Isolation of cDNA encoding a newly identified major allergenic protein of rye-grass pollen: intracellular targeting to the amyloplast.

We have identified a major allergenic protein from rye-grass pollen, tentatively designated Lol pIb of 31kDa and with pI 9.0. A cDNA clone encoding Lol pIb has been isolated, sequenced, and characterized. Lol pIb is located mainly in the starch granules. This is a distinct allergen from Lol pI, which is located in the cytosol. Lol pIb is synthesized in pollen as a pre-allergen with a transit peptide targeting the allergen to amyloplasts. Epitope mapping of the fusion protein localized the IgE binding determinant in the C-terminal domain.

Assessment of ultrastructure in isolated cochlear hair cells using a procedure for rapid freezing before freeze-fracture and deep-etching.

Separated cochlear outer hair cells and isolated strips of organ of Corti containing hair cells and supporting cells have been rapidly frozen before freeze-fracture and deep-etching by immersion of samples sandwiched between two copper plates into liquid nitrogen-cooled propane: isopentane. Assessment of this procedure has shown that no significant freezing damage occurs. The ultrastructure of the hair cells revealed by freeze-fracture of these non-chemically fixed preparations was generally very similar to that seen in fixed material. This indicates that the processing of cochlear tissue normally used for electron microscopy produces few obvious structural artefacts. It also demonstrated that procedures for isolating cochlear hair cells generally do not affect cell structure significantly. However, some isolated hair cells did show abnormalities within the membranes of the lateral cisternae. Such membrane alterations, which would not be identified by light microscopy, occurred to a variable extent but were more commonly present after prolonged periods in maintenance medium. Deep-etching of the preparations to examine extracellular features around stereocilia revealed clearly lateral cross-links between stereocilia. However, tip-links could not be positively identified in either unfixed or prefixed preparations.

Trace element and vitamin deficiency in alcoholic and control subjects.

A wide range of trace elements and vitamins was studied in alcoholic patients admitted for detoxification and in healthy controls. Alcoholic subjects were found to be deficient relative to controls in magnesium and vitamin E, while a relative excess of serum iron and copper, and sweat nickel, was noted. A surprisingly wide range of deficiencies, as compared with standard laboratory ranges, was seen in the control group. This finding emphasizes the need for adequate control groups in nutritional studies of alcoholism, the insufficiency of an adequate diet alone to guarantee adequate nutrition, and the likely high prevalence of undetected nutritional deficiency in the general population. Further research is required on the clinical benefits of nutritional supplementation as part of the treatment of alcoholism, and the value of conventional supplements as a routine treatment is questioned.

Gender differences in perceived intensity and affect of pain between athletes and nonathletes.

The present investigation was conducted to test empirically possible gender differences between athletes and nonathletes on measures of perceived intensity and affect of pain. Athletes were 7 male and 7 female NCAA Division I track athletes. The nonathletes, 7 men and 7 women, were randomly selected from undergraduate classes in physical activities. Responses on visual analogue scales for perceived pain intensity and pain affect were recorded for a cold-water pressor test. Significant group differences examined by Bonferroni contrasts indicated differences between nonathletic women and the other 3 groups on intensity of pain and for male and female athletes on pain affect, as well as for male athletes and nonathletes on pain affect. Univariate analyses of variance indicated significant differences on intensity and affect associated with pain. Research must explore whether differences are related to socialization or personality.

A double blind comparative study of lisinopril and enalapril in patients with essential hypertension.

The efficacy and safety profiles of lisinopril (10-40 mg) and enalapril (5-20 mg) were compared in 169 hypertensive patients during 12 weeks' treatment in a randomised double-blind parallel group study. BP was measured hourly for the first 8 hours following the first dose of lisinopril 10 mg and enalapril 5 mg. The peak reduction in sitting systolic and diastolic BP occurred approximately 6 hours post dose in both groups. At 8 hours post dose lisinopril had reduced sitting systolic and diastolic BP by 2.9 mmHg and 3.5 mmHg (P = 0.02) respectively, more than enalapril with similar results for standing BP. One patient on enalapril developed first dose postural hypotension. After 12 weeks' therapy lisinopril produced a greater decrease (P less than 0.05) in BP than enalapril. Sitting BP decreased by 25/15 mmHg on lisinopril and 17/12 mmHg with enalapril. Standing BP decreased by 24/14 mmHg compared with 16/10 mmHg on enalapril. Eighteen patients did not complete the study, 8 on lisinopril (6 adverse events, 1 uncontrolled BP, 1 protocol violator) and 10 on enalapril (8 adverse events, 1 uncontrolled BP, 1 protocol violator). Overall, the results indicated that while both drugs are well tolerated, the dose range of lisinopril 10-40 mg may produce a greater antihypertensive effect than enalapril 5-20 mg.

