Pharmacokinetic analysis in intraperitoneal hyperthermic chemotherapy of far-advanced gastric cancer patients.

The pharmacokinetics of mitomycin C (MMC) has been evaluated in 10 far-advanced gastric cancer patients by means of an intraperitoneal hyperthermic perfusion (IPHP) using MMC to prevent and/or treat peritoneal dissemination. Further, misonidazole (MIS), which was used as a thermosensitizer, also was evaluated. The IPHP was performed for 120 min right after surgical procedure, using a closed-circuit with an inflow perfusate temperature from 46.3 approximately 47.5 degrees C and an outflow temperature of 44.0 approximately 46.0 degrees C. The MMC level in the perfusate was 10 micrograms/ml at the onset of IPHP and thereafter decreased to 1.7 +/- 0.4 micrograms/ml minutes later. The average AUC (area under the curve) in the perfusate was 3.3 micrograms/ml during the IPHP. The MMC level in the peripheral blood was 0.15 +/- 0.04 micrograms/ml 30 min after start of the IPHP and increased to 0.18 +/- 0.05 micrograms/ml at the end of IPHP. Additionally, the MIS level in the peripheral blood was 64.7 +/- 10.3 micrograms/ml 60 min after the IPHP, 60.6 +/- 9.2 micrograms/ml at 120 min, and then decreased to 32.8 +/- 10.3 micrograms/ml at 12 hours after IPHP. The AUC was calculated as being 975 micrograms.hr/ml. Again, the level in the portal vein blood was calculated from the peritoneal permeability and the peripheral blood level of the drug. From data, it was concluded that the perfusate and portal vein blood level of MMC, the peripheral blood level of MIS, as well as the perfusate temperature, are important in providing a favorable, safe, antitumoral treatment.

[Clinical evaluation of intraperitoneal hyperthermic perfusion in far-advanced gastric cancer].

Nineteen far-advanced gastric cancer patients, including peritoneal dissemination, underwent surgical treatment followed by intraperitoneal hyperthermic perfusion (IPHP). Seven of them had cancerous ascites, 5 had ovarial metastasis, and in 7 the primary tumor extended to the adjacent organs. Postoperatively, ascitic effusion was absent after IPHP, and all patients were discharged. The survival of the IPHP group was compared with that of 27 advanced gastric cancer patients with serosal invasion (control group), who underwent gastrectomy without IPHP. In the IPHP group, the average and 50% survival was 400 and 520 days, respectively, and 1-year survival rate was 73.3%, against 273 days, 250 days, and 31.8%, respectively, in controls. The survival rate for the IPHP group was statistically better than that of the control group (p less than 0.01). In patients with peritoneal dissemination, the average survival and 50% survival for the IPHP group was 396 and 380 days, respectively, and 1-year survival rate was 62.5%, against 204 days, 170 days, and 16.6%, respectively, in controls. Based on these results, IPHP was suggested to be effective for gastric cancer with serosal invasion, especially with peritoneal dissemination.

[Effect of degradable starch microspheres (DSM) on hepatic hemodynamics].

The effect of intra-arterial infusion of degradable starch microspheres (DSM) on hepatic arterial blood flow was measured using a transit-time ultrasonic blood flow meter. Before the infusion of DSM, the mean hepatic arterial flows were 316 +/- 79 ml/min. After infusion, the mean flows decreased to 43 +/- 86 ml/min (86%). RI-angiography using 99mTc-macroaggregated albumin (MAA) was performed to measure hemodynamic changes in the liver. T/N ratio of 99mTc-MAA accumulation was increased from 0.37 to 0.62. Based on the peripheral MMC blood levels after combined infusion with DSM and MMC, the mean AUC with MMC plus DSM was decreased to 55% of that of AUC with MMC alone. These results show that combined use of DSM is effective for intra-arterial chemotherapy against the hepatic cancer.

Hyperplastic cellular components of a hemangiopericytoma. An ultrastructural study.

