Neuromagnetic evidence of pre- and post-central cortical sources of somatosensory evoked responses.

Somatosensory evoked magnetic fields (SEFs), corresponding with electrical median nerve stimulation, were measured using an MRI-linked, whole-head MEG system. In total 184 hemispheres, SEFs were measured, involving 22 normal volunteers and 70 patients with intracranial lesions. The first SEF peak appeared 20.2 +/- 1.5 msec (mean +/- S.D.) after stimulation. The first peak was followed by a second and third peak, 7.7 +/- 3.2 msec and 14.6 +/- 5.4 msec respectively after the first. Corresponding isofield maps suggested a single current dipole oriented in the anterior direction for the first peak, posterior for the second and either anterior or posterior for the third. Using a spherical model and a single current dipole source, the localization of each peak was estimated and superimposed on individually determined MRI images. On a grand average basis, the first-peak dipole was localized on the posterior surface of the central sulcus, "area 3b." The second-peak dipole was localized 3.7 mm medial (P < 0.001) and 2.0 mm superior (P < 0.001) to the first-peak dipole, on the anterior wall of the central sulcus, "area 4." This study with a large number of subjects was able to statistically distinguish two adjacent cortical sources, located on opposite walls of the central sulcus, and within a few millimeters of each other.

Comparative study on the antibacterial activity of phytochemical flavanones against methicillin-resistant Staphylococcus aureus.

Differently substituted flavanones were isolated from Leguminosae and their antibacterial activity was comparatively studied against methicillin-resistant Staphylococcus aureus (MRSA). The minimum inhibitory concentrations (MICs) of phytochemical flavanones to clinical isolates of MRSA were determined by a serial agar dilution method. The structure-activity relationship has indicated that 2',4'- or 2',6'-dihydroxylation of the B ring and 5,7-dihydroxylation of the A ring in the flavanone structure are important for significant anti-MRSA activity and that substitution with a certain aliphatic group at the 6- or 8-position also enhances the activity. Among the thirteen flavanones tested, tetrahydroxyflavanones with these structural characteristics isolated from Sophora exigua and Echinosophora koreensis showed intensive activity to inhibit the growth of all MRSA strains at 3.13-6.25 micrograms/ml. The present hydroxyflavanones would be useful in the phytotherapeutic strategy against MRSA infections.

Purification and structural analysis of extracellular matrix of a skin tumor from a patient with juvenile hyaline fibromatosis.

Juvenile hyaline fibromatosis is a rare mesenchymal dysplasia that is inherited in an autosomal recessive fashion. The histological features of the tumor-like lesions are characterized by the deposition of amorphous hyaline material in the extracellular spaces of the dermis and soft tissues. We have analyzed the hyaline substance in a specimen of a skin tumor obtained from a 4-year-old Japanese girl with juvenile hyaline fibromatosis. It was found to consist mainly of type VI collagen; a small amount of type I collagen was also present. These components were separated by DEAE-cellulose ion-exchange chromatography under reducing conditions. The ratio of the dry weights of type I and type VI collagen was 1:4. Of the three chains of type VI collagen (alpha 1(VI), alpha 2(VI) and alpha 3(VI)), alpha 3(VI) was the most abundant. Glycosaminoglycans in the tumor tissue comprised dermatan sulfate, chondroitin sulfate and hyaluronan, with dermatan sulfate predominating. In contrast, hyaluronan is the most abundant in normal skin.

Determinants of vertebral fracture prevalence among native Japanese women and women of Japanese descent living in Hawaii.

