Difference between effects of chlorpromazine and perphenazine on microsomal phospholipids and enzyme activities in rat liver.

The effects of acute administration of chlorpromazine (CPZ) and perphenazine (PPZ) on hepatic microsomal phospholipids (PLs) and enzyme activities in the male rat were examined in order to elucidate the relationship between individual PLs and drug-metabolizing activity. Cytochrome P-450 and aniline (AN) hydroxylation activity were initially decreased in CPZ-treated rats, but cytochrome P-450 subsequently recovered to a level not significantly different from the control and AN hydroxylation was markedly increased, while in PPZ-treated rats, they remained depressed. CPZ increased the activities of glycerophosphate acyltransferase (GPA) and choline phosphotransferase (CPT), while PPZ increased the activities of phosphatidate cytidylyltransferase (PCT), phosphatidate phosphohydrolase (PPH) and CPT. Concurrently, CPZ raised microsomal phosphatidylcholine (PC), phosphatidylethanolamine (PE), phosphatidylserine-inositol (PSI) and sphingomyelin (SM), while PPZ increased PC and PE, but did not affect the levels of PSI and SM. Acyl components of phospholipids were also modified. CPZ significantly decreased the ratio of saturated to unsaturated fatty acids, particularly in the PC and PE fractions, while the effect of PPZ was only slight. The results imply that an increase of AN hydroxylation activity may involve the incorporation of unsaturated fatty acids into enzyme-associated PC and PE.

An experimental study on direct revascularization of bronchial circulation by microvascular anastomosis.

The effects of direct revascularization of the bronchial artery after bronchoplasty were estimated by laser Doppler velocimetry and india ink injection in dogs. Bronchoplastic surgery at the right main bronchus was performed in all dogs, and the bronchial artery was reconstructed using the internal thoracic artery in the reconstruction group. The mucosal blood flow was measured at the distal side of the anastomosis. India ink was injected into the aorta in the nonreconstruction group and into the internal thoracic artery in the reconstruction group. The peripheral blood flow had diminished immediately after surgeries to 59% of the baseline value and took 14 days to recover to the baseline value in the nonreconstruction group. However, in the reconstruction group, the blood flow recovered at once to 78% of the baseline value and had returned to that value in 5 days. Statistically significant differences were noted between the groups from just after operation to day 7. India ink data confirmed these findings. In the nonreconstruction group, no ink was observed in the peripheral bronchial vessels on day 3; it was noted in part of the vessels on day 7 and in most on day 14. On the other hand, a relatively large number of vessels were stained just after operation in the reconstruction group. Thus reconstruction of the bronchial artery by means of the anastomosis with the internal thoracic artery can be said to be a useful and effective method for preventing airway ischemia.

Intrathoracic fibrin glue for postoperative pleuropulmonary fistula.

Twenty cases of persistent pleuropulmonary fistula after a thoracic surgical procedure were successfully treated with fibrin glue injection into the thoracic cavity through a drainage tube. A new technique, the overlying method, which is a modification of the conventional technique of fibrin glue injection, was devised. Each patient received one to four injections (mean number, 1.6). This method resulted in closure of the pleuropulmonary fistula in all but 1 patient. Minor complications included pain in 1 patient and fever in 3 patients. A control study involving 24 patients with pleuropulmonary fistula was performed. Autoblood, OK-432, and tetracycline were used instead of fibrin glue. The fistula resolved in 12 patients. Complications after injection included pain in 15 patients, fever in 10, and occlusion of the chest tube in 4. With our new method, patients rarely exhibited pain and fever, and occlusion of the chest tube did not occur.

Submucosal dissection of the esophagus: a case report.

A 38-year-old male was admitted to our hospital complaining of dysphagia, sore throat and fever. An upper GI series revealed a "double-barrelled" esophagus. Endoscopic examination of the esophagus showed multiple ulcers and mucosal bridges resulting from laceration of the submucosal layer. An acute ulcer of the stomach was also found. Fasting and total parenteral nutrition for several weeks failed to bring about any changes in the endoscopic findings. As treatment, the mucosal bridges throughout the esophagus were divided with a diathermy knife under endoscopic control. Soon after the procedure, the patient's symptoms disappeared completely, and the massive dissection of the esophagus had improved except for slight depressions.

Role of hydrosulfide ions (HS-) in methylmercury resistance in Saccharomyces cerevisiae.

