Triple endoscopy. A valuable procedure in head and neck surgery.

Synchronous carcinoma in the head and neck, lung, or esophagus may be identified in 10 percent of patients with squamous head and neck carcinoma by routinely performing triple endoscopy (direct laryngoscopy, bronchoscopy, and esophagoscopy) at the time of initial evaluation. In 5.5 percent of patients with squamous head and neck cancer, synchronous lung and/or esophageal cancer was identified. However, in only 1.5 percent of patients was it asymptomatic and would not have been suspected as a result of a thorough history, physical examination, and chest x-ray. The cost-effectiveness of routine triple endoscopy can be questioned, but we think it is indicated to identify those early carcinomas with an excellent chance of cure. The time and money expended to identify these early, asymptomatic cancers is small compared to that spent in treating patients with symptomatic cancer of the same organs, in whom there appears to be a much smaller chance of cure. Multiple superficial squamous carcinomas of head and neck mucosa may be managed by surgical excision, radiation therapy, laser destruction, or cryosurgery. If these methods cannot be applied, combination chemotherapy appears to provide a satisfactory alternative method of short-term control, which we believe merits further clinical investigation.

Combined modality therapy for advanced head and neck cancer.

Seventy-five patients with stage III and IV squamous cell cancer of the head and neck, previously untreated, had two courses of chemotherapy with cis-platinum, Oncovin and bleomycin, followed by radical surgery, radiotherapy or both. Response to chemotherapy (complete and partial) was high (80 percent). In 36 patients radical surgery became possible. The overall survival improved statistically in patients who achieved complete or partial response to chemotherapy compared with those who had minimal or no response. Patients with well to moderately well differentiated cancer, or stage III disease, had a statistically better prognosis than patients with poorly differentiated cancer, or stage IV disease.

Death anxiety among university students in Northern Ireland and Canada.

Male and female university students in Northern Ireland (n = 315) and Canada (n = 255) completed Templer's Death Anxiety Scale (DAS) and the Taylor Manifest Anxiety Scale (MAS). Results of a discriminant function analysis indicated that Canadian students showed more concern about being afraid to die and thoughts of death, while students from the North of Ireland were more concerned about getting cancer, the shortness of life, and the viewing of a corpse. However, total DAS scores failed to show significance between group differences (p greater than .10). Correlations between DAS and MAS total scores were r = .34 for Northern Ireland and r = .31 for Canadian students.

The structure of death anxiety: a factor analytic study.

University students in Canada (n = 255) and Northern Ireland (n = 315), graduate nursing students (n = 124), funeral service students (n = 79), and members of the Unitarian Fellowship (n = 83) completed Templer's Death Anxiety Scale (DAS). The responses of each group were subjected to principal components factor analysis using varimax rotational procedures and the factor scores derived from this analysis were then contrasted using multiple discriminant function analysis. Results indicated that there was more commonality than uniqueness in the factor patterns for these groups. Four common death anxiety patterns were classified as follows: (a) cognitive-affective concerns; (b) concern about physical alterations; (c) concern about the passage of time; and (d) concern about stressors and pain. These factors are consistent with and extend the views expressed in previous research (i.e., Pandy, 1974-75; Pandy & Templer, 1972.

Modification of compensatory swallowing in a supraglottic laryngectomy patient.

Results are presented for repeated videofluoroscopic and scintigraphic examinations of a supraglottic laryngectomy patient, illustrating the successful use and later changes in a compensatory swallow. Issues in patient management are outlined, stressing the importance of interaction between radiologists and dysphagia therapists in the rehabilitation of such patients.

cis-platinum and 5-fluorouracil as initial therapy in advanced epidermoid cancers of the head and neck.

A total of 44 patients with previously untreated Stage IV squamous cell carcinomas of the head and neck were treated with a combination of cis-platinum (100 mg/sq m) and 96-hour infusion of 5-fluorouracil (5-FU) (1,000 mg/sq m/day) in two courses or with cis-platinum (100 mg/sq m) and 120-hour infusion of 5-FU (1,000 mg/sq m/day) in three courses. In the 26 patients receiving the two course regimen, the tumor response rate was 88.5%. In five of the 26 patients complete disappearance of the tumor was achieved; partial remission was obtained in 18 of 26 patients. The main side effects were nausea and vomiting (70%), leukopenia (27%), and mild-to-moderate renal toxicity (27%). In the 18 patients who received the three course regimen of 5-FU and cis-platinum, there were 11 (61%) complete responses, 6 (33%) partial responses, and 1 (6%) minimal response. The incidence of complete response was increased with each course given. No added toxicity was observed with surgery or radiotherapy.

Distribution of fucosylated N-acetyl lactosamine carbohydrate determinants during embryogenesis of the kidney in man.

The monoclonal antibodies AGF4.48 and AGF4.36 raised against the promyeloid cell line HL60 recognise a fucosylated N-acetyl lactosamine determinant. This oligosaccharide sequence has been shown to be present on a variety of tissues at different developmental stages. Using the immunoperoxidase technique and the AGF4.48 and AGF4.36 antibodies on formalin-fixed, paraffin-embedded tissues, the distribution of the corresponding antigenic determinants during human renal embryogenesis has been studied. Both antibodies bind to the surface of the cells of the ampullae of the ingrowing ureteric bud branches, but not to the remainder of the ureteric bud. Reactivity at this site persists until after fusion of the ureteric bud with the S-shaped tubule, but is then lost. The determinants are also found on different segments of the proximal convoluted tubule in the foetal and adult kidney. The determinants are thus found on the cells responsible for induction of renal tubulogenesis, and separately at specific stages and functionally distinct sites on the developing tubule.

Blood lead levels in Toronto children and abatement of lead-contaminated soil and house dust.

South Riverdale in Toronto, Canada, underwent a lead-abatement program. In 1988, lead-contaminated soil was replaced at 970 properties, and in 1989, professional housecleaning for lead removal was conducted in 717 households. The effect of "abatement" on blood lead levels in young children was investigated. Data were analyzed from 12 cross-sectional blood-screening surveys that were conducted during an 8-y period in South Riverdale and in two comparison areas. Responses regarding behavioral, household, lifestyle, neighborhood, and environmental factors, all of which were gleaned from associated questionnaires, were also analyzed. Response rates varied between 32% and 75%. During the years between 1984 and 1992, blood lead decreased in all study areas. There appeared to be a minimal blood lead level of 2-3 micrograms/dl for urban Ontario children who were less than 6 y of age. The significant difference between South Riverdale and the control areas disappeared by 1992. Although abatement activity in South Riverdale was associated with an accelerated decline in blood lead levels, it was difficult to distinguish this from effects of decreased Toronto air lead levels or decreased smelter emissions. Within South Riverdale, abatement appeared to be associated with a slower decline in blood lead levels over time, likely the result of selection bias, soil mixing, or recontamination from the smelter. No difference was observed between the separate effects of housecleaning or soil replacement. The findings could neither strongly support nor refute beneficial effects of abatement.
