Binding characteristics of aromatase inhibitors and phytoestrogens to human aromatase.

We have evaluated the binding characteristics of three steroidal inhibitors [4-hydroxyandrostenedione (4-OHA), 7alpha-(4'-amino)phenylthio-1,4-androstadiene-3,17-dione (7alpha-APTADD), and bridge (2,19-methyleneoxy) androstene-3,17-dione (MDL 101,003)], four nonsteroidal inhibitors [aminoglutethimide (AG), CGS 20267, ICI D1033, and vorozole (R83842)], and two flavone phytoestrogens (chrysin, and 7,8-dihydroxyflavone) to aromatase through a combination of computer modeling and inhibitory profile studies on the wild-type and six aromatase mutants (I133Y, P308F, D309A, T310S, I395F, and I474Y). We have generated two aromatase models based on the x-ray structures of cytochrome P450-cam and cytochrome P450bm3, respectively. A major difference between the cytochrome P450cam-based and cytochrome P450bm3-based models is in the predicted lengths of helices F and G. In the cytochrome P450cam-based model, helices F and G lie antiparallel and extend across the active-site face of the molecule from one edge to the center, so that the carboxyl-terminal residues of helix F and the N-terminal residues of helix G make a major contribution to the structure of the active site. In the cytochrome P450bm3-based model, both helices are longer and so extend almost all the way across the active-site face of the molecule. Considering the size of the androgen substrate, we evaluated our results mainly based on the cytochrome P450cam model. The mutations involved in this study are thought to be at or near the proposed active site pocket. The inhibitory profile analysis has produced very interesting results and provided a molecular basis as to how seven aromatase inhibitors with different structures bind to the active site of aromatase. Furthermore, the investigation reveals that phytoestrogens bind to the active site of aromatase in a different orientation from that in the estrogen receptor.

Gene regulation studies of aromatase expression in breast cancer and adipose stromal cells.

The expression of aromatase in human breast tumors was studied using the reverse transcription-polymerase chain reaction (RT-PCR) method on 70 breast tissue specimens. An RT-PCR analysis using two oligonucleotide primers derived from exon II of the human aromatase gene revealed that aromatase mRNA was detected in all but three tissue specimens. Furthermore, primer-directed RT-PCR was performed to determine the exon I usage in aromatase mRNA in these breast tumor specimens. The analysis revealed that exons I.3 and PII are the two major exons I present in aromatase mRNA isolated from breast tumors, suggesting that promoters I.3 and II are the major promoters driving aromatase expression in breast cancer and surrounding adipose stromal cells (ASCs). Recently, the regulatory properties of a 696-base pair region that contains promoter II, and is situated immediately upstream of exon II of the human aromatase gene, were investigated. Detailed DNase 1 footprinting analysis, DNA mobility shift assays, and chloramphenicol acetyltransferase (CAT) functional studies of this genomic region were performed and led to the identification of a segment (B1) that could act as a promoter (probably promoter I.3) in adipose stromal and breast cancer cells. The study further revealed that the B1 region could be divided into two domains which were designated RE1 and RE2. RE1 was found to have the promoter activity, and RE2 was found to regulate the promoter activity of RE1, but in different manners in MCF-7 cells (as an example of breast cancer cells) and in ASCs. RE2 was found to function as a positive regulatory element in MCF-7 cells and as a negative regulatory element in ASCs, respectively. It was also found that in several breast cancer cell lines, including MCF-7, the promoter activities of both promoter II and promoter I.3 were found to be suppressed by a negative regulatory element, a silencer, present in the 162 bp fragment which is located upstream from promoter II and downstream from promoter I.3. The precise position of the silencer element (termed S1) was localized by deletion mutation and DNase 1 footprinting analysis, and the silencing activity of S1 on promoter I.3 (in B1 fragment) was confirmed by CAT plasmid transfection experiments. UV crosslinking experiments are being performed to examine the regulatory proteins interacting with the silencer element. These studies serve as the basis for the further characterization of the regulatory mechanism of aromatase expression in human breast cancer and ASCs.

Isotypic antibody responses of a population in an endemic area of schistosomiasis japonica and their epidemiologic significance.

