Chemical degradation of dipicolinic acid-C14 and its application in biosynthesis by Penicillium citreo-viride.

Kanie, Matuso (Kagoshima University, Kagoshima, Japan), Shigeo Fujimoto, and J. W. Foster. Chemical degradation of dipicolinic acid-C(14) and its application to biosynthesis by Penicillium citreo-viride. J. Bacteriol. 91:570-577. 1966.-A chemical degradation of dipicolinic acid-C(14) has been worked out, enabling determination of the specific radioactivity of the carboxyl-carbons (carbons-7 and -8), and of the following carbons of the pyridine ring: carbons-2 and -6 combined, carbons-3 and -5 combined, and carbon-4. The degradation was applied to dipicolinic acid synthesized by washed, submerged mycelium of the mold from glucose and C(14)O(2), and from glucose-1-C(14), -2-C(14), and -6-C(14). The distribution of radioactivity within the labeled dipicolinic acids is consistent with operation of respiratory cycles and with the incorporation of one molecule of CO(2) in the pyridine acid. A C(3) compound is a primary building block. The C(7) chain is believed to result from a C(3) plus C(4) condensation, pyruvic acid and aspartic acid beta-semialdehyde being proposed as likely precursors. Other aspects of the biosynthesis of C(7) open-chain compounds and of dipicolinic acid are discussed.

Changes in platelet phospholipase C protein level and activity in Alzheimer's disease.

We have previously demonstrated that PLC-delta was abnormally accumulated in autopsied brains with Alzheimer's disease (AD). As nonneuronal tissue involvement in AD is also suggested and PLC activity is reduced in AD platelets, we examined the changes of the protein level of PLC-delta and its enzyme activity in platelets taken from patients with AD and age-matched controls. PLC-delta in human platelets was detected as a 72 kDa protein using a specific antibody against PLC-delta. Western blots revealed that the protein level of PLC-delta was significantly higher in the cytosolic fraction prepared from AD platelets compared to controls. We investigated the activity of PLC-delta which hydrolyzes phosphatidylinositol and found that the PLC-delta activity in the cytosolic fraction from AD platelets was significantly reduced compared to the control. This finding that the enzyme activity per PLC-delta molecule is reduced in AD platelets is consistent with the study using Alzheimer brains. These results suggest that aberrant phosphoinositide metabolism is present in nonneuronal tissues as well as the brains of patients with AD.

Serial MRI findings of benign poliomyelitis.

A 14-year-old boy presented with incomplete transverse myelopathic symptoms associated with mild encephalitis. Magnetic resonance imaging (MRI) revealed involvement of the central portion of spinal cord extending from the C1 to T5 level on T2-weighted images with expansion of the cervical spinal cord. The boy made a full clinical recovery in a few weeks. Cultures of the pharyngeal exudate were positive for ECHO virus type 11. The lesion on MRI disappeared completely in a few months.

The diagnosis of pancreatic cancer by endoscopic ultrasonography.

Endoscopic ultrasonography was evaluated for detection of tumors of the pancreas. The technique was performed in 42 patients with cancer of the pancreas and 8 patients with nodular fibrosis of chronic pancreatitis. The lesions were clearly identified in all 50 patients, even when the size of the tumor was less than 20 mm in diameter. A tumor of the pancreas was usually visualized as a hypoechoic mass that had a characteristic image depending on the size. Ultrasonographic differentiation of malignant from benign tumors of the pancreas was possible in tumors larger than 30 mm in size but difficult in tumors less than 20 mm in size. Compared with conventional ultrasonography, endoscopic retrograde pancreatography, computed tomography, and angiography, endoscopic ultrasonography had the highest detection rate of tumors of the pancreas, especially in cases of small tumors under 20 mm in size. In cancer of the pancreas, endoscopic ultrasonography also proved to be valuable for the detection of vascular invasion.

TSH-receptor antibodies in mothers with Graves' disease and outcome in their offspring.

Blood was taken from 56 selected newborn babies whose mothers had Graves' disease, to assess the relation between their thyroid function and the presence of thyrotropin (TSH) binding inhibitor immunoglobulins (TBII) and thyroid stimulating antibodies (TSAb) in maternal serum. All the mothers of the thyrotoxic babies had both these TSH receptor antibodies in their serum. However, most of the mothers whose thyroid function had been well controlled in pregnancy gave birth to normal babies. 15 babies had a transient syndrome of low serum thyroxine (T4) and free T4 with normal TSH and this tended to be associated with TSH receptor antibodies in maternal serum (TBII 9/15, TSAb 4/15). 2 infants had transient hyperthyroxinaemia without hyperthyroidism and both their mothers showed strong TSAb activity without TBII activity.

Does the progression of myocardial fibrosis lead to atrial fibrillation in patients with hypertrophic cardiomyopathy?

