Maxillary sinus augmentation using xenogenic bone substitute material Bio-Oss in combination with venous blood. A histologic and histomorphometric study in humans.

The aim of the present study was to evaluate bone formation following maxillary sinus augmentation using bovine bone substitute material Bio-Oss in combination with venous blood by means of histologic and histomorphometric examination of human biopsies. This involved a total of 15 sinus floor elevation procedures being carried out on 11 patients (average age of 49.6 years) according to the technique described by Tatum (1986). The subantral sinus cavity was augmented using bovine apatite combined with venous blood. After an average healing phase of 6.8 months, trephine burrs were used to take 22 bone biopsies from the augmented sinus region. Then 38 Brånemark implants were inserted in both the osteotomies resulting from bone sampling and in regular sites in the augmented posterior maxilla. Histomorphometric analysis of ground sections from the bone biopsies prepared according to the standard method of Donath & Breuner (1982) produced an average percentage of newly-formed bone of 14.7% (+/- 5.0%) and a proportion of residual xenogenic bone substitute material of 29.7% (+/- 7.8%). Some 29.1% (+/- 8.1%) of the surface of the Bio-Oss granulate was in direct contact with newly-formed bone. Histologically, newly-developed bone became evident, partly invaginating the particles of apatite and forming bridges in the form of trabeculae between the individual Bio-Oss particles. Despite the absence of osteoclastic activity, the inward growth of bone indicates slow resorption of the xenogenic bone graft material. When the implants were uncovered, after an average healing phase of 6 months, 4 of the 38 implants had become loose. Of these 4 implants, 1 had to be subsequently explanted, while the others remained as "sleeping implants" and were not included in the implants superstructure. Thus, the resulting clinical survival rate, prior to prosthetic loading, was 89.5%.

Bronchiolitis obliterans organizing pneumonia: a distinct pulmonary complication in cystic fibrosis.

Organizing pneumonia in cystic fibrosis has hitherto been considered a nonspecific reparative process. We report on an adult patient with cystic fibrosis and histologically proven bronchiolitis obliterans organizing pneumonia, who experienced sustained clinical improvement under corticosteroid therapy. This case suggests that bronchiolitis obliterans organizing pneumonia may be a distinct pulmonary complication in cystic fibrosis and improve with specific therapy.

MIB-1 immunohistometry of follicular adenoma and follicular carcinoma of the thyroid gland.

OBJECTIVE: To analyze cellular proliferative activity, MIB-1 immunopositivity of normal tissue (n = 20), follicular adenoma (n = 30) and follicular carcinoma (n = 32) of the thyroid gland was analyzed by means of immunohistometry. STUDY DESIGN: Immunohistochemical reactions were performed on 3-micron sections from routinely formalin fixed and paraffin embedded surgical specimens using an indirect peroxidase method. The rate of immunostained cells was determined using the CM-2 TV image analysis system (Hund, Wetzlar, Federal Republic of Germany). Forty viewing fields (1.94 mm2) were measured with 20:1 objective magnification. An average of 5,965 cells were assessed in each case. RESULTS: Mean MIB-1 immunopositivity was higher in follicular carcinoma (average, 2.30%) and follicular adenoma (0.58%) than in normal thyroid tissue (0.14%). The distribution of single values differed significantly between groups (P < .001). To test the suitability of MIB-1 immunohistometry for the differential diagnosis of follicular adenoma and follicular carcinoma, different four-field tables with varying thresholds were calculated. Using a threshold of 0.9%, follicular carcinoma could be detected with a sensitivity of 75% (24/32) and a specificity of 83% (25/30). If a specificity of 90% is required (27/30), the sensitivity of the test decreases to 69% (22/32), based on a threshold of 1.1%. CONCLUSION: As some overlap of single values has to be considered, MIB-1 immunohistometry, although presenting new insights into the proliferative potential of thyroid lesions, is of only limited value for the differential diagnosis of follicular lesions in routine surgical pathology.

[Comparative studies on the biological significance of the marker for proliferation Ki-67-Antigen and PCNA in primary ovarian carcinoma].

