 17 patients with acute or fulminant hepatitis invariably showed a high activity of 19s IgM anti-HBc (sample/normal ratio 30.6 +/- 13.6). In addition, they all revealed a much lower activity of 7s IgM anti-HBc (4.1 +/- 3.4). In remarkable contrast, 7s IgM anti-HBc activity was higher than 19s IgM anti-HBc activity in the sera from 5 asymptomatic carriers (10.4 +/- 2.4 vs. 1.0 +/- 0.1), 4 patients with chronic persistent hepatitis (11.7 +/- 1.4 vs. 2.1 +/- 1.4), 13 with chronic active hepatitis (13.6 +/- 3.0 vs. 6.0 +/- 5.6), and 10 with liver cirrhosis (8.9 +/- 3.4 vs. 5.4 +/- 2.4). Among these 32 cases of persistent hepatitis B virus infection, there were only 2 (6.3%) in which the 7s IgM anti-HBc was lower than the 19s IgM anti-HBc. The proportion of patients who showed the value of 7s IgM anti-HBc exceeding that of 19s IgM anti-HBc was significantly higher for persistent (30 of 32) than for acute (0 of 17) hepatitis B virus infection (p less than 0.001). On the basis of these results, the determination of IgM anti-HBc in terms of 19s and 7s subpopulations would be useful for differentiating acute from chronic hepatitis B virus infection, especially when the titer of IgM anti-HBc is not high enough to exclude a persistent hepatitis B virus infection.

A case of asymptomatic primary biliary cirrhosis with an initial presenting feature of localized gastric varices.

A case of asymptomatic primary biliary cirrhosis (PBC) with an initial presenting feature of localized gastric varices is reported. The patient, 64 years old female, underwent a barium meal examination because of ill-defined abdominal complaints and was found to have gastric varices localized at the cardia but no esophageal varices. Her blood chemistry showed high values of biliary tract enzymes such as alkaline phosphatase and gamma-glutamyl transpeptidase, but the serum bilirubin level was almost normal. Serum anti-mitochondrial antibody was positive. Histological findings of the surgically biopsied liver specimen were compatible with PBC. The clinical implication of gastric varices in PBC is discussed.

Serum pancreatic oncofetal antigen: its clinical usefulness for screening pancreatic cancer in combination with tests for other tumor markers.

Screening for pancreatic cancer was carried out by serum pancreatic oncofetal antigen (POA) tests in 440 out-patients with abdominal complaints, with concomitant assay of carcinoembryonic antigen, alpha-fetoprotein and ferritin. POA was positive in 13 patients, of whom 3 cases of pancreatic cancer, and 4 of other malignant diseases were diagnosed while the remaining 6 were non-malignant. Of these out-patients, 5 cases were finally diagnosed as having pancreatic cancer by further examinations. Of these 5 patients, POA was negative in 2, of whom one was positive for CEA and AFP. Accordingly, when POA test was applied concomitantly with the other 3 marker tests, the detection rate of pancreatic cancer was elevated to 4/5. Twenty-three patients were found to have malignant diseases and 20 of them were positive for at least one of the 4 markers tested. These results suggest that serum POA assay is useful for screening the pancreatic cancer, especially when the other tumor markers are assayed concomitantly.

Purification and properties of carnitine acetyltransferase from rat liver.

Carnitine acetyltransferase was purified from rat liver after induction of the enzyme by feeding with di(2-ethylhexyl)phthalate. Two enzyme sources were used: the mitochondrial fraction and the homogenate of the liver. The purification procedure was essentially the same for the two enzyme sources. The enzyme purified from the mitochondrial fraction consisted of two different polypeptides with molecular weights of 36,500 and 27,000, whereas that from the homogenate consisted of one polypeptide with a molecular weight of 67,500. Amino acid compositions and peptide maps of the limited proteolytic products of the two enzyme preparations were nearly the same. Their antibodies were cross-reactive. Catalytic properties of the two preparations were nearly the same: the specific enzyme activities, double reciprocal plots of initial velocity study, substrate specificities for acylcarnitines having various carbon chain lengths, apparent Michaelis constants for substrates. On electrophoresis of the immunoprecipitate obtained after incubation of the mitochondrial extract, the two immunoreactive polypeptides with molecular weights of 36,500 and 27,000 were found. But only one polypeptide, with molecular weight of 67,500, was detected when the protease inhibitors were added to the mitochondrial extract. It was concluded that the enzyme in the mitochondrial fraction was a monomeric form but was converted into a dimeric form by proteolytic modification after the disruption of mitochondria. The preparation from the post-mitochondrial fraction, which had a lower specific activity, contained two polypeptides whose molecular weights were 69,000 and 67,500. They could not be separated from each other throughout the purification. The peptide maps of the products of the limited proteolysis were very similar.

Biosynthesis and turnover of carnitine acetyltransferase in rat liver.

Male Wistar rats were fed on a diet with and without di(2-ethylhexyl)phthalate (DEHP) for 2 weeks. Carnitine acetyltransferase in the liver was increased about 100-fold by administration of DEHP. The results of in vivo experiments showed that the incorporation of L-[4,5-3H]leucine into the enzyme was 12-fold higher and the half-life of the labeled enzyme was elongated by a factor 4.6. The results of in vitro translation experiments with total hepatic RNA in a rabbit reticulocytelysate system and the results concerning the synthesis of the enzyme in isolated hepatocytes indicate that the translatable mRNA for the enzyme was increased upon administration of DEHP and that the enzyme is synthesized as a precursor (Mw = 69,000) larger than the mature enzyme (Mw = 67,500). RNA in the free polysomes directed the synthesis of the enzyme precursor five times more actively than RNA in membrane-bound polysomes.

