[Spontaneous dissecting aneurysm of the superficial temporal artery: a case report].

Aneurysms of the superficial temporal artery are rare, and usually traumatic in origin. We present a very rare case of spontaneous dissecting aneurysm of the superficial temporal artery in a 46-year-old man. The patient had pulsatile headache in the right temporal region, and a pulsatile mass in the same region. His headache gradually worsened for which he came to our hospital. On examination, there was a pulsatile mass of 1.5cm x 3cm at the right temporal region, which disappeared when the main trunk of superficial temporal artery was compressed. There was no neurological abnormality. On computed tomographic examination, there was a spotty high density area in the right extracranial region corresponding to the right superficial temporal artery, which was strongly enhanced after contrast medium administration. On MRI examination, the same area showed high intensity, but at inner space there was signal loss on T1WI, T2WI and PDWI. Angiography showed an aneurysmal dilatation at the right superficial temporal artery and both the true lumen and the false lumen were recognized. Excision of the aneurysm under local anesthesia was performed. His complaints disappeared completely post-operatively, and the skin color of that region retained its normal texture. Histopathological examination of the specimen showed hypertrophic intima with fibroblasts, and absence of internal elastic lamina. There was hematoma outside of the intima, but no deposits of hemosiderin. On account of the above findings, the lesion was diagnosed as a spontaneous dissecting aneurysm of the superficial temporal artery.

Morphogenetic study of the eyelids in NC-eob mice fetuses with an open-eyelid malformation at birth.

The NC-eob mice are mutants having open eyelids at birth with complete penetrance. Detailed scanning electron microscopy and light microscopy for study of the eyelid development of NC-eob embryos/fetuses were performed at days 13 to 17 of gestation and compared with normal NC mice, with the aim of investigating the etiopathogenesis of the open eyelids at birth. No difference was observed between NC and NC-eob embryos by scanning electron microscopy and histologic examination at days 13 and 14 of gestation. At day 15 the epithelium at the tip of the eyelids had 2 to 3 layers, and clumps of round periderm cells appeared on the outer surface of the margin of the lids in NC and NC-eob embryos. However, these clumps in NC-eob embryos were not as remarkable in number and in size compared with NC embryos. At day 16 the eyelids of NC fetuses extended over the cornea and began to fuse. The epithelium at the tip of the eyelids consisted of 4 to 5 layers with the following elongated area. The surface of the tip of the elongated epithelium was covered with a large number of round periderm cells. In NC-eob fetuses the epithelium remained stationary at 2 to 3 layers. Only a small number of round cells were noted at the tip of the eyelids, and fusion did not occur. At day 17, although the upper and lower eyelids of NC fetuses were entirely fused, no change occurred in NC-eob fetuses, either in the periderm or in the epithelium.(ABSTRACT TRUNCATED AT 250 WORDS)

[Dysfunction of E-cadherin due to mutation of beta-catenin in a scirrhous gastric cancer cell line].

E-cadherin is a calcium dependent cell-cell adhesion molecule. In cancer tissue, detachment of the adhesion system is indispensable for invasion and metastasis of cancer cells. We have investigated mechanism of the dysfunction of E-cadherin-dependent cell-cell adhesion system in gastric carcinoma cells. Although the high expression of E-cadherin in a scirrhous gastric cancer cell line HSC-39, the function of E-cadherin was completely abolished. Western blotting of beta-catenin in HSC-39 cells demonstrated that a truncated beta-catenin was detected. The truncation was due to partial deletion of beta-catenin DNA. It was concluded that in HSC-39 loss of E-cadherin dependent cell-cell adhesion was due to mutation of beta-catenin gene.

High-molecular-size mite allergens with high carbohydrate content in mite extract containing mite feces of Dermatophagoides farinae.

A series of mite allergens with high carbohydrate content was fractionated from mite extract containing mite feces by ultrafiltration, and successive chromatography on Ultrogel AcA 54, Sepharose CL-6B, and DEAE Toyopearl 650S. The final fractions isolated were termed HM1-2-Eff, -0.1A, -0.1B, -0.3A, -0.3B, and -1M according to the eluted salt concentration. These six fractions were stained weakly with Coomassie Brilliant Blue but strongly with periodate-Schiff's reagent on SDS-PAGE. Their molecular weight were estimated to be 150-155 KD on Sepharose CL-6B. All of these six fractions provoked strong erythema in skin of mite-sensitive and asthmatic patients. HM1-2-Eff, -0.1A, -0.1B and -0.3B reacted with 73% of mite-sensitive patients' IgE by the nitrocellulose dot blotting. One ng/ml of 4 fractions (HM1-2-0.1A, 0.3A, 0.3B and -1M) released histamine from mite allergic patient's blood cells. HM1-2-0.3A induced proliferation of T cell from immunized mouse by mite extract containing mite feces (Dff).

