Parathyroid glands in primary hyperparathyroidism: an ultrastructural morphometric study of 25 cases.

In parathyroid glands removed from patients with primary hyperparathyroidism (pHPT), hyperplasias and adenomas cannot be distinguished from one another by light microscopy when only one gland is available for examination. When a second gland is available, it is necessary to establish whether it is normal, suppressed, or hyperplastic. This distinction may be difficult, and the main criterion is the amount of cytoplasmic lipid in the parenchymal cells. If the lipid is abundant, the gland is considered normal or suppressed, and if it is scanty, the gland is interpreted as hyperplastic. We have performed a morphometric ultrastructural study to test the reliability of this criterion. Twenty-five adenomatous glands removed from patients with pHPT, when compared with glands of normal size from euparathyroid patients, showed a significant increase in the parameters indicative of metabolic activity, namely the size of the Golgi apparatus, the amount of rough endoplasmic reticulum, and the length of plasmalemmas. In addition, the amount of cytoplasmic lipid was significantly reduced. Furthermore, 25 glands of normal size removed from the same patients with pHPT showed an amount of lipid similar to that of normal glands from euparathyroid patients. However, all the parameters indicative of metabolic activity were significantly higher than those in glands from euparathyroid patients and comparable to those found in adenomatous glands. These results suggest that in pHPT, normal-size glands are as active as adenomatous glands, regardless of a higher lipid content.

Steatosis associated with immotile cilia syndrome: an unrecognized relationship?

The present study deals with a case of hepatic parenchymal steatosis in a child with primary ciliary dyskinesia (immotile cilia syndrome) well documented by functional and ultrastructural evaluation of the ciliary epithelia. Hepatic steatosis was associated with ultrastructural evidence of retention of material either in the cisternae of the endoplasmic reticulum or in proximity of the Golgi apparatus of hepatocytes. It is suggested that the absence of dynein in the axoneme is probably part of a diffuse genetic defect which may extend to cytoplasmic, non axonemal, dynein and lead to a disturbance of various microtubule-dependent cell activities.

Occurrence of brown adipocytes in rat white adipose tissue: molecular and morphological characterization.

Brown adipocytes are thermogenic cells which play an important role in energy balance. Their thermogenic activity is due to the presence of a mitochondrial uncoupling protein (UCP). Until recently, it was admitted that in rodents brown adipocytes were mainly located in classical brown adipose tissue (BAT). In the present study, we have investigated the presence of UCP protein or mRNA in white adipose tissue (WAT) of rats. Using polymerase chain reaction or Northern blot hybridization, UCP mRNA was detected in mesenteric, epidydimal, retroperitoneal, inguinal and particularly in periovarian adipose depots. The uncoupling protein was detected by Western blotting in mitochondria from periovarian adipose tissue. When rats were submitted to cold or to treatment with a beta-adrenoceptor agonist, UCP expression was increased in this tissue as in typical brown fat. Moreover, the expression was decreased in obese fa/fa rats compared to lean controls. Morphological studies showed that periovarian adipose tissue of rats kept at 24 degrees C contained cells with numerous typical BAT mitochondria with or without multilocular lipid droplets. Immunocytochemistry confirmed that multilocular cells expressed mitochondrial UCP. Furthermore, the number of brown adipocytes and the density of mitochondrial cristae increased in parallel with exposure to cold. These results demonstrate that adipocytes expressing UCP are present in adipose deposits considered as white fat. They suggest the existence of a continuum in rodents between BAT and WAT, and a great plasticity between adipose tissue phenotypes. The physiological importance of brown adipocytes in WAT and the regulation of UCP expression remain open questions.

[Peripheral neuropathy associated with Castleman's lymphoma].

Castleman's lymphoma is a rare clinicopathological entity and is often difficult to classify nosographically. From the histopathological point of view two variants are recognized, one hyalino-vascular and one plasma cellular. Moreover Castleman's lymphoma may be multifocal or unifocal (located within the thorax in 70% of patients). The plasma cellular variant is often associated with systemic symptoms such as fever, sweating, organomegaly, polyneuropathy, and mono- or polyclonal gammopathy. We describe a rare case of unifocal, plasma cellular-type Castleman's lymphoma located in the mediastinum, asymptomatic at onset but later dominated by neurological symptoms (chronic isolated polyneuropathy whose clinical expression was predominantly motor). A brief review is presented of the main clinico-histological characteristics of Castleman's lymphoma together with some hypotheses about the pathogenesis of the associated neuropathy.