The present study was designed to explore if there exists a correlation between predominant isotype-defined antibody levels and reinfection in low age groups of the population in an endemic area of schistosomiasis japonica in China. One hundred and thirty-eight individuals aged 3-25 years old were selected for serological investigations including the levels of IgG, IgG4, IgM and IgE, detected by ELISA with soluble egg antigen and soluble adult worm antigen. Results show that age is a determinant for SEA-specific IgG, IgG4, and IgE, and SWA-specific IgG and IgG4 antibody levels, which increased with age, and that SEA- and SWA- specific IgG4 antibody levels are risk factors of reinfection, ie, the risk of reinfection occurrence of the population with high level of SEA or SWA-specific IgG4 is 2.83 or 2.40 times, respectively, that with low level of SEA or SWA-specific IgG4, suggesting that in the endemic area of schistosomiasis japonica, there exists a possibility that in the population aged 3-25 years, SEA and SWA-specific IgG4 antibodies mediate a blocking immunity response.

Sexually transmitted disease risk assessment used among low-risk populations in east central Africa: a review.

In sub-Saharan Africa where sexually transmitted diseases (STDs) are prevalent but health resources are scarce, algorithms that act as STD/HIV risk assessment (RA) tools for family planning (FP) and maternal/child health (MCH) clinic attenders may be useful. These RA algorithms generally comprise questions about a person's social, economic, demographic, behavioural and clinical background. The answers can be scored or scaled to indicate a higher probability of current STD/HIV infection and thus the need for testing or treatment. RA can be done alone, or can be combined with clinical examination where laboratory diagnosis is not feasible. But how accurate are RA tools? We reviewed six studies at African sites that examined the use of RA combined with a clinical algorithm for STD screening of clinic attendees. Researchers reported the standard measures of test accuracy: sensitivity (per cent of infected women found positive by the tool); specificity (per cent of uninfected women found negative by the tool), and positive predictive value (per cent of women found positive by the tool who are truly infected). Most algorithms had low sensitivity (< 50%) and low positive predictive value (< 20%). The addition of clinical examination, and of leucocyte esterase (LE) testing, improved accuracy. But algorithms that performed well in one site were less accurate at other sites. For illustrative purposes, we considered a hypothetical population of 1,000 women attending a FP clinic. With a prevalence of cervical gonorrhoea of five per cent, algorithm sensitivity of 40%, and algorithm specificity of 75%, the positive predictive value of the RA tool would be 7.7%. In other words, 20 (7.7%) women diagnosed as infected would actually have gonorrhoea, and 237 false positive women would be treated needlessly, with the attendant costs and risks. However, with a disease prevalence above 50%, as might prevail for vaginal infections, and the same sensitivity and specificity figures, the positive predictive value may reach 70-80%. Risk assessment appears to be a relatively easy way to integrate STD management into family planning and MCH practice. However, there is no evidence to show its effectiveness for screening low-risk populations. We urge improvement of these tools, including information on the behaviour of male partners, and addition of simple screening tests such as leucocyte esterase dipsticks.

The study of IL-1 beta, TNF-alpha, IL-6 gene expression and plasma levels on hemodialysis before and after dialyzer reuse.

