A surgical technique for a vertebral column autograft using the intervertebral disc for cervical disc disease.

We describe a surgical technique for a vertebral column autograft using the intervertebral disc for cervical disc disease for patients whose major problem is not spinal instability. Of a total of 41 patients with cervical disc disease suffering from cervical spondylotic radiculomyelopathy, 33 patients were operated on at one level and 8 patients were operated on at two levels. Postoperative X-ray film showed some movement at the "operated" disc level in all patients (average postoperative follow-up period was 43 months, range two years to 5 years). A significant decrease in motion in the extension position was observed postoperatively (p < 0.0001), but no significant difference was observed between the preoperative motion and the postoperative motion in the flexion position. Anterior angulation was found in two (5%) of the 41 patients. This surgical procedure has two major advantages: 1) no complications related to the iliac donor site, allowing early patient mobilization; 2) the extensive posterior spur can be removed safely and easily under a wide operative field. We believe that this surgical procedure is suitable for preserving the mobility of the spine and may avoid stress concentration at adjacent levels of the "operated" disc. However, in patients whose major problem is spinal instability, anterior cervical fusion should be performed.

Sonochemical polymerization of benzene derivatives: the site of the reaction.

Sonochemical polymerization of benzene and halogen-substituted benzenes has been studied. The difference of absorption spectra of polymerization products can be explained qualitatively using bond energies of the primary products. The relative rate constant of the polymerization reaction is apparently proportional to the inverse of the vapour pressure of the liquids. Using this relation, we analysed the relative rate constant of the polymerization in benzene/chrolobenzene mixtures. From this, we conclude that sonochemical polymerization proceeds in the vapour phase of a bubble.

Archaeon Pyrococcus kodakaraensis KOD1: application and evolution.

Archaea is the third domain which is phylogenetically differentiated from the other two domains, bacteria and eucarya. Hyperthermophile within the archaea domain has evolved most slowly retaining many ancestral features of higher eukaryotes. Pyrococcus kodakaraensis KOD1, which grows at 95 degrees C optimally, is a newly isolated hyperthermophilc archaeon. The KOD1 strain possesses a circular genome, whose size is estimated to be approximately 2,036 kb. KOD1 enzymes involved in the genetic information processing system, such as DNA polymerase, Rec protein, aspartyl tRNA synthetase and molecular chaperonin, share features of eukaryotic enzymes. Rapid and accurate PCR method by KOD1 DNA polymerase and enzyme stabilization system by KOD1 chaperonin are also introduced in this article.

High-efficiency transformation of Chlamydomonas reinhardtii by electroporation.

We have established a high-efficiency method for transforming the unicellular, green alga Chlamydomonas reinhardtii by electroporation. Electroporation of strains CC3395 and CC425, cell wall-less mutants devoid of argininosuccinate lyase (encoded by ARG7), in the presence of the plasmid pJD67 (which contains ARG7) was used to optimize conditions for the introduction of exogenous DNA. The conditions that were varied included osmolarity, temperature, concentration of exogenous DNA, voltage and capacitance. Following optimization, the maximum transformation frequency obtained was 2 x 10(5) transformants per microg of DNA; this frequency is two orders of magnitude higher than obtained with the current standard method using glass beads to introduce exogenous DNA. The electroporation procedure described in this article is of general utility, and makes it feasible to isolate genes by direct complementation of Chlamydomonas reinhardtii mutants.

Serine protease inhibitors expressed in the process of budding of tunicates as revealed by EST analysis.

To identify genes expressed during budding of the tunicate Polyandrocarpa misakiensis, we isolated and sequenced 624 clones from a directionally constructed cDNA library to prepare a catalog of expressed sequence tags (ESTs). A total of 233 ESTs matched genes of known sequence in the SwissProt database. About 24% out of them showed high similarity to ribosomal proteins, twice the value (12%) of pre-budding animals. ESTs involved in the respiratory chain also appeared with significant redundancy, suggesting that tunicate budding is accompanied by the enhancement of energy conversion as well as protein synthesis. Serine protease inhibitor (serpin) afforded another striking example of a gene that was highly expressed in the process of budding. The deduced amino acid sequences of five serpin cDNAs all had two consensus signatures of the Kazal's type of secretory protease inhibitor, one of which had an active site for trypsin and the other for elastase. In line with this, recombinant GST-fusion protein showed both trypsin and elastase inhibitor activities. In accordance with the EST analysis, the hemolymph taken from the budding stage showed the highest activity of trypsin inhibitor. We discuss a possible role that Polyandrocarpa serpins may play in bud development by counteracting trypsin-like serine protease, which could facilitate dedifferentiation of formative tissues.

