Clinical comparison of the SITE IRAS hand-held interferometer and Haag-Streit Lotmar visometer.

The preoperative testing of retinal acuity was evaluated using two instruments in each of 35 eyes having extracapsular cataract extraction. Retinal acuity was measured using the SITE IRAS hand-held interferometer and the Haag-Streit Lotmar visometer. The postoperative findings are given for comparisons of final acuity, prediction, and instrument evaluation. The results showed predicted acuity to be within two lines of the final acuity in 57.1% of eyes with the Haag-Streit unit and in 42.8% of eyes with the SITE instrument. The instrument differences were more in procedure than in results.

Appearance of polychromic endothelial pigment.

Multicolored corneal endothelial pigment was noted in patients on routine specular microscopy, confirming the presence of chromatic pigment deposits at the corneal endothelium. These deposits were polychromic, exhibiting blue, green, red, pink, orange, gold, and yellow colors. These pigment deposits were of different sizes and shapes and often seemed to surround darkened cells or clusters of cells. The variations in color may be the result of differing concentrations of melanin in iris or other pigmented cells that adhere to the posterior surface of the corneal endothelium. To our knowledge, this is the first report of the observation of polychromic endothelial pigment. We encourage the use of color film for specular microscopy to help detect and identify this pigment further.

The punctum pucker procedure for stenosis of the lacrimal punctum.

Stenosis of the lacrimal punctum can be caused by a variety of factors. The Jones one-snip procedure has emerged as the treatment of choice for stenosis that is unresponsive to punctal dilation. A procedure is presented herein that should augment the success rate of the Jones one-snip operation.

Hecht fascia lata needle forceps.

A new instrument is presented for lid tunneling, grasping, and pulling through of the fascia lata in frontalis suspension surgery for ptosis. The instrument, the Hecht Fascia Lata Needle Forceps, is a modification of the small curved hemostat.

Goniomegaly associated with a normal cornea, increased axial length, and minimal refractive error.

The association of a large axial length with a small refractive error and a normal corneal diameter should alert the lens implant surgeon to the possibility that goniomegaly is present. We report a patient whose intraoperative anterior chamber diameter was measured as 15 mm. It was necessary to insert a modified posterior chamber intraocular lens instead of an anterior chamber lens.

Minnesota Multiphasic Personality Inventory: comparison of patients accepting and rejecting myopia surgery.

The Minnesota Multiphasic Personality Inventory (MMPI) was administered to 36 consecutive patients presenting to one surgeon as candidates for myopia surgery. Twenty-one patients proceeded to surgery, and 15 did not. The patients in this series were mature, nonaggressive persons whose basic and clinical MMPI scale values were generally within normal limits. No significant differences were found between the group that underwent surgery and the group that did not. In the group that underwent surgery, no personality characteristics were found to be associated with either a good or a poor surgical result. To our knowledge, this is the first report of the use of the MMPI to objectively evaluate personality characteristics of candidates for myopia surgery.

Punctal ectropion test suture.

We describe here a technique designed to temporarily remove puncta from the lacrimal lake. The "punctal ectropion test suture" (PETS) temporarily rotates the punctum away from the globe, thus interfering with normal tear outflow. The test suture may be used wherever the value of closure of the punctum is contemplated. It is especially useful in symptomatic patients with borderline normal tear outflow.

The skewer technique for anterior capsule remnants.

A technique has been developed which enables the removal of a narrow, long remnant of anterior capsule which is connected inferiorly and inadvertently left during planned extracapsular cataract surgery. The technique is especially useful when the wound has already been partly or completely closed. A partial thickness incision is made at the limbus and a short 30-gauge needle is inserted through the incision into the anterior chamber. The needle then skewers the remnant, tearing it along its edge. The end of the capsular remnant is put on stretch by grasping its edge with a small forceps. This results in a transverse tear, with amputation of the remnant.

Orbital dissection defatting technique for Graves disease.

Five patients with Graves disease and bilateral proptosis were treated with different incisional approaches. They all underwent orbital decompression by removal of the anterior medial orbital walls, the anterior ethmoidal sinuses, the orbital floors, and multiple incisions of the orbital periosteums . The defatting technique, which consists of applying manual anterior orbital pressure with alternate removing of small lobules of fat, was added when it was intraoperatively decided by Hertel exophthalmometer measurement that more decompression was needed. It is estimated that one-third more reduction in proptosis resulted. An average total decrease in proptosis of 9 mm per orbit occurred. Both visual accuities and visual fields returned to normal. The only important complication was the development of hypertropia in down gaze in one patient. A potential value of this technique is its use with orbital floor decompression alone. It may be possible to avoid removing the medial and lateral walls of the orbit, thereby decreasing complications. Defatting may also be a valuable addition in those rare cases where all decompression techniques available are needed to affect an adequate decompression.

Surgical treatment of the denervated or sagging lower lid.

Paralytic ectropion of the lower eyelid and increased curvature of the lower eyelid associated with anophthalmos both cn be optimally treated by use of an autogenous fascia lata sling. Some patients also have problems with prosthesis retention due to lower eyelid deformity with a shortened inferior fornix. In some instances, it is also necessary to perform a horizontal shortening operation on the lower eyelid. In anophthalmic patients, the relationship between prosthesis size and weight and a sagging lower lid is discussed. In some patients when the lower eyelid is elevated, the patient then has an upper lid ptosis for which it is necessary to perform an appropriate levator shortening operation. Surgical technique and illustrative pre- and post-operative photographs are shown.

Prospective evaluation of radial keratotomy using the Fyodorov formula: preliminary report.

Eighteen eyes were preoperatively evaluated and operated on by the method of Fyodorov. Although Fyodorov stated that his formula improves the predictability of radial keratotomy, we know of no previously published studies that included it. They all omitted the Maklakov measurement of scleral rigidity, although Fyodorov stated that it may influence the surgical result profoundly. Maklakov measurement of scleral rigidity does not correlate with Schiotz measurement of scleral rigidity. The overall predictability of the formula at six to 14 months postoperatively was good except in the eyes of a diabetic patient. The formula enables the surgeon to show the patient preoperatively what his likely result will be. Sixteen of 17 eyes (94%) in the refraction range of -2.00 to -7.50 had a visual acuity of 20/50 or better. The patient satisfaction rate was excellent. There were no important complications. Subjective glare and glare independently measured with the Miller-Nadler glare tester was absent in all tested eyes except one eye when it was dilated. Radial keratotomy as described here is safe, effective, and reasonably predictable and may be offered to selected patients who have personal requirements that are not fulfilled by standard optical devices.

Evaluation of the lacrimal drainage system.

Methods of testing for lacrimal drainage insufficiency and a modification of the Jones fluorescein dye tests are presented. Despite their excellence, Jones tests have never been widely employed because of difficulty in dye retrieval. The modification consists of instilling one drop of the dye three times at 15 second intervals, and after a one minute period, placing the head down at a 45 degree angle for ten minutes. Anatomic and roentgenogram studies show that the dye can be easily lost posteriorly unless this is done. No nasal vasoconstrictors are used to avoid false negatives.

Cryotherapy of trichiasis with use of the retinal cryoprobe.

A new method for the treatment of trichiasis is presented. A retinal cryoprobe is used in a freeze-thaw-freeze (double freeze) of the lid. The method is highly successful with no major complications occurring thus far when used on nonpalsied lids.
