Differentiation in virulence patterns of Escherichia coli possessing eae genes.

In this study 98 Escherichia coli strains which belonged to traditional enteropathogenic (EPEC) serotypes and 82 enterohemorrhagic E. coli (EHEC) strains were screened by polymerase chain reaction (PCR) for the presence of E. coli-attaching and -effacing (eae) genes. These strains were also hybridized with the enteropathogenic adherence factor (EAF) probe and examined in the fluorescence actin staining (FAS) test. The results obtained from the individual strains demonstrated that all 26 class I EPEC with localized adherence to HEp-2 cells carried EAF and eae genes. In contrast, of 72 EPEC strains with no or diffuse adherence only 1 strain was EAF positive and 6 strains had eae. Of 82 EHEC strains a total of 75 carried eae sequences. Of considerable interest, 15 of 21 E. coli strains that lost their slt genes during subcultivation were found to be eae positive. As controls a total of 53 enterotoxigenic and enteroinvasive E. coli, and 125 E. coli strains from the normal flora were investigated and all displayed negative results in the eae-PCR. From the 201 strains comprising classical EPEC serotypes, EHEC and E. coli with lost slt genes, a total of 126 displayed a positive FAS test and 122 reacted in the eae-PCR. Only 4 strains were FAS test positive but eae-PCR negative. Our data indicate that E. coli strains possessing the eae genes are heterogenous with respect to their virulence determinants. Loss of virulence plasmids and phage-encoded slt genes either in the host or during storage may contribute to this heterogeneity. DNA fingerprinting of Eikenella corrodens by pulsed-field gel electrophoresis.

In order to conduct molecular typing of Eikenella corrodens strains by macrorestriction fingerprinting, we evaluated different restriction enzymes for digestion of genomic DNA and determined the optimal parameters for separating E. corrodens DNA by pulsed-field gel electrophoresis. Ten E. corrodens strains isolated from oral and extraoral infection sites in different individuals were analyzed. The rare-cutting restriction endonucleases DraI, SmaI and XbaI usually used for pulsed-field gel electrophoresis analyses were not suitable for digestion of E. corrodens genomic DNA because they either did not digest the DNA or produced bands of similar molecular weights that could not be separated. Accordingly, among additional enzymes including BamHI, BglII, EcoRI and Hind III, we found BamHI and BglII to be the most suitable rare-cutting enzymes for pulsed-field gel electrophoresis analysis. They cleaved the genomes of all the above strains into 15-20 fragment bands that were clearly separated by the following pulsed-field gel electrophoresis conditions: 140 V with a running time of 40 h, pulse times of 5 to 50 s with linear ramping and an electrical field angle of 120 degrees. These conditions enabled us to distinguish 8 individual pulsed-field gel electrophoresis patterns from the 10 strains analyzed. However, only 4 identical outer membrane protein profiles were differentiated by sodium dodecyl sulfate-polyacrylamide gel electrophoresis. The data obtained in this analysis showed clonal divergence among members of the E. corrodens species, at the same time revealing this pulsed-field gel electrophoresis as being a highly attractive procedure for epidemiological investigation of this organism, including its acquisition and transmission.

Nucleotide sequence analysis of enteropathogenic Escherichia coli (EPEC) adherence factor probe and development of PCR for rapid detection of EPEC harboring virulence plasmids.

The 1-kb BamHI-SalI fragment from plasmid pMAR2 termed the enteropathogenic Escherichia coli (EPEC) adherence factor (EAF) probe was cloned in pUC19 and pK18. The nucleotide sequence of this fragment was determined, and a set of primers was designed to amplify a 397-bp region associated with pMAR2 by PCR. An analysis of the whole EAF sequence with database libraries indicated no significant homology to any known genes. However, between bases 701 and 787 of the fragment, an 82.8% homology between the EAF and the insertion sequence IS630 of Shigella sonnei exists. The results of PCR with primers of the EAF sequence demonstrated that all of the 151 EAF probe-positive EPEC strains with localized adherence to HEp-2 cells yielded positive EAF PCR results. In contrast, none of the 277 EAF probe-negative strains reacted to the EAF PCR. In addition, the PCR assay was successfully used to generate vector-free digoxigenin-labeled EAF fragments that gave valid results in colony blot hybridization assays. The EAF PCR appears to be a specific and efficient method for the detection of EPEC strains carrying the EAF plasmids.