Distribution of fallover in the carboxylase reaction and fallover-inducible sites among ribulose 1,5-bisphosphate carboxylase/oxygenases of photosynthetic organisms.

The biphasic reaction course, fallover, of carboxylation catalysed by ribulose 1,5-bisphosphate carboxylase/oxygenase (RuBisCO) has been known as a characteristic of the enzyme from higher land plants. Fallover consists of hysteresis in the reaction seen during the initial several minutes and a very slow suicide inhibition by inhibitors formed from the substrate ribulose-1,5-bisphosphate (RuBP). This study examined the relationship between occurrence of fallover and non-catalytic RuBP-binding sites, and the putative hysteresis-inducible sites (Lys-21 and Lys-305 of the large subunit in spinach RuBisCO) amongst RuBisCOs of a wide variety of photosynthetic organisms. Fallover could be detected by following the course of the carboxylase reaction at 1 mM RuBP and the non-catalytic binding sites by alleviation of fallover at 5 mM RuBP. RuBisCO from Euglena gracilis showed the same linear reaction course at both RuBP concentrations, indicating an association between an absence of fallover and an absence of the non-catalytic binding sites. This was supported by the results of an equilibrium binding assay for this enzyme with a transition state analogue. Green macroalgae and non-green algae contained the plant-type, fallover enzyme. RuBisCOs from Conjugatae, Closterium ehrenbergii, Gonatozygon monotaenium and Netrium digitus, showed a much smaller decrease in activity at 1 mM RuBP than the spinach enzyme and the reaction courses of these enzymes at 5 mM RuBP were almost linear. RuBisCO of a primitive type Conjugatae, Mesotaenium caldariorum, showed the same linear course at both RuBP concentrations. Sequencing of rbcL of these organisms indicated that Lys-305 was changed into arginine with Lys-21 conserved.

Cardiac sympathetic nervous disintegrity is related to exercise intolerance in patients with chronic heart failure.

The aim of this study was to clarify the relationship between cardiac sympathetic nervous activity assessed by 123I-MIBG imaging and exercise capacity in patients with congestive heart failure. The subjects were 24 patients with heart failure (NYHA class II to III) and 7 normal controls. A dose of 148 MBq of 123I-MIBG was administered intravenously, and 5-min anterior planar images were obtained 20 min (early) and 4 h (delayed) after the injection of 123I-MIBG. Regions of interest were placed over the left ventricle and mediastinum, and the heart-to-mediastinum ratio (H/M ratio) was calculated as a fraction of the mean counts per pixel in the heart divided by those in the mediastinum. The washout rate from the myocardium was determined by: (early counts delayed counts) x 100/early counts (%). Treadmill exercise tests were performed using a ramp method within 1 week of the 123I-MIBG studies. During exercise, expired gas was analysed, and peak VO2 and VO2 at the anaerobic threshold (ATVO2) were measured. A significant linear correlation was found between the H/M ratio on delayed 123I-MIBG images and exercise duration (r = 0.48, P < 0.05), peak VO2 (r = 0.49, P < 0.05) and ATVO2 (r = 0.56, P < 0.01). The washout rate of 123I-MIBG was inversely correlated with exercise duration (r = -0.45, P < 0.05), peak VO2 (r = -0.51, P < 0.01), and ATVO2 (r = -0.50, P < 0.05). In conclusion, enhanced cardiac sympathetic nervous activity relates to exercise intolerance in patients with congestive heart failure. The non-invasive assessment of cardiac sympathetic nervous activity by 123I-MIBG can predict exercise capacity in human heart failure.

Angiotensin-Converting Enzyme Inhibition Delays Onset of Glucosuria With Regression of Renal Injuries in Genetic Rat Model of Non-Insulin-Dependent Diabetes Mellitus.

