Malignant blue nevus. Report of four new cases and review of the literature.

Malignant blue nevus is a rare melanotic tumor of the skin arising from a pre-existing blue nevus. To date only few cases have been reported. The diagnosis of malignant blue nevus is frequently difficult. Generally accepted criteria do exist, but their application may be controversial. The present paper reports four new cases of malignant blue nevus with follow-up of 3-5 years. Our present data, along with those of the literature, suggest that malignant blue nevus may be more aggressive than generally thought.

6-Methylangelicins: new monofunctional photochemotherapeutic agents for psoriasis.

The monofunctional furocoumarins, the 6-methylangelicins, were tested for their antiproliferative activity with various animal models and for genotoxicity in micro-organisms and in mammalian cells. The most active compound was 6,4,4'-trimethylangelicin, which showed a high antiproliferative effect and reduced genotoxicity in comparison with 8-methoxypsoralen (8-MOP). Some of these compounds were also tested clinically by topical application on 17 patients with psoriasis. They appeared to be more active than 8-MOP in clearing psoriasis without inducing skin phototoxicity. The methylangelicins also caused skin pigmentation.

Metastases to the breast from extramammary malignancies.

Metastases to the breast from all other primary sites are unusual. Twelve female patients were diagnosed between 1971 and 1985 at S. Chiara Hospital-Trento. The majority of these metastases were from malignant melanoma (5) and ovarian cancer (3). In 1 case the lesion was the first sign of malignant disease (adenocarcinoma of the kidney). The development of a breast metastasis is revealed, in our experience, as a negative prognostic factor. Recognition of these lesions as being metastatic is useful in avoiding unnecessary radical operative procedures.

Mucinous sweat-gland adenocarcinoma of the inner canthus: a case report.

Mucinous sweat-gland adenocarcinoma is a rare tumor of the eyelid. Fewer than 30 cases have been reported in the literature. We describe the case of a 77-year-old man with a papillomatous lesion of the inner canthus. The lesion was removed, and the histologic examination confirmed the diagnosis of a mucinous sweat-gland adenocarcinoma. After three years, there has been no recurrence or metastasis.

[Early diagnosis of cutaneous melanoma and its precursors. Importance of the clinical characteristics of thin melanoma].

The diagnosis and removal of precursor and early lesions is the only method to improve the prognosis and survival rate in malignant melanomas of the skin since the aetiology of the disease is unknown and treatment ineffective when the neoplasm is diagnosed at an advanced stage. A campaign for the prevention and early diagnosis of melanoma is based on the health education of the entire population with the distribution of illustrative material that by teaching people to examine their skin will show up suspicious pigmentary lesions that can be examined and removed by the dermatologist. Thin melanoma is always curable when removed and constitutes the basic tumour to be described to the public as it is easily identified and has constant, typical features. Among the precursors the most interesting lesions is the sporadic or familial dysplastic nevus despite the controversy over its pathogenic significance and morphological aspect. The other pigmentary neoplasias (congenital and acquired nevus) only must to be removed when they present changes resembling thin melanoma.

[Breast metastasis as the initial clinical symptom of renal adenocarcinoma].

Renal adenocarcinoma shows an unpredictable clinical course. At autopsy, metastases are present in as many as 95 percent of patients with renal adenocarcinoma. The percentage of metastases from renal adenocarcinoma is greater than would be expected from the frequency of this carcinoma among all carcinomas. The breast is an exceptional site of metastasis, that is not described in literature. In the present report the metastasis in the breast represented the early symptom of renal adenocarcinoma.

Postirradiation sarcoma in a patient treated for testicular seminoma.

A patient with a sarcoma of the left groin at a site where a prior seminoma had been irradiated is presented. Very few postirradiation sarcomas are described in patients treated with radiation therapy for testicular cancers, even if an increased risk of second malignancies has been reported. In contrast with the highly aggressive clinical course of postirradiation sarcomas, an early discovery of the lesion allowed a radical surgical approach with a presumable cure of the patient. A continuous and precise follow-up of patients irradiated for seminoma is suggested.

Reproducibility of histopathologic diagnosis and classification of non-melanocytic skin cancer: a panel exercise in the framework of the multicenter southern European study HELIOS.

AIMS AND BACKGROUND: The reproducibility of histologic diagnoses, and in particular of the distinction between basal and squamous cell forms, has been evaluated in the framework of a multicenter case-control study (in Italy, France, Spain and Switzerland) aimed to assess the causes of non-melanocytic skin cancers. METHODS: A panel composed of 10 pathologists from the collaborating centers was appointed. A total of 1,774 slides of routine diagnoses were blindly reviewed by a second panelist; discordant diagnoses underwent a third examination. Controversial and interesting cases were discussed during general sessions. RESULTS: The results showed a high degree of concordance (99.5%), with a Cohen's kappa of 0.85 (95% CI, 0.77-0.94) in the assessment of malignancy of lesions. The concordance in the distinction between large morphologic groups, basal cell carcinoma and squamous cell carcinoma was also high (Cohen's kappa = 0.85; 95% CI, 0.82-0.89). The assessment of histologic subtypes, degree of invasion and differentiation showed a lower degree of concordance, presumably as a consequence of a weaker consistency in the relevant classifications. CONCLUSIONS: The reproducibility study has therefore validated the epidemiologic study and in particular allowed to correct some misclassifications that could have lessened the analysis of the case-control study. In general, because of its characteristics (number of pathologists and variety of their origins, the large number of cases examined, blind examination of histologic slides), the conclusions of the study may show a certain degree of generalization, at least with regard to the countries represented. Routine histologic diagnoses of cutaneous carcinoma showed a high degree of reliability with reference to the assessment of malignancy and the distinction between basal and squamous cell carcinoma morphotypes.

Malignant cellular blue nevus with true nodal metastases.

A case of malignant blue nevus initially misdiagnosed as cellular blue nevus with atypia is reported: the correct diagnosis was established after the onset of true nodal metastases. Diagnostic criteria in this difficult field of dermatopathology are examined and their limits discussed in view of the not rare pitfalls reported in literature.

Microscopical study of trabeculum after laser cyclotrabeculospasis: case report.

Argon Laser trabeculoplasty and Cyclotrabeculospasis are two common procedures in the treatment of open angle glaucoma. Anatomo-clinical correlation to clarify the exact mechanism of Argon Laser therapy is difficult. We recently used Cyclotrabeculospasis 180 degrees on a patient with open angle glaucoma who developed a choroidal Malignant Melanoma. The eye was enucleated. We studied the trabecular meshwork and Schlemm's canal and measured the inter-trabecular spaces and the opening of Schlemm canal with an ocular micrometer. Comparison of the slides relative to the treated and untreated sections in the same eye seem to confirm the hypothesis of direct traction on the trabecular meshwork by the phocoagulating tissues nearby.
