Case report: acute pancreatitis induced by Clozapine.

Two percent of acute pancreatitis are drug induced. In the present paper, we reported the case of a 39 year-old patient with chronic-hallucinatory schizophrenia who developed symptomatic pancreatitis during the clozapine dose titration performed to reach the therapeutic range. Diagnosis of pancreatitis was suggested by clinical examination and abnormal laboratory values of pancreatic enzymes and confirmed by C-T scan and ultrasonography. The causal incrimination of clozapine in this case seems likely as all other possible causes of pancreatitis were excluded, as AP developed shortly after the introduction of the drug and as the pancreatic enzymes normalized after clozapine was stopped. No rechallenge to confirm the causal relationship was however attempted. So far, only eight cases of acute pancreatitis have been reported in association with clozapine use. Clozapine is an atypical antipsychotic drug which belongs to the chemical class of dibenzodiazepines. The mechanism by which clozapine could produce acute pancreatitis remained unclear. Nevertheless, we advocate a careful biological follow-up (measuring periodically the concentrations of amylase, lipase and triglycerides) during the treatment by clozapine.

[Prognostic factors in chronic lymphocytic leukaemia: contribution of recent biological markers].

Chronic lymphocytic leukemia (CLL) is the most common lymphoid hemopathy in elderly. Diagnosis of CLL is easily made with a full blood count and immunophenotyping, but there is an heterogeneity in clinical evolution. Until now, scheduling of treatment is based on Rai or Binet staging systems. These staging systems can not distinguish patients with a rapid evolution and thus who will need an earlier treatment. In order to detect these patients, it is useful to have some relevant markers to predict disease evolution. This article reviews recent biologic markers that can be used to evaluate long term prognosis of CLL patients.

[Aphasia caused by isolated lesion of the semi-ovale centre: contribution of the measurement of cerebral blood flow].

Sudden onset stroke occurred in a right-handed vietnamese woman speaking, reading and writing french fluently. When first seen in our department, the patient had mild right facial paresis and non fluent atypical aphasia. CT scan and MRI showed a left subcortical infarct in the superficial territory of the middle cerebral artery; only white matter of the semiovale centre was involved. Neurological examination revealed linguistic impairment resembling transcortical motor aphasia, with unusual stuttering, hypophonia, occasional semantic paraphasias and phonological reading and writing abnormalities. Non verbal cognitive function, gestural and buccofacial praxes were normal. Cerebral blood flow study by SPECT was consistent with left sylvian functional deactivation.

[Fundoplication by the Nissen-Rosetti technique using celioscopy. 10 cases].

Ten patients presenting with hiatus hernia and gastro-oesophageal reflux underwent laparoscopic Nissen-Rossetti fundoplication. All patients had either medical treatment failure or invalidating reflux. At present, complete fundoplication is the most effective surgical treatment of gastro-oesophageal reflux. The laparoscopic procedure was rigorously identical with the habitually used "open" procedure, the only difference being the abdominal access. No laparotomy was necessary in our patients, and no major complication was observed. The follow-up showed good clinical results. Laparoscopy has been reported to decrease postoperative pain and ventilatory disorders. Morover, this minimally invasive surgical technique enables rapid resumption of social and professional activities.

[Celioscopic digestive surgery: from the explosion to a critical reflexion].

A critical review of laparoscopic digestive surgery indications is proposed. Patient's benefit by laparoscopy is to undergo the same procedure as by open surgery with identical security conditions but with less postoperative pain and an earlier recovery. Today laparoscopy in digestive surgery is mainly indicated in the fields of functional and benign diseases. Oncology seems for our team to stay a relative contra-indication as yet. Taken into account the future technical progresses, this 1996 state of the art is obviously only a temporary view of the technique before expected mutations.

[Duodenal lipoma causing intestinal subocclusion. Case report].

Duodenum is a rare position for gastrointestinal lipoma which sometimes may lead to severe complications. Authors report the case of a 60-year old woman treated with a transduodenotomic excision through a midline laparotomy. At admission the patient presented epigastric pain for 1 month. Physical examination was negative except for a modest epigastric sensitivity. CT scan showed image with regular features and fat density, and permitted the exact preoperative diagnosis. Final histopathological examination confirmed the diagnosis.
