Mycobacteremia and granulomatous hepatitis following initial intravesical bacillus Calmette-Guerin instillation for bladder carcinoma.

We describe the first case of mycobacteremia and granulomatous hepatitis occurring after the initial intravesical instillation of bacillus Calmette-Guerin (BCG) for bladder cancer. Eight days after BCG instillation, a liver biopsy revealed well-defined granulomas and acid-fast bacilli. A blood culture drawn 8 h after BCG instillation grew Mycobacterium bovis. We summarize the reported complications of BCG intravesical immunotherapy, the associated risk factors, and discuss options for treatment. Although rare, mycobacteremia and granulomatous hepatitis are important systemic side effects of intravesical instillation of BCG for bladder cancer, and should be considered in any patient who presents with persistent fever and abnormal liver function tests after instillation of BCG.

Portal vein thrombosis: a review.

Portal vein thrombosis (PVT) is a rare condition that affects both children and adults. This article reviews the existing literature on PVT, with an emphasis on recent developments. A comprehensive description of etiologic factors and clinical aspects is presented. Treatment issues that remain unresolved are addressed and a framework for the diagnostic work-up and management of patients with PVT is provided.

Ovarian actinomycosis presenting as acute peritonitis.

A patient with acute peritonitis due to ovarian actinomycosis and the association of this disease with the intrauterine contraceptive device is discussed. Because adequate treatment requires prolonged antibiotics the diagnosis should be sure. Unfortunately if antibiotics are given pre-operatively culture is usually unsuccessful and thus careful examination of all removed material is essential. As many of these women are of child bearing age surgical intervention should aim to preserve fertility wherever possible.

Delayed massive hemobilia following percutaneous liver biopsy: treatment by embolotherapy.

A patient developed delayed life-threatening hemobilia after apparently uncomplicated percutaneous needle liver biopsy. An arteriobiliary fistula demonstrated by arteriography was successfully treated by selective transcatheter arterial embolization, with Gelfoam and a Gianturco coil. The diagnosis of liver biopsy-induced hemobilia should be suspected when a postprocedure fall in hematocrit is associated with abdominal pain, hyperbilirubinemia, or unexplained gastrointestinal bleeding. A review of the diagnosis and management of liver biopsy-related hemobilia is presented.

Urinary ascites in the immediate postpartum period.

New onset ascites in the immediate postpartum period is a rare occurrence most often associated with the Budd-Chiari syndrome. We report a patient who developed postpartum ascites secondary to an intraperitoneal bladder perforation.

Heat aggregation studies of phycobilisomes, ferritin, insulin, and immunoglobulin by dynamic light scattering.

Dynamic laser light scattering studies on the heat aggregation behavior of phycobilisomes (PBS), ferritin, insulin, and immunoglobulin (IgG) in dilute aqueous solutions has been reported. Except for PBS, results are reported for heat aggregation trends in these proteins for three different pH environments (4.0, 7.5, 9.1). For PBS, studies were performed only in the neutral buffer medium (pH 7.5). The experiments were performed in the very dilute concentration regime (between 0.23 and 1.8 gL-1). For all these samples heat aggregation and dissociation trends were found to be linear with temperature. Upon temperature reversal (self-cooling), hysteresis-like behavior observed in insulin was found to be predominantly large at pH 7.5. PBS, ferritin, and IgG showed no such behavior at any of three pH values, and retraced their path of aggregation while dissociating on temperature reversal. Heat aggregation and dissociation processes in ferritin were found to be independent of pH. The IgG samples showed smooth aggregation tendency only up to 35 degrees C in the buffer media pH 4.0 and 9.1, whereas for pH 7.0 the same could be observed until 60 degrees C. Low polydispersity in the correlation spectra was observed in case of all these samples.

Hepatic schistosomiasis: report of two cases and literature review.

Two Filipino patients were referred for evaluation of abnormal liver enzymes. Both patients were known to come from regions endemic for Schistosoma japonicum. The clinical laboratory and radiological features of these patients are presented. We highlight the findings of radiologic evaluation, including magnetic resonance imaging (MRI). The epidemiology, clinical manifestations, diagnostic strategies including a comparison of the utility of ultrasound, computed tomography, and MRI are reviewed. The pathophysiology responsible for hepatic manifestations of schistosomiasis is reviewed. An increased influx to the United States of persons from regions endemic for schistosomiasis, such as South America, Southeast Asia, and the Philippines, will result in an increase in the prevalence of the disease in this country. The diagnosis of hepatic schistosomiasis will need to be considered in populations at risk for contracting this common worldwide problem.

Antibiotic prophylaxis in cochlear implantation: current practice.

A postal survey examined the current use of prophylatic antibiotics during cochlear implantation. Seventeen centres, worldwide, replied giving details on 1030 cochlear implant operations: 56.4 per cent were implanted with antibiotic prophylaxis and 43.6 per cent without; 2.9 per cent of devices were removed because of infection, 4.5 per cent of those with prophylaxis and 0.9 per cent of those without. Most cases of infection were associated with technical problems in the design or the positioning of the device. No cases of meningitis were reported. Routine antibiotic prophylaxis does not appear to offer any benefit.
