Ventricular tachycardia with two possible exits from one re-entrant circuit.

A case of sustained ventricular tachycardia (VT) with two possible exits from one re-entrant circuit is described in this paper. The patient was a 27-year-old female who had undergone corrective surgery for tetralogy of Fallot at the age of 8, and developed VT of distinctly different QRS configurations. The exit of one VT was at the right ventricular (RV) outflow tract and the other at the RV apex as determined by endocardial and pace-mapping. Continuous and split activities were observed at the RV outflow tract during both types of VT. One VT was entrained by rapid pacings, and the causal mechanism was thought to be re-entry and common re-entrant circuit was established for two QRS configurations of VTs. A discrepancy was observed between the site of the earliest activation and the site of the pace-mapping which resulted in QRS configuration of VT of the RV apex origin. The findings in this paper should be considered when contemplating aggressive therapy such as electrical ablation.

Estimation of gene frequency and test for Hardy-Weinberg equilibrium in the HLA system.

This paper concerns the testing for Hardy-Weinberg equilibrium and the estimation of gene frequency in the human leukocyte antigens (HLA) system. An extensive simulation study for both testing and estimation is given for investigating the performance of the projection method by Eguchi and Matsuura, which has a closed form, and the method is asymptotically equivalent to the maximum likelihood method. We compare our projection test statistic with the likelihood ratio test and the single degree of freedom chi-square test suggested by Nam and Gart. Actual mean square errors of the projection estimator of gene frequency under the Hardy-Weinberg equilibrium are compared with the maximum likelihood estimator and some other estimators recently discussed by Nam.

[Successful resection and reconstruction of the left atrium, right atrium and atrial septum under extracorporeal circulation in a patient with invasive pulmonary carcinoma].

A 69-year-old man was admitted to our hospital with a complaint of cough and hemoptysis. His chest X-ray showed an obstruction of the right intermediate bronchus and resultant atelectasis of the middle and lower lobes. Cytological examination by bronchoscopy showed squamous cell carcinoma. Although the cancer involvement of the left atrium could not be clearly defined by preoperative chest CT scan, the cancer invasion to the left atrial wall was recognized intraoperatively. Right pneumonectomy along with resection and patch reconstruction of the left atrium, right atrium and atrial septum was performed under extracorporeal circulation. Postoperative hemodynamic state was stable, and echocardiography showed normal volume of the left atrium. Histological examination of the resected specimen showed moderately differentiated squamous cell carcinoma with the involvement of the left and right atrial wall. The resected margins of the left and right atria were free of malignancy. Although he had been clinically in good condition and free from any sign of cancer recurrence, he died of aspiration pneumonia five months after the surgery.

Effect of calcium ion concentration on the permeability of the corneal endothelium.

The effect of free Ca2+ ion concentration on the integrity of the barrier function of the corneal endothelium was studied using the endothelial permeability to carboxyfluorescein (Pac) as a quantitative index according to the method of Araie. Paired rabbit corneas were isolated and mounted in a chamber. To serve as a control, one eye of each pair was perfused with a glucose-glutathione-bicarbonate solution at a Ca2+ concentration of 1.1 mEq, the other with a solution at various Ca2+ concentrations ranging from 0.23-1.1 mEq. The Pac ratio of a solution with a Ca2+ concentration of 0.38 mEq or higher to the control solution was close to unity, and the Pac ratio of a solution with a Ca2+ concentration of 0.33 mEq or lower to the control solution was significantly greater than unity. In a separate experiment, it was found that only slight swelling was seen when a solution with a Ca2+ concentration of 0.23 mEq or higher was used; significant swelling was seen with a solution of a Ca2+ concentration of 0.17 mEq or lower. The lowest free Ca2+ concentration needed for maintaining the barrier function of the corneal endothelium, 0.38 mEq, was found to be higher than that needed for maintaining the corneal thickness, 0.23 mEq.

[Significance of fiberoptic bronchoscopy after pulmonary resection of lung cancer for early detection in second lung cancer].

