Chromosome alterations that correlate with progression to immortality in Syrian hamster embryo cells transformed by gamma-irradiation.

Primary Syrian hamster embryo (SHE) cells can be transformed in vitro by gamma-irradiation or spontaneously and display morphological alterations in discrete colonies as the earliest recognizable transformants. These morphologically transformed colonies can progress to give rise to immortal cell lines. The purpose of the present study was to determine if specific chromosome changes occur that correlate with immortalization. In the non-irradiated culture, 18 transformed colonies were isolated, of which two became immortal. In the irradiated culture, six out of 18 transformed colonies isolated progress to immortalization. Seven out of eight immortalized cell lines exhibited either numerical and/or structural chromosome alterations. Of six radiation-induced immortal lines, four lines showed rearrangements in the long arms of chromosome 3 (3q +) and chromosome 6 (6q +) non-randomly. In all cases, both 3q + and 6q + were detected in the primary transformed colonies from which the immortal cell lines arose. Both 3q + and 6q + occurred always as heterozygotes in the primary-transformed colonies. 3q + and 6q + were never found in the non-irradiated culture, demonstrating that these chromosome changes were induced by irradiation. 3q + and 6q + may correlate with progression to immortality in SHE cells transformed by gamma-irradiation.

The characteristics of laser-Doppler flowmetry for the measurement of regional cerebral blood flow.

The fundamental characteristics of laser-Doppler flowmetry (LDF), especially the depth of cerebral blood flow (CBF) measurement, have not been widely studied in the brain tissue; however, LDF has been widely used in recent clinical and experimental studies. We investigated the depth of CBF measurement and other characteristics related to the use of LDF in the brain. In an animal experimental study, the distribution of laser light and the depth of CBF measurement of LDF were measured by using modified LDF probes. CBF in various conditions was also measured by the LDF and hydrogen clearance method. Laser light of low output lost directivity and was dispersed into a hemispherical form in the brain tissue. The depth of CBF measurement was approximately 100 to 400 microns, depending on the intensity of the emitted laser light, and was affected by changes of CBF. In the physiological condition, the close correlation between the values of CBF by the LDF and hydrogen clearance method was obtained. After cardiac arrest, the CBF value of LDF did not immediately show a 0 value. LDF has several special characteristics, and the sample volume was very small. It is important to pay attention to the several special characteristics of LDF.

Functional image-guided neurosurgical simulation system using computerized three-dimensional graphics and dipole tracing.

This report describes a preoperative simulation and intraoperative localization system for use with three-dimensional functional images in intracranial surgery. The system, which produces three-dimensional functional images by superimposition of the generators of somatosensory evoked potentials derived from dipole tracing, was applied in the preoperative localization of central sulci in 12 patients with intracranial disorders. The preoperative localization of central sulci was assessed by intraoperative cortical recording of somatosensory evoked potentials in three patients. In six patients, three-dimensional computer graphics were reconstructed from magnetic resonance images and allowed visualization of the lesions through the semitransparent views of the scalp surface and the brain. The three-dimensional functional images were created by superimposing the generators of somatosensory evoked potentials on three-dimensional computer graphics. This combined technique provided preoperative data regarding the three-dimensional relationship between the sensorimotor cortex and the lesions. We also developed a three-dimensional digitizer that incorporates a mechanical arm and a laser pointer for use in integrating information obtained in the preoperative simulation into the surgical field. Intraoperative localization can be performed in real time on the three-dimensional computer graphics in combination with the functional images created from dipole tracing. This system was applied in five patients who underwent surgical brain tumor resections with minimal damage to the normal brain tissue. The use of this system for stereotactic craniotomy increased the safety of the surgery by affording preoperative simulation and intraoperative localization with the three-dimensional functional images. Two illustrative case reports are presented.

A highly suspected case of chronic Chagas' heart disease diagnosed in Japan.

