Endothelial modulation of norepinephrine-induced constriction of rat aorta at normal and high CO2 tensions.

Endothelial modulation of norepinephrine (NE)-induced constriction of the isolated rat aorta was studied at normal (PCO2, 41 +/- 0 mmHg) and high CO2 tensions (PCO2, 91 +/- 1 mmHg). In preparations with intact endothelium, increased CO2 tension resulted in rightward shift of the NE dose-response curve with attenuation of maximal contraction. This effect of CO2 was not modified by indomethacin. Treatment with hemoglobin or rubbing of the endothelium meant that increased CO2 tension still resulted in rightward shift of the NE dose-response curve but without altering the maximal contractile response. The basal guanosine 3',5'-cyclic monophosphate (cGMP) levels in control and NE-treated aortic preparations were not affected by increasing the CO2 tension. Thus the inhibitory action of CO2 on NE-induced contraction in the presence of endothelium may not be derived from facilitation of endothelium-derived relaxation factor (EDRF)-induced cGMP synthesis. Increasing the CO2 tension attenuated the sustained contraction induced by the addition of NE and Ca2+ (2.5 mM) to intact endothelium preparations previously bathed in Ca2(+)-free solution. Further addition of Ca2+ (total 5.0 mM) did not increase the contraction. These findings suggest that the intrinsic activity of NE is greatly modified by endothelium at a high CO2 tension. Vasodilation during hypercapnia may be induced at least in part by synergistic actions of EDRF and CO2 on smooth muscle cells.

Correlation between levels of plasma adenosine triphosphate and stress to the fetus at delivery.

Adenosine triphosphate (ATP) and its metabolites are released from intracellular stores and levels of ATP in the plasma are affected by hypoxia and by exercise. We examined levels of plasma ATP in the fetus at the time of obstetrical delivery. The arterial (ACB), venous cord (VCB) and maternal venous blood (MVB) samples were collected immediately after clamping of the cord. ATP was determined by the bioluminescence method. The levels of plasma ATP were significantly higher in ACB than in VCB or in MVB. There were significant correlations between the levels of ATP in ACB and in VCB, between the levels in ACB and in MVB and between the levels in MVB and in VCB. In case of delivery by elective cesarean section, the levels of ATP in ACB were significantly higher than those in ACB of the fetus delivered vaginally. We found a significant correlation between the levels of plasma ATP in ACB and PO2 in VCB, though there was no significant correlation between the levels of ATP and pH and PCO2. These observations suggest that ATP in ACB may be of fetal origin, that the levels decrease concomitantly in response to stress during vaginal delivery and correlate with the oxygen supplied from the placenta.

Active drainage of cardiac lymph in relation to reduction in size of myocardial infarction: an experimental study.

Active drainage of cardiac lymph using hyaluronidase was attempted in dogs. The results were satisfactory and the ischemic myocardium was salvaged. The infarct risk area (I/R) ratio decreased after drainage. Regional myocardial ischemia and infarction were provided by means of ligature of the left coronary artery for 120 and 240 minutes respectively. Cardiac lymph was collected by conventional procedures. Enzymes released from the myocardium increased significantly in the cardiac lymph. The volume of cardiac lymph gradually increased after ligature of the coronary artery. Administration of hyaluronidase further increased the cardiac lymph flow and significantly decreased the I/R ratio as determined by triphenyl tetrazolium chloride (TTC) and methylene blue staining. Drainage of the cardiac lymph salvaged the ischemic myocardium. Reduction of interstitial edema and augmentation of cardiac lymph flow with the hyaluronidase prevented the development of the infarction. This is the first documentation of the effect of active drainage of cardiac lymph on the development of infarction through observation of the I/R ratio.

Circadian rhythm of serum testosterone levels in male beagle dogs--effects of lighting time zone.

In a previous study, the author found that serum testosterone (T) levels in male beagle dogs showed a circadian rhythm which was lowest at 12:00, and increased to a peak at 18:00-6:00, thereafter decreasing until 12:00. The reason was thought to be that dogs were breeding under rigid controlled conditions. The present study was performed to investigate the effects of lighting on the circadian rhythm of T level by means of a reversal of the lighting time zone, because lighting is considered an important factor in modulating T levels. Six male beagle dogs of 2 years were used in this study. The routine lighting time (8:00-20:00) and dark time (20:00-8:00) zone in the breeding room was reversed completely and T levels were measured at intervals of 1-5 weeks for up to 54 weeks. Blood samples were collected at 6:00, 12:00, 18:00 and 24:00. The results showed that the circadian rhythm of the T level and the T level at each blood sampling time did not change significantly within 54 weeks. As a result, it was recognized that the circadian rhythm of the T level in male dogs may not be affected by changes in the lighting time zone.

