[Catheter dilatation of arteries supplying the brain].

Stenoses of the extracranial cerebral arteries can be successfully treated with percutaneous transluminal dilatation (PTD). Early and late results of PTD are comparable to those of vascular surgery, but are not accompanied by its morbidity and mortality rates. In 32 patients with subclavian steal syndrome PTD showed a technical success rate of 88% and a clinical cure rate of 72%. The fibrodysplastic carotid artery stenosis is well suited for PTD. These lesions often extend to the skull base causing difficulties for surgical correction. First experiences with PTD of atherosclerotic carotid artery stenosis are promising, but the efficacy of this procedure must be confirmed by a greater number of treated patients. PTD of the vertebral and basilar arteries present new therapeutic possibilities in the management of vertebrobasilar insufficiency demanding further evaluation.

[Differential diagnostic problems in cerebellopontile angle tumors? A case report].

A patient, suffering from otosclerosis of the right ear, was stapedectomized in 1977. Due to bleeding into the oval window during surgery, a persistent deficit of inner ear performance occurred, causing vertigo and hearing loss of high frequencies, in the right ear. As vertigo increased over the years, the patient underwent extensive otological and neurological examinations without result. Because of anamnestic data all interest was focussed on the right ear, and even gas cisternography was performed on the right side only. Due to a left-side sudden hearing loss, the patient came to the Freiburg ENT Hospital in 1981. ERA examinations and, finally, gas cisternography revealed a tumour--meanwhile confirmed by surgery (acoustic neurinoma)--of the left cerebello-pontine angle. As a consequence of this it must be pointed out that in all cases of unilateral or bilateral loss of hearing of the inner ear and vertigo of unclear etiology, ERA investigations and bilateral gas cisternography should be performed.

Audiometry↗

[Brain stem audiometry and metrizamide-meatocisternography in diagnosis of small neurinomas of the acoustic nerve (author's transl)].

Reviewing the newest literature, an acoustic neuroma can be detected in 96% of the cases, employing brainstem audiometry. The present study aims to answer the question how good small acoustic neuromas can be detected using this method. Fifty patients with an asymmetric hearing loss, with unilateral caloric deficit and short lasting periods of dizziness when changing position were investigated. Brainstem audiometry as well as metrizamide-meatocisternography were carried out. Thus we were able to detect seven small acoustic neuromas; only one tumor could not be detected with the brainstem audiometry. The findings will be shown and discussed.

[Advantages and risks in metrizamide-meatocisternography (author's transl)].

Meatocisternography with iodinated lipoid contrast media has been proven one of the most reliable diagnostic methods in the evaluation of small cerebellopontine angle tumors. Use of this kind of positive contrast media, however, has certain disadvantages. Some of these disadvantages can be avoided using water soluble contrast medium. Possible adverse reactions demand premedication and clinical observation over a period of 24 hours.

Cerebellopontine Angle↗

[Tomoscopy].

UNLABELLED: Report on initial clinical experience with a new form of tomography with fluoroscopy, called 'tomoscopy'. PRINCIPLE: Several grid controlled x-ray tubes firmly mounted (stationary) in circular fashion -- with the foci in equidistance -- are fired in sequential fashion for a short exposure, which creates the impression of tube movement mandatory in conventional tomography, by this sequential firing of several tubes (in 1/50 second total), the mechanical tube motion of conventional tomography can be avoided. In the image intensifier, there is synchronous electromagnetic deflection of the image between input- and output screen; and a tomographic image can be seen at the output screen. This can also be recorded on camera. This new technique allows quick fluoroscopic determination of the ideal 'cut' and thus provides efficient tomography without slowing down the ongoing examination; and it is a useful addition to modern radiographic equipment with image intensification, especially when used with contrast material. Its full future impact on diagnostic radiology can yet not be estimated.

Giant aneurysms of the posterior fossa suspected as neoplasms on computed tomography.

Giant intracranial aneurysms of the vertebral and basilar artery can be mistakenly diagnosed as neoplasms of the posterior fossa on the basis of their clinical presentation and CT findings. The following report of three cases of tumorous aneurysms emphasizes the complementary value of angiography and computed tomography for the localization and correct diagnosis of space-occupying lesions of the posterior fossa.

[Technique and diagnostic value of functional myelography in lumbar discopathy (author's transl)].

A standard myelogram using water soluble contrast with additional functional examination were carried out on 72 patients with suspected disc prolapse. Diagnostic accuracy can be significantly improved by the functional examination and, in some cases, the diagnosis can only be made in this way. There are no false positives. The advantages and limitations of the method are enumerated and its morphological significance is discussed.

