Three-week schedule of irinotecan plus cisplatin in patients with previously untreated extensive-stage small-cell lung cancer.

Irinotecan and cisplatin demonstrated promising outcomes in extensive-stage small-cell lung cancer. According to the dosage and schedule of irinotecan, efficacy and toxicity profiles showed subtle differences. This study was designed to evaluate efficacy and toxicity of 3-week schedule of irinotecan/cisplatin in patients with previously untreated extensive-stage small-cell lung cancer. The primary objective was to evaluate response rate and secondary objectives were overall survival and progression-free survival. Patients with previously untreated extensive-stage small-cell lung cancer were enrolled. Irinotecan 65 mg m-2 was administered on days 1 and 8 and cisplatin 60 mg m-2 on day 1. Treatment was repeated every 3 weeks. Seven out of 54 patients (13.0%) had complete response, and partial response was observed in 33 (61.1%). The overall response rate was 74.1% (95% CI; 62.0-82.2%). Stable disease was observed in eight (14.8%) and no progressive disease was observed. After a median follow-up duration of 28.7 months, the median overall survival and progressive-free survival were 13.6 and 6.5 months, respectively. Major grade 3/4 toxicities were neutropenia (50.0%), anorexia (42.6%), diarrhoea (29.6%), fatigue (29.6%) and vomiting (13.0%). There was one treatment-related death owing to pneumonia. Three-week schedule of irinotecan/cisplatin showed effective antitumour activity and moderate toxicities in patients with previously untreated extensive-stage small-cell lung cancer.

Low molecular weight dicarboxylic acids, ketoacids, and dicarbonyls in the fine particles from a roadway tunnel: possible secondary production from the precursors.

Low molecular weight dicarboxylic acids (DCAs), ketoacids, and alpha-dicarbonyls have been determined for the PM2.5 samples in a Hong Kong roadway tunnel, using a water extraction followed by a butyl ester and/or dibutyl acetal derivatization technique. For the most wintertime sampling runs, outlet and inlet concentrations of the quantified compounds were found to be quite similar (ca. 10% differences), leading to the conclusion that direct emissions of the organic compounds are insignificant from vehicles in hot-stabilized operations although vehicular emissions can provide important precursors to them. In contrast, a significant concentration increase of most compounds was observed at the outlet station compared to the inlet station in the summertime runs, which might be explained by the secondary production of aerosols in the tunnel. The organic compounds studied comprised a small fraction (<1%) of aerosol organic carbon (OC). In winter, their abundances relative to that of OC in outlet samples were found to be significantly less than those in inlet samples. On the basis of the summer data, apparent secondary production factors of the compounds were calculated, which indicate that adipic and m-phthalic acids can be favorably formed in the tunnel. However, like other DCAs, direct emissions of adipic, m-phthalic, and p-phthalic acids from automobiles are suggested to be insignificant.

Source apportionment of PM2.5 in urban area of Hong Kong.

A monitoring program for PM(2.5) had been performed at two urban monitoring stations in Hong Kong from November 2000 to February 2001 and June 2001 to August 2001. PM(2.5) samples were collected once every 6 days at PolyU and KT stations with the sampling duration of 24-h. A sum of 25 chemical species in PM(2.5) were determined and selected for receptor models. Enrichment factors relative to earth crust abundances were evaluated and it was noted that most crustal elements including Al, Ti, Mg, Ca and K have small enrichment factors. Correlation and multivariate analysis technique, such as principal components analysis (PCA)/absolute principal components analysis (APCA) and cluster analysis (CA) are used for source apportionment to identify the possible sources of PM(2.5) and to determine their contribution. Six factors at each site were isolated by using PCA/APCA and cluster analysis. Similar sources (crustal matter, automobile emission, diesel emission, secondary aerosols, tire wear, and non-ferrous smelter) are identified by the PCA/APCA and cluster analysis.

More effects of extracorporeal magnetic innervation and terazosin therapy than terazosin therapy alone for non-inflammatory chronic pelvic pain syndrome: a pilot study.

