Comparative analysis between clinical requests and magnetic resonance imaging results in neurological diseases.

In order to rationalize the use of MR, which is a high cost technology and not widely available, we analysed comparatively the reasons of test requests and MR diagnosis. We selected three groups of motivations for requests: diagnosis, symptoms and aspecific symptoms: they all were related with MR results classifying concordance, negativity and discordance groups. The results were evaluated also considering whether the patients were hospitalized or outpatients and in relation to the brain, the cervical, thoracic and lumbar regions. Within the general group we found a negativity of 36%, which increases to 52.5% if the motivation of the exam was a symptom and to 88% if the symptom was aspecific, whereas it fell to 26% with the request belonging to the group diagnosis. Concordance was higher in the brain and in the cervical spine (62% and 61%) whereas it was 46% in thoracic and lumbar spine and higher, even if not statistically significative, in the group of hospitalized patients compared to the outpatients.

[Cephalic duodenopancreatectomy with preservation of the pylorus in treating a malignant tumor of Vater's ampulla].

The paper reports on the clinical observation of a patient with mechanical jaundice in which the biliary obstruction was caused by an ampulla of Vater. Cephalic duodenopancreatectomy with preservation of the pylorus was used. Distally the duodenum was excised to its intersection with the mesenteric vessels. For recovery of the digestive circuits, a loop excluded "en Y" à la Roux ascending transmesocolically was used. The anastomoses had the following stages: choledochojejunostomy (T-T) the most proximal; pancreatic-jejunal anastomosis (T-L); and the most distal, the duodeno-jejunal one (T-L). After surgery, the evolution was good, and the patient left the hospital with the wound cured. Starting from this observation, the authors discuss aspects connected with the technique of cephalic duodenopancreatectomy with or without preservation of the pylorus.

Sphincteroplasty of the sphincter of Oddi in the treatment of benign distal obstructions of the bile duct. A prospective study of 70 cases managed by a original surgical technique.

Seventy patients in whom sphincteroplasty was performed by an original technique are presented. In 65 cases the indication was stenosis of the sphincter of oddi, associated or not with cholelithiasis or hepatic hydatid disease. There were relative indications in another 5 patients. Sphincteroplasty was achieved with the aid of an original probe, and average length of the incision of the ampullary area was 28 mm. In the immediate postoperative period there was one case of acute postoperative pancreatitis, one duodenal fistula and an upper digestive haemorrhage; also a residual stone was detected. All these complications have responded favourably to conservative treatment. There was a single death in an old patient with bronchopneumonia. The late results were very good or good with the exception of two cases: one which presented with cholangitis episodes maintained by duodenal stasis, and one female patient, who after one year from sphincteroplasty had to be reoperated on for an hepatic abscess.

A case of cortical blindness due to carbon monoxide poisoning.

We report a case of cortical blindness occurring 7 days after acute CO poisoning with no other neurological or psychic deficits apart from denial of the blindness with visual confabulation and slight loss of retentive memory. There was scant correlation between the course of the clinical pattern, which cleared completely within 6 days, and the electroencephalographic pattern with marked and diffuse slowing, which did not recede completely during 3 months observation. CT scanning of the skull, initially within normal limits, displayed 2 weeks later a faint but diffuse hypodensity of the white substance, more marked in the occipital region, which was no longer present 80 days after the first scan.

Sphincteroplasty in the treatment of biliary tract disease. A prospective study of 46 cases.

Forty-six patients in whom sphincteroplasty was performed by an original technique are presented. In 43 cases the indication was oddian stenosis (associated or not with cholelithiasis or hepatic hydatid disease). Only in three patients a "security" operation was performed. Sphincteroplasty was achieved with the aid of an original probe, and average length of the incision of the ampullary area was 27 mm. The postoperative control, made by tube cholangiography and barium investigation, has evidenced the presence of a wide, open choledochoduodenal aperture. Amylase activity was also followed up in 25 patients. One case of acute pancreatitis, two upper digestive haemorrhages and one case of duodenal fistula were recorded as postoperative complications. Healing was obtained in all patients.

[Tomodensitometric studies in multiple sclerosis].

Thirty-five patients with multiple sclerosis were examined with Computed Tomography of the brain. CT was normal in six cases (17%), plaques of low attenuations were shown in ten patients (29%), cerebral atrophy was present in twenty-five cases (71%). Four areas of contrast enhancement appeared in two patients (5%) after intravenous administration of ioide.

[The mechanism of action of phenobarbital on the metabolism of 131J-B.S.P. in cholestasis induced by ANIT in isolated and perfused rat liver (author's transl)].

In order to establish whether the increased B.S.P. secretion in the bile following phenobarbital administration is the consequence of change in the bile output or represents a consequence of the enzymatic induction, the dynamics of the elimination of 131J-B.S.P. was studied in animals treated with ANIT and Phenobarbital in the conditions of isolated perfused liver. At the same time the levels of the conjugation enzyme of B.S.P. were also determined. It was noted that after Phenobarbital the biliary elimination of 131J-B.S.P. is in correlation with the important increase of the output; in the animals treated with ANIT, on the other hand, the biliary excretion of B.S.P. is minimal. The biliary elimination is directly proportional to the increased of the bile output and reflects the changes found in the perfusate. Our findings allow us to assume that the increased of the bile output and in a smaller extent the result of the stimulation B.S.P. conjugation with Gluthation.

Improvement of perfusion flow in the isolated rat liver under the influence of streptase. Autohistoradiographic aspects of 125I-labelled fibrinogen deposition.

The dynamics of the fibrinoplastic process were studied in rat isolated liver, perfused in normothermia with a synthetical medium. For the autohistoradiographical study, 125I-labelled fibrinogen was added to the perfusing medium. The historadiograms obtained suggest that the main link in the rise of trans-hepatic vascular resistance is thickening of the fibrin muff of the sinusoids. Consequent reduction of the capillary permeability determines favourable conditions for stasis and massive precipitation of fibrinogen under the form of fibrin thrombi into the intrahepatic vessels. Streptase addition to the perfusion medium prevents the intervention of these mechanisms during the studied period.

Protecting action of aspartate on the hepatic changes induced by D-galactosamine.

The protective action of aspartic acid on isolated and perfused rat liver was studied. In case of D-galactosamine intoxication the GOT, GPT and SDH activity and the lactate and pyruvate concentration in the perfusion medium were less augmented and the glycogen level in hepatic tissue was less diminished in animals treated with aspartic acid, as compared to controls. The histochemical applied (PAS reaction for glycogen, nucleic acids, NADH2-diaphorase, glucose-6-phosphatase and membrane-ATP-ase), also stated a protecting effect in the treated animals. The protective action of aspartate is hypothetically considered to be exerted by its capacity to reestablish the cellular deficit of pyridine nucleotides and thus to improve the synthesis of nucleic acids, glycoprotein and glycolipids or/and by its participation in various metabolic pathways.

Improvement of perfusion-flow in the isolated rat liver, under the influence of streptase.

Accumulation of radioactive material in the isolated rat liver was noted when 125I-fibrinogen was added to the perfusing medium. Owing to an enhanced production of fibrinogen and to a diminished fibrinolysis, fibrin deposits were found to occur in the capillaries of fhe isolated liver. This would cause a decrease of hepatic flow and an impairment of the nutrition of this organ which leads to subsequent necrosis. Administration of streptase removed the fibrin deposits and restored the perfusive flow in the isolated liver.
