Anti-idiotypes in B-cell tumor therapy.

Thirteen patients with B-cell lymphomas were treated with mouse monoclonal anti-idiotype antibodies. All but 1 of the patients in this study had received extensive prior treatment with conventional therapy for lymphoma. The treatment protocol initially included an escalating dose schedule which was intended to help us evaluate toxicity and pharmacokinetics and, eventually, to achieve appreciable levels of free mouse antibody in the circulation. The last 4 patients received substantial initial doses. Tumor sampling was performed before and during therapy for evaluation of tissue penetration by antibody. Patients received antibodies of gamma 1, 2a, or 2b isotype. None of the patients had serum paraproteins by routine clinical testing, but 6 had an idiotype protein detectable by a sensitive immunoassay at levels greater than 1 microgram/ml, two of which were greater than 200 micrograms/ml. These levels were temporarily reduced by plasma-pheresis. However, the presence of serum idiotype increased the requirement for mouse antibody to achieve tumor penetration. Another obstacle to treatment was immune response to mouse Ig that occurred in 5 of the 13 patients. Once an immune response had begun, further infusions of antibody failed to reach the tumor or induce tumor regression and were associated with toxicity. Our initial patient remains in an unmaintained complete remission 50 months after receiving antibody. Six of 12 additional patients have had objective remissions which also were clinically significant. However, these remissions were not complete. This therapy shows promise as an alternative modality for the treatment of B-cell lymphoma. We will need further studies to determine the mechanisms of the antitumor effect and to improve the clinical results.

Maternally administered antenatal vitamin K1: effect on neonatal prothrombin activity, partial thromboplastin time, and intraventricular hemorrhage.

Infants weighing 1500 g or less at birth are susceptible to intraventricular hemorrhage. This may be due in part to low concentrations of vitamin K-dependent clotting factors. Women in labor between 24-34 weeks' gestation were selected, according to their hospital registration number, to receive 10 mg vitamin K1 intramuscularly at least four hours before delivery. Control women received no vitamin K. The study included only infants born of mothers who were in hospital more than four hours before delivery, who weighed 1500 g or less at birth, and were less than 34 weeks' gestation. Twenty vitamin K1 and 33 control infants qualified for the study. Infants in both groups received routine postnatal vitamin K1. On admission, the infant's prothrombin activity and partial thromboplastin time (PTT) were measured. A head ultrasound was done between days 2 and 4 of life. Results demonstrated significantly improved prothrombin activity, a nonsignificant trend toward improved PTT, and a significantly decreased frequency of intraventricular hemorrhage in infants whose mothers had received vitamin K1. The effect of antenatal vitamin K1 on prothrombin activity and PTT appeared to be more pronounced in female infants.

Sexual quality-of-life of patients with arthritis compared to arthritis-free controls.

The influence of arthritis upon sexual satisfaction and activity and patient receptivity to sexual rehabilitation was investigated by interviewing 169 patients with arthritis and 130 controls. Patients differed from controls in their greater loss of sexual satisfaction over time, but they were comparably satisfied with their current sexual adjustment. They reported similar reductions in frequency of intercourse over time. Joint symptoms and fatigue disturbed the sexual adjustment of patients more than controls, but damaged body image, worry about partner interest, loss of libido and loss of lubrication did not. There was receptivity to a program of sexual rehabilitation among patients and controls that was not dependent upon sexual dissatisfaction.

Transferrin binding of Al3+ and Fe3+.

An understanding of Al3+-induced diseases requires identification of the blood carrier of Al3+ to the tissues where Al3+ exerts a toxic action. Quantitative studies demonstrate that the protein transferrin (iron-free) is the strongest Al3+ binder in blood plasma. Under plasma conditions of pH 7.4 and [HCO3-]27 mmol/L, the successive stability constant values for Al3+ binding to transferrin are log K1 = 12.9 and log K2 = 12.3. When the concentration of total Al3+ in plasma is 1 mumol/L, the free Al3+ concentration permitted by transferrin is 10(-14.6) mol/L, less than that allowed by insoluble Al(OH)3, by Al(OH)2H2PO4, or by complexing with citrate. Thus transferrin is the ultimate carrier of Al3+ in the blood. We also used intensity changes produced by metal ion binding to determine the stability constants for Fe3+ binding to transferrin: log K1 = 22.7 and log K2 = 22.1. These constants agree closely with a revision of the reported values obtained by equilibrium dialysis. By comparison with Fe3+ binding, the Al3+ stability constants are weaker than expected; this suggests that the significantly smaller Al3+ ions cannot coordinate to all the transferrin donor atoms available to Fe3+.

Amyloidosis in a dog: treatment with dimethylsulfoxide.