The objective of this study was to evaluate whether extracorporeal magnetic innervation (ExMI) combined with alpha-blocker therapy is more effective than alpha-blocker monotherapy for patients with non-inflammatory chronic prostatitis (CP)/chronic pelvic pain syndrome (CPPS), category IIIB. Patients were randomized to either terazosin monotherapy (group 1, n=21) or terazosin combined with ExMI therapy (group 2, n=19). Patients in group 2 had 12 treatment sessions of ExMI twice a week during 6 weeks. None of the patients experienced any side effects from treatment. The changes in each domain of the National Institutes of Health (NIH)-Chronic Prostatitis Symptom Index (CPSI) measured on week 6 were not significantly different between the groups. However, the difference (median, 25-75th percentiles) between the two groups in total NIH-CPSI scores was -4 (-11.5, -2) for group 1 and -12 (-17.3, -2.3) for group 2, respectively (P=0.047). At 6 weeks, 47.6% (10 of 21) of group 1 had a >25% decrease in total NIH-CPSI compared with 78.9% (15 of 19) of group 2 (P=0.041). Also, more patients in group 2 (78.9%) were rated as responders with a 6-point decrease in NIH-CPSI compared with group 1 (47.6%) (P=0.041). The early results suggest that ExMI combined with alpha-blocker therapy has better effect than alpha-blocker monotherapy for the treatment of CP/CPPS.

A multicentre randomised phase II study of carboplatin in combination with gemcitabine at standard rate or fixed dose rate infusion in patients with advanced stage non-small-cell lung cancer.

BACKGROUND: Intracellular gemcitabine triphosphate (dFdCTP) levels can be optimised by administering gemcitabine at a fixed dose rate infusion. PATIENTS AND METHODS: Patients with chemonaive advanced non-small cell lung cancer (NSCLC) were randomised to receive gemcitabine at a fixed dose rate gemcitabine 750 mg/m(2) over 75 min (arm A) or gemcitabine 1000 mg/m(2) over 30 min (arm B) on days 1 and 8 every three week cycle. Carboplatin at AUC of 5 was administered in both treatment arms on day 1 of each cycle. End points were activity, tolerability and pharmacokinetics of plasma and intracellular gemcitabine. RESULTS: 76 patients were randomised. Response rate was 34% in arm A and 42% in arm B. Toxicity and quality of life scores were similar for both treatment arms. Mean plasma Cmax(gemcitabine) and mean dFdCTP AUC in arm A was 20.8 microM +/- 17.2 microM and 35,079 +/- 18,216 microM*min respectively and in arm B, 41.2 +/- 13.9 microM and 32 249 +/- 11 267 microM*min respectively. dFdCTP saturation was reached in Arm B but not in Arm A. CONCLUSION: The saturability of dFdCTP accumulation in Arm A suggests optimal delivery of gemcitabine is achieved using fixed rate infusion compared to 30-min infusion. Fixed dose rate gemcitabine is active and feasible, supporting the concept of fixed dosing rate of gemcitabine in advanced NSCLC. However, this entails a longer infusion time with associated higher costs involved.

Seasonal variations and mass closure analysis of particulate matter in Hong Kong.

The chemical characteristics of ambient particulate matters in urban and rural areas of Hong Kong were determined in this study. A monitoring program starting from November 2000 to February 2001 (winter) and June 2001 to August 2001 (summer) for PM10 and PM2.5 was performed at three monitoring stations in Hong Kong. Twenty-four-hour PM10 and PM2.5 samples were collected once every 6 days at two urban sites, PolyU and KT, and every 12 days at a background site, HT, with Hi-Vol samplers. High concentrations of OC, EC (except in PolyU), water-soluble ions and elements were observed in winter among the three sampling sites for PM10 and PM2.5 fractions. Seasonal variations were significant in background HT. Dilution effect due to the increase in mixing depth and precipitation in summer reduced the concentrations of particulate matters. Long-range transport could contribute to the higher concentrations of particulate matter in the winter. Chemical mass closure calculations were performed for PM10 and PM2.5 observed. Mass closure improved when separate factors (1.4 and 1.9 respectively) were used to convert water-soluble organic carbon (WSOC) and water-insoluble organic carbon (WINSOC) into corresponding organic masses. The urban sites showed high percentages of water-soluble ions in winter and high percentages of carbonaceous species in summer. Better results were obtained for the chemical mass closure analysis in winter than in summer. High temperature and solar radiation in summer increased the rate of the complex photochemical reaction in the atmosphere. Therefore the chemical mass closure analysis would underestimate the volatized species and secondary aerosols during summer.