A 50-year-old South American Indian woman, a native of Brazil and now a resident of Shiga Prefecture, was admitted to our hospital because of dyspnea on exertion. We initially suspected dilated cardiomyopathy due to an enlarged and diffusely hypokinetic left ventricle (LV) on echocardiogram. Coronary arteriograms were normal, and histological examination of right ventricular endomyocardial biopsy specimens showed findings compatible with chronic myocarditis. Magnetic resonance imaging revealed localized thinning and a small apical aneurysm at the LV. Since she had previously lived in a high-risk region for Chagas' disease, two immunological examinations for Trypanosoma cruzi were performed. The results of both tests were compatible with the disease. Recently, an increasing number of patients with Chagas' disease have been found in the United States among immigrants from South American countries, and the risk of transmission of the disease through contaminated blood transfusion is becoming a national problem. We report this case with reference to the present state of the problem in the United States and the potential problems it presents in Japan because of the marked increase in the number of immigrants from the affected area.

Neutron generator at Hiroshima University for use in radiobiology study.

A neutron generator (HIRRAC) for use in radiobiology study has been constructed at the Research Institute for Radiation Biology and Medicine, Hiroshima University (RIRBM). Monoenergetic neutrons of which energy is less than 1.3 MeV are generated by the 7Li(p,n)7 Be reaction at proton energies up to 3 MeV. The protons are accelerated by a Schenkel-type-accelerator and are bombared onto the 7Li-target. An apparatus for the irradiation of biological material such as mice, cultured cells and so on, was designed and will be manufactured. Neutron and gamma-ray dose rates were measured by paired (TE-TE and C-CO2) ionization chambers. Contamination of the gamma ray was less than about 6% when using 10-microns-thick 7Li as a target. Maximum dose rates for the tissue equivalent materials was 40 cGy/min at a distance of 10 cm from the target. Energy distributions of the obtained neutrons have been measured by a 3He-gas proportional counter. The monoenergetic neutrons within an energy region from 0.1 to 1.3 MeV produced by thin 7Li or 7LiF targets had a small energy spread of about 50 keV (1 sigma width of gaussian). The energy spread of neutrons was about 10% or less at an incident proton energy of 2.3 MeV. We found that HIRRAC produces small energy spread neutrons and at sufficient dose rates for use in radiobiology studies.

[EEG power and coherence in presenile and senile depression. Characteristic findings related to differences between anxiety type and retardation type].

Quantitative EEG analysis was done on 29 medicated right-handed depressive patients at age over 45 and age and sex matched right-handed 20 normal controls. The patients were all diagnosed as major depression with melancholia by DSM-III-R and were divided into anxiety type (n = 17) and retardation type (n = 12) evaluated with the Hamilton rating scale for depression. Eye closed resting EEGs were recorded on a data recorder from the 16 electrode leads (10-20 system). Artifact rejected 3 minutes EEGs were analyzed by off-line with Fast Fourier Transform from the bilateral frontal (F3, F4), parietal (P3, P4) and occipital (O1, O2) regions. The mean values of absolute amplitude power (microV) and the mean Z scores of inter-, and intrahemispheric coherence were obtained in theta 1 (4.0-6.0 Hz), theta 2 (6.0-8.0 Hz), alpha 1 (8.0-10.5 Hz), alpha 2 (10.5-13.0 Hz), beta 1 (13.0-20.0 Hz), and beta 2 (20.0-40.0 Hz) frequency bands respectively. The main findings were: 1) Beta 1 and beta 2 power were greater in patients with anxiety type depression than in normal controls, and the differences were statistically significant over the parietal and occipital regions in beta 1 and the frontal region in beta 2. The anxiety type was distinguished from the retardation type with the increase of beta 2 power. The retardation type showed higher alpha 1 power over the frontal region and lower alpha 2 power over the occipital region than normal controls. 2) Both types showed greater frontal predominant ratio to the parietal in beta 2 power than did normal controls. The retardation type was distinguished from the anxiety type with the right predominance in beta 1 power over the frontal region. 3) Both groups of patients showed lower frontal interhemispheric coherences than normal controls in each band. In particular, the anxiety type showed significantly lower alpha 2 band and lower beta 1 and beta 2 bands. The parietal interhemispheric coherence was lower in the anxiety type but higher in the retardation type than in normal controls in each band, and this difference was prominent in theta 2 band. 4) The two groups of patients showed higher fronto-parietal intrahemispheric coherences in both hemispheres than the normal controls in each band. Fine structures of utricle of developing chick embryo exposed to 2G gravity.