[Effect of maternally derived antibody levels on antibody responses to canine parvovirus, canine distemper virus and infectious canine hepatitis virus after vaccinations in beagle puppies].

Antibody titers against canine parvovirus (CPV), canine distemper virus (CDV) and infectious canine hepatitis virus (ICHV) in serum were measured in 6 beagle dams and their 38 puppies bred in our colony, in order to clarify the effects of maternally derived antibodies to antibody responses against the viruses after vaccinations in puppies. Correlation coefficient on antibody titers between puppies and dams were CPV: r = 0. 7935, CDV: r = 0.8194 and ICHV: r = 0.8105. Mean maternal antibody positive rates in 7-day-old puppies from their dams were CPV: 67%, CDV: 46% and ICHV: 45%. Mean half-lives of the maternal antibodies in puppies were estimated to be CPV: 13.5 days, CDV: 15.1 days and ICHV: 15.4 days. The antibody response against CPV vaccination in puppies was mainly observed in dogs being titers of less 1:5 and positivity was 39% (15/38 puppies) after 1st vaccination at 42 days after birth, and 82% (31/38 puppies) after 2nd vaccination at 70 days. That against CDV vaccination (at 56 days after birth) was seen highly in dogs being titers of less 1:10 and positivity was 53% (20/38). Also that against ICHV vaccination (at 56 days after birth) was seen frequently in dogs being titers of less 20 holds and the rate was 87% (33/38). From these results, it was estimated that the age when high antibody response against each vaccination could be expected in puppies might be CPV: between 40 and 69 days, CDV: between 32 and 92 days and ICHV: between 31 and 52 days, respectively.

Pulmonary elastic fibers in normal human development and in pathological conditions.

Normal human pulmonary elastic fiber development and development in some pathological conditions were examined using elastic stains by light microscopy, electron microscopy, and immunohistochemistry. In normal development elastic fibers, composed mainly of microfibrils, first appeared around primitive bronchioles at 10 weeks of gestation. As they matured, their appearance became more amorphous, and they extended into the peripheral alveolar walls. Development of elastic fibers was retarded in the hypoplastic lungs of the oligohydramnios syndrome, diaphragmatic hernia, and hydrops fetalis. Elastic development was also retarded in congenital pulmonary lymphangiectasia and in focal areas of lungs with pulmonary dysplasia. Distribution of well-developed elastic fibers was found around the dilated bronchioles and alveoli in cases of congenital cystic adenomatoid malformation and extralobar pulmonary sequestration. Elastic fibers were distributed irregularly and unevenly in the lungs of bronchopulmonary dysplasia and ventilated cases of Wilson Mikity syndrome. In addition, four very immature infants who had progressively deteriorating respiratory function showed an almost total lack of elastic fibers in their alveolar walls.

[Clinical analysis of shunted hydrocephalic neonates and sucklings--observation on obstruction and infection of shunting system].

Although ventriculoperitoneal (V-P) shunt is common as the surgical procedure for hydrocephalus, mechanical malfunction and infection are the most significant problems. Despite the current use of variety of mechanical shunting systems, shunt malfunction remains a major problem in pediatric neurosurgical practice. Pathological lesions associated with malfunctioning shunts have been studied only infrequently. From April 1980 to December 1987, we experienced 96 cases of V-P shunt in neonates and suckling. The therapeutic results of shunting and causes of initial shunt revision in those cases with nontumoral hydrocephalus were studied. Of 96 cases, which included 55 cases of congenital and 41 cases of acquired hydrocephalus, in which a V-P shunt had been performed, those cases with no history of shunt revision or with only elective shunt revision during the 5-year follow-up period were classified as successful shunt. The 56 of 96 cases (58.3% successful shunts included 15 of 24 patients with myelomeningocele, 7 of 10 with encephalocele, 12 of 21 with various types of cerebral anomaly, 12 of 22 with posthemorrhagic hydrocephalus and 10 with postmeningitic hydrocephalus. There were 12 successful shunts performed in 27 premature and 44 in 69 mature infants. The 40 patients (41.7% with non-elective shunt revision included 5 with breakdown of the sutured scalp with cerebrospinal fluid leakage, 20 with obstruction of the shunt system (14 ventricular side, 6 abdominal side, and 15 with infection. During the 5-year follow-up period 11 died and 85 survived 88.5%), of whom 43 (44.8%) showed normal intellectual development and 42 (43.7%) showed developmental disability.

