The association of antiphospholipid antibodies with severe early-onset pre-eclampsia.

OBJECTIVE: To confirm the association of antiphospholipid antibodies with early onset of severe pre-eclampsia before 30 weeks' gestation. STUDY DESIGN: Thirty-four patients with diastolic blood pressure levels > or = 110 mmHg and at least 2+ proteinuria before the 30th week of pregnancy were randomly chosen for inclusion in the study. Blood samples were taken for assessment of anticardiolipin antibodies (ACAs), lupus anticoagulant, syphilitic serology and antinuclear antibodies. Fifteen normal antenatal patients matched for age, parity and gestational age acted as control subjects. RESULTS: Four of the 34 women (11.7%) in the study group had elevated levels of both ACAs and lupus anticoagulant, compared with none in the control group. This was not found to be statistically different. CONCLUSION: Given the low incidence of positive ACAs in early-onset severe pre-eclampsia it is unlikely that they are implicated in its pathogenesis. It is possible that they represent a small subset of patients with alternative or combined pathology.

Na+ transport, H+ concentration gradient dissipation, and system A amino acid transporter activity in purified microvillous plasma membrane isolated from first-trimester human placenta: comparison with the term microvillous membrane.

Our purpose was to isolate microvillous plasma membrane from first-trimester placenta and to measure its transport properties with regard to Na+, H+, and a neutral amino acid. Microvillous membrane was isolated from first-trimester (10 to 13 weeks) and term (38 to 42 weeks) placenta and the purity determined. Uptake of 22Na+ was measured in the presence of an outwardly directly H+ gradient in the presence or absence of amiloride (0.5 mmol/L). The rate of dissipation of an H+ concentration gradient was determined with the H(+)-sensitive fluorescent probe 2',7'-bis(carboxyethyl)-5(6)-carboxyfluorescein. 14C-methylaminoisobutyric acid uptake was measured in the presence and absence of an inwardly directed Na+ gradient. Purity, vesicle volume, vesicle orientation, and electron micrographic appearance of the first-trimester membranes were similar to those obtained from term placenta, but vesicle protein recovery was lower. Amiloride-sensitive Na+ uptake and Na(+)-dependent 14C-methylaminoisobutyric acid uptake was threefold to fourfold lower by first-trimester than by term membranes. The rate of H+ concentration gradient dissipation was greater in the former. The first-trimester microvillous membrane has similar general characteristics to that from term placenta, but its transport activity is quite different.

Comparison of traditional and molecular methods of typing isolates of Staphylococcus aureus.

Fifty-nine Staphylococcus aureus isolates and 1 isolate of Staphylococcus intermedius were typed by investigators at eight institutions by using either antibiograms, bacteriophage typing, biotyping, immunoblotting, insertion sequence typing with IS257/431, multilocus enzyme electrophoresis, restriction analysis of plasmid DNA, pulsed-field or field inversion gel electrophoresis, restriction analysis of PCR-amplified coagulase gene sequences, restriction fragment length polymorphism typing by using four staphylococcal genes as probes, or ribotyping. Isolates from four well-characterized outbreaks (n = 29) and a collection of organisms from two nursing homes were mixed with epidemiologically unrelated stock strains from the Centers for Disease Control and Prevention. Several isolates were included multiple times either within or between the sets of isolates to analyze the reproducibilities of the typing systems. Overall, the DNA-based techniques and immunoblotting were most effective in grouping outbreak-related strains, recognizing 27 to 29 of the 29 outbreak-related strains; however, they also tended to include 3 to 8 epidemiologically unrelated isolates in the same strain type. Restriction fragment length polymorphism methods with mec gene-associated loci were less useful than other techniques for typing oxacillin-susceptible isolates. Phage typing, plasmid DNA restriction analysis, and antibiogram analysis, the techniques most readily available to clinical laboratories, identified 23 to 26 of 29 outbreak-related isolates and assigned 0 to 6 unrelated isolates to outbreak strain types. No single technique was clearly superior to the others; however, biotyping, because it produced so many subtypes, did not effectively group outbreak-related strains of S. aureus.

