Five cases of arrhythmogenic left ventricular aneurysm unrelated to coronary occlusion.

Five among 19 cases of sustained ventricular tachycardia (VT) treated in the last two years had left ventricular aneurysms, but the patients denied any previous attack of chest pains that would indicate acute myocardial infarction. Laboratory findings including serum electrolytes were normal and no signs of inflammation were found. Coronary angiograms were normal but the left ventriculograms showed aneurysms in four patients and akinetic to aneurysmal wall motion in one patient. Electrophysiologic studies (EPS) were done in four patients. VT was induced reproducibly by programmed electrical stimulation in three patients and it was terminated by programmed stimulation within 30 seconds. The foci of VT were determined by EPS. One case who showed acceleration of the VT rate following the second induction of VT developed a fulminant course; Adams-Stokes attacks from VT, more than ten times a night in spite of intravenous administration of a large dose of procainamide, were terminated by DC shock. VT was determined to originate from the aneurysm that was resected operatively. In the other two cases, the foci were resected and the intraoperative EPS confirmed the preoperative foci. The postoperative EPS showed no inducibility of VT in three surgical cases. Though the induction of VT may not be indicated in every case of VT, we believe that EPS is required to determine the focus for the operation and to evaluate the precise drug efficacy in rapid VT. It is further stressed that the sustained VT of our patients including the present five, lasted for several hours until it was terminated in the hospital.

Long-term postoperative status of tetralogy of Fallot.

From 1965 to 1984, 279 patients with tetralogy of Fallot underwent corrective surgery at Niigata University Hospital, and 228 patients were closely followed postoperatively. Late deaths occurred in 12 cases (5.3%), and overall event-free rate at 20 years postoperatively was 76.9%. The most common events were heart failure due to residual cardiac anomaly and symptomatic ventricular arrhythmias. Treadmill test by Ellestad protocol was performed on 74 postoperative patients, 49% of whom could not complete the protocol. These patients had higher RVEDP and lower RVEF than completing patients. Holter ECG was done on 48 long-term postoperative patients, 48% of whom showed complex ventricular arrhythmias. The age at operation was older and postoperative elapsed time was longer in patients with complex ventricular arrhythmias. Although most postoperative TF patients are able to enjoy a normal social life, the treadmill test and Holter ECG revealed that many patients had latent RV dysfunction and complex ventricular arrhythmias with or without significant symptoms. Since the number of symptomatic patients increases after 15 years postoperatively, a regular check-up with cautious observation is important for the patients' care.

A successful electrical ablation of recurrent sustained ventricular tachycardia in a postoperative case of tetralogy of Fallot.

A 26 year old female patient developed recurrent sustained ventricular tachycardia (VT) after undergoing heart surgery for the third time for chronic pulmonary regurgitation following repair of tetralogy of Fallot. At the last surgery, ventriculotomy was again performed in the outflow tract of the right ventricle. At that time, scar formation and thinning of the cardiac wall were obvious. The surgical result was excellent. Three months later, she lost consciousness. ECG showed VT which was refractory to tolerated doses of antiarrhythmic drugs. Electrophysiologic study showed fragmented activity in the outflow tract of the right ventricle during VT which could be induced and terminated by programmed electrical stimulations. DC energy delivery of 200 Joules to catheter electrodes which recorded local activity 20 msec prior to the onset of the QRS of VT was successful in preventing VT. VT could not be induced by extensive programmed stimulation 1 month later.

Transcatheter partial splenic arterial embolization in patients with hypersplenism: a clinical evaluation as supporting therapy for hepatocellular carcinoma and liver cirrhosis.

Eleven cases with hypersplenism, one with liver cirrhosis and ten with hepatocellular carcinoma (HCC) associated with liver cirrhosis, underwent transcatheter partial splenic arterial embolization. In four of ten HCC cases, the spleen was accidentally infarcted during the procedure of transcatheter hepatic arterial embolization (TAE). The mean infarcted area of the spleen was 55.7%. An increase in the peripheral platelet count was particularly remarkable and continued over one year after the embolization. High fever and abdominal pain were observed in all cases. The fever was seen for 18.0 days and pain was noted for an average of 12.8 days after the embolization. Other adverse effects such as pleural effusion and ascites were transitorily observed. Splenic embolization is an effective supporting therapy for hypersplenism in patients with cirrhosis or HCC.

Synthesis and crystal structures of some 8,5'-carbamoyloxy-bridged purine nucleosides.

The X-ray analysis of 8,5'-(N-carbamoyloxy)-2-dimethylamino- methylene-9-(2',3'-0-isopropylidene-beta-D-ribofuranosyl)guanine (2a) has disclosed that some bond angles are significantly larger or smaller than the corresponding angles of normal ribonucleosides. Abnormally longer or shorter bond lengths have also been found. The base-sugar torsion angle of 2a has proved to reside between the C1'-C2' and C1'-C3' lines, and quite near the latter line. Some new 8,5'-carbamoyloxy bridged adenosines were also synthesized as 2',3'-protected forms.

Acute effects of topical forskolin on aqueous humor dynamics in man.

Previous reports concerning the acute effects of topical forskolin on the intraocular pressure (IOP) and the aqueous humor dynamics in human eyes gave conflicting results, and this led us to reevaluate the effects of this drug in a total of 20 young normal Japanese volunteers. Fifty microliter of 1% forskolin was instilled in one eye, and the vehicle to the fellow control eye. After two instillations of the drug at an interval of 5 minutes, the maximum IOP fall of 2.4 +/- 1.3 mmHg occurred after 1 hour; a single instillation had no significant effect. The effects on the aqueous flow rate and iris permeability factor were determined by the oral fluorescein method. Two instillations of the drug reduced the aqueous flow rate to 87 +/- 7% of the control, while the iris permeability factor was increased to 114 +/- 11% (mean +/- SD, N = 10, P less than 0.005). Pretreatment with topical 0.25% 1-timolol in both eyes 1 hour prior to the drug administration did not significantly alter the forskolin effects.

Ultrasonic aid in transvenous instrumental dilatation for patients with membranous stenosis of the hepatic portion of the inferior vena cava.

We report two cases of a membranous stenosis of the hepatic portion of the inferior vena cava. Membranotomy was performed with a specially designed instrument similar to a Tubbs' dilator following balloon membranotomy. During the procedures, ultrasonographic observations were made and this technique was confirmed to be effective for proper placing of a balloon and a dilator in the stenotic portion. Follow-up evaluation of the procedures was also feasible with this ultrasonic aid.
