Hypochlorous acid, which is one of the reactive oxgen species (ROS) produced from human neutrophils, is converted to cytotoxic NH(2)Cl after reaction with ammonia produced by urease in Helicobacter pylori (HP), increasing gastric mucosal injury and with potential development to gastric cancer. We compared the effects of HP on the production of ROS by human neutrophils between two groups-22 patients with gastric cancer and 16 patients without gastric cancer (control group), in whom HP was isolated from stomach biopsy tissues-using a luminol- and lucigenin-dependent chemiluminescence (LmCL and LgCL, respectively). It was very similar in the mean value or variance of mean maximal chemiluminescence intensities (MCI) and peak time in LmCL and LgCL between the two groups. MCI of LmCL was highly correlated with that of LgCL in both groups. These results indicate that there were no differences in the behaviour of HP on human neutrophil chemiluminescence between two groups. The progression from non-malignant mucosa to cancer may be associated with the time-dependent effects of HP via ROS produced by neutrophils on the gastric mucosa.
BACKGROUND: The technique of free jejunal transfers has been widely used for pharyngoesophageal reconstruction. However, secondary infectious and fistulous complications may develop in extensive and preoperatively high-dose irradiated cases. METHODS: A generous jejunomesenteric composite graft was harvested and the mesenterium was effectively applied for head and neck reconstruction. RESULTS: The mesenteric flap connected with the revascularized jejunum was used as means for obliterating dead space in the head and neck region, as a cover for the cover for the cervical vessels, as a vascularized bolstering for the vascular and enteral anastomoses, or as a vascularized bed for skin grafting when the skin flaps are deficient. Only one small fistula occurred in 10 patients who underwent the mesenteric flap. CONCLUSIONS: This versatile technique is expected to reduce the incidence of postoperative complications in pharyngoesophageal reconstruction using free jejunal transfers.

The entry route of viruses into the inner ear is still controversial. Guinea pig cytomegalovirus (CMV) has been used to create an animal model of systemic infection as well as labyrinthitis. In this study, seronegative guinea pigs were administered intracardiac inoculations of varying doses of guinea pig CMV. After three or eight days, the animals were killed and necropsy specimens examined for viral antigen. In the specimens examined three days after inoculation, no label was seen in the cochlea. However, label was found in the spleen with doses of 100 microL, and also in the liver in animals inoculated with 300 microL of virus. After eight days, an animal that received 200 microL of the virus showed labeling in the modiolar blood vessels and perivascular infiltrates. Two of the four animals that received 300 microL of the virus showed labeling in spiral ganglion cells. None of the animals showed viral antigen within the stria vascularis, nor were there signs of acute labyrinthitis. At eight days, animals inoculated with greater than 100 microL of virus showed labeled cells in multiple organs. These data suggest that the entry route of guinea pig cytomegalovirus into the inner ear might involve viremic spread to modiolar blood vessels and subsequent spread to spiral ganglion cells.

In 1995, the Hanshin-Awaji area in Japan was damaged severely by a major earthquake. In this study, the authors administered questionnaires and obtained blood samples to analyze the relationships among lifestyle, psychological stress, and plasma cortisol levels of victims. The authors questioned 107 male inhabitants of Awaji Island about their lifestyles before and after the earthquake, presence of any symptoms of posttraumatic stress disorder, and demographic variables. Plasma cortisol levels were determined with enzyme immunoassay. Cortisol level was correlated strongly with change in lifestyle. The highest cortisol levels were found in the group characterized by a high posttraumatic stress disorder score and by a very profound lifestyle change. This group also contained the highest percentage of subjects who had poor health. In summary, the psychological stress induced by the Hanshin-Awaji earthquake was associated with mean cortisol level; however, this relationship was affected by adjustment of lifestyle.