OBJECTIVE: It was shown in experimental and clinical investigations, that the biological behavior of malignant tumors is reflected by their proliferative activity. PCNA and Ki-67-Antigen are two nuclear antigens and considered to represent important markers of proliferation. We investigated their proliferation index in primary ovarian carcinomas and correlated the results with tumor stage, grading, histological type and survival. MATERIAL AND METHODS: The expression of PCNA and Ki-67-Antigen was immunohistochemically evaluated using the monoclonal antibodies MIB-1 and PC 10 on formalin-fixed, paraffin-embedded tissue of 49 patients. Statistical data were calculated by means of Fisher's Exact Test and Pearson's Chi 2 Test, survival was estimated by Kaplan Meier Curves. RESULTS: PCNA-expression was shown in all ovarian carcinomas and Ki-67-Antigen-expression was detected with one exception (98%) in all tumors, too. No correlation could be found between Ki-67-Antigen-expression and the prognostic factors mentioned above, whereas a high PCNA-expression was significantly correlated with the tumor grading (G3), (p < 0.05). Patients with ovarian carcinomas with high PCNA proliferation index showed the tendency of a shorter overall survival. DISCUSSION: Ki-67-Antigen and PCNA-expression could be detected in almost all primary ovarian carcinomas. PCNA compared to Ki-67-Antigen is considered to be more useful for the determination of the proliferative activity of ovarian carcinomas, although there was shown just a tendency of overall survival dependent on PCNA-expression, and there was a significant correlation only between PCNA-proliferation index and tumor grading.

Prognostic value of DNA image cytometry in resected colorectal hepatic metastases.

OBJECTIVE: To determine the role of DNA image cytometry (DNA ICM) as a useful predictor of outcome following the resection of colorectal hepatic metastases. STUDY DESIGN: In 75 patients (56 R0 resections) with resected colorectal hepatic metastases, DNA ICM was performed on paraffin-embedded specimens. The DNA content of 250 tumor cells was determined in each specimen, and the 2c level was evaluated using 30 granulocytes from the same sample. RESULTS: Common algorithms of DNA ICM, such as maximum DNA content, 5c exceeding rate, 9c exceeding rate, 2c deviation index and the DNA grade of malignancy, identified a group of patients with favorable survival following R0 resection. Clinical findings failed to serve as a prognostic factor. A multivariate analysis revealed the maximum DNA content to be an independent factor influencing postoperative survival. CONCLUSION: DNA ICM is associated with the biologic aggressiveness of colorectal hepatic metastases and is useful as a prognostic marker in patients after resection.

Rapid and prognostically valid quantification of immunohistochemical reactions by immunohistometry of the most positive tumour focus. A prospective follow-up study on breast cancer using antibodies against MIB-1, PCNA, ER, and PR.

The prognostic significance of immunohistochemical markers for cell proliferation [MIB-1, proliferating cell nuclear antigen (PCNA)] and hormone receptor analysis [oestrogen receptor (ER), progesterone receptor (PR)] was tested by means of immunohistometry in a series of 103 breast cancer patients in comparison with the lymph node status N, the tumour size T, histomorphological grading, and the biochemical ER and PR status. Immunohistochemical reactions were performed on 2 microns sections from paraffin-embedded tissue, using an indirect peroxidase method. The proportion of immunostained tumour cell nuclei was determined using a TV-image analysis system. Measurements were performed using a 20 x objective on 40 viewing fields (1.94 mm2, MIB-1 and PCNA) or 20 viewing fields (0.97 mm2, ER and PR). The mean immunopositivity of all viewing fields and the value of the most immunopositive viewing field (MIB-1max, PCNAmax, PRmax, ERmax) were calculated. The mean values and the maximal values were highly correlated (r = 0.903, P < 0.001). After 1:2:1 quantilization, 84.2 per cent of the 412 single measurements revealed mean and maximal values in the same category (P < 0.0001). For each of the four immunohistochemical markers, the prognostic significance of the maximal values was higher than that of the mean values. The highest prognostic significance was found for MIC-1max (P = 0.0002), followed by PRmax (P = 0.0046), ERmax (P = 0.0154), and PCNAmax (P = 0.0161). From the results of a Cox model, a 'prognostic index (PI)' was developed, ranging from -1 to 8: PI = 2 x N + T + MIB-1max-PRmax. The four groups of patients with PI values of < 2, 2-3, 4-5, and > 5 revealed significantly different 7.5-year survival probabilities (P < 0.0001). The simplicity of the PI makes it a clinically useful, routinely applicable, and understandable parameter in the surgical pathology of breast carcinoma.