By the end of 1988, seventy five cases after pulmonary resection for lung cancer and one case after pulmonary resection for basal cell hyperplasia were examined with fiberoptic bronchoscopy as postoperative follow up. Endobronchial tumor was detected in 14 cases. In seven cases, endobronchial tumor was located at site far from the surgical bronchial anastomosis. These tumors were suspected as second lung cancer as long as examined by fiberoptic bronchoscopy. Histological type of all of these was squamous cell carcinoma. Re-operations were performed in five cases and two of them were early lung cancer. In 6 cases (13.6%) among 44 cases examined with fiberoptic bronchoscopy after pulmonary resection for central type squamous cell carcinoma, second lung cancer was detected. In the case of central type squamous cell carcinoma, usually the tumor appears to be of same histological type and at same location. Among those cases those who smoke heavily should be separated as high risk group. If this group is followed with fiberoptic bronchoscopy or sputum cytological examination, it is more possible to detect second lung cancer at an early stage.

[Long-term follow-up (20 to 25 years) of tetralogy of Fallot after correction].

Eighty patients with tetralogy of Fallot who underwent corrective surgery between 1965 and 1970 were followed and evaluated. Late death was observed in 8 patients, and 3 of them were cardiac deaths. The survival rate at 10-year was 96.2%, at 20-year was 91.7%, and at 24-year was 89.0%. Of the surviving patients, 78% to 85% are spending normal social life. Twenty female patients experienced 31 deliveries, and no babies with cardiac anomaly were born. Most of the complaints come from late arrhythmias, and this is the main problem in the long-term period. Right ventricular function is considered to be moderately reduced by catheter analysis. The importance of preservation of right ventricular function during the surgery should be stressed.

[Long-term results of valve replacement in the right side of the heart in congenital heart disease--comparative study of bioprosthetic valve and mechanical valve].

Valve replacements in the right side of the heart (TVR and PVR) were done on 16 patients with congenital heart disease, mainly tetralogy of Fallot and Ebstein anomaly. Including reoperations, 19 operations were performed on them and 20 artificial valves were inserted. Ten mechanical valves (7 St. Jude Medical valves, 3 Starr-Edwards valves) and 10 bioprosthetic valves (7 Carpentier-Edwards valves, 3 Ionescu-Shiley valves) were used. Age at valve replacement ranged from 9 to 52 years (mean 23.0 years), and the follow-up period was 1.28-19.8 years (mean 5.7 years). Including 2 sudden deaths, late death occurred in 4 patients, on all of whom mechanical valve replacements were done at the primary operation. Five-year survival rate of all patients was 76.4 +/- 12.1%, and 10-year survival rate was 63.6 +/- 15.4%. All the patients who received bioprosthesis at the primary operation survived at the time of this follow-up study. On the contrary, long-term results of mechanical valve was unsatisfactory with the 5-year survival of 62.5 +/- 17.1%. In spite of anti-coagulation therapy with warfarin, three patients with mechanical valve complicated thrombotic valves, which necessitated re-operations. Calcified bioprosthetic valve occurred in one patient with I-S valve 8.5 years after the implantation. Five-year complication-free rate was 87.5 +/- 11.7% for bioprosthesis, whereas it was 50.0 +/- 15.8% for mechanical valve (p less than 0.056). It is concluded that the bioprosthesis is the first choice for the valve replacement in the right side of the heart in congenital heart disease.

[Successful repair of an acute type A dissection during pregnancy].

We report a successful repair of an acute type-A aortic dissection during pregnancy. A 29-year-old pregnant woman in the 32 weeks of gestation with Marfan Syndrome was admitted to our hospital for the treatment of anterior chest and back pains. An urgent aortogram through pulmonary artery revealed an aortic dissection beginning from the ascending aorta to the aortic bifurcation. On the following day, an emergent Cesarean section was performed and a baby weighing 1944 g was delivered. Thereafter, cardiopulmonary bypass was instituted and the ascending aorta was opened. An intimal tear was found 2 cm above the aortic valve. The dissected space was closed with reinforcement of Teflon felt strip. A Dacron gusset was sutured to restore aortic continuity. During the weaning from the CPB, a retrograde aortic dissection developed and the heart became arrested. Therefore we immediately converted the aortic perfusion cannula from the right external iliac artery to graft of the ascending aorta, and the operation was completed successfully with a good recovery of the heart. The mother and her child have been alive and well for 3 years and 5 months after the operation.

