Biologic markers and bipolar disease in children.

The diagnosis of manic depressive or bipolar disease is difficult in adolescents. Poorly defined nosology, and a high incidence of schizophrenia-like symptoms contribute to this difficulty. The authors discuss the use of the Dexamethasone Suppression Test (DST) in the diagnosis of bipolar disease in adolescents, presenting three case studies. The existence of phase-dependent DST non-suppression is described for the first time in adolescents.

The dexamethasone suppression test and the Children's Depression Inventory in psychiatric disorders in children.

This open study attempts to evaluate the usefulness of objective diagnostic tests in the child and adolescent age group. Fifty-eight children admitted to a Child and Adolescent Psychiatric service were administered the Dexamethasone Suppression Test (DST), the Children's Depression Inventory (CDI) or both. Rates of DST non-suppression were significantly elevated in children with major affective disorders (10/12) and in children suffering from adjustment disorders (3/18). Children with major depressions and conduct disorders both showed elevated scores on the CDI (p less than 0.025), and these two groups did not significantly differ from each other on this measure. The implications of these findings for future research are discussed.

EEG findings and language patterns in autistic children: clinical correlations.

Several authors have used the EEG as a diagnostic tool to identify distinctive patterns of cerebral dysfunction in autistic children. However, evidence is needed to support the hypothesis that their level of functioning correlates with their EEG profile. In this study EEG's were obtained in 17 autistic children without sedation and following sleep deprivation. These children were also evaluated in a double blind procedure with regard to their speech and communication abilities. We did not find a statistically significant correlation between EEG changes and speech performance. None of the autistic children using an augmentative system of communication had any EEG abnormalities. However, a normal EEG is not sufficient to predict successful response to communication therapy. Since the effect of sedative drugs on brain electrical activity limits the value of tracings, we recommend the sleep deprivation techniques when "difficult" children have to undergo EEG testing. Parents' or guardians' participation during the procedure is encouraged.

Affective disorder in an adolescent.

A case of bipolar disorder presenting in an adolescent girl is presented. Differential diagnosis and treatment is discussed. A number of aspects of this case are outlined including the difficulties of diagnostic accuracy in major disorders of childhood and adolescence, the existence of biological markers, the implication of environmental stressors and cultural-familial factors. An unusual situation of reversal of index case identification is noted.

Part-time residency training.

The development of part-time training in the United States and Canada is described. The flexibility required for young women physicians who attempted to combine childbearing and career development, together with the increasing number of women entering medicine, underlines the importance of part-time training schedules. A variety of flexible training options have been defined and a set of guidelines has been produced by the Canadian Royal College of Physicians and Surgeons. The experiences of part-time psychiatry residents at Queen's University, Kingston is presented. Thus far this has involved a total of six residents over a four year period, including the authors of this paper. The advantages and disadvantages, viewed from both the resident and system viewpoint, are discussed.
