[Changes in immunological capacity of patients with cancer of the large intestine].

The changes of host immunity during the cancer treatment were studied in 27 patients with large bowel cancer. As the result, compared with preoperative status, increasing tendency of serum IgG, IgA levels, normalization of T cell per cent and remarkably lowered CH50 level were found postoperatively. However, in the patients treated with immunochemotherapy, no definite changes were found during the treatment. It was suggested that the CH50 level might have close relation to the curativity or the prognosis in these operative cases.

Assay methods for prekallikrein and kininogens and their applications.

Prekallikrein activity in plasma was assayed using a synthetic peptidyl fluorogenic substrate (carbobenzoxy-L-phenylalanyl-L-arginine 4-methylcoumarinyl-7-amide), after activation of prekallikrein by acetone and kaolin. For total kininogen assay, the pretreatment of plasma at pH 2.0 was the best to eliminate bradykinin potentiators and kininase activity, before addition of trypsin to convert kininogen to bradykinin. Assay method of high molecular weight (HMW) kininogen was established by conversion of HMW-kininogen to bradykinin through activation of Hageman factor by glass powder and that of low molecular weight (LMW) kininogen was also by treatment of HMW-kininogen-depleted plasma in the same way as that for total kininogen. The marked reduction of prekallikrein and HMW-kininogen, not of LMW-kininogen, was found in pleural fluid of rat carrageenin pleurisy, and in plasma after i.v. injection of bromelain in rats. Members of the pedigree of hereditary angioneurotic edema patients also show low levels of prekallikrein and kininogens in plasma.

Sandwich enzymoimmunoassay of hepatitis B surface antigen (HBsAg).

A sandwich enzymoimmunoassay (EIA) procedure was developed for the detection of HBsAg using Fab' of anti-HBsAg conjugated with beta-D-galactosidase from Escherichia coli with anti-HBsAg-coated silicone rubber discs as a solid phase. EIA could detect 1 ng/ml of HBsAg. It was as sensitive as radioimmunoassay (RIA, AusRia II) and about 30-fold more sensitive than reversed passive hemagglutination assay RPHA, ReverseCell). EIA and RIA could detect more HBsAg-positive sera than RPHA.

The potential risk for upper extremity thromboembolism in patients with occluded axillofemoral bypass grafts: two case reports.

Axillofemoral bypass grafts (AxFG) are widely used in the management of poor-risk patients with aortoiliac occlusive disease. On the other hand, AxFGs are associated with a variety complications to the upper extremity (UE). UE thromboembolism represents a significant and specific complication of occluded AxFGs in our series (2.6% of patients, 33.3% of occluded grafts). This article describes two cases of late axillary artery thrombosis caused by the occlusion of externally-supported AxFGs. The two patients were treated by graft disconnection, a distal embolectomy, and patch angioplasty of the axillary artery. Their postoperative courses were uneventful. Based on the authors' experience and a review of the literature, they suggest that an occluded AxFG represents a high risk for use of a donor artery and that such patients must therefore be very carefully followed. To prevent late UE thromboembolism in patients with occluded grafts, the authors strongly advise that such patients undergo a surgical operation with careful follow-up after surgery.

Ileus with incarceration of Spigelian hernia.

An 85 year-old female with ileus due to incarceration of Spigelian hernia, which was diagnosed pre-operatively with the findings of the abdominal computed tomography and plain radiographic pictures, is reported. A simple hernioplasty was made by suturing the internal oblique and transverse muscles to the rectus sheath. The post-operative condition is satisfactory without any recurrence of hernia.

Volvulus of the ileum in adult diagnosed pre-operatively by helical 3-dimensioned computed-tomography.

A 46 year-old male who was strictly diagnosed as having volvulus of the ileum based on the pre-operative information brought by computed tomography (CT) and helical (spiral) 3-dimensioned computed tomography was surgically treated. The post-operative course was satisfactory and the patient is now under observation without any exacerbation of symptoms one year after surgery.

Partial resection for leiomyoblastoma of stomach.

We herein report the case of a 51-year-old male with a submucosal tumor of the stomach which was histopathologically demonstrated to have features compatible with leiomyoblastoma. As the submucosal tumor of the posterior wall of the antrum, which was initially found in the upper gastrointestinal series done during a health examination, had grown 3.8 cm in diameter, the partial resection of the full thickness of the gastric wall containing the tumor with a sufficient tumor margin was performed.