[Pulmonary annuloplasty in functional pulmonary regurgitation associated with mitral stenosis: a case report].

In order to prevent disadvantage of pulmonary regurgitation (PR) on cardiac function, pulmonary annuloplasty (PAP) was performed in a 58-year-old female. She suffered from severe mitral stenosis with right heart failure, and was in a state of cardiac cachexia. She was associated with PR due to pulmonary annular dilatation secondary to severe pulmonary hypertension. She underwent mitral valve replacement, tricuspid annuloplasty and PAP around the pulmonary annulus using a Teflon tape. No PR was observed by postoperative angiograms, and the operative method was proved to be effective on the treatment for PR. The PAP is recommended for patients with functional PR secondary to severe pulmonary hypertension.

[Delayed sternal closure following open-heart surgery in infants and children].

Between January, 1982, and December, 1987, 14 patients failed to approximate sternum after open-heart surgery in infants and children. The indications for keeping sternum open were enlarged heart, myocardial edema, severe depression of myocardial contractility and reduced lung compliance due to pulmonary edema. Of the 14 patients, 9 underwent delayed sternal closure between 2nd and 13th postoperative day, and 4 were long-term survivals. All the rest of five patients who were left their sternum open, died of intractable cardiac failure within 16th postoperative day. During the sternum open, three patients suffered from complications-myocardial bleeding in one, and mediastinitis in two. But none of them directly related to the death, and one patient with mediastinitis successfully healed with closed mediastinal irrigation. Although the employment of the delayed sternal closure is rare and limited to the patient with severe heart failure, its judicious use adds advantages in the management of low cardiac output state in infants and children immediately postoperative period.

Testing the Hardy-Weinberg equilibrium in the HLA system.

A new method of testing the Hardy-Weinberg equilibrium in the human leukocyte antigen (HLA) system is proposed and applied to real data. The derivation is based on the maximum likelihood method and closely related to standard regression theory. The test statistic has a closed representation of residual sum of squares by a projection mapping of data onto the estimated regression plane. Under the Hardy-Weinberg law the noniterative estimates for the gene frequencies are suggested by the use of the projection mapping. The test statistic and gene frequency estimates are shown to be asymptotically equivalent to the maximum likelihood method and to be more efficient than the other suggested test statistic when there are more than two identified alleles.

Role of cholesterol 10-methyl group and effect of "extra" 14-methyl group on silkworm growth and development.

In order to establish the functional importance of the 10-methyl group of cholesterol and the planarity of the steroid ring, silkworms (Bombyx mori) were reared on an artificial diet containing 19-norcholesterol (1), 14 alpha-methylcholesterol (3) or 19,19-difluorocholesterol (2). The former two sterols (1 and 3) only partially satisfied the silkworm sterol requirement; growth and development were seriously retarded. The fluorinated sterol (2) was much more deleterious and was totally inadequate in meeting the sterol requirement.

[Pulmonary complication after resection of lung cancer--difficulty in expectoration and cough dynamics].

From January 1975 to December 1986, 415 patients were operated for primary lung cancer. Postoperative pulmonary complications were observed in 83 patients and among them 48 patients (57.8%) suffered from difficulty in expectoration. Postoperative expectoration mostly depends on the ability of coughing. To evaluate cough dynamics, expiratory flow-rate and volume at voluntary maximal cough were measured. The more expiratory flow rate and volume a cough has, the more effective it is for expectoration. In those patients with decreased FEV1.0, or respiratory muscle weakness because of emaciation and aging, or severe pain in the wound, the cough dynamics was decreased. By cleaning retained secretions in the respiratory tract, postoperative pulmonary complications would be prevented. However in cases where the decrease in postoperative cough dynamics is predictable, application of limited resection should be considered as well.

[The effects of transfusion of irradiated blood upon cellular immune response in patients underwent open heart surgery].