Chloride transport by human placental microvillous membrane vesicles.

Unidirectional uptake of chloride by microvillous membrane vesicles prepared from human term placentas was studied over a range of membrane potentials in the presence and absence of chloride transport inhibitors alone and in combination at maximally effective concentrations. At 0 mV, inhibition of chloride uptake by 0.1 mM DIDS, 0.5 mM DPC, and 0.5 mM DPC plus 0.1 mM DIDS was similar, suggesting a common action upon an anion exchanger; neither 0.1 mM furosemide nor 0.1 mM bumetanide alone had any effect. An inside-positive membrane potential was created by imposing an inwardly directed potassium ion gradient in the presence of valinomycin. Total chloride uptake increased with increasing membrane potential (0, 4.6, 17.3, 25.8 and 32.0 mV). The inhibition of uptake by DPC and DPC/DIDS increased with the membrane potential. The effect of DPC compared to DPC/DIDS was significantly different at 4.6, 17.3 and 25.8 mV, suggesting a degree of additivity of inhibition. Neither furosemide nor bumetanide had any effect at any potential. There was a significant increase in inhibition due to DIDS alone until the membrane potential reached 25.8 mV. But there was no significant difference between the level of inhibition at 32 mV as compared to 0 mV, providing evidence of a DIDS-sensitive conductance similar to that previously seen in patch clamp studies. We suggest that uptake of chloride across the microvillous membrane of the human placenta may be by at least three different pathways; an electroneutral, DIDS-sensitive anion exchanger, a DPC-sensitive chloride conductance and a DIDS-sensitive chloride conductance.

Severe invasive group A streptococcal infections in Ontario, Canada: 1987-1991.

During the past few years, there has been an apparent increase in serious infections due to group A streptococci (GAS) worldwide. We describe our experience with severe invasive GAS infections in Ontario, Canada, during the past 5 years (February 1987 through December 1991). A case was defined as the isolation of GAS from blood or normally sterile tissue in association with hypotension (systolic blood pressure, < 90 mm Hg). Fifty cases were identified in patients ranging in age from 4 to 100 years (median age, 47 years); 29 (58%) of the patients died. A primary focus of infection was identified in 38 cases (76%), with soft tissue being the site involved most frequently (68%). No focus of infection was found in 12 patients, and 36 patients (72%) were bacteremic. Complications included acute respiratory distress syndrome (21 of 50), acute renal failure (20 of 50), hypocalcemia (19 of 24), elevated creatinine kinase values (21 of 27), coagulation abnormalities (15 of 21), and hepatitis (15 of 24). Eleven cases (22%) were nosocomial; one of these was secondary to another nosocomial case. Thirty-three isolates were available for M and T typing and for determination of the presence of the genes for streptococcal pyrogenic exotoxin (SPE). The most frequent types were M1T1 (10) and M12/T12 (8). Twelve isolates possessed the speA gene, and 16 isolates had the speC gene. Only three isolates possessed both speA and speC. All isolates possessed the speB gene.

The ontogeny of peroxisome-proliferator-activated receptor gene expression in the mouse and rat.

The expression of the gene coding for peroxisome-proliferator-activated receptor (PPAR), a novel transacting factor belonging to the steroid superfamily, has been determined in the mouse and rat throughout development using hybridization histochemistry. Messenger RNA is demonstrable in the liver and brown fat from the fetal period onwards and, additionally, in the heart, kidney and gut post-natally. It is proposed that the upregulation of transcription of peroxisomal beta oxidation genes in specific tissues follows binding of the receptor to its natural ligand. Thus PPAR may have an important role in cold adaptation and non-shivering thermogenesis as well as in detoxification.

Etiology of childhood diarrhea in Beijing, China.

