Within the academic medical center providing interdisciplinary, experiential, longitudinal, and mentored learning experiences for students regarding hospice/end-of-life care is a considerable challenge. This article describes an innovative course for medical, nursing, and social work students taught as a partnership among the departments of family medicine, medical history/ethics and three community hospice programs. The goals of the course are: (1) understanding the hospice philosophy of care; (2) understanding that hospice care is an interdisciplinary team process; and (3) development of the caregiver skills of self-reflection as a form of self-knowledge while participating in the care of terminally ill patients. Achieving these goals is challenging for students (especially medical students) and faculty but highly rewarding. The development, implementation and evolution over the past 3 years of this hospice volunteer training course are discussed.

The confluence of enhanced attention to primary care and palliative care education presents educators with an opportunity to improve both (as well as patient care) through integrated teaching. Improvements in palliative care education will have benefits for dying patients and their families, but will also extend to the care of many other primary care patients, including geriatric patients and those with chronic illnesses, who make up a large proportion of the adult primary care population. In addition, caring for the dying, and teaching others to carry out this task, can be an important vehicle for personal and professional growth and development for both students and their teachers.
This study compared the results of renal scintigraphy with simultaneous administration of Tc-99m DTPA and I-131 Hippuran (before and after 25 mg of oral captopril) with the results of the renal arteriogram and renal vein renins (before and after the administration of 25 mg of oral captopril) to evaluate the sensitivity and specificity of renal scintigraphy in the diagnosis of renovascular hypertension. The results of 21 consecutive patients suspected of having renovascular hypertension who underwent scintigraphy and renal arteriography were analyzed. Renal scintigraphy postcaptopril detected all the cases of renovascular hypertension (eight patients) plus two additional patients who had significant renovascular stenosis but no renin overproduction. The results indicate that the renal scintigram, before and after the administration of captopril, is an accurate and sensitive test for the detection of renovascular hypertension and should be used as a screening procedure before arteriography is considered.

An interesting example of a vascular malformation of the mandible has been presented. Early diagnosis made conservative treatment possible. Although the lesion was small, a considerable amount of hemorrhage was encountered during surgery. Adequate treatment was provided with ligation of external carotid arteries bilaterally in conjunction with curettage and packing. After an eight-year follow up, there has been no clinical or radiographic evidence of recurrence.

Thirty-one children (age range, 7-14 years) of 18 probands with a bipolar disorder according to Research Diagnostic Criteria were assessed on clinical and psychological measures and compared with 18 matched normal control children. The authors found higher rates of childhood psychopathology in the children of the probands than in the control children. In addition, the experimental group was characterized by 1) a pattern on the WISC-R of higher verbal than performance IQs, 2) higher ratios of color to movement determinants on the Rorschach test, and 3) an overrepresentation of left-handedness. The implications of these findings are briefly discussed.
We compared 10 commercially available radioimmunoassay kits (American Diagnostics, Becton Dickinson, BioGenex, Clinical Assays, Hybritech, Leeco, Mallinckrodt, Microanalytic, Nuclear Medical Systems, and Radioassay Systems) for determination of human choriogonadotropin (hCG), using serum pools, hCG CR119 (NIH), 2nd I.S. (who), and 1st I.R.P. (who). Criteria were ease of performance, total assay time, sensitivity, potency, and parallelism as compared with reference standards and results for 15 serum pools. The Mallinckrodt kit exhibited the best overall performance, with good low-concentration sensitivity, parallelism with two of the three reference preparations, and good clinical correlation as compared with the reference kit from NIH. Because the antibodies used in the kits are occasionally changed by the manufacturers, these results are necessarily valid only for kits that include reagents identical to those in the kits that we tested.

Intraoperative arteriography was performed in 57 patients undergoing arterial reconstructive surgery of the lower extremity prior to selection of the site for the distal anastomosis. In 42 patients (74%), this procedure proved to be of significant benefit. Prereconstructive intraoperative arteriography influenced surgery for seven patients in the following ways: (1) two patients had visualization of vessels not demonstrated on the preoperative arteriogram; and (2) five patients had demonstration of occlusive disease that favored selection of a more distal site of anastomosis. In the remaining 35 patients, the tibial vessels and arch were visualized more clearly, so that the degree and extent of disease were better appreciated. This may have important prognostic significance. On postreconstructive arteriography, an additional five patients were shown to have an anastomotic defect that was corrected in the operating room. Intraoperative arteriography is a rapid, safe, and simple procedure that should be considered as an adjunct in reconstructive surgery of the lower extremity.

The patient with cancer faces a crisis involving many basic questions and major life adjustments. The natural history of the illness may involve progressive loss in the patient's functional capacity, and the patient needs medical, psychologic, and social support. The family of the patient with cancer can provide much help if their potential resources are fully utilized. Conferences involving the patient, family members, and health care professionals allow for open communication, problem-solving, and support for the patient and family through this difficult time. A case is reported illustrating the value of periodic family conferences in the care of a patient with metastatic disease.