Immune response to Toxoplasma gondii. I. Toxoplasma-specific proliferation response of peripheral blood lymphocytes from patients with toxoplasmosis.

Peripheral blood leukocytes (PBL) from patients with toxoplasmosis were shown to be highly responsive to in vitro stimulation with Toxoplasma gondii extract as measured by incorporation of [3H]methylated thymidine. Analysis of Toxoplasma-specific proliferative cells in PBL by using monoclonal antibodies specific for human T cell subsets revealed that the Toxoplasma-specific proliferation response of PBL from the patients was mediated by Leu 1, Leu 3a positive cells, that is, helper/inducer T cells. Tests for the Toxoplasma-specific proliferation response may provide a readily available method for the diagnosis of congenital toxoplasmosis, especially during the newborn period.

Residual glomerular lesions in postpartal women with toxemia of pregnancy.

Renal biopsies from 10 patients who had residual symptoms of toxemia of pregnancy were examined by light, electron and immunofluorescent microscopy. The biopsies were performed 1 month to 3 months postpartum. The glomeruli showed a similar lesion, even though to a lesser degree, to that found in toxemia of pregnancy. Subepithelial swellings and endothelial cytoplasmic microtubular structures were also observed. Mesangial and monocyte-like cells contained lysosomes, cholesterol clefts and lipid-containing vacuoles with or without myelin figures, suggesting reparative process in the glomeruli. The present study indicates that the glomerular injury persists for a few months after delivery, together with the concurrent reparative process.

Clinical application of the enzyme immunoassay for pancreatic oncofetal antigen.

Pancreatic oncofetal antigen (POA) purified by us has been detected in sera of patients with carcinoma of the pancreas by the micro-Ouchterlony method. In an attempt to improve the sensitivity and clinical usefulness of the serum POA assay, we established an enzyme immunoassay for POA and reported the results with this method. In this study, we investigated serum POA levels in pancreatic cancer and other diseases. The tissue localization of this POA in the pancreas was also studied. For the establishment of the enzyme immunoassay, an anti-POA-F(ab')2 fragment prepared from absorbed antiserum was conjugated with beta-D-galactosidase. The solid-phase "sandwich" principle was used. The normal upper limit of the serum POA level was defined as 400 units/ml. Among 60 patients with pancreatic cancer, 44 had elevated levels (73.3%). Of 22 cases with chronic pancreatitis, 4 had elevated levels (18.2%). In malignant diseases other than pancreatic cancer, elevated levels of serum POA were seen in 17.6% to 48.5% of the patients, most of whom had only slightly elevated levels. These results indicate that enzyme immunoassay for POA is clinically useful for making a diagnosis of pancreatic cancer. Immunoperoxidase staining showed POA to be found at the apical surface of ductular cells in fetal pancreas, at the luminal surface of glandular structures in pancreatic cancer tissue, and also at the luminal surface of the small duct in normal pancreas. Thus it is suggested that a high level of serum POA in patients with pancreatic cancer is derived from pancreatic cancer tissue.

Recent results and problems in the surgical treatment of acquired valvular heart disease.

From June, 1977 to August, 1981, 388 patients underwent open heart surgery for acquired valvular heart diseases. The over-all surgical mortality rate during this period was only 4%, but a high incidence of postoperative complications (26% for major complications) was still observed. It is an urgent concern for all cardiac-surgeons to reduce postoperative complications in order to obtain better surgical results. Even with improved techniques in modern cardiac surgery, 28 patients with a giant left atrium were still in the high risk group, with an over-all mortality rate of 29% and with a high incidence (68%) of postoperative complications. This seemed to be attributable mainly to deteriorated conditions such as cardiac cachexia, but not necessarily to impaired pulmonary function. Therefore, more careful considerations should be paid not only for their circulatory and pulmonary conditions, but also for the improvement of their general condition, including nutrition and metabolism. On the other hand, 27 patients with infective endocarditis showed a remarkable improvement after prosthetic valve replacement in spite of their seriously deteriorated preoperative condition. There were 4 hospital deaths (15% mortality). All transferred to our institution in a far advanced stage of the disease. Early surgical intervention for intractable cardiac failure should be strongly recommended for these patients.

Resection of thorotrast-induced cholangiocarcinoma.

We report a case of successful surgical treatment of cholangiocarcinoma caused by a long standing thorotrastosis. Thorotrastosis should be recognized as a preliminary stage of malignancy. If early diagnosis of the tumor can be made, treatment may be successful.

[Experimental and clinical studies of cefotiam for the treatment of otorhinolaryngological infections].

Basic and clinical studies on cefotiam (CTM) for the infectious diseases in otorhinolaryngological field were performed and the following results were obtained. The peak value in the serum level of CTM was 18.05 micrograms/ml by 0.5 g administration and 35.2 micrograms/ml by 1 g at 30 minutes after a single intravenous injection. CTM was rapidly excreted from the blood after injection and was detected a little amount in serum at 6 hours after the administration. Tissue concentrations of the drug into palatine tonsil and the mucous membrane of the nasal cavity or maxillary sinus were seen in relatively rapid and good manner. Chemotherapeutic results excepting Pseudomonas aeruginosa infections (3 cases) were excellent in 2 cases, good in 7 cases and poor in 3 cases of 12 patients with the infectious diseases in otorhinolaryngological field. No adverse effects and abnormal values in the laboratory findings were revealed in every patients.