[Left ventricular systolic and diastolic function after mitral valve replacement with preservation of posterior chordae tendineae].

Left ventricular function was compared in 14 patients who underwent mitral valve replacement with preservation of the posterior leaflet, chordae tendineae and papillary muscle (preserved group) and 14 patients who underwent conventional mitral valve replacement (non-preserved group). 99mTc radionuclide ventriculography was performed before surgery and one month after surgery. The early diastolic filling rate, an indicator of the efficiency of diastolic function, was measured. The patients in the non-preserved group showed an anteroseptal ejection fraction of 28 +/- 10% before surgery and 32 +/- 10% after surgery, an apical ejection fraction (71 +/- 10% vs 73 +/- 16%), and an inferolateral ejection fraction (56 +/- 16% vs 53 +/- 16%). The global ejection fraction tended to decrease from 56 +/- 9% to 52 +/- 10%. The patients in the preserved group showed an anteroseptal ejection fraction of 37 +/- 13% before surgery and 43 +/- 19% after surgery, an apical ejection fraction (74 +/- 20% vs 77 +/- 17%) and an inferolateral ejection fraction (53 +/- 11% vs 68 +/- 16%). The global ejection fraction, reflecting improvement in the inferolateral ejection fraction, tended to increase from 53 +/- 11% to 58 +/- 11%. The early diastolic filling rate for the preserved group was 1.73 +/- 0.43 before surgery and 1.05 +/- 0.61 after surgery, while that for the nonpreserved group was 1.59 +/- 0.24 vs 0.97 +/- 0.48. Although the early diastolic filling rate normalized in both groups, there was no significant difference between the two groups.(ABSTRACT TRUNCATED AT 250 WORDS)

[A case of pulmonary tuberculoma with pneumoconiosis (graphite lung) that appeared to be lung cancer].

We report a rare case of tuberculoma with pneumoconiosis (Graphite lung). A 62-year-old man was admitted to our hospital for diagnosis of a rapidly growing coin lesion in the right lung (S8). He had worked for 42 years in a foundry and given a diagnosis of pneumoconiosis (Graphite lung). Because transbronchial lung biopsy and percutaneous needle aspiration biopsy were not diagnostic, the right lower lobe was partially resected. Histological examination revealed the presence of caseous necrosis in a solitary mass. Therefore, it is important to consider tuberculoma in the differential diagnosis of a coin lesion in patients with pneumoconiosis.

[Ruptured distal anterior cerebral artery aneurysm and diagnostic dyspraxia: a case report].

A case of ruptured distal anterior cerebral artery aneurysm presenting with diagnostic dyspraxia is presented. A 54-year-old female was referred to our hospital with the complaint of sudden onset of headache followed by disturbance of consciousness. CT and MRI revealed subarachnoid hemorrhage with hematomas in the interhemispheric fissure and the supracallosal area, and CAG revealed a left-sided callosomarginal artery aneurysm. During and after hospitalization, she showed diagnostic dyspraxia characterized by behavior of both her hands opposite to what might be expected e.g. when she tried to pick up a bowl, both her hands moved forward and held it at once; she wiped her head and face with toilet paper after urination. At times her hands behaved in opposite ways. For example, while folding cloths, her right hand tended to fold them while the left hand tended to unfold them; when she put on a sweater, as the right hand put it on, the left hand took it off; when she put her shirt into her trousers, one hand pushed it in while the other hand pulled it out. This unusual behavior was considered to be caused by the impairment of the corpus callosum due to compression by the hematoma. It disappeared gradually over a period of one year. Involuntary motor behavior of the left hand while the right hand is in voluntary action is known as diagnostic dyspraxia. Although this symptom has rarely been reported so far in cases of ruptured distal anterior cerebral artery, it may become noticed more frequently through careful observation.

[The proliferative activity in epithelial hyperplasia of the breast].

