Transportation of newborn infants.

The benefit of an organized neonatal transport system is well established. Over a 30 month period, January 1987 to June 1989, 172 babies were transported to the Dublin maternity hospitals. Birth weights ranged from 640 to 5,180g 106 (62%) were less than 37 weeks gestation. Indications for transport included respiratory distress syndrome (54), prematurity only (43), convulsions and/or neurologic dysfunction (23), jaundice (11) and apnoea (11). One hundred and sixty-eight were transferred by ambulance and four by helicopter. Twenty travelled more than 100 miles. Forty (23%) received assisted ventilation during transport. On arrival 37 (21%) had temperature less than 36 degrees C; 22 (13%) had blood sugar less than 2.2 m mol/l and 34 (20%) had arterial ph less than 7.25. Fifty per cent of referral letters had incomplete information. Treatment and care given en route was recorded in only 28 babies. Twenty-four babies (14%) died. Infants who died were more likely to have been of low birth weight, travelled a long distance, been hypothermic, had poor arterial gases, had blood sugars less than 2.2 m mol/l, and had poor referral letters. This review indicates that death and morbidity continue to be associated with the present system of postnatal transfer of newborn infants. The urgent need for an organized neonatal transport service remains unmet.

Balloon occlusion during coronary angioplasty as a model of myocardial ischaemia: reproducibility of sequential inflations.

In order to evaluate the potential of balloon occlusion during coronary angioplasty as a model of myocardial ischaemia in man we have measured coronary sinus blood flow (CSBF), myocardial oxygen consumption (MVO2), lactate extraction (LER) and electrocardiographic changes in 11 patients undergoing left anterior descending artery (LAD) angioplasty. Baseline measurements were made before balloon crossing and between inflations. Four consecutive inflations each of 60 s duration were made; 5 min return to baseline was allowed between inflations. There was a significant reduction in CSBF and MVO2 (ml min-1) during inflations 2, 3 and 4 (CSBF: 121 +/- 66----94 +/- 53, 113 +/- 49----99 +/- 42, 124 +/- 66----102 +/- 41, P less than 0.02; MVO2: 11.3 +/- 6.6-9.1 +/- 3.9, 10.4 +/- 3.7-8.7 +/- 2.4, 12.2 +/- 4.4----9.4 +/- 2.8, P less than 0.05). However during the first period of balloon occlusion there were inconsistent changes in coronary flow with an overall rise in mean flow (97 +/- 35----128 +/- 80 ml min-1, P = NS) and an overall rise in mean myocardial oxygen consumption (9.6 +/- 3.8----12.5 +/- 7.5 ml min-1, P = NS). There was lactate production during all four inflations but the changes during the first one did not achieve statistical significance. These inconsistent changes during the first inflation were thought to be due to partial obstruction of the stenosis by the deflated balloon before primary dilatation. The changes due to crossing and during the first two inflations were further investigated in another group of 12 patients undergoing LAD angioplasty. Great cardiac vein flow (GCVF), CSBF, MVO2 and LER were recorded at baseline, during crossing and during the first two inflations. With the deflated balloon across the stenosis there were no changes in CSBF or MVO2 but there was a fall in GCVF (103 +/- 28----77 +/- 50, P = NS) and a significant fall in LER (77 +/- 57----16 +/- 37, P less than 0.01). Although there was a fall during the first inflation in CSBF, GCVF, MVO2 and lactate extraction none of these changes were significant. During the second inflation these changes were of greater magnitude and achieved statistical significance. While balloon occlusion during coronary angioplasty has the potential of providing a model of ischaemia in man we have found the first inflation period unreliable, due to the variable degree of occlusion by the deflated balloon. We suggest that only subsequent inflations after the primary dilatation are used for observations.(ABSTRACT TRUNCATED AT 400 WORDS)

A decision scheme for coronary angiography after acute myocardial infarction.