Utricular otoconia of the chick embryos incubated under continuous 2G load were examined under a scanning electron microscope. Morphological changes were confined to the process of otoconial formation and the otoconial size. Otoconial formation was remarkably delayed in the utricle of 2G centrifuged chick embryo. Giant otoconia, which sometimes amounted to over 100 microM in length, existed along the marginal zone of the utricular otolithic membrane. These results indicate that the 2G state influences the otoconical formation.

Clinical study of enlarged infundibular dilation of the origin of the posterior communicating artery.

Although some aneurysmal dilations of the origin of the posterior communicating artery (PCoA) that are revealed on carotid angiogram are true aneurysms or preaneurysmal lesions, the authors warn that diagnosis should not be based only on the size and shape of the dilation, especially when the PCoA does not fill. In the present retrospective study, the authors analyzed intraoperative and angiographic findings in 32 patients with 34 lesions of the PCoA that were preoperatively diagnosed as aneurysms. Only 10 of the lesions were true aneurysms (six cases) or enlarged infundibular dilations with reddish bulges (that is, preaneurysmal bulge; four cases) at the origin of the PCoA. All of the other 24 lesions, including one lesion with PCoA occlusion, were merely enlarged infundibular dilations without any wall abnormality. A well-developed PCoA was observed only in cases of true aneurysm and dilation with reddish bulge. No other clinical or angiographic characteristics proved useful in identifying the type of lesions. These findings may be helpful to the physician in evaluating the clinical features of such lesions and in determining the diagnosis of enlarged infundibular dilation of the PCoA.

[Therapeutic efficacy of zinc picolinate in patients with taste disorders].

We studied the therapeutic efficacy of zinc picolinate in patients with zinc-deficient and idiopathic taste disorders. Efficacy was assessed in a double-blind study. The following findings were obtained: 1. There was a significant difference between the zinc picolinate and placebo groups in the grade of improvement shown by the filter-paper disk method. 2. There was a significant difference between the zinc picolinate and placebo groups in the serum zinc concentration. 3. There was no significant difference between the two groups in the degree of total subjective recovery or the grade of improvement, as shown by in the whole mouth method. 4. We conclude that the administration of zinc picolinate is effective for patients with zinc-deficient and idiopathic taste disorders.

[Influence of aging on electrogustometric threshold].

Electrogustometric testing was carried out, with one researcher examining the electrogustometric threshold at three sites, in 461 normal cases, by TR-06 (Rion Co.), over a wide range. A multiple comparison procedure (Tukey-Kramer) was applied to age groups, revealing a significant increase in the threshold with advancing age; high threshold values were observed in the "60s and older" group for the chorda tympani nerve area (tongue tip) and the glossopharyngeal nerve area (tongue base), and in the "70s and older" group for the superficial greater petrosal nerve area (soft palate). As to the gender difference in women, a significant drop was observed in the late teens whereas a low tendency in all sites tested was not statistically, significant in other age groups. Differences between the right and the left sides were seen in most cases but were within 6dB. As to site differences, the soft palate threshold was higher than those of the tongue tip and the tongue base in all age groups. A significant decrease in the threshold of the tongue tip, as compared to the tongue base, was observed for those in their late teens, whereas minimal differences were noted in the other age groups and at other sites. The soft palate threshold was significantly reduced in smokers in their 30s and 40s as compared with non-smokers.

Pharmacological subtypes of the gamma-aminobutyric acidA receptors defined by a gamma-aminobutyric acid analogue 4,5,6,7-tetrahydroisoxazolo[5,4-c] pyridin-3-ol and allosteric coupling: characterization using subunit-specific antibodies.