The majority of left ventricular (LV) inflow volumes in hypertrophic cardiomyopathy (HCM) depend on atrial contraction because of impaired LV relaxation. If HCM is complicated by atrial fibrillation (AF), heart failure can develop because of the loss of atrial contraction. The purpose of this study was to determine the relationship between the development of AF and myocardial fibrosis or intramyocardial small artery (IMSA) stenosis in autopsied hearts with HCM. Studies were performed in five HCM hearts with AF (AF group) and five HCM hearts without AF (non-AF group). LV specimens were divided into the inner (IT), middle (MT), and outer (OT) thirds. We selected at random 120 fields and 20 IMSAs from each layer and assessed them quantitatively using an image analyzer. We determined the extent of fibrosis (%F) and the degree of stenosis of each IMSA (%L). The %F in the AF group was greater than in the non-AF group (P<.01). In the AF group, the %F of the IT was greater than in the MT and the OT (P<.01). In the non-AF group, the %F of the IT was greater than in the MT (P<.05), and the %F of the MT was greater than in the OT (P<.01). The %L was similar in the AF and non-AF groups. In both groups, the %L of the IT was lower than in the MT (P<.01), which was lower than that of the OT (P<.05). LV fibrosis is more severe in patients with HCM and AF than in those without AF. Therefore, myocardial fibrosis might impair LV relaxation, resulting in hemodynamic intolerance to AF.

Isolation of an excision product of T-cell receptor alpha-chain gene rearrangements.

The genes for the T-cell receptor, like the immunoglobulin genes, are rearranged as DNA. The mechanism of this rearrangement is not clear; unequal crossover between chromosomes and the looping-out and excision of the excess DNA have both been suggested. We isolated small polydisperse circular (spc) DNAs from mouse thymocytes and cloned them into a phage vector. Of the 56 clones we analysed, nine contained sequences homologous to T-cell receptor alpha-chain joining (J alpha) segments. We have characterized one of these clones; it contains one J alpha segment, and the product out of the recombination of a variable region of the alpha-chain gene (V alpha) with a J alpha gene segment. This is the first demonstration of the presence in extrachromosomal DNA of a reciprocal recombination product of any rearranging immunoglobulin or T-cell receptor gene. The finding verifies that V alpha-J alpha joining can occur by the looping-out and excision of chromosomal DNA.

Increased apoptosis of human fetal membranes in rupture of the membranes and chorioamnionitis.

Apoptosis is thought to participate pathophysiologically in the rupture of human fetal membranes (ROM). The aim of this study was to assess apoptosis of the amnion and the chorion in relation to ROM and chorioamnionitis (CAM). The amnion and chorion at the position of the cervical os and fundus of the uterus were obtained from 44 patients. Apoptotic DNA fragmentation was densitometrically determined, and the relative ratio was used for the quantitative evaluation. Among patients without CAM, the relative ratios of apoptosis in the amnion from patients with ROM were higher than those in patients without ROM (P< 0.05). Among patients without ROM, the apoptotic levels in the amnion from patients with CAM were higher than those in patients without CAM (P< 0.05). These were the cases with the amnion at the position of cervical os and fundus, but not with the chorion. The highest ratio of apoptosis was seen in the amnion from patients with CAM and ROM. Among patients with ROM and no CAM, the apoptotic levels at the cervical os in the amnion (P=0.059) and chorion (P< 0.05) was higher than those at the fundus. The increased apoptosis of human fetal membranes was related to ROM and CAM. Apoptosis plays a role in the pathophysiology of ROM.

Clinical pilot studies on pre-operative hyperthermic tumour ablation for advanced breast carcinoma using an 8 MHz radiofrequency heating device.

The multimodality treatment approach for advanced breast cancer provides survival advantages with decreased locoregional and distant recurrences, but these intensive anti-tumour treatments cause severe myelosuppression. Thus, in this study, the usefulness of pre-operative anti-tumour treatment without myelosuppression was investigated. Nine patients with advanced breast carcinoma underwent pre-operative hyperthermic tumour ablation (HTA) using an 8 MHz radiofrequency (RF) heating device (Thermotron RF-8) combined with a grounded needle electrode. The patients had a mean age of 58.3+/-13.9 years and included four patients with stage IIIA, two with stage IIIB and three with stage IV cancer. The target temperature was over 50 degrees C. They tolerated pre-operative HTA therapy well with no early or late complications. The initial mean tumour size was 122.1+/-71.5 cm3 and the post-HTA tumour size was 82.2+/-63.4 cm3; the reduction rate was significant (p = 0.000 293). After the pre-operative HTA, all patients underwent surgery with Level III nodal extirpation. Post-operatively, no locoregional recurrence was observed. Microscopic examination of the primary focus showed complete coagulation necrosis expanding for a diameter of 3.5-5.0 cm. Taken together, the pre-operative HTA was a safe, well-tolerated and effective treatment, achieving tumour reduction as well as complete coagulation necrosis that resulted in a large volume of destruction in breast cancer tissue.