Structural and functional modifications induced by ketamine on synaptosomes in the rat.

This study correlates previous observations on the effects of a general anaesthetic (2-chlorophenyl-2-methylaminocyclohexanone, trade name Ketalar), administered in vivo on the functional properties of synaptosomes from the brain of the rat, with structural modifications detected using freeze-fracture electron microscopy. The anaesthetized rats exhibited a fluidization of the synaptic membranes, as probed by stearic acid spin labels, accompanied by a decrease in the activity of acetylcholinesterase. The freeze-fracture images of synaptosomes from anaesthetized rats showed a clear statistical increase in the number of vesicles with aggregated intramembrane particles, without any significant change of the particle diameter. The hypothesis that a perturbation of the lipid protein interaction is a primary effect, in the mechanism of action of anaesthetics, is supported at present only by indirect evidence.

Rat interscapular brown adipose tissue at different ages: a morphometric study.

Interscapular brown adipose tissue (IBAT) was studied in newborn and 1, 3, 5, 7, 13, 26, 52, 78 and 104-week-old rats maintained under standard laboratory conditions in order to define its morphological modifications and assess previous statements on an involution phase of IBAT after the first weeks of extrauterine life. Serial sections were analysed morphometrically to quantify the multilocular, the monolocular and the connective-vascular components of IBAT. Morphometry shows that the IBAT weight and volume progressively increase up to the twenty-sixth week of age, these figures remaining almost constant thereafter. The volume of the unilocular and connective components increased with age. The volume of the multilocular component increases up to the sixth month of age, and slightly reduces thereafter. The number of the multilocular adipocytes is almost constant from the third week up to the second year of life. Our results suggest that, in IBAT of adult animals, despite a slight prevalence of unilocular adipocytes, the number of multilocular cells is not reduced with respect to younger animals.

The correlation between magnetic resonance imaging and ultrastructural patterns of brown adipose tissue.

The present paper reports on magnetic resonance imaging (MRI) at high spatial resolution of the brown adipose tissue (BAT) in laboratory rodents, in comparison with light and electron microscopy findings. Our aim was to assess whether MRI correlates with the expected ultrastructural differences between newborn and adult BAT. The study was performed on the cervical and the interscapular BAT deposits by means of a nuclear magnetic resonance (NMR)-spectrometer equipped with a high resolution imaging system. Ultrastructural examination of BAT at different ages showed three different patterns of adipocyte ultrastructure in BAT which were associated with different MRI patterns. In BAT, MRI identifies the prevalent type of adipocyte in the tissue providing information consistent with ultrastructural results. Results presented here show that MRI represents a precise and reliable tool to investigate the morphology of tissues in living animals. The safe, non-invasive MRI technique represents a very useful tool in morphological research.

Morphometric evaluation of intracytoplasmic lipid in normal and pathological parathyroid glands.

The chief morphological criterion for classifying parathyroid glands as 'normal', 'suppressed', or 'activated' is the lipid content of the cytoplasm. In particular, cytoplasm lipid deposit is scanty in the active parathyroid cell, according to many authors. In this paper we present the results of a morphometrical study of ten normal parathyroid glands from patients undergoing thyroidectomy, 20 enlarged 'adenomatous' glands from patients with primary hyperparathyroidism, and 20 glands of normal size from the same hyperparathyroid patients. We aimed at assessing the content of intracytoplasmic lipid of secretory cells in all these conditions. The results show that secretory cells of adenomatous glands have less lipid than those from normal glands; however, there was considerable overlapping of the data in the two groups. The mean lipid content of cells in glands of normal size taken from hyperparathyroid patients was not significantly different from the mean value of normal cells, even when ultrastructural evidence of activation was present in the former. These data suggest that caution has to be exercised in assessing the functional status of parathyroid glands on the basis of lipid content of secretory cells, mainly when differentiating between adenoma and hyperplasia.

Increased tight junction width in two children with Ménétrier's disease.

Protein-losing enteropathy (PLE) and edema are usually the most prominent clinical features in children with Ménétrier's disease. However, the changes in gastrointestinal mucosa that can cause PLE have not been described yet in children. We studied by electron microscopy the mucosa of the gastric fundus, which is the site where macroscopic changes are most prominent, in two children with Ménétrier's disease. We found that tight junction width was increased to 10.5 +/- 0.94 nm (mean +/- 1 SD) in one child and to 9.7 +/- 0.7 in the other. Tight junction width returned to normal when PLE and edema subsided. These ultrastructural changes were similar to those described in adults with the disease, although the clinical course of Ménétrier's disease is very different in adults and in children. Both patients showed evidence of cytomegalovirus (CMV) infection, as indicated by increasing IgG antibodies against the virus or recovery of the virus in the urine. Although Helicobacter pylori was found in the antral mucosa of one patient, the clinical course of the disease was not related to this microorganism. We conclude that increased tight junction width plays a role in PLE seen in Ménétrier's disease in children and that CMV, rather than Helicobacter pylori, is associated with the disease.