[Acute pancreatitis and stomach wall necrosis caused by cholesterol embolisms].

A 60-year-old man was hospitalized because of a sudden onset of severe pain in the epigastrium and haematemesis. Acute pancreatitis was diagnosed on the basis of an increased serum amylase concentration (642 U/l). Abdominal ultrasound and computed tomography demonstrated a necrotic zone with central liquefaction in the tail of the pancreas adjoining the stomach wall. Gastroscopy revealed as source of the bleeding an extensive mucosal necrosis at the greater curvature of the stomach. At laparotomy, partial resection of the pancreas, gastrectomy and splenectomy were performed. Histological examination of the resected specimens showed multiple cholesterol emboli in the small arteries of the pancreas and the gastric submucosa.

[Grief, depression and anxiety after spontaneous abortion--a study of systematic evaluation and factors of influence].

Recent studies demonstrate profound and long-lasting adverse psychological and family sequelae of a spontaneous abortion. However, decisive issues of quality, course and determinants cannot be answered sufficiently due to shortcomings of research (e.g. lack of representative samples and adequate measures, reliance on cross-sectional study designs). Grief reactions and their determinants are differentiated in relation to depressiveness and anxiety in 86 patients from a longitudinal study, employing the Perinatal Grief Scale (Thoedter et al. 1988) and standardized symptom checklists. For the majority of the women, around the 10th week of gestation, the embryo is psychologically represented in fantasies, dreams and concrete arrangements in reality. Immediately after the abortion, these women react with painful feelings of "active grief" and "despair". Additional stresses in the pregnancy and lack of social support predict "self-reproachful coping". Women with recurrent abortions who have no children show depressive reactions. Retrospectively, these also present more anxiety and depressive moods during pregnancy. Results support reliability and validity of the grief scale. Implications for counselling and psychotherapy of women after a spontaneous abortion are discussed with respect to these risk constellations.

Ubiquitous transcription factor OTF-1 mediates induction of the MMTV promoter through synergistic interaction with hormone receptors.

Steroid hormones induce transcription from the mouse mammary tumor virus (MMTV) promoter by complex mechanisms requiring binding of the hormone receptors to the hormone responsive element (HRE) of the long terminal repeat region. Here we show that the MMTV promoter contains two degenerated octamer motifs immediately upstream of the TATA box that together bind OTF-1 (Oct-1, NFIII) with an affinity similar to the octamer consensus. In transfection experiments, mutation of these octamer motifs interferes with the hormonal response of the MMTV promoter. In vitro, these mutations do not influence basal transcription but completely abolish the stimulatory effect of purified progesterone receptor. Progesterone receptor and glucocorticoid receptor bound to the HRE facilitate binding of OTF-1 to the two octamer motifs. Thus, OTF-1 is a natural mediator of hormonal induction of the MMTV promoter and acts through cooperation with the hormone receptors for binding to DNA.

[Intraoperative complications in interventions on peripheral arterial vessels].

Between 1980 and 1989, 2395 reconstructive operations were performed for peripheral arterial occlusive disease on vessels above the aortic arch and below diaphragm. 543 (22.6%) procedures of these represented operations upon the supraaortic branches, 756 (13.5%) were operations upon the abdominal aorta and pelvic vessels, 647 (27.0%) were performed on the upper and lower extremity and 449 (18.7%) miscellaneous like e.g. arterial embolectomies, venous embolectomies and others. Intraoperative complications occurred in 2.8% where as there was a 3-fold postoperative complication rate (9.1%). Diffuse bleeding contributed the highest rate of complications followed by injuries different organs. In most cases injuries to the spleen resulted from undue retraction. Other complications were iatrogenic lesions of the ureter, neural injuries and intraoperative bypass-occlusions.

[Aortic replacement with autologous free transplanted small intestine muscle in the dog].