It is important to select patients in the convalescent phase of acute myocardial infarction in whom knowledge of coronary anatomy may identify those potentially suitable for intervention aimed at improving prognosis. However, differing guidelines have been proposed, and by applying some of these guidelines to our large database of patients after acute myocardial infarction, several problem areas were identified. These include lack of considering patients with resting ischemia beyond day 5 of hospitalization, management of patients with reduced ventricular function or patients not exercise tested, and the role of coronary angiography in the elderly. Based on this experience and further analysis in 1,848 patients surviving beyond day 5 of hospitalization, a modified decision scheme for coronary angiography was developed and then tested in a second population (n = 780). In the new scheme, patients over 75 years of age are considered individually. Those under 75 years of age with severe resting ischemia in the hospital at any time beyond the first 24 hours (18% mortality between day 6 and year 1), and hospital survivors with a history of previous myocardial infarction and clinical or radiographic signs of left ventricular failure in the hospital (25% 1-year mortality after discharge), are recommended for coronary angiography. Among the remaining patients, some will perform an exercise test, and those with an ischemic response or poor workload (11% 1-year mortality) are also assigned to coronary angiography. When an exercise test is not performed, a resting radionuclide left ventricular ejection fraction is recommended, and coronary angiography is considered if the value lies between 0.20 and 0.44 (12% 1-year mortality). This relatively simple scheme does not make general recommendations in the elderly, considers patients with in-hospital left ventricular failure or reduced left ventricular function or both, and approaches the problem of patients who do not perform an exercise test. This general approach would avoid early coronary angiography in patients with an average 1-year mortality risk after discharge of 3% and recommend coronary angiography in those at increased risk (average mortality rate, 16%) who make up about 55% of this population under 75 years of age.

Differing circadian patterns of symptom onset in subgroups of patients with acute myocardial infarction.

Circadian variation of the onset of acute myocardial infarction has been noted in many studies and may carry important pathophysiologic implications. However, only a few previous studies have attempted subgroup analyses. In 4,796 patients with documented acute myocardial infarction, the time of symptom onset was recorded. As in other studies, the peak of onset occurred in the morning from 6:01 AM to 12:00 noon, and 28% of the population (1.16 times the average percentage for the other time periods) experienced symptom onset in that period (p less than 0.001). There was a second, lower peak (25%) in the evening between 6:01 PM and 12:00 midnight, which was also observed in some previous studies. We sought to determine whether or not the presence of subgroups with specific clinical characteristics would exhibit different patterns and thereby contribute to these peaks in the overall population. In patients with a history of congestive heart failure (n = 606) or with non-Q wave infarction (n = 832), a pronounced peak (29%) occurred only in the evening. Two nearly equal peaks were observed in patients older than 70 years of age (n = 1,422), smokers (n = 2,057), diabetics (n = 767), women (n = 1,213), and patients taking beta-blocking drugs (n = 847). Finally, in patients with a previous myocardial infarction (n = 1,104), no peaks were observed.(ABSTRACT TRUNCATED AT 250 WORDS)

Comparison of potassium ISFETs with the Radiometer KNA1 and the Corning 902 ion selective electrode analysers for whole blood potassium ion estimation.

Evaluation of the performance of potassium ion sensitive field effect transistors (K+ ISFETs), developed by Thorn EMI in a form suitable for mass production and for incorporation in 'near the patient' analysers, showed only very small constant and proportional biases against the Radiometer KNA1 and the Corning 902 for whole blood potassium ion estimation. Between batch imprecision tests with whole blood showed the K+ ISFET was comparable in performance to the Corning 902 but inferior to the Radiometer KNA1. The evaluation demonstrated that ISFET manufacturing technology has now reached a stage of development at which ISFETs should be considered seriously for use in clinical chemical analysers.

Oxygen tension within the arterial wall: relationship to altered bioenergetic metabolism and lipid accumulation.