Conversion of thyroxine into tri-iodothyronine in zinc deficient rat liver.

Hepatic thyroxine (T4) to tri-iodothyronine (T3) conversion was measured in six animal groups: Group A was fed a severe zinc-deficient diet (1.98 ppm) for 5 weeks; group B was a pair-fed control group for group A; group C was fed a less severe zinc-deficient diet (6.10 ppm) for 5 weeks; group D was a pair-fed control group for group C; group E was fed a zinc-supplemented control diet (90.4 ppm) for 5 weeks; and group F was first fed the severe zinc-deficient diet for 5 weeks and then placed on the zinc-supplemented control diet until a body weight corresponding to the final weight of group E was obtained. Serum T3 and T4 levels and T4 to T3 conversion were significantly reduced in group A. A significant positive correlation was observed between T4 to T3 conversion and alcohol dehydrogenase (zinc-metalloenzyme) activity in liver tissue from the six groups. Thus, it appeared that an appropriate amount of zinc might be one of the factors in thyroxine to tri-iodothyronine conversion in liver tissue normally.

Chemosensitivity test for repeated arterial infusion chemotherapy by reservoir for unresectable hepatocellular carcinoma.

The experimental and clinical usefulness of a chemosensitivity test (Nuclear Damage Assay) was studied. Karyologic degenerative changes were observed as an indicator of drug sensitivity, in repeated arterial infusion chemotherapy (RAIC) using a reservoir for advanced hepatocellular carcinoma (HCC). In the experimental study, this sensitivity test was performed using five liver cell lines against 15 drugs. At the same time, the succinate dehydrogenase inhibition (SDI) test was also performed. Comparison of the results between these two tests gave a high consistency rate of 81%. Clinically, the karyologic sensitivity test was carried out in 135 patients with unresectable HCC. Drug sensitivity could be evaluated in as many as 89% of the total 135 patients. Of the patients, 43 received RAIC on an outpatient basis via a subcutaneously implanted reservoir. The objective response of RAIC on tumours of the 43 patients was evaluated as complete response, partial response, in 3 (9%) and 8 (23%) in 35 patients treated with positive drugs (positive group), and as 0 (0%) and 0 (0%) of 8 patients treated with negative drugs (negative group), respectively. As regards the prognosis, 1 year and 1.5 year survival rates were 70 and 45% in the positive group, and 42 and 0% in the negative group, respectively. As objective response in the positive group tended to be better than that in the negative group, and prognosis in the positive group was significantly better than that in the negative group, this sensitivity test appears to contribute to the improvement of therapeutic results if used to select drugs suitable for RAIC for advanced HCC.

CYP1A1 polymorphism and risk of gynecological malignancy in Japan.

The incidence of endometrial cancer and ovarian cancer in Japan has been increasing in recent years. Results of epidemiologic studies suggest that the onset and multiplication of these cancers are associated with estrogen. Estrogens are metabolized by cytochrome P450 1A1 (CYP1A1) and converted into catecholestrogens, which are carcinogens. CYP1A1 has several polymorphisms, the major one being T6235C transition in the non-coding 3'-flanking region (MspI polymorphism), and another being A4889G transition in exon 7 (Ile/Val polymorphism). These polymorphisms can affect the metabolites of estrogens and contribute to the susceptibility to gynecological malignancy. In this study, to determine whether CYP1A1 polymorphism plays a role in the development of gynecological malignancy in the Japanese population, we assessed the association of CYP1A1 polymorphism in Japanese patients with gynecological malignancy in comparison to that in controls. The odds ratios (ORs) of Ile/Val polymorphism were 1.16 in ovarian cancer patients and 1.70 in endometrial cancer patients. The ORs of MspI polymorphism were 1.33 in ovarian cancer patients and 0.88 in endometrial cancer patients. No significant association was found between these CYP1A1 polymorphisms and gynecological malignancy. Although the frequency of CYP1A1 polymorphism in the Japanese population is higher than that in the Caucasian population, CYP1A1 polymorphism is not related to gynecological malignancies in Japanese population.

Magnetic resonance imaging in athetotic cerebral palsied children.

The magnetic resonance findings in 22 children with athetotic cerebral palsy were studied. Sixteen had perinatal asphyxia, two had neonatal jaundice, and four had no association with predisposing conditions. In six of the children, symmetrical high intensity areas were found in both the thalamus and putamen in T2-weighted images. In five children they were seen only in the thalamus, and in another one only in the putamen. In six children, symmetrical periventricular high intensity areas were seen. In seven of the subjects, no abnormal magnetic resonance findings were seen. Magnetic resonance lesions, possibly caused by asphyxia, were found in the basal ganglia, thalamus and/or cerebral white matter in 14 of 16 children. Three children with lesions only in the cerebral white matter had mild motor abnormality, and six children with lesions in both the thalamus and putamen did not have mild abnormality.
