Application of bis-eugenol to a zinc oxide eugenol cement.

OBJECTIVES: To assess the usefulness of dimerized eugenol (bis-eugenol) in dentistry, the physical properties of zinc oxide eugenol cement (ZOE) with bis-eugenol and the cytotoxicity of bis-eugenol were studied. METHODS: Setting time, compressive strength, solubility and disintegration of ZOE cement with bis-eugenol according to the specifications of JDMAS315 were evaluated. The cytotoxicity of bis-eugenol and eugenol toward two different cell types, HGF (a primary culture of human gingival fibroblast) and HSG (a human epidermoid carcinoma cell line derived from a salivary gland) was evaluated by the MTT test and in terms of cell survival. RESULTS: Addition of bis-eugenol to ZOE did not decrease the physical properties when employed at the ratio of 9:1 or 6:1 (liquid ND:bis-eugenol, w/w). Bis-eugenol was less toxic than eugenol in the cell culture tests. CONCLUSIONS: The results of this assay demonstrated that bis-eugenol is useful in ZOE.

Long-term liver function of recipients with hepatitis G virus infection after bone marrow transplantation.

To clarify the role of hepatitis G virus (HGV) infection in liver dysfunction following allogeneic BMT, we examined cryopreserved serum samples from 33 patients who had a history of blood transfusions before BMT and whose serum samples had been stored periodically, before BMT, on day 100, and thereafter for the presence of HGV-RNA and hepatitis C virus (HCV)-RNA by reverse transcription polymerase chain reaction. Nineteen patients (58%) out of 33 were positive for HGV-RNA before BMT and 10 for HCV-RNA. All patients positive for HCV-RNA were also positive for HGV-RNA. Patients were divided into three groups according to their viral status before BMT; namely, the G+C+ group (n = 10), the G+C- group (n = 9) and the G-C- group (n = 14). Two patients in the G-C- group became positive for HGV-RNA after BMT. One patient in the G+C- group suffered an acute exacerbation of hepatitis, with GPT levels reaching over 1000 IU/l, 2 and 3 years after BMT, showing quite a different clinical course from those in the G+C- group. Excluding these three patients, GPT levels of the patients in the G+C+ group were significantly higher after day 100 and remained higher than those of patients in the G+C- and G-C- groups for at least 4 years. There were no significant differences in post-transplant GPT levels between the G+C- group and the G-C- group at any time point. Of the seven patients followed-up for 5 to 10 years, three patients became HGV-RNA-negative, while four remained positive. In the absence of HCV co-infection, the behavior of GPT values post transplant in patients with HGV infection did not differ from those without HGV infection. With respect to the patient who was G+C- and showed high values of GPT 2 and 3 years post transplant, we suspect that his liver dysfunction might have been caused by some unknown virus or etiology.

Extensive cytogenetic studies of clonality following interferon-alpha therapy in chronic myeloid leukemia occurring in monosomic cells in a patient with Turner syndrome mosaic.

In a 27-year-old female with Turner syndrome mosaic, Philadelphia (Ph) chromosome-positive chronic myeloid leukemia (CML) occurred only in the monosomic cells (45, Xc). Extensive cytogenetic studies, including triple-color fluorescence in situ hybridization (FISH), revealed that Ph-positive monosomic cells (45, Xc), Ph-negative monosomic cells and normal diploid cells (46, XX) were present in her bone marrow at diagnosis. After successful interferon therapy, the non-leukemia cells expanded and reconstituted normal hematopoiesis resulting in complete cytogenetic response, following the selective suppression of the monosomic Ph-positive leukemia clone. The ratio of Xc to XX cells in bone marrow cells was significantly increased to that in skin fibroblasts. Moreover, the ratio of Ph-positive cells to Ph-negative cells was found to be significantly different between karyotyping and FISH. Studies of this quite unique case not only confirmed the clonality of CML, effectiveness of interferon-alpha and X chromosome imbalance among different tissues, but also demonstrated a discrepant increase of the BCR/ABL-positive clone in CML. The latter supports the hypothesis that reduced programmed cell death may be the primary mechanism responsible for the expansion of the leukemia clone in CML. Our study verifies the importance of extensive analysis of a neoplastic disease in patients with a constitutional chromosomal abnormality.

Improvement of quality of life after splenectomy in an HTLV-I carrier with T-cell prolymphocytic leukemia.