MIB-1 immunohistometry on Tru-cut biopsies in inflammatory and cirrhotic liver disease.

OBJECTIVE: To use immunohistometry to analyze the MIB-1 immunopositivity of normal liver parenchyma (n = 25) and of different inflammatory and degenerative liver diseases--including nonspecific inflammation (n = 12), toxic-nutritive (alcoholic) inflammation (n = 12), ascending cholangitis (n = 11), chronic-persisting hepatitis (n = 27), chronic-aggressive hepatitis (n = 28) and liver cirrhosis (n = 23). STUDY DESIGN: Immunohistochemical reactions were performed on 3-micron sections of paraffin-embedded Tru-cut biopsies using an indirect peroxidase method. The rate of immunostained cells was determined using the CM-2 TV image analysis system. Twenty viewing fields (0.97 mm2) were measured with 20:1 objective magnification. An average of 2,600 cells were assessed in each case. RESULTS: The values for normal liver tissue were significantly lower as compared with those of all other groups (< .0001 < P < or = .0008). In nonspecific and toxic inflammation and ascending cholangitis, the values did not differ significantly (P > .05). As compared to chronic-persisting viral hepatitis, however, the MIB-1 immunopositivity of nonspecific (P = .0056) and toxic inflammation (P = .0162) was significantly lower. Apart from ascending cholangitis, chronic-aggressive viral hepatitis revealed significantly increased MIB-1 immunopositivity as compared to all other types of inflammation (P < .0001). The MIB-1 values were significantly higher in liver cirrhosis than in the different inflammatory conditions (< .0001 < P < or = .0105) with the exception of chronic-aggressive viral hepatitis. The highest value for normal liver tissue was 0.4%; it was lower than 96% of the values in the different disease groups. Seventy-two percent of the cases with virally induced hepatitis revealed values > 1.5% as compared to only 14% in the other types of inflammation. In chronic-aggressive hepatitis and liver cirrhosis, 71% and 65%, respectively, of the values were > 2%. CONCLUSION: MIB-1 immunohistometry allowed the detection of significantly different proliferative activity among different types of inflammatory and degenerative liver diseases, indicating diagnostic value in histologically inconclusive cases.

Local expression of cytokines in rat bladder carcinoma tissue after intravesical treatment with Nocardia rubra cell wall skeleton and bacille-Calmette-Guerin.

This study aimed to investigate local immunotherapy with Nocardia rubra cell wall skeleton (N-CWS) and bacille-Calmette-Guerin (BCG) in chemically induced rat bladder carcinoma. After being fed with 0.025% N-butyl-N-(4-hydroxybutyl) nitrosamine for 26 weeks, female Wistar rats were treated once a week by intravesical instillation of 100 microg N-CWS or 5 x 10(6) Colony-Forming Units (CFU) BCG. Tissue specimens were obtained 4 h after the ninth instillation and analyzed by reverse transcription polymerase chain reaction (RT-PCR) for mRNA expression of rat cytokines. The analysis showed high expression of interleukin (IL)-1 alpha, tumor necrosis factor (TNF)-alpha, IL-2, and interferon (IFN)-tau in BCG (7/7, 7/7, 7/7, 5/7) and N-CWS (9/9, 9/9, 8/9, 8/9) treated tumors in comparison to low expression in controls (3/9, 0/9, 3/9, 3/9). Interestingly, intravesical instillation of N-CWS tended to be less effective at preventing local invasion of the tumors. It is speculated that this difference may result from a more strongly induced expression of T-helper cell-derived lymphokines (IL-2, IFN-tau) by BCG.

[Methodologic aspects of a standardized evaluation of mitotic activity in tumor tissues].