Multiple symmetric lipomatosis. Ultrastructural investigation of the tissue and preadipocytes in primary culture.

Multiple symmetric lipomatosis (MSL) is characterized by enlarging, painless fat deposits in the neck and upper trunk. The pathogenesis of MSL is unknown. Owing to localization of MSL fat deposits in the neck and interscapular region, it has been suggested that they could originate from brown fat. However, the histological appearance of MSL adipose tissue is that of white fat, with prevailing monovacuolar adipocytes. Nevertheless, MSL adipocytes are smaller than adipocytes of the common white adipose tissue and show peculiar metabolic features. The ultrastructure of MSL lesions has been not described. The present work investigated the ultrastructural morphology of MSL adipose tissue and lipomatous adipocyte precursors maintained in long-term culture. Samples of lipomatous tissue were obtained from patients affected with MSL undergoing surgical lipectomy. Portion of the tissue was processed for electron microscopy; the rest was digested with collagenase, and isolated preadipocytes from the stromal-vascular fraction were cultured up to 15 days. Cultured cells were prepared for electron microscopy in situ and their morphology compared with human white adipose tissue preadipocytes and rat brown preadipocytes cultured in parallel. Results show the following. 1) Adipocytes of MSL are not monovacuolar and resemble the largest adipocytes that can be found in rat and human brown fat. 2) Some morphological features of MSL adipocyte precursors resemble brown adipocyte more than white: cultured MSL preadipocytes transiently develop large mitochondria with parallel cristae resembling those of the brown fat cell and maintain a multivacuolar lipid deposit in culture, i.e. a typical feature of brown preadipocytes. 3) Some morphological features suggest a neoplastic nature of MSL adipocytes. Taken together, these findings suggest that MSL is a neoplastic disease which could originate in brown fat.

Mitochondria as a feature of smooth muscle differentiation: a study of 70 smooth muscle tumors.

We report that the presence of numerous mitochondria is an ultrastructural feature of smooth muscle neoplasms which is diagnostically useful as a marker for smooth muscle differentiation. The number of mitochondria, as well as the usual features of smooth muscle differentiation, were studied in 70 smooth muscle neoplasms from a variety of body sites. The tumors were sub-classified according to the number of mitochondria (in the majority of the tumor cells) as sparse, moderate, abundant or packed. Thirty-one percent of the leiomyomas had sparse mitochondria and 69% had a moderate number of mitochondria. Seven percent of the leiomyoblastomas contained a moderate number of mitochondria, 33% contained abundant mitochondria and 60% were packed with mitochondria. Twelve percent of the leiomyosarcomas showed sparse mitochondria, 67% showed a moderate number of mitochondria, and 21% had abundant mitochondria. For the above tumor types, the cells with abundant and packed mitochondria contained few or no visible filaments, and these cells were often round or polygonal. By contrast, adjacent spindle cells often contained numerous filaments including dense bodies. The recognition of mitochondria as a feature of smooth muscle differentiation is diagnostically useful in tumor samples showing numerous mitochondria and a paucity of filaments and dense bodies.

S-100 protein in white preadipocytes: an immunoelectronmicroscopic study.

Differentiation of adipocytes from their precursor cells (preadipocytes) is an important problem in the study of the pathogenesis of obesity. Unfortunately, among the immature stages of adipocytes, only relatively differentiated forms can be identified by their fine structure; because early preadipocytes cannot be distinguished from fibroblasts solely on the basis of their morphology, it is impossible to assess the size of the preadipocyte population. S-100 protein has been identified in various mammalian tissues and recently mature adipocytes have been shown to be positive for this protein. Because fibroblasts are negative for S-100 protein, the present study tested the S-100 immunoreactivity of preadipocytes by the peroxidase-antiperoxidase (PAP) preembedding method at the ultrastructural level both in vivo and in culture. Mature adipocytes and early preadipocytes, including fibroblast-like cells devoid of lipid droplets, were positive both in vivo and in culture. Endothelial cells and pericytes were negative; but flattened, lipid-free, fibroblast-like cells surrounding the pericytes were positive. True fibroblasts both in vivo and in culture were negative. Therefore, S-100 protein can be a useful biochemical marker in distinguishing fibroblasts from early preadipocytes.

