Clinical outcomes of uterine sarcomas: results from 14 years worth of experience in the Kinki district in Japan (1990-2003).

To review clinical outcomes and therapeutic varieties, we were invited to submit data from the patients who were treated for uterine sarcomas in Japan from 1990 to 2003. Uterine sarcomas were defined as leiomyosarcoma (LMS), endometrial stromal sarcoma (ESS), and carcinosarcoma (CS). Of a total of 97 patients, 36 (37.1%) were diagnosed with LMS of the uterine corpus, 15 (15.5%) with ESS, 46 (47.4%) with CS. Median age at diagnosis was 59 (21-85) years. Clinical stages based on FIGO were 41 (42.3%) with stage I disease, 6 (6.2%) with staged II, 34 (35.1%) with stage III, and 16 (16.5%) with stage IV. The median follow-up period for all patients was 13 (1-108) months and median disease-free period was 9 (0-96) months. The 1-year survival rate and disease-free survival (DFS) rate were calculated in patients with all sarcomas (overall survival [OAS], 61.3%; DFS, 46.6%). Statistical analysis showed that younger age (less than 50 years), early stage (stages I and II), and surgical procedure (extended hysterectomy [EH] and radical hysterectomy [RH]) were associated with significantly better OAS. Histologic types did not affect the survival period. In conclusion, aggressive surgery including EH or RH at the time of initial operation offers the possibility of prolonged survival.

Tolerance of guinea pig hippocampal slice CA1 neurons to hyperthermia evaluated by orthodromic and antidromic responses.

The tolerance of electrical responses in the CA1 neurons of guinea pig hippocampal slices to elevated temperatures was studied by recording orthodromic and antidromic responses of the population spike (PS). Increasing the temperature of the perfusing medium from 30 degrees C to 49 degrees C resulted in a decreased amplitude of both the orthodromic and antidromic PS, the former disappearing at 42.0 +/- 1.8 degrees C and the latter at 46.2 +/- 1.3 degrees C (n = 8 for both). When the temperature was increased to 44 degrees C, maintained at this level for less than 27 min, then lowered to 30 degrees C, both the orthodromic and antidromic PS recovered within 60 min. When the temperature was increased to 45-49 degrees C, marked irreversible effects were seen with the orthodromic PS, recovery being dependent on the maximum temperature and duration of exposure, the change becoming irreversible after 13 min at 45 degrees C, 6 min at 46 degrees C, 4 min at 47 degrees C or 2.5 min at 48 degrees C. In contrast, the antidromic PS, recorded simultaneously, recovered on lowering the temperature to 30 degrees C in all cases tested, except when the temperature was increased to 46 degrees C and maintained at this level for 25-27 min. These results indicate that, in CA1 neurons, temperatures above 44 degrees C have more potent irreversible effects on synaptic transmission than on axonal or somal function.

Metabolism of 11-deoxycortisol by cytochrome P450(11 beta): identification of reaction products by 1H-NMR and LC/MS-APCI method.

A hitherto-unidentified steroid produced in the reaction mixture of 11-deoxycortisol with bovine cytochrome P450(11 beta) reconstitution system was purified by repeated HPLCs, and subjected to 1H-NMR analysis. The result suggested that the new metabolite was 19-oxo-11-deoxycortisol. This was further confirmed by a new analytical technique using LC/MS equipped with atmospheric pressure chemical ion (APCI)-analyzer.

An experimental study of intramural blood supply network of the stomach wall.

BACKGROUND/AIMS: A fundamental experiment was undertaken with reference to local resection with lymphadenectomy for gastric cancer. The intramural blood supply network of the stomach wall, about which no reports have previously appeared, was surgically investigated. METHODOLOGY: Five pigs were used. The left gastric and the right gastroepiploic vessels and their branches were removed from all the animals. The right gastric vessels were also removed, while the short gastric and the left gastroepiploic vessels were preserved in the 1st pig. Only the short gastric vessels were preserved in the 2nd, and the right and the short gastric vessels in the 3rd. After resection of the whole stomach with the spleen, angiography was performed through the preserved arteries. The 4th and 5th pigs underwent the same procedures as the 1st, with additional local resection of the stomach in the 5th. Both pigs were maintained for 1 year and then, angiography was performed. RESULTS: When the short gastric and the left gastroepiploic vessels, or the short and the right gastric vessels were preserved, the whole stomach continued to receive blood supply through the intramural network. Stomach wall resection therefore can be performed with no complications after these procedures. CONCLUSIONS: The present results confirm the possibility of local resection with lymphadenectomy as a treatment for gastric cancer.

From experimental units to unique experiments: chronobiologic pilots complement large trials.

The timing of treatment affects outcome. For mapping multi-frequency rhythm spectra first to optimize chronochemotherapy, 1000 marker determinations on the subject are more informative than a few determinations on each of hundreds of patients. N-of-6 subgroups should follow, with one subject assigned to each of 6 marker rhythm stages, 60 (e.g. 4 hours on a 24-hour scale) apart. Once the time structure has been mapped, minimal sampling requirements determined, and guidelines for treatment established, the information gained from chronobiologic n-of-1 and n-of-6 test pilot designs can be built cost-effectively into randomized controlled trials to benefit large patient populations. Sequential tests combined with marker rhythmometry and cosinor analysis on single test subjects, small groups and eventually on each patient are powerful tools that can extract information otherwise unattainable even at great cost and bring the P-value from publications to the patients.

Colonic J-pouch decreases bowel frequency by improving the evacuation ratio.