The purpose of this paper is to demonstrate the effect of the transfusion of blood received 1500 rad exposure upon the immune response in 14 patients underwent various type of cardiac surgery. 13 patients received known amounts banked blood and irradiated fresh blood, while one patient received a lot of amounts of banked and irradiated and non-irradiated fresh blood. The authors studied the numbers of lymphocytes as well as lymphocyte subsets such as pan-T cells, B cells, helper/inducer T cells (TH/I), cytotoxic/suppressor T cells (Tc/s), active T cells, natural killer (NK) cells and NK cell activity during two weeks after surgeries. In all 14 patients, pan-T lymphocytes decreased markedly in a few days after surgeries, but increased to higher levels on the eight postoperative day than the levels preoperatively. TH/I and TC/S lymphocytes changed on the similar pattern as pan-T lymphocytes. Active T and B cells did not change significantly in two weeks. The number and activity of NK cells gave the lowest levels on the second postoperative day and did not recover to the preoperative levels in two weeks. One patient received non-irradiated fresh blood showed the similar immune response as other 13 patients, while he gave the lower levels than others did. This patient died of graft-versus-host disease (GVHD)-like syndrome on the 36th postoperative day. It may be thought that the transfusion of irradiated blood would prevent the host from GVHD and gave the better effects on the immune response than that of non-irradiated blood following open-heart surgeries.

Synthesis of 1-(5'-O-benzoyl-3'-deoxy-beta-D-glycero-pentofuran-2'-ulosyl)uracil and its alpha-anomer by [1,2]-hydride shift.

In contrast with direct tosylation of 5'-O-benzoyl- (1d) or 5'-O-pivaloyl-1-beta-D-lyxofuranosyl-uracil (1e) with TsCl/pyridine, tosylation of the 2', 3'-O-dibutylstannylene derivatives (4d,e) of these compounds proved to give the 3'-O-tosyl derivatives 2d, e selectively. Coversion of 2d as a model to 1-(5'-O-benzoyl-3'-deoxy-beta-D-glycero-pentofuran-2'-ulosyl )uracil (5-beta) by base-induced [1,2]-hydride shift was examined under various reaction conditions, and the alpha/beta ratio of the product mixture (5-alpha, beta) determined by 1H NMR spectroscopy in each case. The BzOLi/DMF combination has proved to be most profitable for obtaining 5-beta.

[Intermediate-term results after Fontan type operation for single ventricle].

From 1983 to 1985, Fontan type operation were successfully performed on five patients with single ventricle. Marked cyanosis disappeared soon after the surgery, and normal exercise tolerance was obtained in all cases but one in the late postoperative period. Compared to marked improvement in the exercise tolerance test, cardiac output remained subnormal both at rest and during exercise even in the late postoperative period. Several types of asymptomatic arrhythmias were observed during the acute postoperative days, but they were not recognized in the intermediate-term postoperative period. Routine blood chemistry studies showed that hepatic function was within normal limits, but hepatic dysfunction was suggested by Indocyanine Green clearance test. Renal function was well preserved after Fontan type procedure. Though cardiac output is subnormal, Fontan type procedure is a safe and effective operation, and intermediate-term state is satisfactory without any signs of heart failure.

[A successful biventricular assist for postoperative patient with severe heart failure and hepatorenal dysfunction after mitral and tricuspid valve surgery].

We report a case of successful biventricular assist for severe heart failure after open heart surgery. A 62-year-old man suffering from advanced valvular disease accompanied with hepatorenal dysfunction underwent mitral valve replacement and tricuspid annuloplasty on September 22, 1988. Because of inability of weaning from cardiopulmonary bypass, left heart assisted circulation using a roller pump with heparin-coated tubing system was inserted. Following the left heart assist, an right ventricular assist device (RVAD) was subsequently applied to intractable right ventricular failure. He was successfully weaned from an RVAD after 24 hours, and from left heart assisted circulation after 46 hours. At present, he is doing well without significant complications. Earlier application of biventricular assist might be effective for biventricular failure with hepatorenal dysfunction.