Based on ultrastructural features of cellular components of a hemangiopericytoma, hyperplastic cells are classifiable into fibroblast-like (group I), endotheloid (group II) and pericyte-like (group III) cells. The transformation of the group I cells to the group II, or to the group III cells, is pronounced in our electron micrographs and this may imply that the group I cell is the principal cell of origin in this neoplasm. The smooth muscle-like (group IV) cells comprising the media of the arteries and veins in this neoplasm may represent modified, possibly de-differentiated smooth muscle cells reacted to the neoplastic proliferation of the surrounding adventitial (group I) cells.

[Exercise induced hypoxemia and exercise tolerance in patients with COPD and the benefits of oxygen supplementation].

In order to evaluate the benefits of O2 supplementation during exercise, slowly incremental treadmill exercise tests were performed twice with 30 minutes interval rest in fourteen patients with severe COPD. The patients breathed room air. 31/min of compressed air by nasal prongs, and 31/min of supplemental oxygen in single blind fashion at random. The patients who developed arterial desaturation below 88% on exercise, group D, showed slight but significant increase in walked distance (397 m vs 424 m) and significant decrease in breathlessness (22.9 vs 16.9) on oxygen as compared to on air. On the other hand in patients without significant arterial desaturation, group S, there was no improvement in those parameters. The increase in walked distance on oxygen was closely related with the decrease in mean inspiratory flow (VT/Ti), blood lactate level, and CO2 production at identical work load. Plasma human atrial natriuretic peptide (h-ANP) levels in group D increased with exercise from a resting value of 27.6 +/- 6.9 to 44.0 +/- 9.0 on compressed air whereas the increase was significantly suppressed to 35.4 +/- 9.0 on oxygen. In group S there was no difference in the increase of plasma h-ANP levels between air and oxygen breathing during exercise (33.1 +/- 5.1 vs 31.9 +/- 9.6). A close correlation (r = 0.908) was found between mean pulmonary artery pressures and plasma h-ANP levels at rest and during exercise performed in four patients breathing air and oxygen. Those findings suggested that arterial plasma h-ANP levels reflected the right ventricular afterload and that they could be used to evaluate the effectiveness of O2-supplementation during exercise.(ABSTRACT TRUNCATED AT 250 WORDS)

Plasma concentration and renal effect of human atrial natriuretic peptide in nephrotic syndrome.

To elucidate the pathophysiological role of atrial natriuretic peptide (ANP) in nephrotic syndrome, the plasma level of ANP and renal response to exogenous human alpha-ANP (alpha-hANP) were measured in untreated adult patients with idiopathic nephrotic syndrome (NS) and compared with those of normal volunteers (NL). The plasma concentration of immunoreactive ANP (ir-ANP) in NS (112 +/- 9.8 pg/ml, n = 9, mean +/- SE) was not significantly different from that in NL (98 +/- 8.0 pg/ml, n = 13). However, a significant positive correlation was observed between the plasma ir-ANP level and blood volume in NS (r = 0.714, p less than 0.05). In an infusion study with synthetic alpha-hANP (25 to 100 ng/kg/min), the urine flow rate increased from 0.67 +/- 0.08 to 7.11 +/- 1.08 ml/min in NL (n = 5, p less than 0.01) and from 0.64 +/- 0.16 to 2.88 +/- 0.70 ml/min in NS (n = 9, p less than 0.05) and the urinary sodium excretion increased from 115 +/- 16 to 466 +/- 62 microEq/min in NL (p less than 0.01) and from 51 +/- 8 to 207 +/- 58 microEq/min in NS (p less than 0.01). The absolute and percent changes in urine flow rate and the absolute change in sodium excretion were lower in NS (p less than 0.05) than in NL. The percent change in sodium excretion in NS did not differ from that in NL. In 2 patients with high plasma ir-ANP concentrations, however, infusion of ANP induced poor sodium excretion (59 and 95 microEq/min at 100 ng/kg/min ANP infusion, respectively). Hemodynamic and renal parameters such as blood pressure, pulse rate and creatinine clearance were similarly affected in both NL and NS. We also found that the urinary excretion of protein was significantly increased in NS (p less than 0.05) during infusion of alpha-hANP. Our data suggest that the plasma level of ir-ANP is regulated by blood volume status, and that the renal responsiveness to ANP, at least in part, contributes to water and sodium retention in NS.

[Clinical efficacies of schizophyllan (SPG) on advanced gastric cancer].

