Cytologic and hormonal findings in a carcinoid tumor of the uterine cervix.

A carcinoid tumor of the cervix in a 40-year-old woman was studied by cytology, histology, electron microscopy, immunohistochemistry and hormonal analysis. The preoperative cytologic and histologic findings strongly suggested a carcinoid tumor of the cervix. The serum serotonin level was elevated; immunohistochemical studies demonstrated the presence of serotonin in the cytoplasm of the tumor cells. Following radical hysterectomy, the concentration of serotonin was measured in the excised tumor; it was about 20 times higher than the level seen in normal cervical tissue, confirming that the tumor was a serotonin-secreting carcinoid of the uterine cervix.

Antitumor activity of calcium in combination with antitumor agents against Lewis lung carcinoma.

The antitumor activity of calcium gluconate in combination with mitomycin C and 5-fluorouracil was examined against subcutaneously implanted Lewis lung carcinoma-bearing C57BL/6 mice. The mice were divided into four groups: group 1 received mitomycin C (2 mg/kg) and 5-fluorouracil (50 mg/kg) intraperitoneally once a week for four weeks beginning from the day after implantation of tumors, as well as calcium gluconate (155 mg/kg) twice a week for the same four weeks; group 2 received only mitomycin C and 5-fluorouracil; group 3 received only calcium gluconate; group 4 received a vehicle (physiological saline). Significantly enhanced inhibition of tumor growth was observed neither in a comparison between groups 3 and 4, nor in a comparison between groups 1 and 2 (expect on day 20 post implantation). Thus calcium gluconate given alone or in combination with antitumor agents hardly appeared to possess effective antitumor activity.

Estrogen formation from cholesterol in rabbit preimplantation blastocysts and corpora lutea in vitro.

Rabbit blastocysts and corpora lutea (CL) obtained six days after coitus were incubated with [4-14C]cholesterol. In the CL, the conversion of [4-14C]cholesterol to radioactive progesterone was about twice that to radioactive estrone. On the other hand, in the preimplantation blastocyst, estrone formation from the substrate was far higher than progesterone formation. Both in the blastocyst and CL, [4-14C]cholesterol was least incorporated into estradiol among the steroids identified. It is suggested that steroids biosynthesized in rabbit preimplantation blastocysts and CL are quite different both in quality and in quantity at the time of implantation.

Antitumor activity of ascorbic acid in combination with antitumor agents against Lewis lung carcinoma.

Antitumor activity of ascorbic acid in combination with antitumor agents (mitomycin C and 5-fluorouracil) was examined against subcutaneously implanted Lewis lung carcinoma-bearing C57BL/6 mice by feeding them an ascorbic acid-deficient diet. The mice were divided into four groups: group 1 received intraperitoneally mitomycin C (2 mg/kg) and 5-fluorouracil (50 mg/kg) once a week for four weeks beginning from the day after implantation of tumors, as well as ascorbic acid (1000 mg/kg) twice a week for the same four weeks; group 2 received only mitomycin C and 5-fluorouracil; group 3 received only ascorbic acid; group 4 received a vehicle (physiological saline). Tumor growth of group 1 compared with the other three groups, and that of group 2 compared with groups 3 and 4, was significantly inhibited by day 13 post implantation. Histological examinations of tumor tissues at 10 days after implantation of tumors already showed degenerative changes which indicated these antitumor effects.

Glucose metabolism in pediatric renal transplant recipients: relation to insulin receptors of erythrocytes.

We studied the oral glucose tolerance test (O-GTT) of 13 kidney transplant recipients and compared the results with the insulin binding characteristics of their own erythrocytes. They had mild renal insufficiency with significant increase of serum creatinine concentrations. Body weights were slightly but significantly elevated compared to the controls. All were receiving small doses of prednisone (0.2-0.3 mg/kg/day). Ten of the 13 patients had normal O-GTT and normal binding of 125I-insulin, while the remaining 3 patients showed abnormal O-GTT and significantly reduced maximum binding of 125I-insulin to erythrocytes. Basal insulin concentration and response to O-GTT were significantly elevated in the patients, regardless of O-GTT being normal or abnormal. It is concluded that transplant recipients have an impaired insulin action due to a post-receptor stage abnormality in glucose metabolism which is due perhaps to mild renal insufficiency, mild obesity and prolonged administration of the small dosage of prednisone.