Methylmercury-resistant mutants were obtained from Saccharomyces cerevisiae. They were divided into two complementation groups, met2 (homoserine O-acetyltransferase deficiency) and met15 (enzyme deficiency unknown), as reported previously. It was found that met15 was allelic to met17 (O-acetylserine and O-acetylhomoserine sulfhydrylase deficiency). Methylmercury toxicity was counteracted by exogenously added HS-, and both met2 and met17 (met15) mutants overproduced H2S. On the basis of these results, we conclude that met2 and met17 (met15) cause accumulation of hydrosulfide ions in the cell and that the increased level of hydrosulfide is responsible for detoxification of methylmercury.

[Complete resection of thymic carcinoma supported by cardiopulmonary bypass].

We would like to report on a case of thymic carcinoma that could be completely resected supported by extracorporeal circulation. A 53-year-old female complaining of severe coughing and hoarseness was admitted to our hospital. Her chest X-ray film revealed an abnormal shadow in the upper mediastinum and an elevation of the left diaphragm. Chest CT and MRI showed an anterior mediastinal mass that invaded the main pulmonary artery. The preoperative histological diagnosis of thymoma was made by mediastinoscopic specimen. After 50 Gy irradiation, surgery was performed. During the operation, after resection of the left brachiocephalic vein and a part of the upper lobe of the left lung, the tumor was detached from the wall of the aorta and resected with the invaded part of the pulmonary artery supported by cardiopulmonary bypass. Reconstruction of the pulmonary wall defect was accomplished with Xenomedica. The final pathological diagnosis was thymic adenosquamous carcinoma. The postoperative course has been uneventful 24 months after surgery. Extracorporeal circulation is a useful technique in operations involving malignant diseases when complete resection is able to be accomplished.

[Physical development of patients with spontaneous pneumothorax].

The physical development of 14 patients with spontaneous pneumothorax was studied. At the time of operation, the mean age of these 14 patients was 21.2 years, height 172.5 cm, weight 54.1 kg and Rohrer's index was 104.6, indicating marked ectomorphism. Changes in the height and body weight of these 14 patients were compared with the standard curves estimated from the national averages. The height of these patients was greater than the national average already at the age of 6 years, showed a marked increase at 11-14 years, but the increase slowed down from age 15 on. Their body weight was less than the national average at the ages of 11 and thereafter, and its increase was remarkably slower at the age 15 and beyond. Consequently, Rohrer's index was particularly low at the ages of 11-15 years. These results suggest that the ectomorphism of patients with spontaneous pneumothorax is already apparent in early childhood and is enhanced by the delay in the increase in weight as compared with height in the period of rapid physical development. The rapid increase in the vertical dimension of the thoracic cavity relative to the increase in its horizontal dimension in this period is considered to affect the intrathoracic pressure in the apical region and may promote cyst formation.

[Para-esophageal anastomosis of left atrium in orthotopic heart transplantation in dogs: possibility for electrophysiologic examination of the transplanted heart with transesophageal atrial stimulation].

Para-esophageal anastomosis of left atrium was examined in orthotopic heart transplantation as a new surgical technique for transesophageal electrophysiologic study of the transplanted heart. Seven orthotopic heart transplantations were performed in mongrel dogs. Left atrial appendage of the donor heart was trimmed as a flap and the sinus venosus of right atrium was incised posteriorly. An upper midline incision was made in the posterior wall of left atrium of the recipient after native ventricles were resected. Left atrial appendage flap of the donor heart was anastomosed to the posterior incision of native left atrium, and the remains of left atrium were sutured continuously. Sinus venosus incision of the donor heart was anastomosed to the right atrium of recipient. Ascending aorta and pulmonary trunk were reconstructed in the standard fashion. Transesophageal atrial pacing was possible during and after weaning from cardiopulmonary bypass in all seven cases. The appendage flap was found to occupy one third of the posterior wall of left atrium at the time of sacrifice. It was suggested that para-esophageal anastomosis of left atrium would be an useful technique for transesophageal electrophysiologic study of the transplanted heart.

Effect of amitriptyline or phenobarbital on the activities of the enzymes involved in rat liver.

The effect of phenobarbital (PB) or amitriptyline (AMT) on phospholipid metabolism was studied in rat liver. Administration of PB or AMT caused a marked increase in the microsomal phosphatidylcholine (PC) content. In this context, the activities of glycerophosphate acyltransferase (GAT), phosphatidate cytidylyltransferase (PCT), phosphatidate phosphohydrolase (PPH) and choline phosphotransferase (CPT) in the liver were all increased by PB. In contrast, the activities of GAT and PCT were little affected by AMT, while those of PPH and CPT were significantly increased by AMT. These findings suggest that AMT or PB would increase hepatic microsomal PC synthesis through inducing PPH and then CPT.

