Nodular fasciitis of the bladder.

Nodular fasciitis is a nonneoplastic swelling consisting of fibroblastic proliferation and commonly occurring in the subcutaneous tissue of the extremities. We report a case of nodular fasciitis involving the wall of the bladder. It is important to be aware of the histological characteristics of this essentially benign condition so that unnecessary radical therapies can be avoided.

Percutaneous nephrostomy for treatment of hydronephrosis of pregnancy.

Hydronephrosis of pregnancy is a common phenomenon that rarely requires invasive intervention. We describe a young woman with markedly obstructive hydronephrosis of pregnancy causing forniceal extravasation who was treated successfully by percutaneous nephrostomy. The rationale of such intervention and its advantages over retrograde ureteral catheterization or stenting are discussed.

Haemangioma of the maxilla.

Haemangioma involving the paranasal air sinuses is rare. It presents with severe epistaxis or bleeding due to dental extraction and may mimic malignancy. A case of haemangioma of the maxilla is presented and the diagnostic and operative difficulties together with a review of literature are discussed. Bucy and Capp (1930) described primary haemangioma of bone with special reference to X-ray diagnosis. Wyke (1949) was of the opinion that haemangiomas accounted for only 10 per cent of primary benign neoplasms of skull bones. Batsakis (1979) commented that haemangioma of bone accounted for seven per cent of all osseous neoplasms and that from a review of literature he had found no more than 40 such cases. Dahlin (1967) in a review of 3,947 cases of bone tumours, found 47 cases of osseous haemangioma out of which only three involved the upper jaw. Smith (1959) reported that only 10 cases of haemangioma of the maxilla had been published by 1959 and further isolated cases have been described by Sawhney et al. (1973), Pandhi et al. (1977) and Ahad and Chisti (1977).

Stereoselectivity of kappa-opiate receptor ligands in inhibiting the binding of [3H][3-MeHis2]thyrotrophin releasing hormone to brain membranes.

The effect of (+/-)-, (-)- and (+)-isomers of several ligands for kappa-opiate receptors on the binding of [3H][3-MeHis2]-thyrotrophin releasing hormone ([3H]MeTRH) to rat brain membranes has been determined. [3H]MeTRH bound to rat brain membranes at a single high affinity site with maximal binding capacity (Bmax) of 48 +/- 2 fmol(mg protein)-1, and an apparent dissociation constant, Kd of 4.6 +/- 0.2 nM. At a concentration of 2 nM, the specific binding of [3H]MeTRH was 12.3 +/- 0.6 fmol(mg protein)-1. The isomers of ketocyclazocine, tifluadom (1-methyl-2-(3-thienylcarbonyl) aminomethyl-5-(2-fluorophenyl) H-2,3-dihydro-1,4-benzodiazepine), and alpha-5,9-diethyl-2'-hydroxy-2-(3-furylmethyl)-6,7-benzomorphan [MR 2266 (-), MR 2267 (+)] were used for interaction studies. The (-)-isomer of each of the above drugs was more potent than the (+)-form in inhibiting the binding of [3H]MeTRH to brain membranes, whereas the (+/-)-forms had activity intermediate between (-)- and (+)-forms. The order of activity of kappa-ligands was tifluadom greater than MR 2266 greater than ketocyclazocine. It is concluded that kappa-opiate drugs inhibit the binding of [3H]MeTRH to brain membranes in a stereoselective manner with tifluadom being the most potent drug.

The impact of laparoscopy on modern urologic practice.

The original concept of laparoscopy developed as a branch of cystoscopy. Since the use of the Nitze cystoscope at its inception, laparoscopy has kept pace with the innovations in our cystoscopic inventory. The recently emerging interest of urologists in utility of the laparoscope for a variety of urologic surgery is a welcome renascence of this procedure, which has been used mainly by our gynecologic colleagues, in part because of our own lack of pursuit in its development. Laparoscopy should now become an integral part of our urologic teaching and practice.