Local etoposide injection for treatment of tubal pregnancy with cardiac activity.

Etoposide was injected locally under sonographic management for the treatment of tubal pregnancy with cardiac activity in the distal region of the tube; the serum beta-hCG level was 14,000 mIU/mL. Two doses of 50 mg each, three days apart, were employed. No side effects appeared either during or after the treatment. Hysterosalpingography performed 1 month 3 weeks after treatment demonstrated the patency of the affected tube. Local etoposide instillation may be an option in the management of unruptured ectopic pregnancies, even in the presence of fetal cardiac activity and high trophoblastic activity.

Clinicopathologic characteristics and survival of elderly patients with gastric cancer.

From 1980 to 1991, Japanese patients aged over 49 years with gastric cancer underwent gastric resection in our hospitals. For various studies, two groups, 35 patients over 79 years (advanced age group) and 884 patients aged 50-79 years (mature age group) were prepared. A retrospective study was done with special reference to clinicopathological features and the prognosis. There were no differences in clinicopathological characteristics between the two groups, except for operative procedures and the size, histology and vascular involvement of the primary tumor. The survival rate for the advanced age group was higher than that for the mature age group, with a significant difference at p = 0.00556. However, the postoperative morbidity rate was 48.6% in the advanced age group compared with 5.1% in the mature age group. The most frequent complications were respiratory-related, that is, atelectasis, pneumonia or aspiration pneumonia. Based on these data, advanced age is not a contraindication for resection of a gastric cancer, but patients with serious pulmonary and/or cardiac diseases would not be candidates.

Hyperthermia-induced antitumor activity in human gastric cancer cells serially transplanted into nude mice.

Hyperthermia-induced antitumor activity was assessed histopathologically and findings related to transplantation of human gastric cancer into nude mice were examined. Fragments of human gastric cancer were incubated at 37 degrees C to 47 degrees C for various durations of time, then were evaluated either histologically or with regard to delay in tumor growth and the rates of transplantation into nude mice. Fragments exposed to 39 degrees C for 30 min to 120 min and 41 degrees C for 30 min did not differ from findings in the controls concerning tumor growth and transplantability. In the case of 41 degrees C for 60 min or more, the rates of transplantation decreased significantly and there was a delay in tumor growth. At 43 degrees C for 120 min, 45 degrees C for 30 and 60 min, and 47 degrees C for 15 to 60 min, transplantability was nil. In the groups with a delay in tumor growth, there were irreversible changes in nucleic and cytoplasmic components, and in 6 groups with no evidence of transplantability, complete destruction of the glandular structure, pyknosis and karyorrhexis as well as disappearance of the cytoplasm occurred in almost all cases. Thus, the thermal dose of 43 degrees C for 120 min seems to be the minimally effective one for hyperthermia-induced tumoricidal activity.

Clinicopathologic characteristics of gastric cancer patients with cancer infiltration at surgical margin at gastrectomy.

Although curative surgery is desirable in patients with gastric cancer, tumors adjacent to the esophagogastric and/or gastroduodenal junctions present surgeons with some difficulty in estimating whether or not the lesion has infiltrated beyond the surgical margin. We report herein a retrospective analysis with respect to the clinicopathologic features of the primary lesion and margin positivity for tumor cells. Between 1982 and 1993, 861 gastric cancer patients underwent gastrectomy in our clinics. Of these, 340 had early cancer and the remaining 521 advanced cancer. Cancer infiltration at the surgical margin was determined macroscopically in the fresh resected specimen; re-resection was carried out immediately for positive cases and, subsequently, a rapid histologic examination at the newly-incised edge was carried out intraoperatively. Of the 340 patients with early cancer. 15 (4.4%) had a positive surgical margin which was directly resected successfully. Of the 521 patients with advanced cancer, 73 (14%) had a positive surgical margin and 28 of them had a microscopically negative surgical edge after re-resection; however, 8 others had a positive result at the newly-excised edge after re-resection, and the remaining 37 could not undergo re-resection because of their poor general condition and/or because the tumor had spread to other sites. The positive rate for the final surgical margin was 5.2% (45/861 patients). All of the patients with a positive margin and early cancer had a superficial or excavative type lesion, and 76.7% (56/73 patients) of those with advanced cancer had Borrmann's III or IV type lesion. These findings suggest that in such patients with a tumor adjacent to the esophagogastric and/or gastroduodenal junctions, particular attention should be paid to Borrmann's III or IV lesions in advanced cancer and to superficial or excavated type lesions in early cancer in order to reduce the frequency of positive surgical margin. Additionally, an immediate histologic examination after re-resection is extremely important.