Immunochemotherapy using the antitumor polysaccharide Sizofiran (SPG), an extract from the culture broth of Schizophyllum, was prescribed randomly for unresectable or resectable gastric cancer patients. In case of unresectable cases, the SPG group (treated by SPG plus tegafur--F method--or 5-FU combined with MMC--MF method--) survived longer (p = 0.009 in the MF method and p = 0.003 in the F method) than did the control group (chemotherapy only). Concerning a treatment regimen for resectable cases, all patients were given 0.4mg/kg and 0.2mg/kg of MMC on the day of surgery and the next day, respectively. Oral tegafur was initiated on the 14th postoperative day and was continued as long as possible. In the SPG group, SPG was administered intramuscularly 40mg/week concurrently with the start of oral tegafur and was continued as long as possible. Although the overall survival rates for 5 years did not differ between both groups, the 5-year survival rate for stage III in the SPG group was superior to the findings for stage III in the control (p = 0.0105).

[Primary intracranial T cell type malignant lymphoma: a case report].

Primary intracranial T cell type malignant lymphoma is extremely rare. Only 8 cases are reported in the literature so far. In this paper, a case with this type of malignant lymphoma is reported. A 41-year-old man was admitted because of abnormal behavior. Enhanced CT scan demonstrated high density mass and perifocal low-density area in the right frontal lobe, the right basal ganglia and the periventricular region. Specimen biopsied from right frontal lobe was submitted for histological examination. An immunohistochemical technique using monoclonal surface markers confirmed reactivity for LCA, MT-1, OKT-4 and OKT-8, while there was no reactivity for MB-1 and OKB-2. Pathological diagnosis was diffuse medium T cell type malignant lymphoma. Postoperatively, after 60 Gy irradiation, the tumor disappeared. There is no difference on CT findings between B cell type and T cell type malignant lymphoma.

Combined treatment with surgery and intraperitoneal hyperthermic perfusion (IPHP) for far-advanced gastrointestinal cancer with peritoneal seeding.

In 19 patients with advanced gastric carcinoma intraperitoneal hyperthermia combined with mitomycin C was administered. After this therapy in repeated cytological examinations of the peritoneal effusion malignant cells could be determined. From 19 patients 5 died innert the first 10 months. The other patients lived 12 months without recurrence.

[A case of the thyroidea ima artery arising from the aortic arch].

The present report describes a case of the thyroidea ima artery observed in a 65-year-old Japanese male on dissection in 1988. The thyroidea ima artery arose from the aortic arch between the branchiocephalic trunk and the left common carotid artery. The thyroidea ima passed to the left lobe and isthmus of the thyroid gland for the supply of these. The inferior thyroid artery on the right side was of common type in its origin and course, though that on the left side was absent. Either of the superior thyroid arteries arose from the common carotid artery.

[Somatic cell hybrids used in cytogenetic analysis of transformation mechanism of uterine endometrial cell].

6-Thioguanine and Neomycin resistant human endometrial carcinoma cells were established from parental HHUA 95 cells in the present study. The karyotype was 46,XX although chromosome abnormalities had been observed occasionally. Cell hybrids between 6-TGr neor 95 and rat immortalized, non-tumorigenic 3Y1 cells were formed to analyze for cosegregation of chromosomes and tumorigenicity. However, the morphology of 3 hybrid clones was a spindle form, similar to the 3Y1 cells. In spite of the consistent presence of human chromosomes, negative anchorage independent growth in these clones suggested that none of the chromosomes involved in fused cells was associated with tumorigenicity of endometrial carcinoma. Loci on a few human chromosomes of a normal cell cause suppression in several hybrid systems. Cell fusion between 6-TGr 95 and normal human fibroblasts (HF) was performed to confirm whether this is also true in endometrial carcinoma. Hybrid cells had four copies of homologous chromosomes (two from each of the parent cells). These had a polygonal appearance, being similar to the HHUA 95 cells. Both anchorage independence and tumorigenesis were negative. Those results suggested that malignant transformation of endometrial cells was the result of multiple genetic changes. In this respect, the loss of genes implicated in the suppression plays an important role in endometrial carcinogenesis.

[Pharmacokinetics and materno-fetal influence of ritodrine hydrochloride during the continuous treatment of threatened premature labor].