Exposure to PM2.5 and PAHs from the Tong Liang, China epidemiological study.

Chemically speciated PM2.5 and particle-bound polycyclic aromatic hydrocarbon (PAH) measurements were made at three sites near urban Tong Liang, Chongqing, a Chinese inland city where coal combustion is used for electricity generation and residential purposes outside of the central city. Ambient sampling was based on 72-hr averages between 3/2/2002 and 2/26/2003. Elevated PM2.5 and PAH concentrations were observed at all three sites, with the highest concentrations found in winter and the lowest in summer. This reflects a coupling effect of source variability and meteorological conditions. The PM2.5 mass estimated from sulfate, nitrate, ammonium, organics, elemental carbon, crustal material, and salt corresponded with the annual average gravimetric mass within +/-10%. Carbonaceous aerosol was the dominant species, while positive correlations between organic carbon and trace elements (e.g., As, Se, Br, Pb, and Zn) were consistent with coal-burning and motor vehicle contributions. Ambient particle-bound PAHs of molecular weight 168-266 were enriched by 1.5 to 3.5 times during the coal-fired power plant operational period. However, further investigation is needed to determine the relative contribution from residential and utility coal combustion and vehicular activities.

Carbonyl compounds in the roadside environment of Hong Kong.

The levels of carbonyl compounds were determined at the roadside urban station at the Hong Kong Polytechnic University (HKPU) campus during January 2002 to February 2002. Nine carbonyl compounds were quantified in this study. Temperature and solar radiation were found to affect the photochemical reactions of the carbonyls. Formaldehyde/acetaldehyde ratio ranged from 1.27 to 1.35. Strong correlations between formaldehyde and acetaldehyde were found, in the time period 1800-2100, which indicated that they were originated from the same sources during this time period. Roadside carbonyl samples were also collected at four other roadside environments during 2001. Kwai Chung (KC) station showed the highest average formaldehyde and acetaldehyde concentrations due to its highest traffic flow, especially for diesel vehicles. High concentration of toluene emitted from gasoline-fueled vehicles was believed to be the cause of high benzaldehyde level at the Central (CT) station through the photochemical oxidation of toluene. The average concentrations of formaldehyde and acetaldehyde in Hong Kong are well within the ranges reported in roadside environments of other urban cities. However, Mexico City in Mexico and Cairo in Egypt had much higher concentration levels of formaldehyde and acetaldehyde than in Hong Kong roadside environment due to the incomplete combustion of different fuel compositions.

Human and mouse microglia express connexin36, and functional gap junctions are formed between rodent microglia and neurons.

Microglia, the tissue macrophages of the central nervous system (CNS), intimately interact with neurons physically and through soluble factors that can affect microglial activation state and neuronal survival and physiology. We report here a new mechanism of interaction between these cells, provided by the formation of gap junctions composed of connexin (Cx) 36. Among eight Cxs tested, expression of Cx36 mRNA and protein was found in microglial cultures prepared from human and mouse, and Cx45 mRNA was found in mouse microglial cultures. Electrophysiological measurements found coupling between one-third of human or mouse microglial pairs that averaged below 30 pico-Siemens and displayed electrical properties consistent with Cx36 gap junctions. Importantly, similar frequency of low-strength electrical coupling was also obtained between microglia and neurons in cocultures prepared from neocortical or hippocampal rodent tissue. Lucifer yellow dye coupling between neurons and microglia was observed in 4% of pairs tested, consistent with the low strength and incidence of electrical coupling. Cx36 expression level and/or the degree of coupling between microglia did not significantly change in the presence of activating agents, including lipopolysaccharide, granulocyte-macrophage colony-stimulating factor, interferon-gamma, and tumor necrosis factor-alpha, except for some reduction of Cx36 protein when exposed to the latter two agents. Our findings that intercellular coupling occurs between neuronal and microglial populations through Cx36 gap junctions have potentially important implications for normal neural physiology and microglial responses in neuronopathology in the mammalian CNS.