Oxygen tension (pO2) was measured in upper thoracic arteries and in muscular foci at the celiac bifurcation from atherosclerosis-susceptible White Carneau and atherosclerosis-resistant Show Racer pigeons at 6, 12, and 24 weeks of age. At each site the pO2 was measured within adventitial (outer), medial (middle), and subendothelial (inner) zones with polarographic oxygen microelectrodes while the animals were under dissociative anesthesia. The atherosclerosis-resistant Show Racer pigeons exhibited no significant age or site differences in pO2 levels. The medial pO2 was approximately 25 mm Hg, while the adventitial and subendothelial zones had pO2 levels of approximately 30 mm Hg. The pO2 profiles through the various zones of the arterial wall of 6-week-old atherosclerosis-susceptible White Carneau pigeons were similar to those of the Show Racers. However, significantly lower oxygen tension was found in the subendothelial zone from 12- and 24-week White Carneau celiac bifurcations (27 and 21 mm Hg) than in corresponding zones in Show Racer sites. Also, the medial zone in 24-week White Carneau celiac foci exhibited a significantly lower pO2 (20 mm Hg) than the medial zone of younger White Carneau. When correlated with previously described biochemical and morphological changes in White Carneau pigeons, the findings presented herein indicate that focal decreases in oxygen availability within arterial tissue occur after the onset of spontaneous atherogenesis, and probably result from decreased diffusion of oxygen into the arterial wall.

Plasma sodium and potassium measurement: minimising ISE-flame differences using specimens from patients.

Experience in clinical chemistry of direct reading ion-selective electrode analysers for the measurement of plasma Na+ and K+ concentrations has shown that the results obtained on any specimen may vary significantly from one make of instrument to another. To overcome these differences, the European Working Group on ISEs of the IFCC Expert Panel on pH, Blood Gases and Electrolytes have proposed that instruments should be standardised to report Na+ and K+ concentrations in undiluted plasma to agree with flame emission spectrometry in samples with normal plasma water bicarbonate concentrations and normal pH. In designing and setting the calibration of a new ISE analyser for Na+ and K+, the Corning 614, the manufacturer has attempted to satisfy this proposal using a large number of specimens obtained from patients. This paper presents details of the procedure used and the results of an independent evaluation of its validity in routine clinical practice.

Antibody responses of sheep vaccinated with foot rot vaccines.

Enzyme-linked immunosorbent assay (ELISA) and crossed immunoelectrophoresis (IEP) were used to investigate antibody responses of sheep vaccinated with a double adjuvanted or single adjuvanted commercial foot rot vaccine. ELISA detected an antibody response of greater magnitude to the double adjuvant vaccine compared with the single adjuvant vaccine. Sera from sheep vaccinated with double adjuvant vaccine recognised at least six antigens of Bacteroides nodosus in crossed IEP while sera from the single adjuvant vaccinated sheep recognised one antigen. The use of non-denatured antigens of B nodosus in ELISA and crossed IEP enabled quantitative comparisons of antibody responses to the different foot rot vaccines to be made.

A rapid method for observing the internal morphology of amphibian embryos.

A simple, rapid method for visualizing the internal morphology of amphibian embryos is described. Fixed embryos of Ambystoma mexicanum are surrounded with commercial embedding material and are frozen. Internal structure is revealed by cutting the embryos with a cooled scalpel or razor blade, removing the frozen embedment with an aqueous buffer, and processing the halved embryos for scanning electron microscopy (SEM). This technique allows the internal anatomy of the embryo to be viewed but is much simpler and more rapid than previously described techniques. It should therefore prove useful for understanding and teaching the three-dimensional relationship between tissues in developing embryos. In addition, we believe that this technique could be used, with minor changes, as a rapid method for viewing the internal morphology of a variety of specimens.

A monoclonal antibody to the phosphorylated form of phenylalanine hydroxylase. Definition of the phosphopeptide epitope.

Monoclonal antibody PH7 has specificity for the phosphorylated form of the human liver phenylalanine hydroxylase and negligible reactivity towards the dephosphorylated form of the native enzyme by enzyme-linked immunoassay. PH7 binds specifically to the phosphorylated form of the liver enzyme after SDS/polyacrylamide-gel electrophoresis and transfer to nitrocellulose. Competitive blocking assays have been applied in conjunction with reversed-phase h.p.l.c. of purified tryptic fragments of human liver phenylalanine hydroxylase to localize the epitope. The major immunoreactive tryptic peptide cross-reacting with PH7 had an amino acid analysis corresponding to the first 41 amino acids of the human liver phenylalanine hydroxylase sequence and included the serine residue that is thought to be the phosphorylation site. The monoclonal antibody recognized the phosphorylated form of the synthetic decapeptide corresponding to the local phosphorylation-site sequence Gly-Leu-Gly-Arg-Lys-Leu-Ser(P)-Asp-Phe-Gly, but not the dephosphodecapeptide. Thermolysin digestion of the peptide demonstrated the monoclonal antibody bound to the pentapeptide Leu-Ser(P)-Asp-Phe-Gly. Monoclonal antibody PH7 recognized the phosphodecapeptide at concentrations 10(3)-fold higher than with phenylalanine hydroxylase, compared with 10(4)-10(7)-fold higher for other phosphopeptides and phosphoproteins. The results demonstrate that monoclonal antibody PH7 has specificity for the phosphorylated form of phenylalanine hydroxylase at the phosphorylation site.