OBJECTIVE: To investigate the biocompatibility of dialyzer reuse. METHODS: Twenty-two hemodialysis patients were randomized into cuprophan (CU, 7), polymethylmethacrylate (PMMA, 7) and polysulphone (PS, 8) membrane groups to observe IL-1 beta, TNF-alpha, IL-6 gene expression and their plasma levels by using themselves as control with enzyme-linked immunosorbent assay (ELISA), reverse transcription-polymerase chain reaction (RT-PCR) and in situ hybridization. RESULTS: Plasma levels of interlukin-1 beta (IL-1 beta), tumor necrosis factor-alpha (TNF-alpha), interleukin-6 (IL-6) were 14.07 +/- 3.32 pg/ml, 67.41 +/- 19.79 pg/ml, 83.67 +/- 40.34 pg/ml; 12.80 +/- 3.01 pg/ml, 49.65 +/- 9.75 pg/ml, 33.36 +/- 12.14 pg/ml and 14.41 +/- 3.16 pg/ml, 80.56 +/- 23.22 pg/ml, 48.14 +/- 16.01 pg/ml, respectively, after patients dialyzed with CU, PMMA and PS membranes. Plasma cytokine levels decreased after reuse compared with those before reuse in each group. But no significant difference was found between them (P > 0.05); the levels of IL-1 beta, TNF-alpha, IL-6 gene expression after reuse were 5.61 +/- 0.33, 2.11 +/- 0.12, 5.04 +/- 0.19%; 2.43 +/- 0.19, 1.29 +/- 0.11, 3.48 +/- 0.20% and 2.48 +/- 0.20, 1.24 +/- 0.11, 3.22 +/- 0.20% respectively by in situ hybridization, and 0.92 +/- 0.07, 0.63 +/- 0.05, 0.53 +/- 0.05; 0.61 +/- 0.06, 0.47 +/- 0.04, 0.37 +/- 0.03 and 0.59 +/- 0.05, 0.44 +/- 0.04, 0.38 +/- 0.03 by RT-PCR, respectively. After reuse there was significant decrease as compared with that before reuse (P < 0.001, P < 0.005 and P < 0.05, respectively). CONCLUSIONS: This suggested reprocessing dialyzer with formaldehyde reduced cytokine release and gene expression in peripheral blood mononuclear cells and enhanced dialyzer biocompatibility. It would be beneficial to reduce the dialysis cost and may reduce the complication related to a long term hemodialysis.

Significance of P-selectin expression in human glomerulonephritis.

OBJECTIVE: To examine the relationship between P-selectin expression and human glomerulonephritis. METHODS: P-selectin expression was investigated in renal biopsies of glomerulonephritis patients (n = 133) by immunohistochemical and in situ hybridization analyses. They were divided into three groups based on the degrees of cells proliferation or sclerosis. Group I: histological lesions in glomeruli are mild (n = 23); Group II: glomerular cells proliferation is prominent (n = 91); Group III: glomerular sclerosis is severe (n = 19). The Kruskal-Wallis test, Chisquare test and Spearman's correlation were used in our study. RESULTS: In normal controls, renal P-selectin expression was negative (n = 10). In glomerulonephritis, the up-regulated P-selectin expression on tubular epithelium was significantly higher than that on glomeruli and interstitium (P < 0.01). P-selectin was expressed predominantly on platelets in glomeruli, and glomerular P-selectin expression was more significantly up-regulated in Group II than in Group I or Group III (P < 0.05; and P < 0.01, respectively). There were strong correlations between the degree of tubulointerstitial lesions and the expression of P-selectin on tubular epithelium or within interstitium (rs = 0.395 and rs = 0.337, P < 0.01). Furthermore, P-selectin messenger RNA (mRNA) signals were also detected in the cytoplasm of glomerulus, tubular epithelial cells, interstitium and vascular endothelium. CONCLUSIONS: P-selectin might mediate intraglomerular platelets aggregation, activation and leukocyte accumulation in the early stages of human proliferative glomerulonephritis. The up-regulation of P-selectin in interstitium was associated with interstitial fibrosis and tubular atrophy and may contribute to the progression of glomerulonephritis.

Biological and clinical significance of cytogenetic study on 100 acute lymphoblastic leukemia and 219 acute non-lymphoblastic leukemia.

OBJECTIVE: To further investigate the role that cytogenetic may play in the diagnosis and prognosis of leukemia, a study was conducted in 319 acute leukemias. METHODS: 100 patients with acute lymphoblastic leukemia (ALL) and 219 patients with acute non-lymphoblastic leukemia (ANLL) were from Rui Jin Hospital, Xin Hua Hospital, Ren Ji Hospital and Shanghai Children's Hospital. Their cytogenetic data were analyzed together with those of morphology, immunology and clinical prognosis. RESULTS: In ALL group, 48 cases were karyotypically normal whereas 52 cases revealed chromosomal changes, among which 32 had quantitative abnormalities and 20 had qualitative abnormalities. The translocation t(9; 22) was identified in 11 out of 20 cases of structural aberrations (55%). Specific structural aberrations t(9; 22) and t(8; 14) were detected to be related to B-lineage associated differentiation antigens and t(8; 14) also with ALL-L3 according to FAB classification. With regard to clinical prognosis, the survival rate of structural aberration subset decreased significantly compared with the normal karyotype subset (P < 0.05). However, no statistically significant difference was found between hyperdiploidy subset (not including near-triploidy) and normal karyotype subset (P > 0.75). In ANLL group, 80% of de novo patients and relapsed patients had chromosomal abnormalities. Importantly, structural aberrations accounted for 73% of these abnormalities and frequently corresponded to specific types of FAB classification. Relevant prognostic studies demonstrated that t(15; 17) subset had the best overall survival probability, followed by t(8; 21) and normal karyotype subset, while the numerical aberration subset showed a relatively poor prognosis. CONCLUSION: Our data confirmed that cytogenetic study is important for the molecular study of the leukemogenesis. On the other hand, it also provides an independent parameter for prognosis in acute leukemia.