The cardiovascular and metabolic effects of ritodrine hydrochloride on the mother and fetus were investigated during the 28-day and/or longer continuous treatment of threatened premature labor. Concentrations of ritodrine in maternal venous blood during the treatment, and placental transfer of the drug into the fetal circulation and amniotic fluid were also determined to ascertain the effectiveness and safety of the drug. Seventeen volunteer patients at 27-34 weeks of gestation were given a total dose of 2,850-7,350mg of ritodrine intravenously at a mean infusion rate of 72.9 micrograms/min for 28-66 days, when the maternal serum concentration of ritodrine was maintained at approximately 200ng/ml throughout the treatment period. A significant but slight increase in the maternal heart rate and serum glucose, and a slight fall in maternal blood pressure were observed during ritodrine infusion. The fetal heart rate did not change significantly. Although ritodrine also produced decreases in the red blood cell count, hemoglobin and hematocrit, these changes were transient and recovered to the control level thereafter. The ratios of umbilical venous blood and the amniotic fluid concentration of ritodrine to maternal venous concentrations determined at cesarean section were 0.68 and 1.64, respectively, suggesting that a gradual accumulation of placentally transferred ritodrine may occur when long-term continuous infusion is carried out. There were no noticeable drug-related findings in mothers and newborns during and after labor. These results indicate that threatened premature labor may be well effectively controlled by ritodrine without severe side-effects on the mother or fetus in treatment for along period.

[A case of chronic obstructive pulmonary disease associated with central alveolar hypoventilation diagnosed by exercise test].

We report a case of chronic obstructive pulmonary disease associated with central alveolar hypoventilation diagnosed by exercise test. A 61-year-old man with chronic obstructive pulmonary disease was admitted to our hospital because of respiratory failure caused by bacterial pneumonia. After recovery from pneumonia, we examined his functional ventilatory response to incremental exercise. Exercise was discontinued because of his maximal target heart rate, and there was no complaint of dyspnea throughout the exercise test. Although the patient had enough ventilatory reserve during exercise (VEmax/MVV = 50%), there was a marked progressive retention of carbon dioxide disproportionate to the extent of his air flow obstruction. Moreover the patient was able to reduce the carbon dioxide tension to a subnormal level by voluntary hyperventilation, and his ventilatory response to carbon dioxide and mouth occlusion pressure at rest was significantly diminished as compared with normal subjects. These findings suggest that this patient has several features compatible with central alveolar hypoventilation. Exercise test was useful for the diagnosis as well as exercise prescription because the patient could develop dangerous hypoxemia without alarming symptoms like dyspnea.

[The importance of uterine vascular systems in the maintenance of pregnancy].

The mesometrial artery (MA), which is the final part of the uterine artery in guinea pig, supplies nutrients and oxygen to the fetus and placenta during pregnancy. Accompanying fetal growth, MA increases 8-fold in diameter, and 30- to 50-fold in weight, protein content and deoxyribonucleic acid (DNA) content. The importance of MA in the maintenance of pregnancy was examined by comparing the proliferative rate of MA during pregnancy and abortion induced by antiprogesterone RU38486 30 mg/kg. MA was incubated in Medium 199 with 185kBq/ml 3H-thymidine for 4h and the rate of 3H-incorporation into MA was used as the proliferative rate. The proliferative rate of MA during the first trimester (day less than 20) was 2,825 +/- 1,036 Bq/mg.h, which is almost 100 times higher than the rate in diestrus animals. During the later course of pregnancy, the proliferative rate decreased logarithmically, being 352 +/- 58 (40 less than or equal to d less than 50), 158 +/- 23 (50 less than or equal to d less than 60) and 75 +/- 10 (60 less than or equal to d). The proliferative rate of MA was measured also in animals which received RU38486 30 mg/kg at 24-48 h prior to the measurement and whose fetuses were still alive. The proliferative rates of MA in such animals decreased markedly to 141 +/- 21 (40 less than or equal to d less than 50) and 37 +/- 8 (50 less than or equal to d less than 60) (p less than 0.01), which means that the proliferation of MA is reduced even before the death of the fetus.(ABSTRACT TRUNCATED AT 250 WORDS)