BACKGROUND: The Otsuka Long-Evans Tokushima Fatty (OLETF) rat is a new genetic model of non-insulin-dependent diabetes mellitus (NIDDM). We investigated whether angiotensin inhibition influences the onset of NIDDM and brings about a regression of renal injury in diabetes mellitus. METHODS AND RESULTS: Six-week-old OLETF rats were treated with the angiotensin-converting enzyme (ACE) inhibitors imidapril or enalapril for 16 weeks. Systolic blood pressure is increased in an age-dependent manner in OLETF rats. In this study, the elevation in systolic blood pressure was dose-dependently reduced by ACE inhibitor treatment. In OLETF rats, plasma concentrations of insulin and glucose increased and the glucosuria occurred at the age of 22 weeks. Simultaneously, OLETF rats exhibited proteinuria and nodular lesions in glomeruli. The ACE inhibitor treatment almost completely reduced glucosuria, and also decreased plasma concentrations of insulin and glucose in OLETF rats. ACE inhibitor treatment lessened the proteinuria and attenuated morphologically the severity of nodular lesions in OLETF rats. Moreover, increases in plasminogen activator inhibitor 1 (PAI-1) in OLETF rats were reduced by the ACE inhibitor treatment, and the improvement of glomerular lesions was related to decreases of PAI-1 and angiotensin II levels in plasma but not to improvement of glucose metabolism. CONCLUSIONS: ACE inhibitors delay onset of NIDDM with attenuation of kidney injury. The regression of kidney lesions is probably due to angiotensin reductions but not to glucose metabolism per se. ACE inhibitor drug therapy may be useful in preventing NIDDM and the subsequent renal injury in patients with NIDDM.

The snail repressor establishes a muscle/notochord boundary in the Ciona embryo.

Previous studies have identified a minimal 434 bp enhancer from the promoter region of the Ciona Brachyury gene (Ci-Bra), which is sufficient to direct a notochord-specific pattern of gene expression. Here we present evidence that a Ciona homolog of snail (Ci-sna) encodes a repressor of the Ci-Bra enhancer in the tail muscles. DNA-binding assays identified four Ci-Sna-binding sites in the Ci-Bra enhancer, and mutations in these sites cause otherwise normal Ci-Bra/lacZ transgenes to be misexpressed in ectopic tissues, particularly the tail muscles. Selective misexpression of Ci-sna using a heterologous promoter results in the repression of Ci-Bra/lacZ transgenes in the notochord. Moreover, the conversion of the Ci-Sna repressor into an activator results in the ectopic induction of Ci-Bra/lacZ transgenes in the muscles, and also causes an intermixing of notochord and muscle cells during tail morphogenesis. These results suggest that Ci-Sna functions as a boundary repressor, which subdivides the mesoderm into separate notochord and tail muscle lineages.

Quantitative assessment of myocardial 99mTc-sestamibi uptake during exercise: usefulness of response rate for assessing severity of coronary artery disease.

An increase of 99mTc-sestamibi uptake in the myocardium during exercise was defined as a response rate, and the feasibility of a response rate for detecting coronary artery disease (CAD) was tested. Eighty-seven patients with suspected CAD had myocardial perfusion imaging with 99mTc-sestamibi during exercise and at rest. A dose of 370 MBq of 99mTc-sestamibi was injected at the maximal level of exercise, and a myocardial image was obtained 90 min later (exercise image). Then, 740 MBq of 99mTc-sestamibi was administered at rest, and myocardial imaging was repeated (rest image). The exercise and rest images were corrected for physical decay and injected doses, and the exercise image was subtracted from the rest image to obtain the corrected rest image. A response rate was calculated as follows: (exercise image-corrected rest image)x100/corrected rest image (%). The global response rates of 20 patients without significant coronary stenosis (< or =50%) were higher than those of 67 patients with significant coronary stenosis (81+/-33% and 50+/-28%, p<0.01). Global response rates were correlated with the maximal rate pressure products during exercise (r=0.56, p<0.01) and delta rate pressure products (r=0.53, p<0.01). Regional response rates in myocardial areas perfused by stenotic coronary arteries of < or =50%, 75%, 90% and 99-100% were 60+/-24%,* 56+/-33%,* 40+/-23%* and 30+/-23%,* respectively, (*p<0.01 vs without significant coronary stenosis). The response rates decreased as the severity of coronary artery stenosis advanced, and distinguished between coronary stenoses of graded severity. Accordingly, the response rate from myocardial perfusion imaging with 99mTc-sestamibi may provide complementary information to the conventional inspection with myocardial tomography regarding the severity of CAD.

Isolation of acriflavine resistant Erysipelothrix rhusiopathiae from slaughter pigs in Japan.