Lack of evidence of low ionized calcium levels in systemic hypertension.

An epidemiologic screening survey was conducted in 325 male industrial workers to investigate the relation between serum total and ionized calcium concentrations and blood pressure. No relation was found. Previous reports of lower serum ionized calcium levels in hypertensive patients may be related to methodologic deficiencies both in the selection of subjects and in ionized calcium measurement. These data do not support the concept that increased blood pressure levels are related to calcium deficiency or to abnormal plasma calcium homeostasis.

The role of aggregation in embryonal carcinoma cell differentiation.

Cultures of the P19 line of embryonal carcinoma cells differentiate into various cell types including cardiac muscle when aggregated and exposed to medium containing 1% dimethylsulfoxide (DMSO). DMSO-treated aggregates became completely covered with an epithelial cell type 3 to 4 days following drug exposure. This epithelial cell was tentatively identified as primitive extraembryonic endoderm by its ultrastructural appearance and its possession of cytokeratin intermediate filaments. Muscle cells developed within the interior of DMSO-treated aggregates. They first became apparent 5 to 6 days after DMSO exposure and were characterized by the presence of striated muscle-specific myosin, immature myofibrils, and intercalated discs. We determined the proportion of cells developing into epithelium and muscle in aggregates of various sizes and showed that the proportion of epithelium was highest in small aggregates whereas muscle cells developed only in aggregates of relatively large size. The muscle was usually associated with necrotic areas which developed within the interior of large aggregates. Our results suggest that cardiac muscle differentiation in the aggregates requires both the DMSO-induced formation of an epithelial cell coat and one other condition which may be the proximity to necrotic areas.

Clinical acceptability, use-patterns and use-effectiveness of the vaginal contraceptive sponge and Neo Sampoon tablets--an international multi-center randomized clinical trial.

This paper describes the results from a randomized clinical trial comparing the Collatex vaginal contraceptive sponge (a predecessor of the Today sponge) and Neo Sampoon foaming vaginal contraceptive tablets; the trial was conducted from 1979 to 1983 in four centers located in three countries (two in Yugoslavia and one each in Taiwan and Bangladesh). The sponge was associated with more insertion and retention problems than the tablet, especially in the two Asian centers. More Neo Sampoon users complained of a burning or stinging sensation. This complaint, however, seemed to be well-tolerated and was not a frequent reason for irregular use and/or discontinuation of use of the tablets. Clinically significant medical complications were rarely reported for either method. Sponge users were more likely to report irregular use than tablet users, primarily due to inconvenience of use. Rates of discontinuation at six months of use were also consistently higher among sponge users than Neo Sampoon users in the four centers. Life-table pregnancy rates at 12 months of use ranged from 3.8 to 18.2 per 100 sponge users and 6.2 to 29.9 per 100 Neo Sampoon users, based on data from the two Yugoslavian centers and the Taiwan center (data from the Bangladesh center were excluded from analysis of pregnancy rates). Practical implications of these findings are discussed.

A clinical trial of Neo Sampoon vaginal tablets and Emko foam in Alexandria, Egypt.

