Antagonism of the effects of (+)-PD 128907 on midbrain dopamine neurones in rat brain slices by a selective D2 receptor antagonist L-741,626.

1. The ability of PD 128907 to activate dopamine receptors in the ventral tegmental area, substantia nigra pars compacta, and striatum was investigated by use of in vitro electrophysiological recording and fast cyclic voltammetry. The affinity of a novel D2 selective antagonist L-741,626 for receptors activated by this agonist was measured to determine if its effects were mediated by D2 or D3 receptors. 2. The active (+) enantiomer of PD 128907 bound with high affinity and selectivity to rat D3 dopamine receptors. The Ki values for (+)-PD 128907 were 620 nM at D2, 1 nM at D3 and 720 nM at D4 receptors. 3. (+)-PD 128907 inhibited cell firing in both the ventral tegmental area and substantia nigra pars compacta with EC50 values of 33 nM (pEC50 = 7.48 +/- 0.10, n = 10) and 38 nM (pEC50 = 7.42 +/- 0.15, n = 5), respectively. No effects of (+)-PD 128907 (100 nM) were observed on glutamate or GABA-mediated synaptic potentials elicited by focal bipolar stimulation. 4. L-741,626 antagonized these effects of (+)-PD 128907 in a concentration-dependent and surmountable manner with an affinity, determined from Schild analysis, of 20 nM (pKB = 7.71 +/- 0.14) in the ventral tegmental area and 11 nM (pKB = 7.95 +/- 0.18) in the substantia nigra pars compacta. 5. (+)-PD 128907 also inhibited dopamine release in the caudate-putamen with an EC50 of 66 nM (n = 5). The affinity of L-741,626 for these nerve terminal autoreceptors (pKB = 7.71 +/- 0.06; = 20 nM) was identical to that observed on midbrain dopamine neurones. 6. These data demonstrate that the D3 receptor ligand (+)-PD 128907 is a potent agonist on rat midbrain dopamine neurones. However, its lack of regional selectivity, and the high affinity of the selective D2 receptor antagonist L-741,626 for receptors activated by (+)-PD 128907, was more consistent with an action on D2 autoreceptors rather than upon a D3 dopamine receptor subtype.

Corneal coloboma, aphakia and retinal neovascularization with anterior segment dysgenesis (Peters' anomaly).

An infant with bilateral Peters' Anomaly, with corneal opacification in the right eye and an opacification with an axial defect in the left also had an associated hydrocephalus and cardiac anomalies that proved fatal; histopathology of the right eye showed retinal neovascularization in addition to findings consistent with a diagnosis of Peters' Anomaly; in the left eye, pathologic findings were suggestive of a corneal coloboma associated with aphakia.

Cognitive-behavioural treatment of shoplifting in a depressed female.

This paper describes the successful treatment of a depressed shoplifter, using cognitive therapy combined with exposure, an approach not previously described in the literature for this problem. Definitions of shoplifting behaviour are discussed, and other treatment methods examined.

The School Enrollment-Based Health Insurance program: socioeconomic factors in enrollees' use of health services.

OBJECTIVES: The School Enrollment-Based Health Insurance program is designed to reduce financial barriers to children's health care use. This study sought to determine if any socioeconomic measures differed between enrollees with at least one health care encounter and those with no encounters. METHODS: Logistic regression was used to assess the impact of various predictors on the odds that a child would use health care services. RESULTS: Children receiving free insurance premiums were less likely to use health care than those receiving partial subsidy. African-American and Hispanic children were less likely than Whites to use health care. Age, sex, and months enrolled also influenced the likelihood of health care use. CONCLUSIONS: Financial and non-financial factors must be considered when developing children's health care programs.

Infant with mos45,x/46,XY/47,XYY/48,XYYY: genetic and clinical findings.

We describe an infant with mos45,X/46,XY/47,XYY/48,XYYY who presented with ambiguous genitalia. Her phenotype was also remarkable for minor ear and eye anomalies and coarctation of the aorta with bicuspid aortic valve. Laparoscopy revealed bilateral Fallopian tubes and a left infantile testis with epididymis. Chromosomal analyses of blood, skin, aorta, right Fallopian tube, and left gonadal tissue showed mos45,X/46,XY/47,XYY/48,XYYY. The 46,XY cell line was identified with routine trypsin-Giemsa banding only in cultured cells from an aortic biopsy. Fluorescence in-situ hybridization (FISH) was utilized to identify the presence of 46,XY cells in other tissues. The clinical manifestations of this patient are discussed and compared with those of similar cases of Y chromosome aneuploidy. To our knowledge, this is the first report of a patient with this unusual karyotype.

Chronic pancreatitis: a complication of systemic lupus erythematosus.

