[Mucosa-associated immune system in HIV-1 infection. T-cell subpopulations compared in different segments of the gastrointestinal tract].

T-lymphocyte subsets in duodenal, ileal, coecal, colonal, sigmoidal, and rectal lamina propria were determined in order to compare the distribution of CD2+, CD4+, and CD8+ cell subsets as well as the CD4/CD8 ratio in six different compartments of the gastrointestinal tract. In both HIV1-infected and not HIV1-infected patients, no differences in the distribution of T-lymphocyte subsets could be noted between the compartments examined. The HIV1-infected patients showed a decrease of CD4+ helper cells and CD4/CD8 ratio and an increase of CD8+ suppressor/cytotoxic cells in all examined segments. In terms of lamina propria T-lymphocyte subsets, the intestinal immune system seems to react to HIV1-infection homogenously.

Analysis of c-myc, bcl-1 and bcl-2 translocations in human lymphoma by pulsed-field gel electrophoresis.

Translocations of the c-myc, bcl-2 and the putative bcl-1 oncogene are recurrent events in B-cell lymphoma. Since it is likely that the rearranged genes contribute to the malignant phenotype of the tumor cells, such oncogene translocation is of major interest. The molecular detection of translocations using conventional Southern hybridization analysis is complicated by the fact that translocation breakpoints are dispersed over large chromosomal regions. In order to overcome this problem we used pulsed-field gel electrophoresis (PFGE) to detect c-myc, bcl-2 and bcl-1 translocations in 29 lymph node biopsies. C-myc translocation could not be detected in this group, either with standard Southern analysis of PFGE. Translocations of the bcl-2 gene were detected by PFGE in 5 samples and the breakpoints were mapped in all cases to the third exon of bcl-2 by standard Southern analysis. Furthermore, we also found rearrangements of the bcl-1 locus in 3 samples. Mapping of the breakpoint failed in one of these cases, which strongly indicates the existence of a breakpoint outside the bcl-1 major breakpoint region. Thus, PFGE allows the rapid detection of translocations in human lymphomas within large stretches of DNA.

Chronic erosions of the antral mucosa: a sequela of Helicobacter pylori-induced gastritis.

The etiopathogenesis of chronic erosions of the antral mucosa still remains to be clarified. In order to investigate the question as to whether chronic erosions might be a sequela of Helicobacter pylori-induced gastritis, we submitted biopsy material obtained from 250 patients with chronic erosions of the antral mucosa to a histological evaluation. The grading of Helicobacter pylori colonisation, and gastritis was compared with the results of histological evaluation of the biopsy material obtained from 1,196 patients with Helicobacter pylori gastritis, but no lesions of the gastric mucosa. The results of a histological analysis showed that Helicobacter pylori gastritis was present in 99.1 percent of the patients with chronic erosions of the antral mucosa. A comparison of the histological findings in patients with no lesions of the gastric mucosa revealed, that the density of Helicobacter pylori colonization, the severity of the antral gastritis together with its activity were statistically significantly more pronounced (p < 0.001) in patients with chronic erosions. In contrast, no differences in the morphological parameters were seen in the case of corpus mucosa. On the basis of these results, we conclude that chronic erosions of the antral mucosa represent a sequela of Helicobacter pylori gastritis, and that these Helicobacter pylori-induced chronic erosions should, in future, be differentiated from other erosions.

[Immunoproliferative disease of the small intestine. A rare differential diagnosis of Crohn's disease].

For 2 years a 39-year-old Turkish man had had abdominal pain as well as weight loss of more than 10 kg over 4 months, associated with a raised ESR (33 mm/h) and recurrent gastrointestinal bleedings which endoscopy revealed to have been caused by a marked erosive duodenitis. Biopsies from the duodenum and terminal ileum demonstrated lymphoplasmocytic infiltration of the entire small intestine in the sense of immunoproliferative small intestinal disease (IPSID). By immunohistochemistry the lymphoplasmacellular infiltrate showed polyclonal expression of IgA without IgA increase in serum, urine and duodenal juice. The patient was treated with tetracycline (500 mg twice daily) for one year. The symptoms and histological lesions were much improved after 4 months. One year after diagnosis he was symptom-free.

The diagnosis of early gastric lymphoma.

In analogy to the early carcinoma of the stomach, early gastric lymphoma is defined as a tumour with a maximum depth of infiltration extending into the submucosa. Among 94 consecutive gastric surgical specimens investigated, 45 (47.9%) early lymphomas were diagnosed. Macroscopically, most of these tumours present a non-uniform "variegated picture". There appears to be no relationship between the largest transversal diameter of the gastric lymphoma and its depth of penetration. In contrast, a statistically significant relationship was detected between the degree of malignancy and the depth of infiltration. Involvement of the regional lymph nodes by lymphoma is increased significantly with a high degree of malignancy and greater depths of infiltration, while the incidence of lymph follicles decreases. An analysis of the morphology of the surgical specimens reveals that - in common with early cancer of the stomach - early lymphomas, too, pass through a "malignant cycle". Depth of infiltration and degree of malignancy also have an influence on the number of endoscopic/biopsy examinations necessary to permit a definitive diagnosis of lymphoma.

Differences in Helicobacter pylori associated gastritis in the antrum and body of the stomach.

