Congenital hypothalamic hamartoblastoma, hypopituitarism, imperforate anus and postaxial polydactyly--a new syndrome? Part I: clinical, causal, and pathogenetic considerations.

We report on six infants with a neonatally lethal malformation syndrome of hypothalamic hamartoblastoma, postaxial polydactyly, and imperforate anus. Some, but not all, patients had laryngeal cleft, abnormal lung lobulation, renal agenesis and/or renal dysplasia, short 4th metacarpals, nail dysplasia, multiple buccal frenula, hypoadrenalism, microphallus, congenital heart defect, and intrauterine growth retardation. The infants also had hypopituitarism and hypoadrenalism. All were sporadic cases, parents were not consanguineous, chromosomes were apparently normal. Family histories were unremarkable. There was insecticide and/or herbicide exposure in several of the cases, but no exposures were common to all 6 mothers. Five of the patients were born within an 8-month period, but all in different geographic locations. It is postulated that this is a previously apparently unreported syndrome of presently unknown cause.

Some observations on the cholesterol esterifying and cholesterol ester hydrolyzing activities in dog plasma.

Cholesterol esterification and cholesterol ester hydrolysis in dog plasma were investigated. Esterification proceeded linearly for 60 min, and the amounts of cholesterol esterified were in the range of 0.13-0.18 mumol/ml/h. No change of acyl composition had occurred in newly formed cholesterol esters during incubation. With the addition of Na taurocholate (10 mM), complete inhibition of the esterifying activity and maximal activation of the hydrolase activity were observed. Approximately 50% of cholesterol esters present in plasma was hydrolyzed in 10 min of incubation, and the reaction was completed within 60 min. The maximal rate of hydrolysis was estimated to be 4.0-5.4 mumol/ml/h, and polyunsaturated esters were hydrolyzed more rapidly than saturated ones. The esterifying activity was detected in high density (HDL) and very high density lipoproteins (VHDL), while the hydrolytic activity was found only in VHDL. Each lipoprotein fraction served as a good substrate for hydrolysis, while HDL was the sole substrate for esterification. The optimal pH of the hydrolytic activity in VHDL lay in a broad range between 6.8 and 7.2 and the apparent Km was determined as 12.5 x 10(-3) mM for cholesteryl oleate.

Characterization of 5'-terminal methylation of human alpha- and beta-globin mRNAs.

After incubating peripheral blood of various anemic individuals with [methyl-3H]methionine, 5'-terminal cap structures and the sequential methylation of human globin mRNAs were determined. Human alpha- and beta-globin mRNAs have the same methylated nucleotides at their 5' ends as the alpha- and beta-globin mRNAs of mouse and rabbit: m7G(5')ppp(5')[m6Am/Am]pCmp. The order of methyl group addition to human globin mRNA is also similar to that of mouse globin mRNA in that methylation of the 5'-terminal guanosine and the ribose of adenosine occurs earlier than that of the base of adenosine and the ribose of cytidine. Thus, both the cap structures and the order of methylation are conserved in the globin mRNAs of these species, suggesting that variation in the structure of the 5' terminus of globin mRNAs may alter the function of these molecules.

Effect of different levels of dietary alpha-tocopherol and linoleate on plasma and liver lipids in rats.

The influences of dietary alpha-tocopherol, in combination with three different levels of linoleate (low, moderate or high levels), on the concentrations of plasma and liver lipids in rats were examined. Male Wister rats were fed diets ad libitum for 2 weeks. dl-alpha-Tocopheryl acetate was added to the diets in 0, 20 or 200 mg tocopherol/100 g diet. The following results were obtained. (1) Higher dietary levels of alpha-tocopherol tended to cause higher plasma lipid levels in proportion to the amounts added. This phenomenon is most remarkable in rats fed a diet containing cholesterol and low linoleate (0.8% of calorie). (2) The concentrations of plasma lipids were influenced significantly than those of liver lipids by the levels of alpha-tocopherol under the conditions of this experiment.

The terminal pathway of the lymphatic system of the human heart.

Aantomical dissections in 9 human cadavers revealed the terminal pathway of the lymphatic system of the left ventricle to be constituted mainly by channels emptying into the right angulus venosus (junction of the internal jugular and subclavian veins) at the base of the right side of the neck. This observation has clinical implications because it has been shown that a sampling of cardiac lymph provides the best method for analyzing myocardial metabolic abnormalities and that drainage of cardiac lymph alleviates the myocardial changes produced by ischemic injury.