Although estimation of mitotic activity is generally a recognized clinical practice in the diagnosis and grading of tumors, there are several methodologic aspects that merit special consideration. Our results, in relation to those of others, lead to the following conclusions: 1. The values attained by calculating the mitotic activity of tumor cells in paraffin-embedded tumor tissue are lower than those obtained in vivo. The results cannot always be interpreted clinically due to the complexity of some of the factors associated with it which require special attention. That the method still has clinical relevance and value in spite of the associated problems may be due to the fact that tumor tissues generally undergo long periods of hypoxia before being fixed for histomorphological analyses--a condition which by itself is a decisive factor in the reduction of mitotic activity--so that additional changes in some individual factors from case to case do not make much difference to the estimated or calculated value. 2. Mitotic activity should be calculated in areas of tumor sections where there are numerous mitotic tumor cells. Additionally, only cells with nuclei showing lysed nuclear membranes and identifiable single chromosomes at higher magnification ought to be considered in the calculation. Apoptotic and pyknotic elements should not be calculated. 3. The specification of the final magnification (e.g., x 400) is not a sufficient basis for comparison of optical conditions for the mitotic estimation, since the visual fields of standard light microscopes may differ up to 2.6-fold. A temporary streamlining of the standard may be the application of a 0.159 mm2 visual field (x 40 objective, x 10 ocular by visual field value 18). As an intermediate goal, mitosis should be calculated in percentages of the tumor cells or areas of the total tumor tissue. 4. The interspecific reproducibility of the quantified mitosis makes it reliable for clinical application. It compares well with cytometric and morphometric methods in assessment of tumor cells and is of higher clinical relevance than the conventional histomorphological tumor grading systems.

[Incidence of inflammatory placental changes in threatened premature labor with and without additional antibiotic therapy].

Considering the causal association of silent intrauterine infection and prematurity we investigated the possible effect of adjuvant antibiotic treatment of women with preterm labour on the appearance of inflammatory placental lesions. 140 patients with preterm labour in the 30 + 2 week of gestation (median; range: 17 + 2-34 + 6) without premature rupture of the membranes and detection of facultative-pathogenic micro-organisms in the vagina and/or in the canal of the cervix were enrolled in the study. 74 women were treated vaginally (polyvidone-iodine) in addition to intravenous tocolysis, 66 women were given ampicillin, cefotiam or erythromycin intravenously. After delivery the placentas were examined histologically and the frequency of inflammatory lesions was evaluated by use of 4 scores of classification. For statistical analysis the Fisher Exact- and the Wilcoxon Rank Sum Test were used. We found no differences concerning amnamnestic and perinatal parameters comparing the 2 groups of patients. With only one of the histological scores used (according to Salafia et al. [18]) we found a higher frequency of inflammatory placental lesion in the antibiotic treated group (12/66) in comparison to the vaginal treated group (4/74). Fifty patients of the antibiotics' group received the antibiotic during the last 10 days before birth. No differences in the frequency of inflammatory placental lesions were detectable in these patients when compared with the local treated group. However, we found a lower prolongation of gestation (calculated from the day of admission to the day of delivery, median: 7; range: 1-92 days) and a lower gestational age at delivery (median: 33 + 0; range: 22 + 2-39 + 6 weeks) in the patients receiving antibiotics during the last 10 days before birth in comparison to the local treated women (22; 1-138 days and 35 + 0; 23 + 4-41 + 5 weeks, respectively). There is the same incidence of inflammatory placental lesions in patients with preterm labour and facultative-pathogenic micro-organisms in the vagina and/or in the canal of the cervix who received adjuvant antibiotic treatment during pregnancy compared with patients who were treated vaginally with polyvidone-iodine.

[Uterine metastasis of invasive lobular breast carcinoma. Case report and review of the literature with reference to differential diagnostic problems and clinical consequences].

Uterine metastases of malignant tumours are rare. Among extragenital malignancies breast cancers of the lobular type most likely spread out to this site. The respective diagnosis is rendered more difficult by the late onset of clinical symptoms that may imitate the picture of primary uterine cancer. We report on a patient with advanced invasive lobular breast cancer (ILC) where this form of metastazation was detected after conization for PAP IVa. Especially in patients with advanced lobular breast cancer and suspicious symptoms (such as pathologic PAP-Smear, conspicuous colposcopic finding, vaginal bleeding, growth-progressive uterus and hyperplastic endometrium) the possibility of uterine metastases should be included in preoperative planning.