Single night studies in obstructive sleep apnea.

The role of single night studies and the determinants of effective nasal continuous positive airway (CPAP) pressures were determined in 412 consecutive patients between 1984 and 1989. Patients chosen for analysis had an apnea index (AI) of greater than or equal to 20 hr-1 prior to CPAP. The AI was 67 +/- 30 hr-1, the body mass index (BMI) was 36 +/- 9 kg/m2, the age was 51 +/- 13 yr and the lowest oxygen saturation was 72 +/- 14%. Effective CPAP (9 +/- 3 cm H2O) was documented in 320 patients on single night studies and resulted in a 99% reduction in the frequency of obstructive events and improvement in the lowest O2 saturation to 94 +/- 5%. Only 18% of the variability in effective CPAP could be explained by AI and BMI. Single night studies are sufficient to establish effective CPAP in 78% of patients and offer considerable conservation of resources compared to routine multiple night studies. Effective CPAP pressures are variable and must be determined by incremental CPAP trials.

Recovery of Mycoplasma hominis from blood culture media.

Although several workers have demonstrated that Mycoplasma hominis is responsible for 2 to 20% of cases of septicaemia, reports from Britain do not support this premise--possibly because many of the blood culture systems used fail to sustain growth of this organism. Previous work suggests that this is probably due to the inhibitory effects of sodium polyanethol sulphonate (Liquoid) on M. hominis. In an extension of an in-vitro evaluation of blood culture systems, gelatin was shown to give some protection against the inhibitory effect of polyanethol sulphonate on M. hominis. Of 20 systems evaluated, Becton Dickinson's Supplemented Peptone Broth in their Vacutainer collection system was the most successful at recovering M. hominis.

Reactive type II pneumocytes in bronchoalveolar lavage fluid from adult respiratory distress syndrome can be mistaken for cells of adenocarcinoma.

A growing body of literature illustrates that bronchoalveolar lavage is a reliable and efficient means of diagnosing primary and secondary malignancies in the lung. Its safety in severely compromised patients often makes it preferable to other biopsy procedures. However, a variety of reparative and degenerative pulmonary disorders may result in cytologic alterations so severe that pneumocytes resemble cells of malignancy. We describe four patients with the adult respiratory distress syndrome from whom lavage fluid showed gland-like groups of malignant-appearing cells morphologically consistent with adenocarcinoma. Transbronchial biopsy sections in one case and lavage fluid electron microscopy in another showed that these pseudomalignant cells were reactive Type II pneumocytes with surface microvilli, cell junctions, and numerous cytoplasmic myelin figures. Careful clinicopathologic correlation is the best way to ensure accurate diagnosis in these cases.

The irradiation of V79 mammalian cells by protons with energies below 2 MeV. Part II. Measurement of oxygen enhancement ratios and DNA damage.

The effectiveness of low-energy (below 2 MeV) protons at inducing DNA damage in the form of single- and double-strand breaks has been determined. Protons with mean energies of 1.90 MeV, 1.15 MeV and 0.76 MeV corresponding to track average LETs of 17 keV/microns, 24 keV/microns and 32 keV/microns, respectively, have been used and compared with 250 kVp X-rays and 3.8 MeV 238Pu alpha-particles. Although there was variation in the RBE for DNA ssb induction with LET, the RBEs for dsb induction at all three proton energies and for 3.8 MeV alpha-particles were all around 1.0. This suggests that, if DNA dsb are important in radiation-induced cell lethality, the probability of an induced dsb leading to a lethal event increases with increasing LET of radiation. Oxygen enhancement ratios were measured for both cell survival and DNA dsb induction, and in both cases a decrease in OER with increasing LET was observed.

Pneumocystis carinii pneumonia treated with eflornithine in AIDS patients resistant to conventional therapy.

Eflornithine (DFMO) was used to treat 31 AIDS patients with confirmed Pneumocystis carinii pneumonia who had clinically failed treatment with pentamidine, sulphamethoxazole-trimethoprim or both agents as their first-line therapy. Twenty-one of 31 (68%) responded to second-line treatment with 400mg/kg per day of eflornithine. Five patients discontinued treatment because of bone marrow toxicity. Eflornithine appears to be a useful salvage therapy in patients failing first-line treatments.

Anxiety and depression: discrete diagnostic entities?

