Plasma electrolytes in elderly patients taking fixed combination diuretics.

Plasma potassium and sodium concentrations were measured in a group of elderly patients taking maintenance thiazide diuretic therapy alone, with a potassium supplement or in combination with a potassium sparing diuretic. Fixed dose combinations of a thiazide and potassium sparing diuretic did not significantly reduce the prevalence of hypokalaemia and the combination of amiloride-hydrochlorothiazide was associated with a disproportionate number of cases of hyponatraemia. The desirability of the current widespread use of fixed dose combination diuretics over less expensive single agents is questioned.

Effect of suture technique on blood velocity waveforms in the microvascular anastomosis of autogenous vein graft.

In using autogenous vein grafts in microvascular surgery, maintenance of compliance at the site of anastomosis is of importance in achieving patency. Using high-frequency, 20 MHz, pulsed Doppler ultrasonography to monitor blood flow velocity through the vein graft, we have evaluated the effects of continuous and interrupted suture technique on anastomotic compliance in vessels less than 1.5 mm in diameter. In 15 dogs, 15 procedures of each technique, using the continuous technique in one limb and the interrupted technique on the contralateral limb, patency rates for both techniques, documented by arteriography, are 100% at a mean follow-up of 11 weeks. There were no significant differences in the blood velocity profile and pulsatility proximally, distally, and at the graft site between continuous and interrupted technique. This study confirms the suggestion that autogenous vein graft has sufficient elasticity to maintain normal arterial blood velocity waveforms.

Compliance with neuroleptic medication in outpatients with schizophrenia; relationship to subjective response to neuroleptics; attitudes to medication and insight.

The relationship between compliance, subjective response to neuroleptics, attitudes to medication, and insight was assessed in a selected group of individuals with schizophrenia who were attending a catchment area outpatient psychiatric service. Regular compliers with medication reported a significantly better subjective response to neuroleptics and were more frequently prescribed depot medication compared with individuals who were irregularly compliant. There was no difference between regular and irregular compliers in terms of attitudes to medication or level of insight. There was a significant correlation between the measures of insight obtained using a self-report questionnaire (the Insight Scale [IS]) and a semistructured interview (the Scale for Assessment of Insight [SAI]).

The effect of aspirin on serum immunoglobulins.

After discontinuing aspirin products for seven days, 21 aspirin tolerant adults were given 650 mg of aspirin daily for one week. Serum immunoglobulins (IgG, IgA, IgM, and IgE), complete blood count (CBC), and total eosinophil counts (TEC) were determined before aspirin challenge, four hours after initial challenge, on day seven of aspirin ingestion, 24 hours after discontinuing aspirin, and three weeks after discontinuing aspirin. No significant changes were noted in any of these categories. Although aspirin is an anti-inflammatory drug similar to corticosteroids, it does not produce immunological changes that corticosteroids produce as determined by these parameters.

Is there a relationship between the structure of Irish psychiatric multidisciplinary teams and patient outcome?

This study evaluated the structure of psychiatric multidisciplinary teams within eight Irish Health Boards over a two year period (1997 and 1998). Furthermore, it assessed the relationship between team structure and three measures of psychiatric outcome within individual Health Board areas. The findings suggest a significant variation between Health Boards in relation to the staffing of psychiatric multidisciplinary teams. Although there was no evidence that the structure of multidisciplinary teams influenced suicide rates or duration of hospitalization, it had a significant independent effect on admission rates to psychiatric units.
