Study of antibodies to adenoviruses in patients with tumors of the urogenital system.

The presumable relationship between viruses and malignant disease has been studied. Approximately 4000 assays for antibodies to adenovirus were carried out with sera of patients with malignant and nonmalignant diseases of the urogenital organs and other organic diseases. The search was directed in the first place at antibodies to the early non-virion antigens of adenovirus-12 of oncogenic properties. On the evidence of the complement fixation reaction antibodies to adenoviruses were found to be less frequent in malignant tumors and in hypertrophy of the prostate than in the control group. Antibody to the non-virion antigen of adenoviruses was found in 53 per cent of patients with neoplastic diseases and prostatic hypertrophy, in 18 per cent of patients with urogenital diseases other than tumors and in 4 per cent of those with other organic diseases. The results point to a possible role of adenoviruses in tumors of the urogenital organs.

[Angiomyolipoma of the kidney].

The case of a woman with angioleiomyolipoma is reported. In the light of published data a brief review is presented on the aetiology, pathology and clinical aspects of this extremely rare disease. Angioleiomyolipoma is a benign disease which can be cured by removal of the kidney. In the case at issue, there occurred no tuberous sclerosis in the patient.

[Modified surgical solution to female urethral diverticulum].

Nine cases of female urethral diverticuli were successfully operated in the years between 1978 and 1980. The anatomic pathophysiology of female urethral diverticuli is discussed according to the currently accepted international and Hungarian terminology. A new modified surgical method is described and the possibilities opened by its adequate application are outlined. No postoperative urethrovaginal fistula has ever been observed. It is emphasized that the new surgical method can only be applied to large diverticuli with a narrow neck. In the interest of the patient co-operation between the urologist and gynaecologist is indispensable.

[Acute suppurative kidney inflammation].

In 12 years the authors treated 97 patients with suppurative nephritis in their Department. They performed 78 conservative operations and 19 nephrectomies. The symptomatology and diagnosis of the disease are briefly discussed. Attention iscalled to the importance of early diagnosis and the earliest possible surgical intervention. The dangers of unfounded antibiotic treatment and of chemotherapy are pointed out as these might mask the classical symptoms and render early diagnosis more difficult.

[Treatment of urinary tract infections after prostatectomy].

Patients subjected to transvesical prostatectomy were given before and after the operation Nebcin (Tobramycin) intramuscularly in daily doses of 160 mg for 8 days to prevent infections of the urinary tract which develop almost without fail after operations of this type. The urine of the patients participating in the trials was sterile before operation. Two hours before the operation the patient was given his full daily dose and the serum and urine levels, as well as the concentration of the antibiotic in the tissue were determined. The removed organs were processed under aseptic conditions. Bacteriological assay ensured the measurement of the biologically effective quantity of the drug, in contrast to chemical methods. The 18-hour slanting agar culture of the strain of the species susceptible to the antibiotic under investigation was used. The optical density of the suspension of the culture washed with physiological saline was photonephelometrically adjusted to that of a standard barium sulphate suspension. Compared to the controls the results indicate that the pre- and postoperatively administered antibiotic has a favourable effect on the healing of the wound, and on the progress of the postoperative phase.

[Rapidly growing carcinoma of the penis].

After a brief review of the cancer of the penis the case of a 24-year-old man is described. After five months of the rapidly progressing cancer of the penis no radical operation could be performed in the intact tissue. The importance of biopsy is stressed which has to be repeated, occasionally involving the preparation of series of slides.

Rehabilitation renal failure due to poisoning.

The cases of acute renal failures due to poisoning treated at the artificial kidney department of the University, Department of Urology in Budapest are received. The patients were readmitted later to check their renal function. Prognosis was best in the group with mercury poisoning where renal function of the survivors was completely restored. Similarly promising are the results in other types of poisoning if the patients can be made to survive the acute period. It is emphasized that if poisoning results in symptoms indicating acute renal failure, the patient must be transferred to an artificial kidney department as soon as possible.

[Anemia of chronically hemodialyzed patients].

The correlation between the circulating erythrocyte count, and the NPN level, the number of performed dialyses and the quantity of transfused blood was investigated in 10 patients subjected to haemodialysis for more than six months. No evaluable relationship was found between the circulating erythrocyte count and the NPN level and the number of dialyses. Though the majority of patients had received more transfusions than usual, their anaemia persisted. The quantity of transfused blood can considerably be reduced by carefully chosen indications.

Acute renal failure caused by leptospirosis.

Three patients with severe leptospirosis leading to anuria and treated with haemodialysis are reported. One patient died. The pathomechanism and the underlying pathological and histological changes of the renal failure are discussed. It is stressed that the clinical diagnosis of leptospirosis is often difficult, as other infectious diseases,first of all infectious hepatitis, frequently present the same symptoms.