[Cellular adhesion molecule (CD44) in ovarian tumors].

OBJECTIVE: To investigate a method of detecting adhesion molecule (CD44) contents in ovarian tumors and its clinical significance. METHODS: In 106 patients with ovarian tumors (50 benign and 56 malignant), the adhesion molecule (CD44) contents in peripheral blood lymphocytes (PBL) and in neoplastic tissues were analysed (quantitatively by using flow cytometric-immunological method). RESULT: (1) The CD44 contents in the PBL of the patients with malignant tumor were obviously higher than those of patients with benign tumors (P<0.05) and than those of the control (P < 0.01); (2) Within the group of patients with malignant tumors, the CD44 contents in the neoplastic tissues were higher than those in the PBL (0.01 < P < 0.05); There was no significant difference in CD44 contents among the different histopathological types, whether benign or malignant; (4) The CD44 contents in the PBL decreased gradually after surgery. CONCLUSION: To analyse the CD44 contents in PBL and neoplastic tissues by flow cytometry may be an important method or parameter to differentiate benign ovarian tumors from malignant tumors and to discover recurrence of metastasis of malignant tumors in early stage.

Serological investigation on human trichinellosis spiralis in Hubei Province of PR China.

PVC-ELISA for detection of human trichinellosis and its value of field application was explored during 1988-1994. Sera of 56 cases of clinically confirmed trichinellosis were tested for serum Trichinella antibodies by way of PVC-ELISA, and the positive rate was 100%. When the sera of 80 cases of healthy persons from non-endemic areas were tested for trichinellosis, they were all negative. For the purpose of exploring the cross-reaction of PVC-ELISA used for detection of trichinellosis, sera from 144 cases of other parasitoses were tested. Among them, the sera of 80 cases of schistosomiasis japonica for trichinellosis were negative; 30 cases of paragonimiasis and 30 cases of clonorchiasis were also negative. Among sera of 4 cases of cysticercosis, there was only one positive. The results showed that PVC-ELISA for trichinellosis had a high sensitivity and specificity. For field application, a population of 800 from endemic areas were examined by PVC-ELISA, 85 (10.63%) were positive. The infected persons were mainly in Xiangfan region of Hubei Province. The percentage of the positive of trichinellosis in Xiangfan region was 64.71% (55/85). The positive rate of males and females was 13.41% (57/425) and 7.47% (28/375), respectively. There was a difference among the age groups, with the highest in those > or = 20 years. The findings of this field investigation suggested that the infection rate of trichinellosis spiralis was high in this province, especially in the Xiangfan region; and that PVC-ELISA was suitable for field application. The emphasis of trichinellosis control and research of this province should be in the Xiangfan region.

[Behaviour after transplantation of brain cells into monkey models of Parkinson's disease].

OBJECTIVE: To observe the improved degree of pathogenic behaviour in monkey models of Parkinson's disease after transplantation of substantia nigra cells of human fetus. METHOD: 1-methy-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) was used to prepare monkey models of hemiparkinson's disease, i.e., early substantia nigra cells of human fetus were stereotaxically transplanted into PD monkey's behaviour for six months, using immuno-electromicroscopy to prove the transplanted survived neuron. RESULTS: After transplantation, the model monkey's motion slow and muscle hypertonia was apparently improved. Limb tremor almost disappeared, Right spontaneous rotation behavior relieved. Rotating frequency caused by APO became less than before transplanytation, and its effect could last a year. Under the immunoelectromicroscope, we observed that transplanted tyrosine hydroxylase positive neuron was connected with the host brain cells in synapse. CONCLUSION: Substantia nigra neuron transplanted into the PD monkey brain can establish synaptic connection with the host nervous cells. Pathogenic symptoms are improved and the effect may remain for a longer time.

