[Dietary behavior and digestive diseases].

The influence of alimentation on the digestive pathology is very important. In this report the authors review the principal results of epidemiologic studies and animals experimentations. According to this survey of the literature it can be stated that some presumptions exist for: -- the responsibility of diet without vegetal fibers in the frequency of constipation, colonic divercitular disease, piles and hiatal hernia. The comparison of the alimentary habits in the western Europe with rural Africa is very instructive on that matter; -- the responsibility of alcohol consumption, use of hypercaloric regimen and hyperlipidic ingestats as causative factors for chronic pancreatitis; -- the importance of an hypercaloric, hyperlipidic and low residue regimen as etiologic factors in biliary gallstones; -- the role of denutrition and alcoholism in liver steatosis and cirrhosis in developed country; -- more important, perhaps, is the suspicion of the role of nutrition in the development of digestive cancer: alcohol will facilitate oesophageal cancer, alimentary nitrites gastric cancer meanwhile fiberless regimen and biles salts will promote colonic cancer. Impairments of nutrition observed after digestive resections in case of inappropriate alimentation are also analyzed as well as the principal alimentary disturbances related to allergy or enzymatic deficiency.

[Zollinger-Ellison syndrome treated medically by an inhibitor of H2 histamine receptors].

Metiamide an histamine H2-receptors antagonist has been used to treat a case of Zollinger-Ellison syndrome characterized by a long standing diarrhea, an important gastric hypersecretion and a moderatly elevated plasma gastrin but without digestive ulceration. At the dose of 600 mg per day, Metiamide induced a complete suppression of acid secretion, an effect which lasted for 15 days after stopping the drug. Accordingly and since the only finding at time of laparotomy was a small lymph node enlarged with endocrine metastatic tissue, the stomach was left intact and Metiamide pursued. During the first 4 months of chronic administration of Metiamide, acid secretion was maintained at levels below 25 p.cent of initial values. Ulteriorly however, although dosages of Metiamide were increased, acid hypersecretion resumed and a duodenal ulcer developed. Total gastrectomy was then performed 11 months after the beginning of Metiamide. In spite of the failure of Metiamide treatment, the long term follow up of this case of Zollinger-Ellison Syndrome, allowed us to get theoretical and practical informations.

Secretin effect on gastrin, gastric secretion in dogs with chronic antral stimulation.

Antrocolic transposition in four dogs with Heidenhain pouch and gastrojejunostomyresulted in a marked increase in fasting plasma gastrin concentration and sustained highacid secretion, closely related to gastrin levels. A marked production of pepsin output could not be correlated with plasma gastrin. Reduction in plasma gastrin concentration was more than twofold less pronounced than the reduction in acid output for 1 and 2 U/kg-h, while the 0.5 U/kg-h no effect was noted. For both acid output and gastrin concentrations, close correlations were noted between presecretion level and remaining level upon secretin infusion. Despite the reduction in the secretory volume of the pouch, 0.5 and 1 U/kg-h of secretin induced a 1.5- and 2-fold increase in pepsin output, respectively. Two untis per kilagram-hour decreased the secretory volume as well as the pepsin output. If the physiological release of secretin in dogs does not exceed the equivalent of the lowest dose studied, our results would indicate that acid inhibition is a physiological activity of secretin, while the effect on circulating gastrin concentration seems to be phamacological.