Age-adjusted prevalence of vertebral fracture has been reported to be higher among native Japanese women than among women of Japanese descent living in Hawaii. In this cross-sectional population-based study, we examined a variety of potential risk factors for associations with prevalent vertebral fractures and investigated whether these factors could explain the difference in vertebral fracture prevalence between native Japanese and Japanese-American women. Spine radiographs and data on spine bone mineral density (BMD) and other potential risk factors were collected among 802 Japanese women aged 50-88 years living in Hiroshima and 840 Japanese-American women aged 52-88 years living in Hawaii. In logistic regression analysis, BMD was a major predictor of prevalent vertebral fracture. In linear regression models, weight, age, and menstrual history (age at menopause or years between menarche and menopause) were significantly associated with BMD and thus might contribute to fracture risk indirectly through their effects on BMD. However, age and menstrual history provided additional and complementary information about fracture prevalence after adjusting for BMD. These variables together explained much of the difference in vertebral fracture prevalence between the two study populations. We conclude that the observed difference in age-adjusted prevalence of spine fracture between native Japanese and Japanese-American women was accounted for primarily by the differences in BMD, duration of estrogen exposure, and/or duration of estrogen deficiency. Thus, current BMD is a major but not the sole risk factor for vertebral fractures. Age-related and menopause-related mechanisms may also play an important role in spine fracture independent of BMD.

Flavones with antibacterial activity against cariogenic bacteria.

Methanolic extracts obtained from 13 plants were studied for their antibacterial activity against cariogenic bacteria. Among them, the extract from Artocarpus heterophyllus showed the most intensive activity. Serial chromatographic purifications offered two active compounds which were identified as 6-(3-methyl-1-butenyl)-5,2',4'-trihydroxy-3-isoprenyl-7-methoxy flavone and 5,7,2',4'-tetrahydroxy-6-isoprenylflavone. Both isolates completely inhibited the growth of primary cariogenic bacteria at 3.13-12.5 micrograms/ml. They also exhibited the growth inhibitory effects on plaque-forming streptococci. These phytochemical isoprenylflavones would be potent compounds for the prevention of dental caries.

Fatty acid metabolic imaging with 123I-BMIPP for the diagnosis of coronary artery disease: application to patients with diabetes mellitus and hyperlipidaemia.

The aim of this study was to assess the relation of plasma substrate concentration to 123I-beta-methyliodophenyl-pentadecanoic acid (123I-BMIPP) kinetics in the myocardium and to test the application of 123I-BMIPP imaging to patients with diabetes mellitus (DM) and/or hyperlipidemia (HL). 123I-BMIPP imaging was performed on 78 patients with suspected coronary artery disease, 22 with HL, 11 with DM, 12 with HL and DM, and 33 with neither HL or DM. Significant coronary stenosis (defined as > or = 50% of luminal diameter) was documented in 49 patients. After an overnight fast, blood samples were drawn for blood glucose, insulin, cholesterol, triglycerides and free fatty acid levels. Then, 148 MBq 123I-BMIPP was injected intravenously and flushed rapidly with saline. Data were obtained for 60s in a standard anterior projection in list mode, at a rate of 1 frame per second. Myocardial single photon emission tomographic (SPET) images were obtained 20 min and 4 h post-injection. The myocardial uptake of 123I-BMIPP was calculated using the Ishii-MacIntyre method. Regional accumulation of 123I-BMIPP was scored semi-quantitatively from 0 (normal) to 4 (no activity), and the sum of regional scores in each patient was defined as the total defect score (TDS). Myocardial uptake and clearance of 123I-BMIPP had no relation to the levels of blood glucose, insulin, cholesterol, triglycerides or free fatty acids. Myocardial uptake and clearance of 123I-BMIPP, TDS and the sensitivity of detecting significant coronary stenosis were not significantly different between the four groups of patients. We conclude that 123I-BMIPP can be used to detect impaired fatty acid metabolism in patients with diabetes mellitus and/or hyperlipidaemia.

Identification of a 450-kDa human epidermal autoantigen as a new member of the plectin family.