Epithelial hyperplasia of the breast has been recognized as an indicator of the risk for occurrence of invasive ductal carcinoma. In the future, however, its biological nature as well as precancerous potency remains unclear. Therefore, in order to elucidate a part of its biological nature, we attempted to evaluate proliferative activity in 62 lesions of epithelial hyperplasia of the breast, by means of immunohistochemistry of Ki-67 and silver stain of nuclear organizer region (NOR). Consequently, the labelling index (LI) of Ki-67 and the number of AgNOR became larger according to the increased grade of stratification, but cellular atypia without significant stratification also showed large LI of Ki-67 and number of AgNOR. Moreover, it was suggested from the discrepancy in the results of Ki-67 and AgNOR that AgNOR and Ki-67 reflected different phenomena.

[Normal-sized ovary carcinoma syndrome: histopathological analysis of 14 cases].

Fourteen cases of "normal-sized ovary carcinoma syndrome" (diffuse metastatic malignant disease of the abdominal cavity of the female, with normal-sized ovaries, with no origin assigned definitively by intraoperative or preoperative evaluation, Feuer et al., 1989) were analyzed histologically, histochemically, immunohistochemically and ultrastructurally. Through these studies, 14 cases were divided into 11 primary peritoneal tumors (4 diffuse malignant mesotheliomas and 7 serous surface papillary carcinomas) and 3 metastatic peritoneal tumors (2 ovarian tumors and 1 appendicular tumor). To distinguish serous surface papillary carcinomas from epithelioid diffuse malignant mesotheliomas, examinations such as hyaluronidase digestion test, electron microscopy, and immunohistochemical studies using antibodies for Ber-EP4 and Vimentin were found to be useful. In order to achieve an accurate prognosis of normal-sized ovary carcinoma syndrome, it seems necessary for us to accumulate more information on this syndrome.

The correlative evidence suggesting that trehalose stabilizes membrane structure in the yeast Saccharomyces cerevisiae.

The contribution of trehalose and hsp104 to membrane fluidity and the mobility of non-freezing cell water were examined on the basis of whole cell NMR analysis of the yeast Saccharomyces cerevisiae. Membrane fluidity was dependent on the accumulation of trehalose not hsp104 and non-freezing cell water was dependent on the accumulation of hsp104 not trehalose. Thus, the correlative evidence suggesting that trehalose protects yeast cells from temperature extremes by stabilizing the membrane structure was observed in vivo.

[Effect of tepid cardiopulmonary bypass in coronary artery bypass operation].

The effect of systemic temperature during cardiopulmonary bypass (CPB) surgery was evaluated in 100 patients. The patients were divided into three groups, based on systemic temperature during CPB; 28 degrees C, 30 degrees C, or 32 degrees C (tepid). Multidose cold crystalloid cardioplegia was administered for myocardial protection. Pump flow was maintained at 75 ml/kg/min. Methoxamine hydrochloride and phenothiazine were used to maintain systemic perfusion pressures between 60 and 80 mmHg. Preoperatively, there were no differences between groups in left ventricular ejection fraction or extent of coronary artery disease. The time required for CPB and weaning from CPB were significantly shorter in the 32 degrees C group than in either the 28 degrees C or the 30 degrees C group. There were significant differences in the doses of methoxamine hydrochloride and phenothiazine required in each group. Postoperatively, there were no significant differences in the incidence of myocardial infarction, stroke, or 30-day mortality between groups. In conclusion, tepid systemic perfusion shortens the length of CPB and does not differ significantly from cold perfusion with respect to mortality and morbidity.

Heterogeneous responsiveness of the in situ rat vascular endothelial cells to mechanical stretching in vitro.

En face endothelial cell (EC) sheets from various vessels of the rat vascular tree were stained with rhodamine-phalloidin to examine the response of the ECs to mechanical stretching, especially from the viewpoint of actin stress fiber (SF) dynamics. Arterial ECs were classified into three types according to the distribution pattern of their SFs before and after stimulation. In both ECs with few SFs from arteries near the heart and those having many SFs from arteries distributed in the uterus, ovary or kidney, new SF formation was not induced by stretching. ECs with various amounts of SFs from the abdominal aorta to the femoral artery developed many SFs in response to stretching. On the other hand, the response of venous ECs with initially few SFs was generally low, although active SF formation was observed in ECs from the uterine vein, portal vein and anterior facial vein, which are normally subjected to higher levels of mechanical stress in situ. The amplitude of stretching necessary for SF formation in ECs was about 40% in both vascular systems, although the time necessary for SF expression was 10 min and 30 to 40 min in arterial and venous ECs, respectively. A rapid increase in the number of ECs with SFs for a given amplituding of stretching suggests the presence of a threshold-like value in this reaction system. It was clarified that notable heterogeneity of ECs in their response to mechanical stress exists among the arterial and venous trees.

