Tunica vaginalis patch graft urethroplasty.

An ideal free graft substance for urethroplasty should: 1, be well accepted at the site of urethroplasty with rapid healing; 2, not invite secondary complications, such as cysts, hair growth and calculus formation; 3, not cause scarring or recurrent stricture disease, and 4, be easy to procure and handle. Tunica vaginalis satisfies these criteria and, in this experimental study, proved to be an excellent graft material for urethroplastic procedures.

Light scattering studies on interaction of Ca2+ with phosphatidylserine liposomes: triggering action of pH.

A novel dependence of interaction of Ca2+ with phosphatidylserine liposomes on pH at different temperatures has been observed. Triggering action of pH, in the range of pH 7 to 9, on Ca2+-induced aggregation/fusion of phosphatidylserine liposomes, with probable involvement of the amino group in the polar headgroup of the lipid, is reported here. Probable role of bound water layer around polar headgroup in the initial stage of Ca2+-induced aggregation/fusion has been suggested. Possible use of triggering action of pH as a mechanism to control fusion processes in biological systems is also suggested.

[Hypertrophic cardiomyopathy: distribution of the hypertrophy evaluated by bidimensional echocardiography and observations on the genesis of the obstruction].

Two-dimensional echocardiography (2-DE) was performed in 38 patients with hypertrophic cardiomyopathy (HCM) in order to study the distribution of hypertrophy. Its importance in the genesis of the obstruction was evaluated in 26 of these patients who also underwent cardiac catheterization. The hypertrophy was confined to the septum only in 14 patients (37%) and involved both septum and free wall in 24 patients (63%). In the 26 patients who underwent cardiac catheterization, extensive hypertrophy (septum + free wall) was found in 16 of 18 patients with a pressure gradient and in 3 of 8 patients without a pressure gradient. Systolic anterior motion of mitral valve (SAM) was present in all patients with extensive hypertrophy and a pressure gradient. Three patients without a pressure gradient had systolic anterior motion of mitral valve. The two-dimensional echocardiographic finding of systolic cavity elimination was present in 7 of 18 patients with pressure gradient and in none of the patients without. We conclude that in hypertrophic cardiomyopathy: Septal hypertrophy is always present and also involves the free wall in a high percentage of the patients (63%). The data are in agreement with previous observation, apart from isolated free wall hypertrophy which was not seen in our series. The presence of extensive hypertrophy involving the free wall seems to be an essential determinant of a gradient. Systolic anterior motion of mitral valve seems to be the most sensitive (sensitivity = 89%) and cavity elimination the most specific (specificity = 100%) echocardiographic sign of outflow pressure gradient.

Ectopic production of placental hormones (human chorionic gonadotropin and human placental lactogen) in carcinoma of the uterine cervix.

Serum hCG and hPL levels were assayed simultaneously in patients with cervical carcinoma and were compared with those of a miscellaneous group of malignant conditions. It is apparent from the results that hCG secretion is of a much wider occurrence as compared to hPL. There seems to be a definite relationship between the clinical staging and ectopic hCG and hPL production. While hCG secretion is predominant in the later stages, hPL is produced in the earlier stage. It therefore seems that hPL might prove to be helpful in early diagnosis of carcinoma of the uterine cervix.

Primary vesical pheochromocytoma.

Pheochromocytoma of the urinary bladder is a rare neoplasm that often occurs with the unique symptom complex of micturitional attacks due to increased catecholamine secretion during detrusor activity. Analysis of 3 of our patients is added to a discussion of 97 cases reported in the world literature. Therapeutic decision is complicated by the absence of unequivocal histologic criteria of malignancy for these tumors. Future perspectives in determining the malignant potential of pheochromocytoma are presented and our treatment plan elaborated.

Dorsovertical lumbotomy approach for surgery of upper urinary tract calculi.

During a 30-month interval 26 patients underwent pyelolithotomy or upper ureterolithotomy through the dorsovertical lumbotomy approach. The intraoperative course and postoperative performance were compared to those in a similar group of patients operated upon using the standard flank incision. Our analysis established the superiority of the dorsovertical lumbotomy incision for all factors evaluated, especially postoperative drainage, analgesic use and hospital stay. The surgical steps are described in detail and the relative contraindications are discussed.

Inflammatory ureteral strictures after ureteroileal diversion.

We describe 2 patients in whom histologically proved inflammatory strictures of the ureter developed after ureteroileal diversion. The dense impassable strictures were located proximal to the ureteroileal junction. The clinical onset was preceded by acute urosepsis and septicemia. Judicious use of percutaneous nephrostomy and antegrade studies is emphasized in establishing the diagnosis.

Surgery of male genital lymphedema.

Incapacitating male genital lymphedema most commonly results from filariasis, which is endemic in the tropical and subtropical countries. However, with the advent of extensive ablative surgical and radiotherapeutic measures against abdominopelvic malignancies, more cases of iatrogenic lymphedema of the genitalia can be expected in other parts of the world as well. Surgical treatment of male genital lymphedema is essentially divided into 1) excision of subcutaneous lymphedematous tissues with genital reconstruction using the remaining skin and 2) complete excision of lymphedema followed by split thickness skin grafting of the denuded phallus. The rationale behind our preference for the latter procedure is discussed with illustrative case profiles and important salient surgical steps are outlined.

Structure and tissue distribution of some retinoid-binding proteins.

Vitamin A has, apart from its function in the visual pigments, general effects on several organs. Early signs of vitamin A deficiency include keratinization of epithelia and hyperkeratosis of the skin. To elucidate a generalized function for vitamin A, we have taken the approach of tracing the vitamin from its storage site in the liver via its blood transport by the retinol-binding protein (RBP) to its uptake by susceptible cells. We have also examined the intracellular occurrence of vitamin A as regards its binding to specific receptor proteins. Here we summarize data on the amino acid sequences of several vitamin A-binding proteins. The finding that CRBP and CRABP, the two intracellular proteins, are homologous to each other, to a myelin protein, and to a fatty acid-binding protein may shed light on the functions of these proteins. Retinoic acid, which binds to CRABP but not CRBP, induces differentiation of teratocarcinoma cells. This is accompanied by a lowering of the CRABP concentration, an increase of the CRBP level, and an increase in the uptake of retinol from RBP. The epidermis contains both CRBP and CRABP, and their distributions are rather similar. However, in contrast to CRBP, CRABP is most abundant in cells lining the hair follicles. CRBP occurs in greatest relative amounts in the outer layers of the epidermis. Since techniques have been developed to measure CRBP and CRABP, normal and disease-affected skin may now be explored as to quantity and cellular distribution of the retinoid-binding proteins.