BACKGROUND/AIMS: To compare the functional outcome of ultra-low anterior resection for rectal cancer with colonic J-pouch reconstruction with that of straight reconstruction. METHODOLOGY: Twenty-three patients who underwent ultra-low anterior resection with or without J-pouch reconstruction underwent bowel transit study, videodefecography, and answered a questionnaire survey 4 months and 1 year after surgery. Eleven healthy subjects underwent similar testing as controls. RESULTS: Patients with a J-pouch had less frequent stools than patients with straight reconstruction 4 months after surgery (p<0.05), but the two groups were similar at 1 year. Bowel transit time was similar at both study points. The evacuation ratio was higher after J-pouch than straight reconstruction 4 months after surgery (p<0.05). However, the ratio improved in the straight group, and no difference existed at 1 year. Colonic contraction was seen only near the anastomosis 4 months after surgery, but the contraction proximal to the anastomosis improved over the next 8 months. CONCLUSIONS: J-pouch reconstruction facilitates evacuation by improving the evacuation ratio. Although straight anastomosis caused excessive stool frequency 4 months after surgery, colonic function continued to improve and was comparable with J-pouch and straight reconstruction 1 year after surgery because the contraction ratio proximal to the anastomosis improved.

Glucose and lipid metabolism during long-term antihypertensive treatment with indapamide in non-insulin-dependent diabetic patients.

Thirty-nine patients with diabetes and hypertension were treated with indapamide for 24 weeks to study the effects of that drug on glucose and lipid metabolism. The drug was administered at a dose of 2 mg once per day in the morning as a single drug (26 patients) or in combination with other antihypertensive drugs (13 patients), including calcium antagonists, angiotensin-converting enzyme inhibitors, an alpha-blocker, or a beta-blocker. Blood pressure was reduced in both groups during treatment, and no alteration of glycemic control or lipid metabolism was observed. One patient complained of a mild headache, but treatment was continued. The results indicate that indapamide is useful for the long-term treatment of hypertension in diabetic patients, either alone or in combination with other antihypertensive agents.

Spectral differences between epitopes in the cronomes of salivary CA130 and CA125.

Ovarian tumor markers CA125 and CA130 are here investigated in the search for harbingers of an elevated cancer risk or pre-cancer. A circadian rhythm of large amplitude is demonstrated for salivary CA125 and CA130 of a clinically healthy 28-year-old woman (AP). The circadian rhythms of both markers found in AP have a pattern similar to that of a patient (EH) with a müllerian cancer involving the ovary, with high values in the early morning and low values in the afternoon. Whereas acrophases occur approximately at the same circadian stage in both AP and EH, the MESOR and circadian amplitude are higher in EH as compared to AP. The circadian rhythm of salivary CA130 of AP is similar to that of other clinically healthy women. CA130 concentrations are lower than CA125 concentrations in unstimulated saliva (before rinsing of the mouth) in this clinically healthy woman (AP), and in other women studied herein. This difference between CA130 and CA125 concentrations is observed irrespective of circadian stage and irrespective of the concentration range of CA125 at the outset. After rinsing of the mouth, a statistically significant decrease in both CA130 and CA125 concentration is observed in AP and in another healty woman (GC) who collected saliva both before and after rinsing of the mouth.

Toward salivary-urinary chronosensitivity testing: chronomes of OVX1, M-CSF and CA130.

Several rhythmic components were previously mapped for salivary and/or urinary CA125 and CA130. On the background of such reference standards, OVX1 and M-CSF were assayed on 242 urine samples and 160 saliva samples provided by a 71-year-old patient with a Müllerian/ovarian adenocarcinoma. Serum OVX1 correlates with serum CA125 (P = 0.002); when circulating CA125 concentrations decreased (from 122 to 14 U/ml), the urinary excretion rate of OVX1 decreased (P = 0.005), whereas the urinary excretion rate of CA125 increased (P < 0.001). Salivary OVX1 and urinary M-CSF show ultradian variations (with a frequency of one cycle in 14-17 hours), which could be utilized to guide treatment timing targeted first to optimize treatment efficacy and as a second consideration to minimize treatment toxicity.

Management of malignant tumor with intracaval extension by selective clamping of IVC.

BACKGROUND/AIMS: Malignant tumors with retrohepatic intracaval extensions are difficult to treat. We report five cases of intracaval tumor emboli (3 hepatocellular carcinoma, 2 renal cell carcinoma). MATERIAL AND METHODS: The tumor embolus is removed by the following methods according to site: in the right atrium, by open heart surgery after clamping of the inferior vena cava between the superior vena cava and the intrahepatic inferior vena cava and of the portal vein, in combination with a cardiopulmonary bypass using a pump oxygenator; above the confluence of the hepatic vein with the inferior vena cava, by inferior vena cava clamping between its suprahepatic and intrahepatic portions, shunting from the inferior vena cava and the portal vein to the axillary vein; below the inferior vena cava-hepatic vein confluence, by inferior vena cava clamping below the confluence and in the infrahepatic portion; and around the confluence, by side clamping of the inferior vena cava, maintaining both hepatic and systemic circulations. RESULTS: Pulmonary emboli were diagnosed in one patient. However, the patient's condition improved with anti-coagulant therapy. No major complication was observed in any other patient. All patients were discharged after a mean postoperative period of 32.8 days. One patient with HCC died of lung metastasis at 5 months and the other two, of recurrence in the residual portion of the liver at 4 and 16 months, and the two with RCC are still alive without recurrence of the carcinoma 9 and 14 months later. CONCLUSIONS: Preoperative recognition by ultrasonography, computed tomographic scanning, cavography and especially trans-esophageal endoscopic ultrasonography is important. Vascular exclusion may also be performed by various techniques depending on the site of the tumor embolus.