A 34-year-old woman of HTLV-I carrier with T-PLL, whose quality of life improved and survival was prolonged after splenectomy, is described. The patient had marked splenomegaly, generalized lymphadenopathy and marked proliferation of abnormal lymphocytes in the peripheral blood with an irregular nucleus, deeply basophilic cytoplasm and a single prominent nucleolus, which were positive for CD2, CD3, CD5, CD7, CD4 and CD8. Although the patient had serum antibody against HTLV-I, HTLV-I proviral DNA integration was not detected. She was diagnosed as an HTLV-I carrier with T-PLL and received combination chemotherapy and 15.1 Gy splenic irradiation. However, the generalized lymphadenopathy and splenomegaly did not improve. The patient underwent splenectomy to palliate abdominal distension and hypersplenism. After the operation, her symptoms improved dramatically and within a week her hemoglobin concentration and platelet count normalized. She was discharged from hospital two weeks after the splenectomy, however 11 months later, she relapsed and despite treatment with chemotherapy and alpha-interferon, she died two months after the second admission. Autopsy findings revealed that PLL cells had invaded the bone marrow, lymph nodes, liver, lungs, kidneys, uterus, ovaries and adrenal glands.

Acute minimally differentiated myeloid leukemia (M0) with inv(3)(q21q26).

We describe a 55-year-old Japanese man with acute minimally differentiated myeloid leukemia (M0) with an inversion in the long arm of chromosome 3, i.e., inv(3)(q21q26). Patients with this chromosomal abnormality usually show normal or elevated platelet counts. However, our case had a low platelet count with megakaryocytic dysplasia at diagnosis. Furthermore, the 3q21q26 aberration is generally detected in patients with acute myelogenous leukemia (AML) or myelodysplastic syndrome. To the best of our knowledge, it has also been reported in two cases of AML-M0 with 3q21q26 and this is the third case of AML-M0 with 3q21q26. Thus it is suggested that there is some relationship between this type of karyotype abnormality and leukemogenesis and/or thrombopoiesis.

Free radical scavenging activity in the nonenzymatic fraction of human saliva: a simple DPPH assay showing the effect of physical exercise.

Free-radicals and other reactive oxygen species (ROS) have been implicated as being major damaging species in pathology and they have been widely investigated. Using 1,1'-diphenyl-2-picrylhydrazyl (DPPH), we estimated total free radical scavenging activity in the low-molecular-weight nonenzymatic fraction (LMNEF) of human whole saliva. The activity of the whole saliva and serum were measured in terms of the rate of decrease in the absorbance at 517 nm in a 40% ethanol DPPH solution (pH 7.4) at room temperature. The DPPH activity of saliva and serum showed a significant linear relationship. The mean DPPH activities of saliva from 257 subjects aged 4-72 was found to be 0.389+/-.190 micromol/ml and bore no relation to age or sex. The activity in saliva of 86 subjects aged 4-11 was significantly different before and after exhaustive aerobic dance exercise for 1 hr. Physical exercise markedly decreased free radical scavenging activity in whole saliva of children. On the basis of the above results, we concluded that DPPH is useful for evaluating the total antioxidant capacity of LMNEF of human saliva.

Low-dose cytarabine-induced hepatic and renal dysfunction in a patient with myelodysplastic syndrome.

We report a 49-year-old female patient with secondary myelodysplastic syndrome who developed liver dysfunction and acute renal failure caused by low-dose cytosine arabinoside (Ara-C) therapy. Treatment of low-dose Ara-C has widely been used for acute myelogeous leukemia and myelodysplastic syndrome, and it is thought to be a low toxicity except for myelosuppression. The patient complained of a transient adverse reaction in the second and third course of therapy. This rare case indicates that careful observation should be carried out during low-dose Ara-C therapy in view of allergic reactions.

Mechanical stress-induced Ca2+ entry and Cl- current in cultured human aortic endothelial cells.