Various alpha and beta 3 subunit-specific antibodies were used to characterize some of the heterogeneous ligand-binding properties of gamma-aminobutyric acidA receptors. Polyclonal antibodies that were raised against the cytoplasmic amino acid sequence (380-392) of the rat beta 3 subunit recognized a single polypeptide of molecular mass of 58 kDa in Western blots with Ro7-1986 affinity-purified GABAA receptors from the rat brain, and a doublet of molecular mass of 54 kDa and 56 kDa in receptors from the bovine cortex, hippocampus, and cerebellum. Deglycosylation of purified receptors from the bovine cortex with N-glycanase resulted in a single band immunostained at molecular mass of 52 kDa. These anti-beta 3 subunit antibodies immunoprecipitated approximately 50% of [3H]flunitrazepam binding sites from soluble extracts of bovine cortex, whereas beta cyto antibodies, which probably recognize all beta subunit isoforms, precipitated almost 100% of benzodiazepine binding sites. These results indicate heterogeneity of GABAA receptor subunit composition with respect to the nature of beta subunits. The GABA analogue 4,5,6,7-tetrahydroisoxazolo[5,4-c]pyridin-3-ol (THIP), like GABA, shows heterogeneous binding affinities in brain homogenates. The higher affinity sites were previously suggested as corresponding to a 58-kDa polypeptide in rat that is photoaffinity-labeled with [3H]muscimol, a band that comigrates with the one stained by anti-beta 3 antibodies. However, THIP affinity was not significantly different between receptors containing beta 3 subunits and those lacking beta 3, as demonstrated by similar affinities in receptors that ere immunoprecipitated by anti-beta 3 antibodies and those that were not. Also, THIP displaced [3H]muscimol binding with similar multiple affinities across brain regions where different beta subunit variants are expressed with varying abundances. These observations suggest that the property of high affinity THIP binding cannot be explained solely by beta 3 subunits. The coupling efficiency between GABA and benzodiazepine binding sites appears to be determined by the nature of alpha subunits rather than of beta subunits. GABA enhanced [3H]flunitrazepam binding with different efficacies and potencies in receptors immunoprecipitated by anti-alpha 1, -alpha 2, and -alpha 3 subunit antibodies. In contrast, beta 3 subunit-enriched and disenriched receptors did not differ in this property. [3H]Flunitrazepam binding in GABAA receptors containing alpha 2 and alpha 3 subunits was enhanced to a significantly greater extent than were those with alpha 1. In addition, receptors containing alpha 1 and alpha 3 subunits had higher potencies of enhancement than did those with alpha 2 subunits.(ABSTRACT TRUNCATED AT 400 WORDS)

[Whole mouth gustatory test (Part 2)--Effect of aging, gender and smoking on the taste threshold].

The whole mouth test method was employed for examining changes in gustatory senses due to aging. Participants were 314 male and 356 female healthy volunteers not complaining of taste disorders. Simultaneously, the effects of smoking were examined, comparing 71 male smokers to 142 male non-smokers, ranging from 20 to more than 60 years of age. A multiple comparison procedure (Tukey-Kramer) was applied to age groups, revealing a significant increase in the threshold with age; high threshold values were observed in the "70s and older" group in comparison to the younger group for all tastes other than the sweet taste. Concerning the differences between males and females, the gustatory thresholds of males were found to be higher than those of females for some in the "20s and older" group and for all tastes other than the sweet taste, whereas the thresholds of males were lower than those of females for all taste qualities in the late teen group. As to male subjects in whom the effects of smoking could be studied, smokers in their 20s demonstrated a slight rise in the threshold only for the bitter taste.

[An operative case of the dumbbell type retromediastinal schwannoma].