Erysipelothrix rhusiopathiae is the causative agent of swine erysipelas. Although an attenuated vaccine is used in Japan, recent increases in disease occurrence have cast doubts on its efficacy. We investigated the similarity between the vaccine strain and E. rhusiopathiae field isolates by the analysis of acriflavine resistance (the vaccine strain marker), serotype, DNA fingerprinting and pathogenicity to mice. Although 7 acriflavine resistant E. rhusiopathiae isolates were separated from arthritic lesions of slaughter pigs, we were unable to prove that they were identical to the vaccine strain.

Myocardial tomography with technetium-99m-tetrofosmin during intravenous infusion of adenosine triphosphate.

UNLABELLED: The purpose of this study was to determine the biodistribution of 99mTc-tetrofosmin during intravenous infusion of adenosine triphosphate (ATP) and to evaluate the potential diagnostic value of myocardial tomography with 99mTc-tetrofosmin during ATP infusion for the detection of coronary artery disease. METHODS: Myocardial 99mTc-tetrofosmin imaging with ATP infusion and coronary arteriography were performed on 65 patients with suspected coronary artery disease. ATP was infused intravenously at a rate of 0.16 mg/kg/min for 5 min, and 370 MBq of 99mTc-tetrofosmin was injected 3 min after the start of ATP infusion. Myocardial SPECT imags were obtained 60 min later. Then, 740 MBq of 99mTc-tetrofosmin was administered at rest, and myocardial SPECT was repeated. Regional uptakes of 99mTc-tetrofosmin were scored from 4, normal, to 0, no activity. Serial 5-min planar images were obtained in the anterior projection at 15, 30, 45 and 60 min after the 99mTc-tetrofosmin injection in 10 patients. Heart-to-lung and heart-to-liver count ratios were defined from the serial planar images. RESULTS: Adverse effects of ATP infusion were mild and transient. A heart-to-lung ratio after ATP infusion was high even at 15 min (3.40 +/- 0.33) and gradually increased with time. A heart-to-liver ratio after ATP was 0.53 +/- 0.40 at 15 min and increased with time. A heart-to-liver ratio reached 0.99 +/- 0.25 (p < 0.01) after 45 min and 1.32 +/- 0.36 (p < 0.01) after 60 min. The sensitivity and specificity for detecting coronary artery disease by myocardial SPECT with ATP were 89% (39/44) and 86% (18/21), respectively. CONCLUSION: This study shows the favorable biodistribution of 99mTc-tetrofosmin after intravenous infusion of ATP. A one-day imaging protocol of 99mTc-tetrofosmin tomography with ATP is feasible and has high diagnostic accuracy for coronary artery disease.

Effect of adherence on antimicrobial susceptibility of Pseudomonas aeruginosa, Serratia marcescens, and Proteus mirabilis.

A simple method was used for testing the antibiotic susceptibility of adherent bacteria to plastic surfaces. Pseudomonas aeruginosa, Serratia marcescens, and Proteus mirabilis cells adhering to the bottom of a plastic tissue culture plate were incubated in serially diluted antibiotic solutions. After 24-h incubation the solutions were removed and a fresh medium without antibiotics was added to each well. The viability of the cells was judged by their growth after a further 24-h incubation. In our assay system, we employed a short incubation time (1-h) involving adherence of bacteria to a surface for the purpose of minimizing the effect of the glycocalyx on antibiotic activity. Even if the bacteria did not form a biofilm, the minimal bactericidal concentrations for adherent bacteria (MBCADs) markedly elevated. The MBCADs of ofloxacin well correlated with the bacteriological eradication by ofloxacin treatment for urinary tract infections (UTIs) associated with indwelling urinary catheters, whereas the minimal inhibitory concentrations did not show a correlation. Kinetic studies showed that adherent Pseudomonas aeruginosa had a 2 h-lag time before logarithmically growing when these bacteria were incubated in Mueller-Hinton broth without antibiotics. The tolerance demonstrated by adherent cells is likely to play a role in the difficulties encountered in the antimicrobial chemotherapy of biofilm infections. Moreover, our assay system was considered to be useful in the therapeutic selection of antibiotics for these infections.

[Interhemispheric asymmetry exists in female in the N100m source position of the auditory evoked magnetic fields].