Non-grafting method of vaginal construction for patients of vaginal agenesis without functioning uterus (Mayer-Rokitansky-Küster syndrome).

We have treated 18 patients with vaginal agenesis without functioning uterus (MRK syndrome) during the period between January 1984 and December 1995 by Frank's dilatation method followed by the surgical method without grafting. Clinical records were retrospectively examined in regard to the size and functions of the constructed vagina and problems of the treatment procedure. The treatment by non-invasive dilatation was first carried out starting at the age of 21 years 2 months +/- 2 years 9 months (Mean +/- SD) for the duration of 10.9 +/- 9.8 months. It was successful (constructed vaginal length by pressure: 60 mm or more) in 6 patients (35.3%). The non-grafting surgical method was then performed. The operation time was 2:11 +/- 0:37 hours and no complications were observed. The vaginal lengths 1 month after operation and at the latest follow-up were enough to have coitus and no severe complications including shrinkage were observed. Our method to construct a new vagina is simple and useful, since it left no operation scar in contrast to McIndoe's method and other methods with grafting.

Peritoneal cytology and its prognostic value in endometrial carcinoma.

BACKGROUND: There has been a controversy about the prognostic significance of positive peritoneal cytology in endometrial carcinoma. MATERIALS AND METHODS: Peritoneal cytology was obtained at the time of surgery, including systematic retroperitoneal lymph node dissection, in 114 patients. RESULTS: The incidence of positive peritoneal cytology was 35.1%. The 5-year survival rates of the stage IIIA and IIIC (FIGO, 1988) cases were 82.8% and 58.3%, respectively. In pathological stage I (the disease was histologically confined to the uterine corpus), there was no significant difference in 5-year survival rates between patients with and without positive peritoneal cytology. Though the patients in stage IIIA who had only positive peritoneal cytology were given no postoperative therapy unless they had extrauterine disease, no patients developed recurrence. In stages IIIC and IV, the prognosis was significantly poorer for patients with positive peritoneal cytology than for those with negative cytology. CONCLUSION: Positive peritoneal cytology is not an adverse prognostic factor endometrial carcinoma if disease is limited to the uterus.

Intraperitoneal administration of cytomegalovirus hyperimmunoglobulin to the cytomegalovirus-infected fetus.

Twenty-five percent of cytomegalovirus (CMV)-infected fetuses had sequelae and 8% of those in the recurrent-infected group had sequelae. There is no report yet on the fetal therapy for CMV infections. A Japanese pregnant woman with intrauterine fetal CMV infection diagnosed at 26 weeks of pregnancy is presented. CMV culture of amniotic fluid was positive. A CMV DNA assay using the polymerase chain reaction method of the cord blood and the amniotic fluid was positive during the pregnancy; however, testing for fetal serum CMV-specific IgM was negative. The CMV IgG titer of fetal serum at 27 weeks of pregnancy was a third of that of the maternal serum. CMV hyperimmunoglobulin was injected into the fetal abdominal cavity at 28 and 29 weeks of pregnancy. A second administration of CMV hyperimmunoglobulin increased the titer of CMV IgG in the fetal circulation. At birth, the urine culture was positive for CMV. However, CMV DNA of the ascites became negative. A brain CT scan performed 2 weeks after birth revealed some small calcifications beside the right ventricle. CMV hyperimmunoglobulin injection to the fetal abdominal cavity has been shown to increase the IgG in the fetal serum. This is the first report of fetal therapy of congenital CMV infection.