The serum from an individual with a subepidermal blistering disease was previously shown to recognized a 450-kDa epidermal autoantigen. The molecular structure of this antigen was investigated by screening a human keratinocyte cDNA library with the patient's serum. One clone, with a 276-bp cDNA insert, that encoded an epitope recognized by the serum was isolated. Rabbit polyclonal antibodies that were prepared against the corresponding fusion protein recognized the 450-kDa epidermal antigen and stained the basal keratinocytes in human epidermis. This clone was used for further screening of the original keratinocyte and HeLa cell cDNA libraries. Two different, but closely related, 0.8- and 2.0-kb cDNAs were isolated, and their deduced amino acid sequences indicated that the encoded proteins belonged to the plectin family. Northern blot analysis of total RNA from human keratinocytes with these cDNA inserts as probes detected RNAs of approximately 12-13 kb. The 0.8-kb cDNA hybridized to polyadenylated RNA species from human skeletal muscle, heart, lung, and kidney, whereas the 2.0-kb cDNA hybridized to transcripts present only in kidney and lung. Southern blot analysis of genomic DNA from the human placenta revealed similar, but not identical, patterns of hybridization with the 0.8- and 2.0-kb cDNAs. Data suggest that the 0.8- and 2.0-kb cDNAs encode two different proteins but are derived from the same gene.

Intracranial aneurysms: diagnostic accuracy of MR angiography with evaluation of maximum intensity projection and source images.

PURPOSE: To determine whether evaluation of source images from magnetic resonance (MR) angiography in addition to maximum-intensity projection (MIP) images improves the detection of aneurysms. MATERIALS AND METHODS: Conventional and MR angiography were performed in 193 patients with various intracranial vascular lesions or normal findings. Images were evaluated in a blinded manner. Two readings were performed 6 weeks apart by evaluating MIP images with and without source images. Results were evaluated with receiver operating characteristic analysis. RESULTS: Sensitivity for the detection of aneurysms increased slightly when source images were included. The detection rate of internal carotid artery aneurysms was most improved with the addition of source images. No statistically significant differences in performance were found between the readings with MIP images alone and with source images. CONCLUSION: Sensitivity may improve with combined reading of nonselective MIP and source images.

Reverse redistribution of technetium-99m-sestamibi following direct PTCA in acute myocardial infarction.

UNLABELLED: A pattern of reverse redistribution (RR) has not been documented in myocardial 99mTc-sestamibi imaging. The purpose of the study was to clarify the time-related changes in myocardial distribution of 99mTc-sestamibi in patients with acute myocardial infarction. METHODS: Myocardial SPECT with 99mTc-sestamibi was performed in 27 patients with acute myocardial infarction within 1 wk after the onset. Twenty-three patients received direct percutaneous transluminal coronary angioplasty (PTCA) and 4 patients did not. Myocardial images were obtained 1 hr (early) and 3 hr (delayed) after the injection of 99mTc-sestamibi. Regional myocardial uptake of 99mTc-sestamibi was scored from 4 (normal) to 0 (no activity), and the RR pattern was defined as a decrease of more than 1 in the regional score at the 3-hr delayed images. Regional myocardial uptake and clearance of 99mTc-sestamibi was also assessed quantitatively. Coronary arteriography and left ventriculography were performed 1 mo later. RESULTS: Out of 22 patients with successful PTCA, RR of 99mTc-sestamibi was observed in 15 patients (68%). Persistent defects (PD) were seen in 12 patients (7 patients with successful PTCA, 1 patient with unsuccessful PTCA, and 4 patients who did not receive angioplasty). In patients with RR, regional uptake of 99mTc-sestamibi in the area of myocardial infarction decreased from 54% +/- 10% in the early images to 43% +/- 8% in the delayed images (p < 0.01). Technetium-99m-sestamibi clearance from the myocardium was faster in the infarct area than in the normal area (26% +/- 7% versus 9% +/- 6%, p < 0.01). Coronary arteriography performed 1 mo later revealed that the patency of the infarct related artery was 100% (15/15) in patients with RR and 50% (6/12) in those with PD (p < 0.01). The extent and severity of a wall motion abnormality were less in patients with RR than in those with PD (extent: 24 +/- 10 versus 36 +/- 9 chord, p < 0.01; severity: -2.7 +/- 0.4 versus -3.4 +/- 0.6 s.d./chord, p < 0.01). CONCLUSION: The RR of 99mTc-sestamibi was observed in 68% of patients after successful direct PTCA and was associated with the accelerated clearance of 99mTc-sestamibi from the myocardium. The presence of RR in 99mTc-sestamibi imaging indicates the patency of the infarct-related artery and predicts the preserved left ventricular function.

