[Evaluation of exanthemas following high-dosage intravenous penicillin combinations in intensive care].

118 patients of a non specialized intensive care unit have been studied, all of them under high dose bactericidal cover (10 MIU of Na-Penicillin G and 2,0 Ciclacillin q 12 hrs.) for a period of days to 4 weeks. In 17 (14,4%) a skin rash was observed. 10 of these could be studied using special techniques (radial immunodiffusion, passive hemagglutination, RIST and RAST), however in none of these cases there was a hint of the existence of penicillin specific antibodies. In 6 patients also skin tests were performed. There was no immediate type reaction, only twice delayed type reactions occured to Na Pencillin G.6 patients had continuing treatment on spite of the rash and without further steps the effluorescences vanished within 3-6 days. Therefore continuation of the antibiotic therapy in spite of rash along with strict clinical and laboratory monitoring seems to be preferable to a hastened change of antibiotic regime

[Measurement of bronchopulmonary elastance and airway resistance: comparison between common methods and automatic continuous measurement by the system AUPREM (author's transl)].

The system AUPREM is a new measurement device, which allows continuous measurement of the mechanical properties of the lung and bronchial system. Pulmonary elastance and airway resistance can be measured continuously even in extremly debilitated or unconscious patients. Transpulmonary pressure change, airflow and tidal volume are used as input. From these values airway resistance and pulmonary elastance are calculated. Comparative measurements in a group of healthy subjects and in a group of patients with pathological resistance and elastance values revealed that there was no significant correlation between the system AUPREM and the commonly used methods bodyplethysmography and elastance measurement by oesophageal balloon. However, the differences are in a low order of magnitude.

[On the application of electric stimulation currents in operations. Concept of a clinically applicable device (author's transl)].

With the use of high voltage electric currents in patients who initally have been anaesthetised, the anaesthesiologist faces new aspects. For this reason basic principles of physical medicine and medical engineering with respect to special problems are described. The majority of impulses current generators used so far do not even comply with the first principal requirement of any therapy namely knowledge of the dose of the agent applied. A system for electric currents crossing the body is outlined, which is characterized by constant current and isovoltage-free outlets in connection with a special ECG-amplifier and a control pattern. This apparatus for the first time guarantees maximal safety in clinical use. First animal experiments demonstrate the possibility of the risk if this method is applied in a inexperienced manner: in all animals ventricular fibrillation could be achieved when the electrodes were used in a position near the heart and there appeared to occur nonspecific reactions when current was directed through the brain (ear acupuncture)

[Pharmacokinetics and pharmacodynamics of vecuronium in children].

Pharmacodynamics of vecuronium have been studied in children 1 to 10 years old. Doses from 0.015 to 0.1 mg/kg were given and the effects measured mechanically and by electromyography. For 0.1 mg/kg the pharmacokinetics were established. A smaller distribution volume and higher plasma clearance in children as compared to adults have been found, while the dose-effect ratio is the same. The action of vecuronium is shorter in children than in adults.
