Electrostatic environment of the tryptophylquinone cofactor in methylamine dehydrogenase: evidence from resonance Raman spectroscopy of model compounds.

Methylamine dehydrogenase (MADH) utilizes its endogenous tryptophan tryptophylquinone (TTQ) as a cofactor in enzymatic catalysis, with the C6 carbonyl of the quinone implicated as the site of attack by substrates and other nucleophiles. Resonance Raman (RR) spectroscopy provides an ideal method for investigating the state of this carbonyl group whose C==O stretch is distinct from other vibrational modes of the cofactor and is readily identified by its shift to lower energy in H218O. In a series of indole 6,7-quinone models for TTQ, the in-phase stretching vibration of the two C==O groups occurs at 1650 cm-1 in nonpolar solvents and shifts to 1638 cm-1 in H2O. The absorption maximum undergoes an analogous shift from 400 to 425 mm. The spectral properties of the indole quinones in H2O approach the corresponding values in Thiobacillus versutus MADH (C==O stretch at 1612 cm-1, lamdamax at 440mm) and are indicative of strongly hydrogen bonding of the C==O and NH groups of the cofactor in the native enzyme. Addition of monovalent cations [NH4+,Cs+, and (CH3)3NH+] to MADH causes further increases in the lamdamax and decreases in the frequency of the C==O stretch[1590 cm-1 with (CH3)3NH+]. This implies a strong electrostatic interaction between monovalent cations and a carbonyl oxygen (most likely at C6) in TTQ. The fact that these cations behave as competitive inhibitors of the methylamine substrate suggests that methylamine binds to the same location in the enzyme prior to its covalent reaction with the cofactor. Addition of monovalent cations to the one-electron-reduced semiquinone form MADH results in RR spectral shifts for a number of vibrational modes of the cofactor. Thus, the ability of monovalent cations to promote and stabilize the formation of the semiquinone intermediate is also due to their direct electrostatic interaction with the TTQ cofactor.

Serious chemical sclerosing cholangitis associated with hepatic arterial 5FU and MMC chemotherapy.

A case of iatrogenic sclerosing cholangitis secondary to hepatic intra-arterial 5-fluorouracil (5FU) and Mitomycin C (MMC) chemotherapy is described. When any unexplained elevation of liver function results in alkaline phosphatase and bilirubin level, chemotherapy should be discontinued, and further examination carried out using ultrasonography, transhepatic cholangiography and endoscopic retrograde cholangio-pancreatography (ERCP). Although percutaneous transhepatic biliary drainage has been effective in some cases, in our case, the clinical course was irreversible and the patient died of hepatic failure and gastrointestinal bleeding. When clinical signs of hepatic dysfunction occur in the absence of tumor progression, iatrogenic sclerosing cholangitis must be suspected.

[Changing trends in the surgical management of breast carcinoma in Japan].

Modified mastectomy, which involved removal of the breast, overlying skin and axillary contents but spares the pectoralis muscle, has recently been established as an entirely satisfactory approach for Stage I and Stage II breast carcinoma. In a sequential series of patients, we noted that the traditional mastectomy followed by radiation to axilla and adjacent area failed to confirm the efficacy to support the prevention for local relapse in the past decade. The concepts of adjuvant chemotherapy evolved primarily from clinical trials: CMF and CAMF have been shown to influence favorably the therapy of premenopausal women with positive. nodes. Additional endocrine adjuvant treatment in postmenopausal women with positive nodes was significantly superior to that with CAMF. Immediate breast reconstruction following modified mastectomy or subcutaneous mastectomy was safe, simple, and possible to produce the patient's satisfaction. There was a positive effect on life style and on sexual, social or work relationships. Finally, in recent years, several studies have reported that more conservative treatment, such as partial mastectomy and nodes dissection with postsurgical radiotherapy are as effective as modified mastectomy. Present research will likely continue to modified current treatment as more data are gathered and analyzed clinically and histologically.

Augmentation of circulation of pedicled transverse rectus abdominis musculocutaneous flaps by microvascular surgery.

The biggest problem of the TRAM flap for breast reconstruction is distal necrosis or fat lysis due to poor circulation. In order to utilise the entire TRAM flap tissue in extensive tissue defects, the contralateral rectus muscle is used as a pedicled carrier and the ipsilateral superficial or inferior epigastric vessels are anastomosed with appropriate recipient vessels in the axilla. This procedure has been performed in three cases with no necrosis, fat lysis or hardening of the flap tissue, proving adequate circulation in the flap.