Noninvasive evaluation of malignancy of brain tumors with proton MR spectroscopy.

PURPOSE: To test clinical proton MR spectroscopy as a noninvasive method for predicting tumor malignancy. METHODS: Water-suppressed single-voxel point resolved spectroscopy in the frontal white matter of 17 healthy volunteers and 25 patients with brain tumors yielded spectra with peaks of N-acetyl aspartate (NAA), choline-containing compounds (Cho), creatine/phosphocreatine (Cre), and lactate. These peak intensities were semiquantitated as a ratio to that of the external reference. The validity of the semiquantitation was first evaluated through phantom and volunteer experiments. RESULTS: The variation in measurements of the designated region in the volunteers was less than 10%. Normal ranges of NAA/reference, Cho/reference, and Cre/reference were 3.59 +/- 0.68, 1.96 +/- 0.66, and 1.53 +/- 0.64 (mean +/- SD), respectively. In 17 gliomas, the Cho/reference value in high-grade gliomas was significantly higher than in low-grade gliomas. Levels of NAA/reference were also significantly different in low-grade and high-grade malignancy. In eight meningiomas (four newly diagnosed and four recurrent), the level of Cho/reference was significantly higher in recurrent meningiomas than in normal white matter or in newly diagnosed meningiomas. CONCLUSIONS: Higher grades of brain tumors in this study were associated with higher Cho/reference and lower NAA/reference values. These results suggest that clinical proton MR spectroscopy may help predict tumor malignancy.

Delayed metabolic recovery of hibernating myocardium after percutaneous transluminal coronary angioplasty: assessment with iodine-123-betamethyl-p-iodophenyl-pentadecanoic acid imaging.

The time course of improvement in fatty acid metabolism after percutaneous transluminal coronary angioplasty (PTCA) was investigated using echocardiography and fatty acid metabolic imaging with iodine-123-betamethyl-p-iodophenyl-pentadecanoic acid (123I-BMIPP) before, 1 week and 3 months after PTCA in 31 patients with angina pectoris. Decreased left ventricular wall motion before PTCA improved 1 week after PTCA in 13 of 31 patients. 123I-BMIPP uptake was reduced in these 13 patients before PTCA, and did not improve 1 week after PTCA. Decreased myocardial uptake of 123I-BMIPP improved 1 week after PTCA in eight of 23 patients (group A). Thirteen patients in whom 123I-BMIPP uptake had not improved 1 week after PTCA showed a delayed recovery of 3 months after PTCA (group B). All patients in groups A and B showed improvement in wall motion 1 week after PTCA. Patients in group B had a higher incidence of unstable angina (77% vs 25%, p < 0.01), 99% or 100% stenosis (62% vs 13%, p < 0.01) and collateral vessels (46% vs 13%, p < 0.05) than those in group A. Serial fatty acid metabolic imaging with 123I-BMIPP after PTCA showed delayed metabolic recovery after improvement in wall motion in 13 of 23 patients. The presence of severe myocardial ischemia before PTCA enhanced the chronological discrepancies between the recovery of wall motion and fatty acid metabolism.

[Quantitative assessment of an increase of myocardial 99mTc-MIBI accumulation during exercise--usefulness of response rate].