Results are reported for a comparative 12-month study of Neo Sampoon foaming vaginal tablets containing 60 mg of the spermicide, menfegol, and Emko vaginal foam containing an 8.0% concentration of the spermicide, nonoxynol-9. Conducted in cooperation with the Family Planning Association in Alexandria, Egypt, the trial included 349 women who were randomly allocated to use one of the two contraceptive products. The twelve-month cumulative life-table rate for accidental pregnancy (per 100 women) was 2.8 for Neo Sampoon tablet users and 2.1 for Emko foam users. The 12-month continuation rates were 77.6 and 77.2 per 100 women for the tablet and foam groups, respectively. In both groups, the majority of discontinuations from the study were for personal reasons, including lack of confidence in the method, messiness, partner's objection and a burning sensation. Few women reported a product-related complaint while using their assigned contraceptive method. The most commonly reported complaint for both methods was that use of the product led to an uncomfortable burning sensation for the woman and/or her partner. This complaint, however, was cited by less than 5% of the women in each group. Thus, a combination of low pregnancy rates, few complications and complaints and high continuation rates confirm the relative acceptability, effectiveness and short-term safety of these methods of contraception among this sample of Egyptian women.

Real-time analysis ambulatory electrocardiography--clinical evaluation of cardiac arrhythmias by the Aegis system.

To evaluate the technical reliability, clinical applicability, and arrhythmia accuracy of one manufacturer's real-time analysis ambulatory ECG instrument (Aegis Medical Systems), 164 patients were simultaneously examined through a "Y" cable by both a real-time analyzer and a conventional Holter recorder. Technical failure was similar for both recorders (2% in each), and the real-time analyzer was applicable to all patients encountered. Using a randomly selected hand-counted database of 799 hours as the standard of truth, accuracy of the real-time analyzer for hourly mean heart rate, isolated ventricular ectopic beats, ventricular couplets, ventricular tachycardia, isolated supraventricular ectopic beats and supraventricular tachycardia was determined. Mean heart rate showed 96% agreement and a high correlation (r = .986) to hand-counted values. Real-time analysis overall sensitivity, positive predictive accuracy, and false positive rate for the Aegis Medical System were (a) for isolated ventricular ectopic beats--92%, 92%, and 8%, (b) for ventricular couplets--80%, 97%, and 3%, (c) ventricular tachycardia--81%, 92%, and 8%, (d) supraventricular ectopic beats--81%, 82%, and 18%, and (e) for supraventricular tachycardia--75%, 89%, and 11%. Arrhythmia analysis was valid for mean heart rate and ventricular arrhythmias, but showed lower sensitivities in detection of low prevalence (1-60 b/Hr) ventricular couplets (72%) and ventricular tachycardia (43%). Significant inaccuracies for some densities of supraventricular arrhythmia were also found. This study found the real-time analysis ambulatory ECG similar to conventional Holter recording for technical reliability and patient applicability. Given the limitations of currently available real-time analysis ambulatory ECG systems, it is a promising technology.

The development of skill in producing word-final English stops.

This study examined production of word-final English /p/ and /b/ by subjects whose native language does not possess voiced stops in word-final position. Native Chinese adults resembled native English adults, native English children, and native Chinese children in producing /p/ with greater peak oral air pressure than /b/. However, unlike subjects in the other groups, the Chinese adults' /b/ was sometimes misidentified as /p/. This may have occurred, at least in part, because the Chinese adults produced a much smaller difference between /p/ and /b/ in labial closure duration and voicing than the other subjects. The English adults sustained voicing in /b/ significantly longer than subjects in the other three groups. To help determine the basis for this ability, the shape of oral air pressure waveforms was examined systematically. The percentage of "delayed" and "bimodal" waveforms, in which pressure stopped increasing, or decreased, prior to the release of labial constriction, was calculated for each group. Only the English adults showed more such waveforms for /b/ than /p/. Voicing continued 18 ms longer in /b/ tokens with delayed and bimodal waveforms than in tokens in which oral pressure increased continuously. The duration of closure voicing was correlated with the rate at which pressure increased in the English adults' /b/ waveforms. Previous aerodynamic modeling has shown that delayed and bimodal waveforms may result from an active enlargement of the supraglottal cavity. This, together with the pattern of between-group differences observed here, suggests that the English adults learned to enlarge the supraglottal cavity to sustain voicing in /b/. It appears that neither the children nor the Chinese adults had as yet acquired this skill.
