Unusual association of upper gastro-intestinal anomalies in a newborn.

We are describing one case of persistent nonbilious post-prandial vomiting in a newborn due to hypertrophic pyloric stenosis, intestinal malrotation and gastric antral diaphragm. Those three anomalies were successively diagnosed as causes of persistent upper gastrointestinal subobstruction.

Evaluation of esophageal transit in children and in infants by means of Krypton-81m.

This paper presents the results of Krypton-81m esophageal transit study in a series of children and infants presenting various esophageal disorders. The advantage of Kr-81m, compared to other radionuclides, is that the radiation dose to the patient is extremely low. The study can therefore be repeated as often as necessary and a high dose can be given to ensure good quality results. The method is very easy to perform and allows a quantitative assessment of esophageal transit in physiological conditions. The method gives information about the severity of altered transit and it is particularly useful in studying the effects of treatment.

Copper deficiency in infants with active celiac disease.

Celiac disease was diagnosed in two unrelated infants aged 7 and 7.5 months with severe malnutrition. They showed typical clinical, biological, and histological signs of the disease. Moreover, accompanying copper deficiency was suggested by severe hypocupremia and persistent neutropenia; bone radiographs were also compatible with this diagnosis. Rapid and complete correction of these anomalies could only be obtained after addition of oral copper sulfate to the gluten-free diet. Mechanisms possibly involved in the development of copper deficiency in young infants with celiac disease are: chronic malabsorption; high copper needs in rapidly growing infants; and possibly increased biliary and digestive losses. It is therefore suggested that young children with severe celiac disease should be monitored for their copper status.

Evaluation of a new method for routine in vitro cultivation of Giardia lamblia from human duodenal fluid.

Although a method for in vitro cultivation of Giardia lamblia was described as early as 1927, only a few clinical isolates were cultured in vitro due to the complexity of the techniques. We developed a method which allows for the routine isolation of Giardia trophozoites from human duodenal fluid and maintenance of the organisms in axenic culture. This study evaluates the method in 198 patients. Seventeen strains of Giardia were isolated and cultivated axenically. The method was more sensitive than the microscopic examination of aspirated fluid and examination of an impression of mucosal biopsy. Five patients, however, excreted cysts in the stool, although no trophozoites could be demonstrated in the duodenal fluid. G. lamblia were cultivated from one patient who did not excrete cysts. The method will enable the collection of G. lamblia strains from clinical material in large numbers and can offer an important advance in epidemiological, biochemical, immunological, and therapeutic investigations of giardiasis.

Quantitation of esophageal transit by means of 81mKr.

A method for quantifying esophageal transit by means of 81mKr is described. The principal advantage of 81mKr compared with 99mTc is its ultra-short half-life with its consequences on radiation dose and counting statistics. Factors which may influence the esophageal transit time such as body posture, volume of the tracer, and the nature of the tracer were studied. The reproducibility of the technique is quite good and its capacity to detect minor esophageal transit disorders is demonstrated.

Endoscopic injection sclerosis of oesophageal varices in children--indications and techniques.

Endoscopic injection sclerosis (EIS) of oesophageal varices is a valuable alternative treatment of recurrent variceal haemorrhage in childhood, since derivative treatment is not always possible; the spontaneous regression of varices occurring in some cases could also be promoted by EIS. A modified technique using a specially designed silicone rubber tube around the pediatric-type fiberscope in order to compress the injected varix is described. Eight children aged between 15 months and 12 years were treated with this method and 2 to 4 varices were sclerosed at each procedure using the Olympus NM3K injector. A constant reduction of the volume of the treated varices was observed, and the risks of variceal bleeding diminished significantly.

Gastroesophageal scintiscanning in children.

Four patients' positions were tested in search of increased sensitivity of gastroesophageal scintiscanning for the detection of reflux in children: supine, prone, left lateral, and 30 degrees right posterior oblique. The sensitivity was highest when the child was placed in supine position. A 60-min recording period increased the sensitivity of the technique, and is thus preferred to a shorter recording time.

Significance of persisting IgM anti-HBc antibodies in hepatitis B virus infection.

Igg and IgM antibodies to the core antigen of hepatitis B virus (HBV) were measured in 136 patients who developed acute HBV hepatitis and who were followed prospectively. After acute hepatitis all the patients developed transiently IgM anti-HBc lasting for two to five months. In contrast, IgM anti-HBc persisted 8 and 9 months in two patients who developed persistent hepatitis and were continuously detected for two years in nine patients who developed aggressive hepatitis. The results suggest that the determination of IgM anti-HBc might be useful to predict the outcome of chronic hepatitis B infection.

Gastro-oesophageal scintiscanning in children.

99mTc colloid scintigraphy is a sensitive technique for the detection of gastro-oesophageal reflux, compared to X-ray studies. A quantification index of reflux is used and can be of value when there is concern about the significance of a positive result. Continuous monitoring during one hour increases by 25% the detection of reflux in comparison with a 30 min test. In our series, no case of lung aspiration was recognized using this technique. The scintiscanning index cannot predict the existence of an associated oesophagitis.

Fiberendoscopy of the gastrointestinal tract in children. A series of 100 examinations.

This article reports the results of our first 100 examinations of the upper gastrointestinal tract and the colon in children and infants, using standard fiberscopes and also pediatric models. The procedure could be performed under sedation in 80% of the cases and general anesthesia was required mainly with standard fiberscopes. Compared to roentgenographic findings, fiberendoscopy proved to be more reliable in superficial mucosal lesions and gastrointestinal bleeding. Colonoscopy though difficult, is valuable in diagnosing rectal bleeding lesions and as a follow-up technique in inflammatory bowel diseases.

Fiberendoscopic monitorised dilatation of oesophageal strictures in children.

Oesophageal dilatations were performed 55 times in 10 children aged 2 months to 8 years presenting with acquired strictures of several origins. Classical bougienage with rubber material and newer technique with the Eder-Puestow's metallic olives system were used successively. The conjunction of the latter with systematic use of fiberendoscopy enhances considerably the safety of the procedure and permits better functional results.