Systemic lupus erythematosus (SLE), an autoimmune disease with multisystem involvement, has been reported to be associated with a number of gastrointestinal complications. Acute pancreatitis is an unusual manifestation of this disorder. This report presents two cases of patients with SLE who developed chronic pancreatitis with no other identifiable etiology.

A method for the continuous measurement and display of pulmonary resistance in man.

1. We have validated a method for the continuous display and 'on-line' measurement of total pulmonary resistance in man, using a hybrid digital/analog computer. 2. The basic variables, which are measured by standard techniques, are flow rate of the mouth (V) and oesophageal pressure (Poes), and these are the only analog inputs necessary to the computer. 3. Resistance (RL,cont) is calculated continuously as: RL,cont = (Poes-V/C)/V where V is tidal volume and C is the dynamic compliance. RL,cont is continuously displayed as an analog signal on an oscilloscope. 4. Changes in resistance, measured by this method over a wide range of values in three subjects, showed an almost exact correlation with those measured by a standard method (Mead-Whittenberger).

Recombinant human growth hormone therapy does not increase microalbuminuria in children with short stature.

OBJECTIVE: Concerns have been raised about possible adverse effects of growth hormone on renal function. We measured microalbuminuria as a sensitive index of early glomerular damage in children being treated with recombinant human growth hormone. DESIGN AND PATIENTS: Microalbuminuria was measured in a group of 17 children with short stature being treated with recombinant human growth hormone and in a group of 13 patients with idiopathic short stature not receiving therapy. MEASUREMENTS: Microalbuminuria was measured by a commercially available ELISA and urinary creatinines were determined using a Beckman creatinine analyser. RESULTS: The level of microalbuminuria was 0.484 +/- 0.275 g albumin/mol creatinine (mean +/- SD) in the patients receiving growth hormone and 0.681 +/- 0.574 g albumin/mol creatinine in the untreated controls. There was no statistically significant difference between these values. CONCLUSIONS: Treatment with recombinant human growth hormone does not cause an increase in microalbuminuria in children with normal renal function. This supports the safety of this medication in growth hormone deficient children with normal renal function.

Alterations in endothelial superoxide dismutase levels as a function of growth state in vitro.

Damage to the retinal vasculature in retinopathy of prematurity is primarily at the level of the growing neovascular front. This clinical observation in combination with experimental observations that correlate the induction of superoxide dismutase (SOD) activity with differentiation led to the hypothesis that the basis for the relative vulnerability to oxygen lies in the relative "undifferentiated" nature of proliferating endothelial cells (EC). To test this hypothesis, an in vitro model of microvascular EC was used and levels of SOD activity were assayed as a function of growth state and differentiation. The SOD activity was elevated significantly in EC that stopped growing as a contact-inhibited monolayer compared with its activity in cells in their log phase of growth. In addition, SOD levels were elevated in microvascular cells that stopped growing and were organized into a network of capillary-like tubes, the ultimate differentiated state of microvascular EC. To understand the mechanism of increases in SOD activity with differentiation, the effect of extracellular matrix synthesized by a confluent monolayer of EC was examined as was artificial growth arrest caused by mitomycin. Both treatments led to an increase in SOD activity over control cells. Thus, it is possible that SOD activity in cells is modulated by information provided from the extracellular matrix and "intracellular" signals that indicate cessation of cell growth. These data support the hypothesis that the growing front of retinal microvessels is more vulnerable to effects of oxygen-induced damage because of their relatively undifferentiated state with respect to the oxygen radical-scavenging enzyme system of SOD.

Characterization of a GTPase-activating protein for the Ras-related Ral protein.

We have demonstrated the presence of a GTPase-activating protein (GAP) for the Ras-related Ral A protein in the cytosolic fraction of brain and testis. This protein, designated Ral-GAP, was distinguished from Ras-GAP by its behavior in two chromatography systems and by the fact that the two GAP proteins did not stimulate the GTPase activity of each others target GTP binding proteins. The lack of effect of Ral-GAP on Ras GTPase activity also distinguished it from the product of the neurofibromatosis gene NF-1. Ral-GAP also differed from Rho-GAP and Rap-GAP by virtue of its elution from a gel filtration column with proteins of Mr greater than 10(6). This was likely an overestimate of the protein's molecular mass, however, since it sedimented in sucrose gradients between standard proteins of 150 and 443 kDa. Ral-GAP failed to promote the GTPase activity of mutant Ral proteins containing amino acid substitutions that in Ras lead to GAP-insensitive proteins.

Antagonism of antihypertensive drug therapy in pregnancy by indomethacin?

Two women with preeclampsia treated with pindolol and propranolol became profoundly hypertensive when indomethacin was added because of premature contractions. The interaction of nonsteroidal antiinflammatory agents and beta-blockers and their role in the control of blood pressure in obstetrics are discussed. Indomethacin should not be given to pregnant patients with hypertension treated with beta-blockers.