Mitotic frequency as a prognostic factor in breast cancer.

Grading of tumor malignancy in breast cancer should contribute essential information both for the prospective outcome of the individual patient as well as for TNM staging. In a series of 104 breast cancer patients we tested the prognostic validity and reproducibility of mitotic figure counting compared with TNM staging, Bloom and Richardson grading, DNA single cell cytometry, and morphometry. Four-micrometer thick hematoxylin-eosin-stained routine slides were investigated. Mitotic figures were counted in representative areas of the tumor in 10 159-microns 2-sized high power fields (HPFs) at a 400X magnification; the median value was seven and the threshold for the 25th percentile was three. This value should replace the common but prognostically invalid threshold of 10. Univariate survival analysis showed that mitotic figure counting allows the identification of three groups of patients (< or = 3, 4 to 20, > 20 mitoses per HPF) with significantly different survival probabilities (P < .0001; P = .0178). Depending on the number of mitotic figures, length of survival was significantly different within the group of T1N0 tumors (P = .0082) and the group of T1N1 or T2N0 tumors (P = .0251). In a Cox stepwise regression model mitotic frequency counting added prognostic information to tumor size and was of higher prognostic significance than lymph node status, DNA ploidy, or mean nuclear area. The 95% confidence limit for interobserver reproducibility, tested in 20 cases, was plus/minus 8 mitoses. After quartilization an agreement of 75% was observed.

Experimental arterial stent placement. Comparison of a new Nitinol stent and Wallstent.

RATIONALE AND OBJECTIVES: To compare technical aspects and neointima formation of a new Nitinol stent and a Wallstent. METHODS: A Nitinol sten and a Wallstent were placed into the iliac arteries of 14 sheep (total of 28 stents) and followed angiographically. Seven sheep were killed after 1 month and 7 after 4.5 months. Examination of histologic specimen by light microscopy was performed to determine neointimal and medial thickness inside the stents. Measurements were analyzed statistically. RESULTS: Neointima was thicker in Nitinol stents (N) than in Wallstents (W) (P < 0.004), both adjacent to (W = 0.265 mm, N = 0.596 mm) and between the stent wires (W = 0.177 mm, N = 0.361 mm). Because of poor radiopacity and the Crochê releasing technique, delivery was more difficult with Nitinol stents. High-grade stenosis was found in two Nitinol stents at 1 and 4.5 months of follow-up, respectively. One Wallstent was insufficiently incorporated into the vessel wall. CONCLUSIONS: The observed difference in neointima formation that may be caused by differences in design, mechanical properties or the metal alloy of the stents probably is too insignificant to be relevant to patency of large arteries. However, it may be relevant in small vessels such as coronary arteries.

Aneuploidy in actinic keratosis and Bowen's disease--increased risk for invasive squamous cell carcinoma?

The value of DNA single cell cytometry for the detection of aneuploidy was assessed in 100 specimens of actinic keratoses and 39 specimens of Bowen's disease. Ten seborrhoeic keratoses and 10 samples of normal epidermis served as negative control groups. Monolayer smears, prepared from formalin-fixed, paraffin-embedded tissues, were Feulgen-stained and used for interactive DNA-cytometry. In each specimen, the DNA content of 150 randomly chosen squamous epithelial cells was measured, using a TV-image analysis system (TAS-plus, Leica, Germany). Aneuploidy was diagnosed if at least three nuclei with a DNA content above 5c (5cEE > or = 3) were found. The aneuploidy rate in actinic keratosis was 69% (69 of 100) and in Bowen's disease was 95% (37 of 39). Another 20 specimens of actinic keratoses and the remaining two specimens of Bowen's disease were diagnosed as suspicious for aneuploidy (0 < 5cEE < 3). The 20 specimens of seborrhoeic keratoses and normal epidermis did not show any nuclei above the 5c level, and were classified as non-aneuploid. This indicates a sensitivity of 76% (106 of 139) and a specificity of 100% (20 of 20). The frequent occurrence of aneuploidy in actinic keratoses and Bowen's disease underlines the character of the lesions as epidermal carcinomas in situ, but does not explain the long-term low incidence of invasive growth.