Neurogenic tumors of the mediastinum with an intraspinal component connected by a narrowed segment in the intervertebral one are generally described as dumbbell or hour-glass tumors, which need cautious and precise diagnoses and remedies, compared with other neurological tumors. A 62-year-old male was admitted to our hospital for abnormal tumor shadow in the chest X-ray film. We diagnosed this case as dumbbell type neurogenic tumor by MRI and CT. An operation was performed by modified Grillo's method: One-stage operation. With patient in prone position, L-shaped skin incision was made. Through total laminectomy of T-5 and T-6 and resection of the 6th rib, paravertebral portion of the tumor was removed, and thoracotomy in the same position under the same view enabled us to remove the residual tumor. Histopathological diagnosis was schwannoma. After the operation, no neurological complications were detected.

Relationship between alpha-tocopherol and polymorphonuclear leukocyte elastase in septic adult respiratory distress syndrome.

To elucidate the interrelationship between active oxygen and polymorphonuclear leukocyte elastase (PMN elastase) in patients with septic adult respiratory distress syndrome (ARDS), we examined the relationship between alpha-tocopherol, which has an antioxidant action, and PMN elastase. Serial blood samples were collected from 16 patients with septic ARDS. Serum levels of alpha-tocopherol were determined by HPLC. PMN elastase was determined by ELISA. The serum alpha-tocopherol level in a nonsurviving group was significantly lower than that in a surviving group at the time that septic ARDS was diagnosed (p = 0.0068). The level of PMN elastase in the nonsurviving group was significantly higher than that in the surviving group (p = 0.0058). A significant negative correlation existed between the serum levels of alpha-tocopherol and those of PMN-elastase (r = -0.659, p = 0.0043). Both the serum alpha-tocopherol level and the PMN elastase level appeared to be good indices of the severity of ARDS. The results suggested that an interaction between active oxygen and PMN elastase may increase the clinical severity of this disease.

Plasma interleukin 8 and polymorphonuclear leukocyte elastase concentrations in patients with septic shock.

We determined the plasma concentrations of interleukin 8 (IL-8), polymorphonuclear leukocyte elastase (PMNE), and endotoxin in patients with septic shock in order to investigate the role of IL-8 and PMNE in the development of septic shock, especially in septic adult respiratory distress syndrome (ARDS). The IL-8 concentration in patients with septic shock was 6.28 +/- 9.00 ng/mL (mean +/- SD, n = 29), which was significantly higher (P < 0.0001) than the concentration in septic patients without shock (0.35 +/- 0.35 ng/mL, n = 40). There was a significant correlation between the IL-8 concentration and the PMNE concentration at the onset of septic shock (r = 0.6916, P < 0.0001). The IL-8 concentration was also significantly correlated with the endotoxin concentration (r = 0.5584, P = 0.0016). There was a significant negative correlation (r = -0.8237, P < 0.0001) between the serum PMNE concentration and the oxygenation index (PaO2/FiO2) at the onset of septic shock. These results indicate that IL-8 and PMNE are produced in large quantities when septic shock occurs, and may play a role in the development of septic ARDS.

Inhibitory effect of FUT-175 on the production of interleukin 8 and polymorphonuclear leukocyte elastase.

We investigated the inhibitory effects of a protease inhibitor, FUT-175, on the production of interleukin 8 (IL-8) and polymorphonuclear leukocyte elastase (PMNE) by polymorphonuclear leukocytes (PMN) and vascular endothelial cells. IL-8 production by PMN and vascular endothelial cells stimulated with lipopolysaccharide (LPS) was inhibited by FUT-175. This compound also inhibited PMNE production by PMN following LPS stimulation.

[A case of spontaneous rupture of the stomach].

We experienced a case of rupture of the stomach which was a 80-year-old female, and had suffered from vomiting after lunch followed by abdominal pain and distention. Physical examination revealed muscle guarding and radiography depicted massive free gas in the abdominal cavity. At laparotomy, a rupture was located close by the anterior wall of the greater curvature of the stomach. There was no evidence of ulceration, tumorigenesis, or traumatic history. The operation consisted of debridement around the rupture site and subsequent closure. The rupture was considered to have occurred at a fragile site due to circulatory disturbance of the gastric wall.