To determine the role of recently recognized enteropathogens in childhood diarrhea in China, 221 children with diarrhea and 108 controls seen at the Beijing Children's Hospital were studied during April and May 1989. Stools were examined for ova, parasites, and rotavirus, cultured for bacterial pathogens, and probed for enterotoxigenic Escherichia coli (ETEC), enteroinvasive E. coli (EIEC), enterohemorrhagic E. coli (EHEC), and enteropathogenic adherence factor-positive (EAF+) E. coli. Pathogens were identified in 56.5% of children with diarrhea and 43.5% of controls (P = 0.04). Detection of enteropathogens was significantly greater in patients examined within 1 week of symptom onset (65%) than in patients examined later (39%; P = 0.01). ETEC was the most frequently detected pathogen in children with diarrhea, accounting for 20% of the cases. Other agents identified in patients included the following: salmonellae, 12%; rotavirus, 7%; EIEC, 7%; EHEC, 7%; members of the Aeromonas hydrophila group, 6%; EAF+ E. coli, 5%; Ascaris lumbricoides, 3%; shigellae, 3%; campylobacters, 2%; and Vibrio spp., 0.5%. The isolation rates of salmonellae (P = 0.02), EAF+ E. coli (P = 0.04), and mixed pathogens (P = 0.05) were significantly greater for diarrhea patients than for controls. Resistance to multiple antimicrobial agents occurred in 39% of the Salmonella isolates, 22% of the Aeromonas isolates, and 17% of the Shigella isolates. Multiresistant salmonellae (P = 0.05) and shigellae were recovered from diarrheal stools only. Ciprofloxacin, cefotaxime, and imipenem were the only agents tested to which all bacterial isolates were susceptible in vitro. These results suggest that both traditional and newly recognized agents are important causes of childhood diarrhea in Beijing and that therapy may be complicated by indigenous antimicrobial resistance.

The expression of IGF-II in the early post-implantation rat conceptus.

Insulin-like growth factor II (IGF-II) gene expression was investigated in the early rat conceptus using in situ hybridisation. IGF-II mRNA has been detected as early as 7 days post coitum in the extraembryonic ectoderm of the egg cylinder (including the ectoplacental cone but excluding the trophoblast bordering the implantation chamber). There was no expression either in the early embryo at this stage or in the implanting blastocyst (5.5 days). At 9.5 days transcription was also detected in extra-embryonic mesodermal derivatives and the visceral yolk sac endoderm, but not in the embryonic region. At 10.5 days transcription was detected in embryonic mesodermal derivatives. High levels were found in the primitive heart, hepatic diverticulum and septum transversum; lower levels were found in the early somites and other primitive mesenchymal derivatives and midgut endoderm. No expression was detected at any stage in derivatives of the embryonic ectoderm, including the neurectoderm. Recent studies of IGF-II, the IGF-II/M-6-P receptor and enzymic extracellular matrix remodelling suggest that the expression of IGF-II in the early rat conceptus may be related to hyaluronate breakdown (with, or without, sulphated glycosaminoglycan synthesis), linked with the aggregation and differentiation of previously migrating mesenchymal cells.

Arterial thrombosis in nephrotic syndrome.

The nephrotic syndrome is characterized by profound changes in the turnover and concentration of most plasma proteins, including those involved in the coagulation pathways. Thromboembolic complications, especially venous, have been widely reported. Arterial thrombosis is a relatively rare complication and has been reported mainly in nephrotic children. In this report, an adult nephrotic patient who developed thromboses of his right middle cerebral and left femoral arteries is described. The patient died, and at postmortem no underlying arterial disease was found. Histology of the kidneys showed minimal change disease.

Expression of the IGF-II/mannose-6-phosphate receptor mRNA and protein in the developing rat.