[Clinical analysis of shunted hydrocephalic neonates and sucklings. Observation of postshunt complication due to overdrainage from intraventricular CSF].

During the past 3 decades hydrocephalus has been treated effectively with valve-regulated shunts and it would appear at present that this means of treatment will continue despite the fact that it probably has a higher complications rate than any other neurosurgical operation. The complications are generally categolized as those due to obstruction, those caused by infection, and those related to overdrainage. It appears, however, that the complications due to overdrainage such as the slit ventricle syndrome, the isolated unilateral lateral ventricle, and the isolated fourth ventricle and are poorly defined and understood and, therefore, the treatment has been subject of confusing literature. In this paper, the characteristics of neonatal and infantile hydrocephalus were discussed in an analysis of postoperative complications of cerebro-spinal fluid shunt procedures which focuses on the overdrainage by the shunt system. Ninety six cases of hydrocephalus which included 55 congenital and 41 acquired, were treated during a period of 7 years and 9 months. Patients with congenital hydrocephalus included 24 with myelomeningocele, 10 with encephalocele, and 21 with aqueductal stenosis associated with various cerebral anomalies. Patients with acquired hydrocephalus included 22 with posthemorrhagic and 19 with postmeningitic. There were 29 cases (30.2%) in which various complications due to overdrainage developed. In 12 of these additional conservative treatment and/or additional surgical therapies were necessary. Patients with complications included 19 (19.8%) with the slit ventricle, 6 with the unilateral isolated lateral ventricle, and 4 with the isolated fourth ventricle.(ABSTRACT TRUNCATED AT 250 WORDS)

[Anesthesia for pectus excavatum].

The incidence of arrhythmia, postoperative complication and pulmonary oxygenation (PaO2) were studied in 48 patients with pectus excavatum scheduled for the Ravitch operation under halothane-nitrous oxide-oxygen (GOF) and enflurane-nitrous oxide-oxygen (GOE) anesthesia. Preoperative abnormalities of ECG were observed in 36 of 18 cases. Main abnormalities were incomplete right bundle branch block, left atrium enlargement, and sinus arrhythmia. Ventricular arrhythmia was observed in 4 of 12 cases during GOF anesthesia, whereas no arrhythmia was observed during GOE anesthesia. In postoperative chest X-ray, pulmonary atelectasis (60%), pleural effusion (48%), and pneumothorax (8%) were observed. The results suggest that GOE is more advantageous for pectus excavatum operation than GOF. Postoperative pulmonary surveillance is important for pectus excavatum operation.

[Expression of oncogenes by smooth muscle cells in atherosclerotic lesions].

Smooth muscle cells in the thickened intima of the aorta also expressed sis, mye, and c-fos, in addition to collagen types I, III, IV and type V. In the cultures of smooth muscle cells isolated from human aorta, collagen I, III and gelatinase were produced without any other special conditions for the culture. In the presence of PDGF, collagen type V and collagenase production was detected in the culture. Oncogene expression was not detected in the cultured cells when tested by the indirect immunofluorescence technique. Immortalized smooth muscle cells with SV 40 expressed myc, SV 40 large T antigen, and p53 without inhibition of collagen and collagenolytic enzymes production.

[Two cases of rectal carcinoma that developed as a late complication of pelvic radiotherapy].

Two cases of a pelvic evisceration have been performed, due to an irradiation-induced rectal cancer. Described are the cases of two women who had been treated with irradiation for a cervical cancer following a hysterectomy, one patient being 65 years old and the other 67. After a latent period that lasted for 12 and 24 years, respectively, each had developed a rectal cancer. In each case, case excised specimen showed diffuse fibrosis and a hyaline change, reflecting the effect of radiation on the tumoral tissue. Cases of an irradiation-induced rectal cancer are uncommon, and the symptoms of enterocolitis caused by irradiation are similar to those of a colorectal cancer. The authors therefore suggest careful and long-term follow-up of patients that have received pelvic radiation.

Effect of linoleic acid hydroperoxide on adenovirus DNA replication.