Some forms of anxiety and affective disorder, such as panic disorder and major depression, appear distinct, while other forms, such as generalized anxiety disorder and chronic depression or dysthymia, may lie on a continuum and blend with each other. However, even panic disorder and major depression have many common features. Moreover, for reasons not yet clear, they occur together frequently, and their combined occurrence in the same patient has been associated with greater severity and chronicity, decreased treatment responsiveness, and, possibly, increased familial prevalence of anxiety and/or depression. Finally, studies of primary care patients suggest the frequent occurrence of a mixed anxiety-depressive disorder that may often be subsyndromal by DSM-III-R criteria but is nevertheless associated with prominent distress and/or impairment.

Measurement of filtration coefficient in single cerebral microvessels of the frog.

1. This study reports the first results of measurements of filtration coefficient (Lp) and osmotic reflection coefficient to sucrose (sigma suc) in single brain microvessels. 2. Microvessels on the surface of frog brain were cannulated with a micropipette and perfused with an artificial cerebrospinal fluid (CSF) containing the low molecular weight impermeant dye carboxyfluorescein (MW 376). The superfusing solution was a similar CSF which could be made hypertonic by the addition of 40-125 mmol l-1 sucrose. 3. Vessels were assessed for dye retention using video-intensified microscopy after occlusion with a glass microneedle. Only six vessels out of a total of ninety-five were tight under the experimental conditions used. Those vessels which were tight were occluded while an osmotic load was applied across them. When this load was 50 mosmol l-1 and less, the steady-state dye concentration within the vessel lumen was similar to that predicted assuming the endothelium behaves as a perfect semipermeable membrane, with concentration polarization of solute. 4. The product Lp sigma was estimated in two ways: (i) from the fitted monoexponential function that described the rising dye concentration within the occluded segment, and (ii) from the initial rate of increase in dye concentration. The two values obtained were similar and it was concluded that sigma NaCl = sigma suc = 1, and the best estimate for filtration coefficient Lp = 2.0 x 10(-9) cm (cmH2O s)-1. 5. At the osmotic loads of 100 mosmol l-1 and more, the initial rate of increase estimate of Lp sigma was less than half of the whole curve estimate, the axial dye distributions were dissimilar from those predicted by a mathematical model based on the perfect semipermeable membrane, and the steady-state concentration was less than 70% of that expected. These findings are consistent with a diffusive pathway having opened. The model was modified to include patches of vessel wall which had developed leaks and a good fit to the data was obtained with a sucrose permeability and an Lp similar to skeletal muscle endothelium. 6. The possibility that water passes through a paracellular pathway across the intact blood-brain barrier is discussed. It is concluded that this pathway could not be detected by the methods used and can carry no more than 50% of the water driven by a hydrostatic pressure gradient.

Elderly mobility and mobility outcomes: households in the later stage of the life course.

Recent research has demonstrated that elderly residential mobility is conditioned more often by economic factors than is mobility in general. This article explores these issues, specifically those related to the effects of mobility, aging in place, and migration on elderly economic well-being. Using the American Housing Survey national file for 1985, different types of mobility and migration for households in the later stages of the life course are examined. The evidence reiterates central city/suburban locational differences for the elderly, emphasizes the role of house costs, and house cost/income ratios in creating stress for the elderly, and strongly suggests that the elderly population is not simply grouped into young-old, old, and old-old, but is better treated in a continuum of life course changes.

Sodium D-lactate infusion of panic disorder patients.

Nineteen patients with panic disorder received sodium D,L-lactate (racemic) and pure sodium D-lactate infusions in a pilot study. Sodium D-lactate, which is less metabolically active than L-lactate, produced panic attacks in half the patients. D-Lactate also produced hypocapnia and alkalosis, indicating hyperventilation. These findings suggest that metabolism of lactate is not necessary for the induction of panic in susceptible patients. D-Lactate appears similar to other agents that cause panic in its capacity to stimulate respiration in the preliminary study.

Induction of mutations in mouse lymphoma L5178Y cells by ethylmethanesulphonate, benzidine and benzo[a]pyrene.

As part of the third UKEMS collaborative trial, the induction of mutations in mouse lymphoma L5178Y cells was analysed by an agar cloning method. The method used was based on the published methods of Clive and co-workers and Amacher and co-workers. Mutations at the thymidine kinase (tk) locus were analysed following exposure to ethylmethanesulphonate (EMS) in the absence of S9 mix, benzo[a]pyrene (B[a]P) in the presence of S9 mix and benzidine (BZD) in the absence and presence of S9 mix. Mutations were induced under all these conditions and no difference was found between a 48-h and 72-h expression period. Small and large colonies on trifluorothymidine (TFT) selective plates were expanded and found to be stably resistant to TFT. The effects of varying the S9 level and the composition of the cofactor mix on the mutagenicity of BZD and B[a]P were also analysed. The ability of BZD to induce mutations at the hprt locus was investigated. No mutations were detected either in the presence or absence of S9 mix.