Solubilization and characterization of a ouabain-sensitive protein from transverse tubule membrane-junctional sarcoplasmic reticulum complexes (TTM-JSR) in cat cardiac muscle.

A new glycoprotein of 31,500 dalton, which has a high affinity for ouabain, and is independent of (Na+-K+)-ATPase, was solubilized from transverse tubule membrane and junctional sarcoplasmic reticulum complexes (TTM-JSR) of cat cardiac muscle. This protein could be extracted only in small amounts from sarcolemma-plasma membrane (SLM-PL) fragments, suggesting that it indeed originates from the TTM-JSR.

Acute effect of amitriptyline, phenobarbital or cobaltous chloride on delta-aminolevulinic acid synthetase, heme oxygenase and microsomal heme content and drug metabolism in rat liver.

Cobaltous chloride (CoCl2) caused very marked decreases of cytochrome P-450, b5 and total heme contents and an increase of heme oxygenase activity. On the contrary, phenobarbital (PB) increased hepatic drug-metabolizing enzymes, but the total heme content remained unchanged. On the other hand, amitriptyline (AMT) caused a marked increase of delta-aminolevulinic acid (delta-ALA) synthetase activity at 12 and 24 hr. In addition, the contents of total heme and cytochrome b5 and the activities of aminopyrine (AM) N-demethylase and aniline (AN) hydroxylase at 24 hr were also increased by AMT, whereas cytochrome P-450 content did not change. This may be explained by the fact that AMT would increase hepatic heme synthesis through the prolonged induction of delta-ALA synthetase, but it may not cause an increase in cytochrome P-450 heme because there are increases in the contents of cytochrome b5 and total heme.

5-Aminolevulinate Synthetase

Effects of perphenazine, chlorpromazine or CoCl2 on the activities of delta-aminolevulinic acid synthetase and heme oxygenase and on the content of hemoprotein in rat liver.

Administration of CoCl2 caused a marked decrease of hepatic drug-metabolizing enzymes and the induction of delta-aminolevulinic acid (delta-ALA) synthetase and heme oxygenase. Under the same experimental condition, the inverse relationship between the decrease of hepatic drug-metabolizing enzymes and delta-ALA synthetase activity and the increase of heme oxygenase activity was observed in perphenazine (PPZ)- or chlorpromazine (CPZ)-treated rats. However, the decrease of cytochrome P-450 and aniline hydroxylase by CPZ was later restored or increased over the control level. In addition, CPZ resulted in a marked decrease of total heme content, but this content was not changed by PPZ.

5-Aminolevulinate Synthetase

Preparation of the bifidus milk powder.

Malt extract and skim milk were used as the culture medium for bifidobacteria. Bifidobacterium longum E194b (ATCC 15707) selected from among four strains grew well in a nitrogen atmosphere regardless of the initial dissolved oxygen level of the culture medium. The optimum temperature range for growth was 34 to 37 degrees C. The viable cell numbers of Bifidobacterium in freeze-dried powder from neutralized bifidus milk were 20 times those found in powder from nonneutralized milk after storage for 30 d at 35 degrees C. The optimum residual water content in the powder for bacterial stability was about 3.5% or less. Bifidus milk powder prepared under these optimum conditions contained at least one billion viable cells even after storage for 30 d at 35 degrees C. The powder must be sealed with material made of aluminium for storage.

Effects of perphenazine, diazepam and nitrazepam on amitriptyline metabolism in rat.

Repeated coadministration of amitriptyline (AMT) and perphenazine (PPZ) decreased the AMT-induced increase of hepatic microsomal aminopyrine N-demethylase, aniline hydroxylase, NADPH-cytochrome c reductase and UDP-glucuronyltransferase (UDP-GT) activities in rats. This may be explained by a fact that inhibition of AMT metabolism in hepatic microsomes by PPZ led to increase the non-conjugated formation of AMT and its metabolites in the liver, brain, urine and feces. On the other hand, a slight decrease of drug metabolizing enzyme activity by diazepam (DZP) but not by nitrazepam (NTZ) was enhanced by combined treatment of AMT, whereas the enhancement of UDP-GT activity by DZP or NTZ was suppressed by coadministration of AMT. At the same time there was no clear increase of non-conjugated formation of AMT metabolites in rat urine and feces during combined treatment.