Drought-inducible-but ABA-independent-thaumatin-like protein from carrot (Daucus carota L.).

Drought treatment induces the accumulation of dcTLP, which is similar in structure to the thaumatin-like proteins (TLPs) found in the embryogenic calli, seedlings, and mature plants of carrot (Daucus carota). We isolated a full-length dcTLP cDNA clone from carrot and characterized the 5' upstream sequences. The coding region of dcTLP consisted of 645 nucleotides; the theoretical pI value was 4.9, and its molecular weight was approximately 22 kDa. The production of dcTLP transcripts in the seedlings increased dramatically with dehydration treatment but was not affected by abscisic acid (ABA), salicylic acid, or jasmonic acid. The expression patterns of dcTLP mRNA at different developmental stages and in response to a variety of signal molecules was analyzed using reverse transcriptase-PCR and promoter analysis with fused genes of 0.5-kb 5' upstream sequences in which beta-glucuronidase (GUS) reporter genes (gus) were established. The induction of dcTLP was found to be highly specific to drought stress in the embryogenic calli, seedlings, and mature plants. Our results suggest that this new isoform of TLP that has been isolated from carrot is a drought-specific, ABA-independent, non-organ-specific, and non-developmental-stage-specific protein.

Indoor radon levels in selected hot spring hotels in Guangdong, China.

Guangdong is one of the provinces that have most hot springs in China, and many hotels have been set up near hot springs, with spring water introduced into the bath inside each hotel room for hot spring bathing to attract tourists. In the present study, we measured radon in indoor and outdoor air, as well as in hot spring waters, in four hot spring hotels in Guangdong by using NR-667A (III) continuous radon detector. Radon concentrations ranged 53.4-292.5 Bq L(-1) in the hot spring water and 17.2-190.9 Bq m(-3) in outdoor air. Soil gas intrusion, indoor hot spring water use and inefficient ventilation all contributed to the elevated indoor radon levels in the hotel rooms. From the variation of radon levels in closed unoccupied hotel rooms, soil gas intrusion was found to be a very important source of indoor radon in hotel rooms with floors in contact with soils. When there was spring water bathing in the bathes, average radon levels were 10.9-813% higher in the hotel rooms and 13.8-489% higher in bathes compared to their corresponding average levels when there was no spring water use. Spring water use in the hotel rooms had radon transfer coefficients from 1.6x10(-4) to 5.0x10(-3). Radon in some hotel rooms maintained in concentrations much higher than guideline levels might thus have potential health risks to the hotel workers, and technical and management measures should be taken to lower their exposure of radon through inhalation.

Differential regulation of ca(2+) signaling and membrane trafficking by multiple p2 receptors in brown adipocytes.