[Prognostic factors of dermatomyositis: analysis of 119 cases].

In order to know the prognosis of dermatomyositis, 119 patients with dermatomyositis were investigated from June to September 1995 by Shanghai Medical Cooperation Center on Dermatomyositis. Ten prognostic factors were studied with Lifereg Procedure multiple factor analysis. The results showed that 2 and 5-year survival rates were 75.3% and 53.2% respectively. The most common cause of death was lung infection and then was malignant tumor. Unfavorable prognostic factors were old age, malignant tumor, interstitial lung disease, dysphagia, cardiac involvement, fever, failure of remission and steroid therapy alone. The survival curve showed that mortality rate was high in the first three years, less between three and seven years and none after seven years. The results showed that dermatomyositis is a disease with poor prognosis. Patients with dermatomyositis can be adversely affected by many factors and should be treated regularly with steroid and immunosuppresive drugs as early as possible.

[Study of dermatomycosis and survey of pathogens in troops of Hainan area].

Subtropical area is the prevalent area of dermatomycosis with natural conditions suptable for the growth and proliferation of fungi causing suterficial dermatomycosis. Dermatomycosis not only brings about certain sufferings to the military personnel in peacetime, but also causes nonbattle loss in manpower in war time. In the present work, a survey of dermatomycosis in Hainan subtropical area of China and isolation of the pathogens were carried out. The results were as follows: The morbidity of superficial dermatomycosis was 34.1% and it was manifested clinically as tinea pedis, tinea versicolor, tinea corporis, tinea axillaris, tinea cruris, etc.; The main pathogen causing dermatomycosis in this area was Trichophyton rubrum which accounted for 50.4% of the pathogens isolated and the next was Trichophyton gypseum which accounted for 20.3%; Trichophyton rubrum could cause dermatomycosis of many sites of the body in this area, but the main lesious were tinea corporis and tinea cruris.

Alpha1-adrenergic receptor signaling via Gh is subtype specific and independent of its transglutaminase activity.

Tissue transglutaminase (TGase II) is a Ca2+- and thiol-dependent enzyme that catalyzes the post-translational modification of proteins via the formation of epsilon(gamma-glutamyl) lysine bonds. We have shown previously that the GTP-binding protein, Gh, is a TGase II that mediates intracellular signaling by the alpha1B-adrenergic receptor (AR) (Nakaoka, H., Perez, D. M., Baek, K. J., Das, T., Husain, A., Mison, K., Im, M.-J., and Graham, R. M. (1994) Science 264, 1593-1596). Here, we evaluated the ability of Gh as compared with Gq to mediate receptor-stimulated inositol phosphate turnover by the three alpha1-subtypes (alpha1A, alpha1B, and alpha1D). In addition, we questioned if the transglutaminase function of Gh is involved in its receptor signaling activity. A mutant form of a human TGase II cDNA in which the codon for the active site cysteine (Cys277) was replaced by serine was cloned into the mammalian expression vector pMT2'. Compared with wild-type TGase II, no transglutaminase activity was observed with transient transfection of this Cys-->Ser mutant in COS-1 cells. However, like wild-type TGase, the Cys-->Ser mutant mediated receptor-stimulated inositol phosphate turnover when cotransfected with an alpha1B-AR cDNA. Galphaq supported alpha1-AR-mediated inositol phosphate turnover by all three receptor subtypes. By contrast, although both the wild-type and Cys-->Ser construct mediated receptor signaling by the alpha1B AR and alpha1D AR, the alpha1A-AR was unable to interact with Gh. However, a Gh-dependent signaling phenotype could be rescued by a chimeric alpha1A construct in which the third intracellular loop of the alpha1A-AR was replaced by that of the alpha1B-AR. Thus, the signaling function of Gh is independent of its transglutaminase activity and is alpha1-AR subtype specific. This subtype specificity of the interaction between alpha1 ARs and Gh involves important determinants in their third intracellular loops.
