[CT and conventional radiography in the evaluation of bony lesions (author's transl)].

Several cases selected from a group of 67 patients with bony lesions (primary benign and malignant neoplasia, secondary neoplasia and metabolic disorders) serve to demonstrate the significant information gained by CT compared to that gained by conventional radiographic methods including angiography and bone scanning. CT is advantageous in showing the exact intra- and extraosseous extent of the lesion--helpful for further diagnosis and therapy. Measurements of length, area, volume and density can help to narrow down the differential diagnosis and they aid in follow-up examinations. CT supplements conventional radiographic methods and in some cases may even replace them, e.g. angiography.

Adenocarcinoma↗

[Radiology disease of rare liver tumors (author's transl)].

The past decade has shown an increase in the number of rare primary hepatic tumors, such as focal nodular hyperplasia (FNH), and hepatocellular adenoma and carcinoma. Some examples serve to demonstrate the value of scintigraphy, ultrasound, angiography and CT in the diagnosis of these lesions. Furthermore, the value of the radiographic finding for treatment and follow-up studies is pointed out--in conservative as well as surgical treatment.

Adult↗

[Primary varicosis of the lower extremity (author's transl)].

Primary varicosis is due to congenital or acquired defects of the venous wall and valves. Subjective patient complaints are often different and even in contrast to clinical findings. The combination of insufficiency of the large superficial veins and insufficiency of the perforating veins often leads to chronic venous insufficiency with crural ulceration. Phlebography is indicated prior to surgery in order to evaluate the deep and the perforating veins and to check for variants. The recurrance rate for varicosis is low if all insufficient superficial and perforating veins are made functionless.

Adult↗

[Post-thrombotic syndrome of the lower extremities (author's transl)].

The postthrombotic syndrome with chronic venous insufficiency develops either gradually over a number of years or after a latent period of varying length. The clinical manifestations correlate with morphological and functional phlebographic findings of the superficial and deep veins. The conservative therapy of the post-thrombotic syndrome, consisting of anti-coagulant therapy, compression dressing or active physical exercise, must be consistently enforced. Indication of vascular surgical intervention should be carefully noted. Optimal examination of the patho-anatomical findings in the phlebogram is imperative as an important aid in decision-marking.

Collateral Circulation↗

[Computerized tomography in the evaluation (author's transl)].

CT can clearly demonstrate dilation of intra- and extra-hepatic bile ducts due to mechanical obstruction. Note is made that the intrahepatic bile must not necessarily participate in dilation in obstructive jaundice. The cause in 27 cases observed in our institutions was as follows: 16 pancreatic tumors; 1 stone; 2 extrahepatic bile duct obstructions; 4 liver lesions (tumor and cirrhosis) and 4 with cause unknown. Furthermore, CT is helpful in the evaluation of hepatogenic non-obstructive jaundice such as due to primary liver cell carcinoma (hepatoma), metastases to the liver and advanced cirrhosis of the liver. The value of CT in the evaluation of different types of cholestasis is demonstrated by several exemplary cases; and the problems of differential diagnosis are pointed out.

[Radiographic findings of periosteal reactions in systemic bone disorders (author's transl)].

Periosteal reaction is frequently the first sign of systemic disease affecting the skeleton, including generalized osteopathia. It can be generalized, focal, monostotic or polyostotic and shows solid, lamellary or interrrupted spiculae-like reaction. A reliable diagnosis is possible from the interpretation of these changes, as for instance with spiculae: Very dense and evenly arranged spiculae are only seen in hemolytic anemias and metastases of neurogenic tumors. Onionskin like periosteal reaction with simultaneous transformation of the diaphyseal cortex are difficult to interpret. However, in case of hyperparathyreoidism, subperiosteal resorption and transformation of the diaphyseal cortex permit precise diagnosis. Follow-up examinations have prognostic and therapeutic value. The knowledge of the different morphology of periosteal reaction associated with systemic disease (including generalized osteopathias) is important in diagnostic radiology.

Does long term haemofiltration provoke secondary hyperparathyroidism?

One thousand nine hundred and forty-nine haemofiltration procedures have been performed since 1976; 4 different types of haemofilters have been tested in gravimetric and volumetric haemofiltration machines. Balance studies revealed a diminished calcium intake of about 1/3 of that found in conventional dialysis. Magnesium balance is negative and leads to decreasing serum Mg values. Inorganic phosphate is removed in similar quantity to dialysis. PTH levels increase during 3--18 months of treatment, whereas 25-OH-D3 levels rapidly fell, gc-globulin- and HCT-levels did not change markedly. g-c globulin could be detected in filtrate samples in remarkable amounts, 25-OH-D3 was found in traces. Radiologically no apparent alteration occurred. We recommend Vitamin D supplementation to compensate for the losses and thereby avoid development of secondary hyperparathyroidism.