Extracellular ATP triggers changes in intracellular Ca(2+), ion channel function, and membrane trafficking in adipocytes. The aim of the present study was to determine which P2 receptors might mediate the Ca(2+) signaling and membrane trafficking responses to ATP in brown fat cells. RT-PCR was used to determine which P2 receptors are expressed in brown fat cells. Responses to nucleotide agonists and antagonists were characterized using fura-2 fluorescence imaging of Ca(2+) responses, and FM 1-43 fluorescence imaging and membrane capacitance measurements to assess membrane trafficking. The pharmacology of the Ca(2+) responses fits the properties of the P2Y receptors for which mRNA is expressed, but the agonist and antagonist sensitivity of the membrane-trafficking response was not consistent with any P2 receptor described to date. Brown adipocytes expressed mRNA for P2Y(2), P2Y(6), and P2Y(12) metabotropic receptors and P2X(1), P2X(2), P2X(3), P2X(4), P2X(5), and P2X(7) ionotropic receptors. The agonists ATP, ADP, UTP, UDP and 2', 3'-(benzoylbenzoyl) ATP (BzATP) increased intracellular Ca(2+), while 100 microM: suramin, pyridoxal-phosphate-6-azophenyl-2' 4'-disulfonic acid (PPADS), or Reactive Blue 2 partially blocked Ca(2+) responses. ATP, but not ADP, UTP, UDP or BzATP activated membrane trafficking. The membrane response could be blocked completely with 1 microM: PPADS but not by the antagonist MRS2179. We conclude that multiple P2 receptors mediate the ATP responses of brown fat cells, and that membrane trafficking is regulated by a P2 receptor showing unusual properties.

Solitary fibrous tumors of the pleura: clinical, radiological, surgical and pathological evaluation.

AIMS: To report a clinicopathological series of cases of solitary fibrous tumor of the pleura from Taiwan. METHODS: Clinical data was collected from a review of medical records and telephone interviewing for follow-up. RESULTS: Eight patients, three men and five women aging from 34 to 71 years, underwent tumor resection and were followed in a period from 7 months to 13.5 years. Six patients underwent standard thoracotomy and two had VATS for tumor excision. Tumors were pathologically benign in seven patients and malignant in one. Patients were all alive with no evidence of tumor recurrence at the time of this report. CONCLUSIONS: One should always consider SFTPs as potentially malignant tumors. Complete resection remains the mainstay of cure. Standard thoracotomy should always be considered when a high suspicion of malignancy is raised, whereas VATS may be a preferred approach for smaller tumors.

Effective dose evaluation for chest and abdomen X-ray tests.

The radiation doses resulting from diagnostic X-ray examinations are routinely measured in terms of entrance skin exposure (ESE). In this study, for the purpose of radiation protection, the radiation doses received from chest and abdomen X-ray tests were evaluated in terms of equivalent dose and effective dose. The dose calculations were conducted by using the MCNP Monte Carlo code and an adult hermaphrodite mathematical phantom. The effects of both operating high voltage and projection geometry on the effective dose were investigated. The absolute values of the effective doses may be provided from the national average ESE.

Indoor/outdoor relationships for PM2.5 and associated carbonaceous pollutants at residential homes in Hong Kong - case study.

UNLABELLED: Six residences were selected (two roadside, two urban, and two rural) to evaluate the indoor-outdoor characteristics of PM(2.5) (aerodynamic diameter <2.5 microm) carbonaceous species in Hong Kong during March and April 2004. Twenty-minute-averaged indoor and outdoor PM(2.5) concentrations were recorded by DustTrak samplers simultaneously at each site for 3 days to examine diurnal variability of PM(2.5) mass concentrations and their indoor-to-outdoor (I/O) ratios. Daily (24-h average) indoor/outdoor PM(2.5) samples were collected on pre-fired quartz-fiber filters with battery-powered portable mini-volume samplers and analyzed for organic and elemental carbon (OC, EC) by thermal/optical reflectance (TOR) following the Interagency Monitoring of Protected Visual Environments (IMPROVE) protocol. The average indoor and outdoor concentrations of 24 h PM(2.5) were 56.7 and 43.8 microg/m(3), respectively. The short-term PM(2.5) profiles indicated that the penetration of outdoor particles was an important contributor to indoor PM(2.5), and a household survey indicated that daily activities were also sources of episodic peaks in indoor PM(2.5). The average indoor OC and EC concentrations of 17.1 and 2.8 microg/m(3), respectively, accounted for an average of 29.5 and 5.2%, respectively, of indoor PM(2.5) mass. The average indoor OC/EC ratios were 5.8, 9.1, and 5.0 in roadside, urban, and rural areas, respectively; while average outdoor OC/EC ratios were 4.0, 4.3, and 4.0, respectively. The average I/O ratios of 24 h PM(2.5), OC, and EC were 1.4, 1.8, and 1.2, respectively. High indoor-outdoor correlations (r(2)) were found for PM(2.5) EC (0.96) and mass (0.81), and low correlations were found for OC (0.55), indicative of different organic carbon sources indoors. A simple model implied that about two-thirds of carbonaceous particles in indoor air are originated from outdoor sources. PRACTICAL IMPLICATIONS: Indoor particulate pollution has received more attentions in Asia. This study presents a case study regarding the fine particulate matter and its carbonaceous compositions at six residential homes in Hong Kong. The characteristics and relationship of atmospheric organic and elemental carbon were discussed indoors and outdoors. The distribution of eight carbon fractions was first reported in indoor samples to interpret potential sources of indoor carbonaceous particles. The data set can provide significant scientific basis for indoor air quality and epidemiology study in Hong Kong and China.