In 1,265 patients with Helicobacter pylori (HP)-associated gastritis, the degree of HP colonisation correlated in a highly significant manner with the degree and the activity of gastritis in the antrum and body. The degree of HP colonisation is identical in both body and antrum in 49.3%, the antrum being more markedly colonised in 38.7% and the body in 12.0%. In comparison, the degree and activity of gastritis is more pronounced in the antrum in 71.3% and 72.5%, respectively, the corresponding figures for the body being only 3.4% and 7.3%, respectively. The degree of HP colonisation, and also the degree and activity of gastritis, are not age-related. In the body, the degree of colonisation by HP increases with advancing age. In parallel to this, there is also a statistically significant increase in the degree and activity of gastritis in the body. In comparison with the antrum, however, the inflammatory reaction in the body is less pronounced than in the antrum, even with increasing age. The different milieu in the antrum vis-a-vis the body, with changes in acid secretion behaviour is discussed as a possible cause of these differences.

Lymphoid follicles in antral mucosa: immune response to Campylobacter pylori?

The prevalence of lymphoid follicles in endoscopic biopsy specimens from normal antral mucosa (n = 220), mucosa with reflux gastritis (n = 104), and in cases with Campylobacter pylori-associated gastritis (n = 2544) was studied. In the latter group whether there were associations between degree and activity of gastritis and the prevalence of lymphoid follicles and between the occurrence of lymphoid follicles and the presence of intestinal metaplasia in the antrum were investigated. In cases with normal mucosa and in those with reflux gastritis lymphoid follicles were not detected, but mucosal lymphoid follicles were found in 1297 (54%) of the cases with C pylori-associated gastritis. The prevalence of lymphoid follicles in the antral mucosa depended on the degree and activity of the gastritis and also correlated with the presence of intestinal metaplasia. The development of lymphoid follicles in the mucosa of the antrum probably represents, primarily, an immune response to the colonisation of the mucosa by C pylori.

Gastric glandular cysts--investigations into their genesis and relationship to colorectal epithelial tumors.

One thousand five hundred cases presenting with glandular cysts of the gastric mucosa (Elster's polyps) (64.2 per cent women, 38.8 men) were investigated. The average age of 57.1 +/- 14.5 years militates against the possibility of these polyps being Hamartomas. In 41 out of 100 cases submitted to follow-up gastroscopies, these Elster's polyps were either reduced in number and sitze, or else had newly formed in previously unremarkable mucosa--which is further argument against the hamartoma hypothesis and in favour of a functional-secretory genesis. The results of 596 cases submitted to examinations of colorectal epithelial tumours (24.8 per cent in the overall group, and 35.6 per cent in those patients submitted to examination of the entire colon by means of colonoscopy and/or colon contrast enema). In 21 patients with adenomatosis coli (including 11 patients with Gardner's syndrome), in addition to Elster's polyps, a higher incidence of adenomas in the upper gastrointestinal tract was also observed (57.1%), in 5 cases taking the form of initial tubular adenomas in the apex of Elster's polyps. After surgical treatment, some of the patients (28.6%) revealed a reduction in the number of polyps in the stomach. On the basis to these results, a thorough diagnostic work-up of the colon in patients with Elster's polyps is recommended.(ABSTRACT TRUNCATED AT 250 WORDS)

Cyclin-dependent kinase-inhibitor 1 (CDKN1A) in the squamous epithelium of the oropharynx: possible implications of molecular biology and compartmentation.

The cdknlA gene encodes CDKN1A, a protein that regulates cell cycle progression, terminal differentiation, and apoptosis. Polymorphisms or loss of heterozygosity of this usually biallelically expressed gene have no major impact on carcinogenesis. The prevalence of somatic mutations in malignancies is low. Gene rearrangements involving cdknlA are scarce. CDKN1A is expressed in both premalignant and malignant lesions. While the prognostic value of nuclear CDKN1A expression is controversial, the prognostic value of its recently discovered cytoplasmic accumulation is simply unknown. CDKN1A translocates from the nucleus to the cytoplasm when cleaved by caspase-like activities during early apoptosis. The presence of cytoplasmic catabolites (e.g.: p14) might therefore indicate apoptosis. We found no correlation between nuclear and cytoplasmic anti-CDKN1A immunoreactivity in our samples of oropharyngeal squamous cell carcinoma. Morphogenesis in chronic Pneumocystis carinii infection--discussion of three cases.

The clinical and histological features of three HIV1-positive patients are reported of whom two showed persistent focal lesions on serial chest X-rays without any demonstrable lesion on repeated bronchoscopies. The third patient presented with recurrent pneumothorax. By thoracoscopy peripheral lung tissue was obtained in all cases, showing localized histomorphological findings compatible with a prolonged interstitial reaction around Pneumocystis carinii (PC) organisms and their breakdown products. Based on the finding of foreign body granulomas and an increased number of alveolar macrophages, we propose a hypothetical pathway for the development of this unusual reaction to PC infection focusing on the possible secretion of tumor necrosis factor alpha by activated macrophages. By this model the unusual features of PC infection in our patients and in the literature could be explained. Furthermore, in these cases possibly an additional oral or intravenous treatment might be indicated.