Ultrastructure of pituitary folliculo-stellate cells of lactating rat during treatment with 2-bromo-alpha-ergocriptine.

Folliculo-stellate cells of adenohypophysis were studied by transmission electron microscopy in lactating rats during treatment with the dopaminergic agonist 2-bromo-alpha-ergocriptine. Folliculo-stellate cells showed marked activation in control lactating rats, were unaffected by a single injection of the drug (3 mg/kg), regressed to the morphology of those found in non-lactating animals upon 6 days of drug treatment (1 mg/kg/die) both in presence and in absence of the suckling stimulus. However, morphometric analysis showed a proportion of folliculo-stellate cells in the gland which was similar in the four experimental groups (range: 11.3-13.5%), despite the increasing reduction of prolactin cells (55, 44, 32, 30%). In chronically treated animals to which suckling had been denied, extruding and degenerating prolactin cells were found, as well as extracellular material in non follicular spaces; the latter were lined by both folliculo-stellate and endocrine cells. These data suggest a dual reactivity of folliculo-stellate cells in the adenohypophysis according to the functional status of the gland.

Pachydermoperiostosis (primary hypertrophic osteoarthropathy): report of a case with evidence of endothelial and connective tissue involvement.

A case of pachydermoperiostosis characterised by the presence of finger clubbing, periostosis, sweating of hands and feet is described. Modifications of capillaroscopic pattern and of arteriovenous anastomoses are reported. The periungual border and finger tip tissue showed diffuse endothelial hyperplasia, hyalinosis, and sclerosis with packing of collagen fibres. Electron microscopy showed hypertrophic and activated endothelia (numerous and hypertrophic Golgi complexes, several Weibel-Palade bodies, vesicles of micropinocytosis, and glycogen particles), the basal membrane thickened and reduplicated, perivasal infiltrate in superficial derma, reticulation and segmentary reduplication of basal membrane in arteriovenous shunt. In the perineural connective tissue numerous Luse bodies (long spacing collagen) were evident. The data indicate that in the early phase of pachydermoperiostosis morphological endothelial and collagen fibre abnormalities are present, though there is a normal peripheral blood flow.

Immunoelectron microscopical identification of the uncoupling protein in brown adipose tissue mitochondria.

The distribution of the uncoupling protein (UCP) in brown adipocyte mitochondria of the hibernant Muscardinus avellanarius was obtained by ultrastructural immunocytochemistry. In both cryosections and sections of Lowicryl-embedded material UCP was localized in the mitochondrial cristae of brown adipocytes, but not in liver mitochondria. It should now be possible to easily identify the morphology of cells committed to BAT differentiation in the tissue as well as in cell culture.

Scanning electron microscopy of the small intestine mucosa in children with celiac disease after long-term dietary treatment.

Jejunal mucosal specimens from twenty children with celiac disease were studied by light microscopy (LM), scanning electron microscopy (SEM), and transmission electron microscopy (TEM) after one year of dietary treatment. An ultrastructural morphometric study was performed in five patients who had an intestinal permeability (IP) test. Seventeen patients were tested for serum antigliadin antibodies (AGA). In ten children, in whom LM showed partial villous atrophy, SEM and TEM examination confirmed the lesion. In the second group (10 children) with normal morphology at routine LM, SEM showed lesions of variable degree in 70% of cases. The morphological ultrastructural investigation showed good correlation with the immunological and functional data (IP test): ultrastructural damage of the jejunal mucosa after one year of a gluten-free diet was found in patients with positive serum AGA and an abnormal IP test. Furthermore, the morphometric study of the ultrastructural alterations allowed a quantitative, closer correlation between morphological and functional data. Our results suggest: 1) SEM and TEM investigations offer additional and more complete information on celiac patients, over LM alone. 2) The morphometric evaluation of the ultrastructural alterations highlights quantitative and reproducible correlations between morphological and clinical data, not strengthened by the subjective, qualitative study.

Morphological identification of brown adipose tissue by magnetic resonance imaging in the rat.

Morphological distinction between brown and white adipose tissue was obtained in the rat by MRI at 7 Tesla. Both superficial (subcutaneous) and deep (mediastinal) brown fat was visualized; the level of resolution was less than 1 mm. These results show that MRI is a potentially useful tool for studying the biology of brown adipose tissue in the experimental animal and, prospectively, in man.