Phenotypic switching of Cryptococcus neoformans can produce variants that elicit increased intracranial pressure in a rat model of cryptococcal meningoencephalitis.

Increased intracranial pressure (ICP) plays an important role in the morbidity and mortality of cryptococcal meningoencephalitis. The microbial and host factors that contribute to the development of increased ICP are poorly understood. We found that phenotypic switch variants of Cryptococcus neoformans (smooth and mucoid) differed in their abilities to promote increased ICP in a rat model of cryptococcal meningitis. Rats infected with the mucoid variant developed increased ICP, whereas rats infected with the smooth parent did not. This trend correlated with a shorter survival time and a higher cerebrospinal fluid (CSF) fungal burden for mucoid variant-infected rats, although brain fungal burdens were comparable between mucoid variant- and smooth parent-infected rats. Magnetic resonance imaging revealed enhanced T2 signal intensity over the surfaces of the brains of mucoid variant-infected rats. In addition, more polysaccharide accumulated in the CSF and brains of mucoid variant-infected rats. The accumulation of glucorunoxylomannan was associated with elevated levels of MCP-1 (CCL2) and, accordingly, a more pronounced but ineffective monocytic inflammatory response in the meninges of mucoid variant-infected rats. In summary, these findings suggest that strain-specific characteristics can influence the development of increased ICP and indicate a manner in which phenotypic switching could influence the outcome of a central nervous system infection.

Enhanced photocatalytic degradation of VOCs using Ln3+-TiO2 catalysts for indoor air purification.

Two types of lanthanide ion-doped titanium dioxide (Ln3+-TiO2) catalysts including La3+-TiO2 and Nd3+-TiO2 were prepared by a sol-gel method. The effects of the lanthanide ion doping on the crystal structure, surface area, adsorption properties, pore size distribution, and surface chemical state of the catalysts were investigated by means of XRD, BET, and XPS. As results, the crystal size decreased significantly, while the specific surface area, t-plot total surface area, micropore volume, and the total pore volume increased owing to the lanthanide ion doping. The nitrogen adsorption-desorption isotherms of the catalysts showed that the N2 adsorption ability of the Ln3+-TiO2 catalysts was better than the TiO2 catalyst. Among them, the 0.7% Ln3+-TiO2 catalysts demonstrated the highest adsorption ability. The photocatalytic activity of the catalysts was investigated in the experiments of the photocatalytic degradation of benzene, toluene, ethylbenzene and o-xylene (BTEX) in a gaseous phase. The photocatalytic efficiency of the TiO2 catalysts with the lanthanide ion doping was remarkably enhanced by BTEX removal. The 1.2% Ln3+-TiO2 catalysts achieved the highest photocatalytic activity. The enhanced photodegradation of BTEX is possibly due to the improved adsorption ability and the enhanced electron-hole pairs separation due to the presence of Ti3+ on the surface of Ln3+-TiO2 catalysts and the electron transfer between the conduction band/defect level and lanthanide crystal field state.