Free transplantation of 60 grafts from muscle of small intestine has proved to work well for adequate replacement of larger vessels even in cases in which vessel beds were affected by infections. Patency of implants for smooth blood flow was angiographically verified in 86.7 per cent of all cases, whereas aneurysms occurred in only 15 per cent and septic complications in 11.7 per cent. With due consideration given to some technical aspects in preparation and implantation of transplants (complete removal of mucosa and pretension by something between 30 and 60 per cent), new therapeutic approaches might come to the fore to cope with infected synthetic protheses in vascular surgery.

Replacement of dog's aorta by autologous intestinal muscle in the infected retroperitoneum.

The free transplant of intestinal muscle lacking mucosa into an infected vascular bed has been shown to be a suitable replacement for the aorta in dogs. This type of transplant is incorporated into the infected area and shows a high tolerance against progressive infection. Therefore, an autologous transplant may be suitable to ensure continuous blood flow during the healing phase of a deep infection when a synthetic prosthesis is unsuccessful.

[Femoropedal bypass: a step forward in peripheral bypass surgery?].

Femoropopliteal and femorocrural bypasses are standardized procedures. There is given a patency rate of 70-90% after a period of 5 years according to the progress of the disease and to the used graft material. Bypassing the lower limb in AOD (stage III and IV by Fontaine) it is possible to implant a femoropedal bypass. In our patients we had a patency rate of 81.3% during a follow-up time of 13 months. An av-shunt nearby the distal anastomosis seems to be important.

[Prognosis of perforating gastroduodenal ulcer: simple closure or definitive ulcer operation?].

95 patients are reported, who had perforation of a gastroduodenal ulcer. In 47 cases the perforation was oversewn only; primary or secondary high selective vagotomy (HSV) were done in 33 cases and resection in 15 cases. Lethality of the first group was 23.3%; it was 9.1% after primary HSV and resection. No death was recorded in the group of 26 patients, who had secondary HSV or resection. In the group of patients surviving, which had symptoms for more than 3 months prior to surgery, 42.4% had new symptoms of ulcer diseases after surgery and 27.2% actual relapse, if the perforation had been oversewn only. These numbers were 16.2% after HSV and 5.4% after resection respectively. In patients where the ulcer was located intra- or postpylorically symptoms of ulcer disease were observed in 42.1% after simple oversewing, and in 15.4% after HSV or resection, the difference being significant. There was no difference between incidence of postoperative symptoms when ulcers was located prepylorically.

[Results of the surgical treatment of bleeding gastroduodenal ulcers].

Within 11 years 279 operations for acute bleeding from gastroduodenal ulcers were performed at the surgical University Clinic in Würzburg. Of the 279 patients, 140 had to be operated on immediately because of acute severe bleeding. Of the 158 bleeding duodenal ulcers, 104 had a selective proximal vagotomy (SPV), 29 had gastric resection, and in 25 the ulcer was simply closed. The overall mortality after SPV was 8.6%, 62% after gastric resection, and 40% after simple suture ligation. Recurrent bleeding occurred in 8.7% after SPV, 10.3% after gastric resection, and in 24% after suture ligation. Of the 89 bleeding gastric ulcers, 25 had a selective proximal vagotomy, 30 a gastric resection, and 34 excision of the ulcer. The overall mortality after SPV was 16%; after gastric resection it was 23.3% and 41.2% after ulcer excision. Recurrent bleeding was observed in 8% after SPV, in 10% after gastric resection and in 11.7% after ulcer excision.

[The value of diagnostic peritoneal lavage in perforating abdominal injuries].

From July to October 1980 40 patients with deep torso injuries caused by shrapnel or high velocity missiles were treated in the ICRC-Field Hospital at Kao-I-Dang near the Cambodian border in Thailand. In 17 cases indication for laparotomy was evident, whereas in 23 patients paracentesis and lavage of the abdomen were performed. In 12 cases with positive results, immediate laparotomy revealed significant intra-abdominal injuries. Of ten patients with negative or weakly positive results nine were treated conservatively and their recovery was uneventful. Only one patient was operated and a retro-peritoneal hematoma not requiring surgery was found. It is concluded that peritoneal lavage is a useful diagnostic procedure in evaluating penetrating abdominal injuries.