[Adverse reactions in metrizamide-meatocisternography (author's transl)].

To avoid disantvantages and possible late reactions of oily contrast medium we performed 18 Meatocisternographies using the watersoluble contrast medium Metrizamide (Amipaque). Anti-convulsive premedication and clinical observance over a period of 24 h are necessary due to adverse reactions.

[Long-term follow-up study of the absorption of gas after encephalography in dilated intracranial cerebrospinal fluid spaces (author's transl)].

The absorption of gas after encephalography in different groups of patients was investigated in a long-term follow-up study. Group I (11 patients) showed no pathological findings in the encephalogram. Group II (8 patients) had encephalographic signs of diffuse cerebral atrophy. Group III (7 patients) demonstrated in the encephalogram characteristics of normal pressure hydrocephalus with variously located blocks of air in the subarachnoid space. In group I the gas located in the subarachnoid space had usually disappeared after 24 h, in some cases after 72 h. In group II the pattern of resorption was basically similar, although in one case cortical air could be seen at 96 h. In group III the absorption of air from the subarachnoid space began after 72 h in radiological relevant amounts. In none of the cases of group III could similar short times of resorption be seen in comparison to group I. It is considered that these two groups can be clearly distinguished from each other in the duration of resorption of air from the subarachnoid space. The importance of encephalography as a pathophysiological tool for investigation of both the disturbed resorption and circulation of the cerebrospinal fluid in normal pressure hydrocephalus is stressed.

[Neuroradiologic and clinical correlations in asymmetric lateral ventricles and posterior cornua in pneumo-encephalograms (author's transl)].

Retrospective study of 452 out of 4155 pneumoencephalograms continuously investigated by two independent observers over 20 years. Follow-up and statistics of frequency and sex-differences of asymmetries of lateral ventricles and posterior cornua. Criteria of selection: history and clinical findings in favor of a general cerebral cause and against circumscribed changes due to local damage. Technique standardized and equal bilateral filling in pneumoencaphalography. Results of the numerous findings: 1. Posterior cornua were symmetrical in 370 patients 81.9% (256 male, 114 female, 18.1%). Definite asymmetry of posterior horns in 72 males and 10 males. 2. These asymmetries showed variations from unilateral to bilateral, but clearly different sizes of posterior horns. 3. Unilateral posterior horn "anlagen" seen in 64 (14.1%) of all patients. Males predominated 8:1, left more common than right 3.5:1. 4. Bilateral "anlagen", with one posterior horn predominating, in 18 patients (3.9%). Once more males more common 8:1. Larger "anlagen" on left: 8:1. 5. Accompanying further asymmetries of lateral ventricles with equal bilateral posterior horns were of similar size in 73.7%. In 21.1% the lateral ventricle was larger on the right, in 5.1% on the left. The left side was more common 4.15:1. 6. Large ventricles were of equal size in 45.1 of patients with asymmetry of the posterior horns, 45.1% on left, 9.7% on right the ventricle was larger. Once more the left side predominated 4.6:1. 7. Trying to correlate clinical-neurologic, neurophthalmic, neurophysiologic and tachistocopic findings with pneumoencephalograms with or without asymmetry showed no regular relation with handedness, ocular dominance, limitations of visual fields or--neglect or shapes of ventricles. In all subgroups frequency and sex-distribution corresponded to the norm. 8. Numerical frequency of coupling of right-handedness and right predominant eye with left increased size of ventricle was within the statistically expected and was, therefore, not interpreted as a sign of functional dominance of the left cerebral hemisphere, as done by others. 9. When eliciting from the occiput visually evoked potentials 8 patients showed important findings, in spite of their small number. In 3 patients with symmetrical "anlagen" of the posterior horns, latency and amplitude of potentials were the same; in 5 patients, with asymmetry of posterior horns the side of the larger horn showed significant reduction of amplitude without exception. The investigations show, quantitative statistical findings of frequency and sex-distribution and their negative correlation with examined clinical parameters apart, the anatomic-morphologic variation of asymmetry of posterior horns corresponds to a correlate which can also be shown with a neurophysiologic technique.

[Meatocisternography with water-soluble contrast medium for diagnosis of cerebellopontine angle tumors (author's transl)].

First experiences with Metrizamide in Meatocisternography for evaluation of cerebellopontine angles tumors are reported. The diagnostic value of this method to demonstrate space occupying lesions in this area could be shown. Severe side effects had not been observed. Simultaneous monitoring of EEG, ENG and ECG showed no pathological alterations.