Model building of DNA repair enzyme 3-methyladenine DNA glycosylase-DNA complex.

We report the model structure of enzyme-DNA complex based on X-ray structure of 3-methyladenine-DNA glycosylase II and mutagenic studies. The enzyme structure does not have any recognizable DNA binding motifs. The DNA was located in the large cleft between the two all alpha domains with the damaged adenine base moiety (3-methyladenine) of DNA duplex positioned in the entrance region of the cleft including the tryptophan and aspartic acid residues. For the target 3-methyladenine to interact with these residues, the 3-methyladenine has to flipped out from the DNA helix. A number of the positively charged amino acid residues adopt at the negatively charged phosphate moieties. It is necessary to kink the DNA helix slightly to form the good interactions between both molecules. The damaged base must be held in place by a number of interactions including the weak van der Waals and electrostatic attractions.

Three-dimensional model of a hairpin ribozyme.

On the basis of the mutational and chemical-modification analysis, we propose the new secondary structure of a hairpin ribozyme, which contains three stems, a triple helix and the reverse Watson-Crick g+1C44 base pair at the 3' side of the cleavage site. The computational approach to the tertiary structure of a hairpin ribozyme have been carried out.

Failure of down-regulation of estrogen receptors and progesterone receptors after medroxyprogesterone acetate administration for endometrial hyperplasias.

BACKGROUND: To determine the biologic difference between normal endometrium and endometrial hyperplasias, the authors assessed the expression of estrogen receptors (ER) and progesterone receptors (PR) in the subjects before and after oral administration of medroxyprogesterone acetate (MPA) (Hysron-H, Kyowa Hakko Kogyo Co., Ltd., Tokyo, Japan). METHODS: The authors studied the expression of ER and PR in pathologic conditions of the endometrium such as cystic, adenomatous, and atypical hyperplasias before and after MPA treatment, 600 mg daily, for 8-14 weeks, and compared this expression with that of normal endometrium. RESULTS: Stromal and epithelial components of the normal endometrium in the proliferative phase were positive for ER and PR. In the secretory phase and after MPA treatment, the epithelial component showed either negative or faint staining for ER and PR, but the stromal component showed positive staining for PR and negative or faint staining for ER. All hyperplasias exhibited ER and PR in the stromal and epithelial components, although the intensity of the staining was variable in atypical hyperplasias. After MPA treatment, in the epithelial component of cystic and adenomatous hyperplasias, down-regulation of PR was prominent, but many cells continued to express ER. The epithelial component usually was surrounded by PR-positive decidual cells. In atypical hyperplasia, many cells continued to express ER and PR in the epithelial component surrounded by both ER and PR positive stromal cells after MPA treatment. CONCLUSIONS: Among the pathologic conditions of the endometrium, cystic and adenomatous hyperplasias have mechanisms of down-regulation of PR but not ER in the glandular component after MPA treatment, whereas in atypical hyperplasia, down-regulation of ER and PR does not occur in the epithelial component. Compared with normal endometrium, the majority of endometrial hyperplasias have an impairment of down-regulation of ER in the epithelial component by MPA treatment, and atypical hyperplasia has an impairment of down-regulation of ER and PR. Endometrial hyperplasias seem to have an abnormality of down-regulatory mechanisms of sex steroid receptors in the epithelial component when treated with exogenous progesterone.

Somatic mutations of the von Hippel-Lindau tumor suppressor gene in sporadic central nervous system hemangioblastomas.

Hemangioblastoma is one of the benign tumors in the central nervous system. It is often associated with the von Hippel-Lindau (VHL) disease, a well known hereditary tumor syndrome. It is believed that inactivation of both alleles of VHL tumor suppressor gene is essential in the tumorigenic processes in hemangioblastomas associated with VHL disease. The molecular basis for the development of sporadic hemangioblastomas is not known. Here, we analyzed 13 cases of primary sporadic hemangioblastomas for somatic mutations of VHL gene with single strand conformational polymorphism analyses of the tumor DNAs. We detected abnormal single strand conformational polymorphism pattern in 7 tumors (54%). Of these 7 possibly mutated tumors, we successfully characterized 3 tumors by direct sequencing. We were unable to sequence 4 tumors because of the poor quality of DNA obtained from paraffin blocks. Somatic mutations in the 3 tumors were 2 missense mutations and 1 microdeletion. These mutations were observed in 1 tumor in exon 1 and 2 tumors in exon 2. Our results suggest that mutations of VHL tumor suppressor gene are involved in the development of at least 20% of sporadic central nervous system hemangioblastomas.