A fluid stream through a microtube was applied to cultured human aortic endothelial cells to investigate the endothelial responses of both the ionic currents and intracellular Ca2+ concentration ([Ca2+]i) to mechanical stimulation. The fluid stream induced an increase in [Ca2+]i that was dependent on both the flow rate and the extracellular Ca2+ concentration. Gd3+ and niflumic acid inhibited the fluid stream-induced increase in [Ca2+]i, whereas Ba2+ and tetraethylammonium ion exhibited no effect. The fluid stream-induced [Ca2+]i increase was accompanied by the activation of an inward current at -52.8 mV. The reversal potential of the fluid stream-induced current shifted to positive potentials when the external Cl- concentration was reduced but was not affected by variation of the external Na+ concentration. During the exposure to the fluid stream, [Ca2+]i was voltage dependent, i.e., depolarization decreased [Ca2+]i. We therefore conclude that the fluid stream-induced current is largely carried by Cl- and that the Cl- current may thus play a role in modulating the Ca2+ influx by altering the membrane potential of endothelial cells.

On the inotropic actions of arginine vasopressin in ventricular muscle of the guinea pig heart.

Arginine vasopressin (AVP) (0.03-3 microM) produced transient negative responses that were followed by slowly developing positive inotropic responses in guinea pig papillary muscles. The inotropic responses were antagonized by OPC-21268 (10 microM), but not by OPC-31260, and suppressed by neomycin (3 mM). The negative inotropic response was hardly affected by staurosporine (10 nM) and H-7 (10 microM). AVP (1 microM) elicited a transient decrease followed by a slowly developing increase in I(Ca). The latter increase in I(Ca) was not elicited in the presence of staurosporine (1 nM). These results indicate that AVP stimulates V1 receptors that couple to phosphoinositide hydrolysis and thereby increases intracellular Ca2+ and activates protein kinase C.

De novo acute myelogenous leukemia with trilineage myelodysplasia associated with t(8;21)(q22;q22).

We describe a rare case of de novo acute myelogenous leukemia with trilineage myelodysplasia (AML/TMDS) associated with t(8;21)(q22;q22). The patient was admitted to our hospital with leukocytopenia. AML/TMDS was diagnosed by excess myeloblasts and morphological findings of bone marrow. The karyotype revealed 45, X, -Y, t(8;21)(q22;q22) in 17 of 20 analyzed mitoses, and also AML1/MTG8 transcripts were detected by the reverse transcription polymerase chain reaction (RT-PCR) method. The patient achieved a complete remission with a combination chemotherapy of daunorubicin, cytarabine, and prednisolone. This case suggests that t(8;21)(q22;q22) may participate in the pathogenesis of AML/TMDS, although this type is usually found as one of the chromosomal abnormalities in de novo acute myelogenous leukemia (AML) with maturation.

Primary esophageal T cell lymphoma.

A 60 year-old woman with primary esophageal T-cell lymphoma in clinical stage I(E)B is presented. Immunohistologic examination showed tumor cells to be positive for anti-LCA, anti-UCHL-1, anti-MT-1, anti-CD3 antibodies, and negative for anti-L26 antibody. Disappearance of dysphagia and improvement in esophageal findings were noted after 65 Gy of irradiation, and biopsy specimens from the esophagus revealed no malignancy. Primary esophageal lymphoma is extremely rare, and this T-cell lymphoma is only the fourth case reported in the literature.

1H-NMR studies of the interaction of dental adhesive monomer, 4-META with calcium.

Our objective was to determine whether high-resolution proton-nuclear magnetic resonance (500 MHz) could be utilized for detection of ionic binding interaction of the 4-META resin system with calcium derived from hydroxyapatite. The stability of 4-META in aqueous medium was studied, findings indicated that 4-META was rapidly converted to 4-MET, a hydrate product of 4-META in 10% D2O/DMSO-d6. The 1H-NMR signals of the methacryloyloxyethoxy group of 4-MET remained intact following the addition of both monocalcium phosphate (MCP) and dicalcium phosphate dihydrate (brushite) solution, whereas those of its trimellitic portion were markedly shifted upfield depending on the phosphate concentration. The shielding effect followed by upfield shifts was due to the localization of electron density surrounding the carboxylate anions that were dissociated by the interaction with calcium counter cation. The shielding effect of 4-MET with brushite was larger than that with MCP. An ionic interaction of 4-MET derived from 4-META with calcium was demonstrated.

[Precision and accuracy of white blood cell differentiation by an automated blood cell analyzer].

