An obstructed death and medical ethics.

In this case analysis deception or lying to a dying patient is discussed within the context of different relationships: the relationship between the patient and her family doctor, the relationship between the patient and the surgeon and the relationship between the patient and her family. It is suggested that the principle of veracity is not only a core feature in the patient-doctor relationship but is also fundamentally connected with the basic element of trust between the patient and doctor. The surgeon, in deceiving the patient, even at the request of her husband, betrayed her trust in violating the principle of veracity. The primary responsibility of the surgeon was towards his patient rather than her family. In lying to his patient the surgeon failed to give Mrs Fasper something she needed most: respect for her as a person.

Sequence analysis of the major outer membrane protein gene of an ovine abortion strain of Chlamydia psittaci.

The major outer membrane protein (MOMP) gene from an ovine abortion strain of Chlamydia psittaci (S26/3) has been cloned and sequenced. The gene shows the features of other chlamydial MOMPs but comparison with the previously reported sequence for the ovine abortion isolate A22/M has revealed substantial sequence divergence which is clustered into the same four intramolecular regions as the sequence variation found between C. trachomatis serovars. Subsequent restriction enzyme analysis of A22/M DNA has shown that it has an avian-type genomic profile and thus the comparison is between types rather than between strains.

Quality: from imperatives to innovations.

I have outlined the quality issues facing critical care nurses today and have also presented some challenges for the future. Critical care nurses must now enter a new era characterized by unification, collaboration and involvement, increased professional autonomy, and caring to meet the customer's expectations for quality in fact and quality in perception. Through innovation, critical care nurses can control the pace of change and influence quality in critical care. Recall the kaleidoscope and remember that its intent is to make the whole seem more than the sum of its parts. The image on the horizon is a dynamic and powerful nursing profession as a whole, with critical care nurses assuming an essential and more important role. In fact, the image on the horizon looks bright to me.

Malalignment of the shoulder after stroke.

One hundred and ten consecutive patients (51 men, 59 women) admitted to a stroke unit were studied for radiographic changes at the shoulder on the affected side. Malalignment was found in 51 (46%) patients, of whom 37 (72.5%) had changes on the initial x-ray film and a further 14 (27.5) developed malalignment over the following 12 months. These findings indicate that malalignment of the shoulder is common in the early stages of a stroke and may be missed unless radiographs are taken with the patient erect and the arm unsupported. Malalignment may lead to delay and limitation in restoration of function. The consequences of malalignment can be prevented by correct handling, positioning, and full passive movement of the shoulder from the onset of the stroke.