The increase of myocardial 99mTc-MIBI accumulation during exercise was evaluated quantitatively, and the feasibility of response rate as a noninvasive marker of coronary stenosis was tested. Myocardial perfusion imaging with 99mTc-MIBI during exercise and at rest was performed in patients with suspected coronary artery disease. A dose of 296 MBq of 99mTc-MIBI was injected intravenously at maximal treadmill exercise, and myocardial image was obtained 90 min later (1st image). Then, 740 MBq of 99mTc-MIBI was administered at rest, and myocardial image was repeated (2nd image). These images were corrected for a decay and injected dose, and the 1st image was subtracted from the 2nd image to obtain the rest image. An increase of myocardial accumulation of 99mTc-MIBI during exercise was defined as (exercise image-rest image) x 100/rest image (response rate). A response rate of a patient with normal coronary artery was 102%, whereas a response rate in the area of severe coronary stenosis was 21% in a patients with angina pectoris. After successful PTCA to a stenosed coronary artery, a response rate increased to 75% in this patient. Coronary perfusion reserve during exercise can be assessed noninvasively by 99mTc-MIBI. Response rate of 99mTc-MIBI provides additional information to conventional perfusion imaging and may be a new marker of severity of coronary artery disease.

Intracranial epidural abscess: late complication of allograft cranioplasty.

A case of intracranial epidural abscess, 20 years after allograftic cranioplasty was presented. The abscess was caused by bacterial implantation after a minor stab wound of the scalp over the cranioplastic plate. It seemed quite rare that the intarcranial abscess occurred 20 years after an allograftic cranioplasty.

[Right hemispheric dominancy in the auditory evoked magnetic fields for pure-tone stimuli].

It is well known that the auditory evoked N100m response is dominant in the contralateral hemisphere to the stimulated ear: N100m latency is shorter and N100m amplitude is larger in the contralateral hemisphere than the ipsilateral hemisphere. Interhemispheric asymmetry of the N100m responses were reported in terms of the equivalent dipole localization: the right hemispheric N100m dipole is more anterior to the left hemispheric dipole. In the present study, we investigated interhemispheric asymmetry of the auditory evoked P50m and N100m response in terms of signal amplitude. A helmet-shaped whole head magnetoencephalography system was employed to see small but significant differences between two hemispheres. Thirty-seven right-handed healthy subjects, 31 males and 6 females, participated in the study. The helmet-shaped whole head MEG system (CTF Systems-Osaka Gas) consists of an array of 66 MEG sensor sites uniformly distributed over the entire head. The monaural auditory stimuli, 2000 Hz tone burst of 50 msec duration were delivered to the left and then the right ear sequentially, with an intensity of 80 dB SPL at the ear. White noise of 50 dB SPL was delivered to the opposite ear. The inter-stimulus intervals were randomized in the range from 2.5 to 4.7 sec. In a total of 37 subjects, N100m dipole pattern appeared on both hemispheres either left or right ear stimuli. When P50m peak amplitude was compared between contralateral responses, the right-hemispheric response (141.5 +/- 16.3 fT, mean +/- s.e.m.) was slightly larger than the left-hemispheric response (134.3 +/- 10.2 fT). Differences, however, were not significant. When P50m peak amplitude was compared between ipsilateral responses, the right-hemispheric response (135.1 +/- 11.0 fT) was slightly larger than the left-hemispheric response (115.3 +/- 10.9 fT). Differences were not significant, either. When N100m peak amplitude was compared between contralateral responses, the right-hemispheric response (484.4 +/- 28.4 fT) was statistically (p < 0.01) larger than the left-hemispheric response (375.6 +/- 28.0 fT). When N100m peak amplitude was compared between ipsilateral responses, the right-hemispheric response (405.8 +/- 27.9 fT) was statistically (p < 0.02) larger than the left-hemispheric response (315.1 +/- 24.8 fT). Previously, we reported that the right hemispheric latency is shorter than the left hemispheric latency in the auditory evoked field N100m responses. Present study indicated another evidence of the right hemispheric dominancy in the auditory evoked responses, namely the lager amplitude in the right hemispheric response at least for pure-tone stimuli. The whole-head MEG system is suitable to analyze differences of auditory function between two hemispheres.