Precision and accuracy are important to assure the quality of clinical laboratory tests. We investigated the precision and accuracy of white blood cell (WBC) differentiation by automated blood cell analyzers. The coefficients of variation (CVs) of neutrophil, lymphocyte, monocyte, eosinophil and basophil differentiation by GEN.S (Coulter), based on a flow system and VCS technology, were 1.1%, 1.5%, 4.6%, 2.1%, and 33.3%, respectively. Between-run precision for neutrophil, lymphocyte, monocyte, eosinophil and basophil differentiation by GEN.S using cell control, 5C in 26 serial days was 2.0%, 2.8%, 6.5%, 9.2%, and 29.5%, respectively. The precision of WBC differentiation by blood cell analyzers based on the flow system was excellent except for basophil. We studied the accuracy of WBC differentiation by blood cell analyzers based on the flow system in contrast with eye-count. The correlations between neutrophil, lymphocyte, monocyte, eosinophil, and basophil percentages counted by GEN.S and by eye-count were r = 0.968, 0.971, 0.902, 0.940, and 0.391, respectively. The agreement rate between WBC differentiation by Microx, a blood cell analyzer based on pattern recognition, and eye-count cell by cell was good except for basophils. The accuracy of WBC differentiation by blood cell analyzers was also excellent excluding basophils.

[Successful emergency operation for subdural hematoma and acute epidural hematoma in a patient with acute promyelocytic leukemia].

A 22-year-old woman was admitted with purpura. Acute promyelocytic leukemia (APL) with disseminated intravascular coagulation (DIC) was diagnosed. On the 17th day after treatment with all-trans retinoic acid (ATRA), left subdural hematoma developed. Although coagulation abnormalities were still observed, emergency surgery was performed. Acute epidural hematoma was confirmed by computed tomographic scan after the operation. A second operation for drainage was successful. Post-operative intracranial hematoma may be caused by rapid decompression induced by surgery, but DIC could also be involved. This case underscored the need for careful consideration of the indications for surgical treatment of such DIC patients, with close follow-up monitoring for the postoperative development of neurological symptoms.

[Pulmonary function after allogeneic hematopoietic stem cell transplantation].

This study was undertaken to identify the factors influencing pulmonary function in patients who underwent hematopoietic stem cell transplantation (HCT). Pulmonary function tests were evaluated before and after HCT in 51 adult patients who underwent HCT between 1993 and 1998. The patients with hematologic malignancies were given total body irradiation. Graft-versus-host disease (GVHD) prophylaxis consisted of cyclosporine A and short-term methotrexate. Six patients suffered from acute GVHD above grade II and 27 patients suffered from chronic GVHD. The post-transplant % diffusing capacity (%DLco) 100 days after HCT was significantly lower than pretransplant values (82 +/- 21% versus 71 +/- 15%, p < 0.01). The %DLco at 100 days was significantly lower in patients with chronic GVHD than in patients without chronic GVHD (66 +/- 16% versus 77 +/- 9%, p < 0.05). These findings suggested chronic GVHD is related to the decreased %DLco values observed 100 days after HCT.

Cytotoxicity of photosensitizers camphorquinone and 9-fluorenone with visible light irradiation on a human submandibular-duct cell line in vitro.

The cytotoxic effect of two types of photosensitizers (camphorquinone, CQ, a widely used aliphatic type and 9-fluorenone, 9F, an aromatic type) in the presence of 2-dimethylaminoethyl methacrylate (DM) as a reducing agent with exposure to visible light (350-550 nm) was examined in a human cell line. Cytotoxicity was evaluated in terms of the percentage of cell survival, and the production of reactive oxygen in living single cells was measured with an adherent cell analysis and sorting laser cytometer and a peroxide indicator. The amount of reactive oxygen generated in the cells irradiated in the 9F (1 mM-3 min) system was about 9-fold greater than under the same conditions in the CQ system. Similarly, the decrease in cell survival in the 9F system was about 10-fold greater than in the CQ. Both the production of reactive oxygen in the cells and the decrease in cell survival paralleled the concentration of photosensitizers and the irradiation time. Although the cell-damaging effects with the CQ system were mild, at a higher dose (10 mM) and longer irradiation time (24 min) it produced cell survival equal to that in the 9F (1 mM-3 min) system. These results suggest that in the case of irradiated photosensitizer systems, 9F was much more damaging to the cells than CQ, which damage probably occurred via free radicals involving reactive oxygen generation.