The insulin-like growth factor II (IGF-II) receptor is identical to the mannose-6-phosphate receptor (M-6-P), but its role as a somatomedin transducer is uncertain. IGF-II/M-6-P receptor expression was studied by in situ hybridization (ISH) in the developing rat. Expression occurs in extra-embryonic membranes at the time of IGF-II mRNA induction and later at paracrine/autocrine sites of IGF-II action (skeletal muscle and perichondrium) in the embryo. Highest levels of receptor mRNA occur in heart and major vessels. Postnatally transcription is strongly down-regulated. This suggests a role for the IGF-II/M-6-P receptor in IGF-II action or turnover during development distinct from its role in lysosomal transport.

MR technique for localization and verification procedures in episcleral brachytherapy.

Spatial definition of an intraocular tumor and subsequent determination of the actual position of an implanted eye plaque are essential for adequate ocular brachytherapy treatment planning. However, a method for verification of the plaque placement which would provide required 3-dimensional information is not available at present. In addition, tumor localization procedures, including ultrasonography and CT techniques, cannot always offer the precision needed for 3-dimensional definition of an intraocular target. This communication describes a magnetic resonance imaging technique specifically developed for both localization and verification procedures. A 1.5 Tesla magnetic resonance scanner, spin-echo pulse sequence (echo time 30 msec, repetition time 700 msec), and commercially available surface coil were used to obtain a series of transverse, coronal, and sagittal images of a slice thickness of 3 mm. Usually, eight scans in each of the three planes were needed for adequate coverage of the orbit. The required patient set-up and data acquisition time did not exceed 40 minutes. With a data matrix size of 256 X 256 pixels and 13 cm field of view, localization and verification were accomplished with a precision of 0.5 mm. Our results suggest that the magnetic resonance imaging technique permits precise integration of diagnostic and therapeutic procedures, and in addition provides adequate data for accurate treatment planning. We conclude that magnetic resonance imaging is the preferred diagnostic technique for episcleral brachytherapy.

Histochemical localization of IGF-I and IGF-II mRNA in the rat between birth and adulthood.

We describe the postnatal ontogeny and localization of insulin-like growth factors I and II (IGF-I and -II) in the rat. We have used oligodeoxyribonucleotide probes for in situ hybridization (hybridization histochemistry) and for Northern blotting. IGF-II mRNA is strongly expressed in liver, skeletal muscle, perichondrium, leptomeninges and choroid plexus of the newborn. Demonstrable levels fall dramatically in the liver at 18-20 days postnatally but persist for longer periods in muscle and remain undiminished throughout life in the pia/choroid plexus, indicating that different control mechanisms operate in these tissues. IGF-I mRNA is predominantly found in the liver. Its level in this organ rises well before levels of IGF-II fall. This suggests that distinct factors govern the expression of IGF-I and -II genes.

Control of IGF-II mRNA levels by glucocorticoids in the neonatal rat.

Insulin-like growth factor-II (IGF-II) is thought to be a major growth factor during fetal and neonatal development. Levels of IGF-II mRNA fall dramatically in the liver - the major site of endocrine production - between 18 and 20 days post-natally. No information concerning the control of gene expression post-natally has hitherto been available. Using Northern blotting and in-situ hybridization, we show here that cortisone acetate rapidly extinguishes IGF-II mRNA expression in the neonatal rat liver. The effect at putative autocrine/paracrine locations such as skeletal muscle and choroid plexus is much less marked or absent. The repression by cortisone acetate is discussed in the light of the available IGF-II gene sequence.

Antimicrobial cream susceptibility testing.

Utilizing a recently described rapid needle extrusion method for determining the sensitivities of burn wound isolates, we have compiled data on the resistance patterns of over 250 isolates from our burn unit. Major isolate groups were Staphylococcus aureus, Pseudomonas aeruginosa, various Enterobacteriaceae and Enterococci. Excellent correlation was exhibited between the inhibitory zone sizes and the minimal inhibitory concentration for 120 organisms tested. Utilizing the needle extrusion sensitivities to facilitate the selection of the burn creams, a significant reduction in the microbiologic flora of the burn wound was noted. The utilization of this technique in the selection of burn creams deserves controlled trials to assess whether changes in topical therapy might alter clinical outcome.