The effect of linoleic acid hydroperoxide on replication of adenovirus type 2 DNA in HeLa cells was examined. The level of the DNA was extremely reduced when the cells immediately after the infection with adenovirus type 2 were incubated for 24 h in the presence of 1 nmol/ml (measured by the thiobarbituric acid reaction and expressed in terms of malondialdehyde) of the hydroperoxide. The levels of the messenger RNA for adenovirus type 2 DNA polymerase and the expressed polymerase were equally reduced. The inhibition of DNA replication by lipid hydroperoxides in animal cells was discussed in relation to degenerative disorders.

[Molecular basis of mucopolysaccaridosis type VII (beta-glucuronidase deficiency)].

We have identified several mutations causing beta-glucuronidase (beta Gl) deficiency in three cases with mucopolysaccaridosis type VII (MPS VII). Enzyme assay of lysates of these lymphocytes or cultured fibroblasts showed little residual activity and that the beta Gl-specific mRNA levels were normal, as revealed by Northern blot analysis. Mutated cDNA clones including the entire coding sequencing were isolated from a library in case 1 and PCR (polymerase chain reaction) products in case 2 and 3 derived from cultured fibroblasts. Sequencing of the full-length mutated cDNA revealed C----T transitions, an event causing a single Ala619----Val change (cases 1 and 2) and Arg382----Cys and Pro649----Leu changes (case 3). The former change is detected by loss of the cleavage site for the enzyme Fnu 4 HI in the mutated cDNA. On the basis of the loss of Fnu 4 HI restriction site, the patients (cases 1 and 2) were shown to be a homozygote with this mutation and the parents and brother in case 1 were heterozygotes. The Ala619----Val and Arg382----Cys mutations disrupt a functional domain consisting of a region of sequence highly conserved among human, rat and bacterial beta Gl's, and they lower the enzyme activity, as tested by transfection of COS cells with expression vectors harboring the mutated cDNA. However the Pro649----Leu mutation does not lower the enzyme activity.

[Long-term results of coronary artery bypass grafting: comparison of angiographic evaluation of internal thoracic artery and saphenous vein grafts].

The long term (10 to 15 years) results of coronary artery bypass grafting (CABG) were studied in 20 patients. The duration of follow-up was ranged from 130 to 170 months with mean 146.4 months. Ten out of 20 patients underwent coronary angiography (CAG), which disclosed that the late patency of saphenous vein (SV) grafts was 68.8% (11/16), but 54.5% (6/11) of patent SV grafts showed atherosclerotic changes such as irregularity and localized narrowing. On the other hand, internal thoracic artery (ITA) grafts were all patent without any atherosclerotic luminal changes. We recognized that ITA grafts were superior to SV grafts from an angiographic standpoint of view in the long term in Japan.

[Left ventricular rupture following the procedure of percutaneous transluminal coronary angioplasty: a case report].

A 50-year-old man with 90% concentric stenosis at the proximal portion of the left anterior descending artery was tried to be treated with percutaneous transluminal coronary angioplasty (PTCA). The guide wire did not pass through the stenotic portion, and the staining out of coronary artery was recognized. Four hours after the procedure, he fell to shock status with a sign of electro-mechanical dissociation. An emergent operation confirmed the left ventricular rupture. Repair of the tear and saphenous vein bypass to the left anterior descending artery were performed rapidly, however, he was not weaned from cardiopulmonary bypass. Close communication between cardiologist and surgeon, and immediate surgical back up are essential for saving these patients from severe ischemic events following PTCA failure.

A statistical study of autopsied cases of neonatal intraventricular hemorrhage.

We have performed a statistical study of 1218 autopsies of neonates to extract the factors related to fatal neonatal intraventricular hemorrhage (IVH). Prematurity indicated by a short gestational period or a low birth weight was the most significant factor for IVH. Many other factors considered to be related to IVH were also related closely to prematurity. Therefore, we pose the following question throughout the study: Did the factors independently relate to IVH without any confounding effects of prematurity? To obtain the answer, we tried a statistical adjustment for gestational age to eliminate the confounding effects of prematurity. After the adjustment was made, we concluded that the following 10 items, in order, were the actual factors related to fatal IVH: prematurity, subependymal hemorrhage, subarachnoidal hemorrhage, respiratory distress syndrome, hyaline membrane disease, respiratory acidosis, intracerebral hemorrhage, cyanosis, respirator care, and low Apgar score. There were two distinctive categories in the factors that were eliminated by the adjustment. One was a group that has no significance whatsoever in any subdivided gestational groups, such as meconium aspiration syndrome, due to purely the result of the confounding effect. The other was a group that showed its statistical significance only in a certain period of gestation, such as breech presentation.