A dog with hypoalbuminemia, proteinuria, and renal amyloidosis was treated with dimethylsulfoxide for 2 years. During a 1-year period, serum albumin concentrations increased to within normal range. The 24-hour urinary protein excretion was 5.7 g before treatment with dimethylsulfoxide. Two years after initiation of treatment, the 24-hour urinary protein excretion was 3.7 g. Removal of a testicle containing a Sertoli cell tumor and treatment of interdigital pyoderma may have been factors in the dog's clinical improvement. The only adverse effect from the dimethylsulfoxide treatment was the characteristic disagreeable odor.

Microsomal epoxide hydrolase activity in human x mouse hybrid cells.

Microsomal epoxide hydrolase (E.C.3.3.2.3) activity has been measured in human x mouse hybrid cells prepared from human cells expressing 6-7 x the activity of the mouse cells. Rabbit antihuman and antimouse antisera raised against purified enzymes were used to discriminate between human and mouse enzymes. All twenty five clones examined did not express human enzyme and this correlated with the loss of human chromosome 6 from each cell line. Four hybrids expressed 2-3 x the activity expressed by the mouse cell parent and these all retained more human chromosomes, specifically chromosome 19, than those with low activity. It is concluded that the human gene for epoxide hydrolase may be on chromosome 6 and that other gene products can affect the level of activity expressed by a cell.

T-cell depletion of allogeneic bone marrow prevents acceleration of graft-versus-host disease induced by exogenous interleukin 2.

Highly purified human recombinant interleukin 2 (IL-2) markedly accelerated lethal GVHD in the H-2-identical B10.BR----CBA combination, but had no effect when the donor cells were depleted of mature (Thy-1.2-positive) T lymphocytes, indicating a strong immunopotentiating effect of IL-2 on mature T cells causing GVHD. In the same donor-host combination, IL-2 did not influence the recovery from the post-transplantation bone marrow aplasia. The results suggest that IL-2 could be considered for adjuvant hormonal therapy to enhance immune recovery in recipients of T-cell-depleted allogeneic marrow.

Perceptual plane geometry: collinearity judgments probe the perceived orientation of an angle's sides.

The task was to position a dot to lie one line length beyond the vertex of an angle by mentally extending one line segment forming the angle (and later the other line segment). Eight angles, variously oriented, provided judgmental errors attributable to the size of the subtended angle and line orientation. Collinearity errors are consistent with the hypothesis that the sizes of all subtended angles are underestimated. Horizontal-vertical assimilation describes the line-orientation effect. The largest error component is associated with the interaction of the two independent variables. Judgmental errors were modeled by a set of theoretically meaningful additive terms. Modeling was facilitated by symmetry assumptions (associated with odd and even mathematical functions) about the absolute magnitude and sign of judgmental errors that lead to the comparison of judgments derived from "interesting pairs" of stimuli.

Pancreoblastoma in a neonate associated with Beckwith-Wiedemann syndrome.

A cystic abdominal mass was removed from a male child born at 32 weeks gestation. Histological examination showed this to be a pancreoblastoma. Both clinical and histological features of Beckwith-Wiedemann syndrome developed within the first 12 weeks of life. The child remains well and thriving with no tumour recurrence at 10 months of age.

Morbidity and mortality in duodenal atresia.

98 babies were admitted to the Hospital for Sick Children, Great Ormond Street, London, with duodenal atresia between 1967 and 1981. The overall mortality was 32%, although it fell from 39% in the first half of the series to 26% in the second half. This reflected the better management of babies in risk group C (Watterson et al 1962). However, the high incidence of associated anomalies, involving particularly the alimentary tract, was demonstrated to carry a grave prognosis.

The sterility testing of dispensed radiopharmaceuticals.

The benefits of retrospective sterility tests on radiopharmaceuticals, were assessed by following the survival of bacteria in non-radioactive and radioactive kits and generator eluate over a period of 5 days. The response of bacteria inoculated into non-radioactive materials ranged through death, survival and growth. The response of bacteria inoculated into radioactive materials was either death or survival. The value of retrospective sterility testing is doubtful and an improved test method is suggested.

Transplantation of fetal fibroblasts and correction of enzymatic deficiencies in patients with Hunter's or Hurler's disorders.

An attempt was made at correcting the specific lysosomal enzyme deficiencies in 7 children with Hunter's or Hurler's diseases by transplantation of fetal fibroblasts. In spite of pretreating the young patients with stored blood, following a procedure employed successfully to avoid rejection of kidneys from incompatible donors, the use of serum-free media for culturing the cells before being harvested and incubation of the cells with chorionic gonadotrophin, the transplantation of fetal fibroblasts was not associated with biochemical or clinical changes. None of the seven patients showed immune reactions against the transplanted cells, HLA antigens, or the missing enzymes.