The N100m source of the auditory evoked magnetic fields (AEFs) is located more posterior in the left than the right hemisphere in male subjects. However, whether this asymmetry exists in female subjects is controversial. The present study, analyzed the N100m source positions of the AEFs due to monaural tone stimuli using a helmet-shaped 66-channel magnetoencephalography system (CTF Systems) in 62 right-handed normal adults (24 females and 38 males). The best fit sphere was calculated for each subject from the head shape reconstructed from three-dimensional magnetic resonance images. N100m source positions were estimated using a double-dipole model. The contralateral N100m response to the stimulated ear was measured relative to the midpoint of the bilateral auricular points. Statistical analysis used a standardized head size for each subject based on the mean value of all subjects (r = 8.27 cm). In females, the N100m dipole positions were 0.00 +/- 1.02 cm anterior in the left hemisphere and 0.35 +/- 0.74 cm in the right (p < 0.02). In males, the N100m dipole positions were -0.31 +/- 0.81 cm (mean +/- standard deviation) anterior in the left hemisphere and 0.61 +/- 0.78 cm in the right (p < 0.0001). The interhemispheric difference in the N100m positions was 0.35 +/- 0.65 in females and 0.92 +/- 0.77 in males (p < 0.005). In conclusion, the left hemispheric N100m is located more posterior to the right in both genders. However, this functional asymmetry is more evident in males, like previous findings of more evident anatomical asymmetry in males.

[Differential diagnosis of hyponatremia following subarachnoid hemorrhage].

Hyponatremia is a common complication after subarachnoid hemorrhage (SAH). Although the mechanism of hyponatremia is still controversial, cerebral salt-wasting syndrome (CSNS) is currently regarded as being more responsible than the syndrome of inappropriate secretion of antidiuretic hormone (SIADH). The aim of our study was to assess the plasma volume status of a patient with hyponatremia following subarachnoid hemorrhage. In doing this it may be possible to indirectly differentiate its pathogenesis. Fifty patients with SAH were studied. Twenty patients demonstrated hyponatremia (serum sodium < 135 mEq/L) during day 7 to 13 after subarachnoid hemorrhage. Patients with hyponatremia were categorized on the basis of their daily body weight, and central venous pressure. Group A consisted of patients with hypovolemia (16 patients), with the onset time of hyponatremia being day 7 to 9. Group B included those with hypervolemia (4 patients); hyponatremia was observed during day 10 to 11 and was corrected in all patients within 72 hours after induction of fluid restriction. Our findings suggest that hyponatremia following subarachnoid hemorrhage usually occurs due to CSWS, although SIADH remains as a minor pathogenesis. We conclude that the combination of daily body weight and CVP measurements is a simple and practical method to distinguish promptly SIADH from CSWS.

[Epidemiology--risk factors and preventive strategy].

Many factors, including age, sex, menopause, genetic makeup, calcium intake, and physical exercise, influence the development of osteoporosis. Bone density is a major risk factor for fracture at spine and hip. However, risk of hip fracture is affected by factors not limited to bone strength, for example, factors related to falling. These risk factors can be classified as those that are controllable and those that are not. Although one has uncontrollable risk factors, the risk of hip fracture can be decreased by reducing the controllable risk factors. To prevent osteoporosis and fracture, it is necessary to be familiar with an individual's risk factors and offer guidance to reduce the controllable risks. Maintaining or increasing bone mass are essential for those with uncontrollable factors.

[Diffusion-weighted MRI detectability of acute cerebral ischemia: comparison with eventual infarction].

To clarify the clinical significance of diffusion-weighted magnetic resonance imaging (DWI) hyperintensity in acute cerebral ischemia, 34 patients were investigated with cerebral ischemia that had lasted no more than 8 hours. Either a spin echo sequence with navigator echo (n = 21) or an echo planar sequence (n = 13) was used for DWI using 1.5 tesla magnet. The eventual infarction was judged from T2-weighted image (T2 WI) or CT performed 2-12 days after the onset and was classified into cortical (n = 24) or perforating artery (n = 10) area. The eventual infarction was compared in size with the acute DWI finding and expressed as larger, same or smaller. Thirty patients showed ischemic hyperintensity lesions on acute DWI (6 patients on T2 WI). In cortical artery areas, the eventual infarction was same as hyperintensity area of acute DWI in 15 cases, larger in 8 cases, smaller in 2 cases. In perforating artery areas, the eventual infarction was same as hyperintensity area of acute DWI in 8 cases, and larger in 2 cases. These results suggest that in the perforating artery areas, the acute DWI hyperintensity is a good predictor of the eventual infarction and that in cortical artery areas, dynamic ischemic process may continue for hours before resulting in the eventual infarction.
